Triple tube drainage for “difficult” gastroduodenal perforations:A prospective study

AIMTo prospectively study the outcome of difficult gastroduodenal perforations (GDPs) treated by triple tube drainage (TTD) in order to standardize the *** presenting to a single surgical unit of a tertiary hospital with difficult GDPs (large, unfavourable local and systemic factors) were treated with TTD (gastrostomy, duodenostomy and feeding jejunostomy). Postoperative parameters were observed like time to return of bowel sounds, time to start enteral feeds, time to start oral feeds, daily output of all drains, time to clamping/removal of all drains, time for skin to heal, complications, hospital stay, and, mortality. Descriptive statistics were *** December 2013 and April 2015, 20 patients undergoing TTD for GDP were included, with mean age of 44.6 ± 19.8 years and male:female ratio of 17:3. Mean pre-operative APACHE II scores were 10.85 ± 3.55; most GDPs were prepyloric (9/20; 45%) or proximal duodenal (8/20; 40%) and mean size was 1.83 ± 0.59 cm (largest 2.5 cm). Median times of resumption of enteral feeding, removal of gastrostomy, removal of duodenostomy, removal of feeding jejunostomy and oral feeding were 4 d (4-5 IQR), 13 (12-16.5 IQR), 16 (16.25-22.25 IQR), 18 (16.5-24 IQR) and 12 d (10.75-18.5 IQR) respectively. Median hospital stay was 22 d (19-26 IQR) while mortality was 4/20 (20%).CONCLUSIONTTD for difficult GDP is feasible, easy in the emergency, and patients recover in two-three weeks. It obviates the need for technically demanding and riskier procedures.

Accuracy of preoperative CT liver volumetry in living donor hepatectomy and its clinical implications

Background: An accurate preoperative volumetric assessment of donor liver is essential for successful living donor liver transplant by ensuring adequate remnant and graft recipient weight ratio (GRWR). Methods: The study cohort consisted of 744 right lobe (RL), 65 left lobe (LL) and 33 left lateral sector (LLS) grafts from July 2010 to January 2014. A semi-automated interactive commercial software called AW Volume share 6 was used for volumetry. Bland Altman plot was used for assessing the agreement between estimated graft weight (EGW) and actual graft weight (AGW). Results: There was no statistically significant difference between EGW and AGW for RL graft weight (722±134 vs. 717±126 gm;P=0.06). Although Bland Altman graph showed that 95% limits of agreement was more in LL (?164 to +110) than RL (?156 to +147) and LLS grafts (?137 to +239), CT scan significantly overestimated LL graft weight (EGW =460±118 gm vs. AGW =433±102 gm;P=0.003) and underestimated LLS graft weight (EGW =203±48 gm vs. AGW =254±49 gm;Pking higher GRWR.

Beach pea or beach cowpea(Vigna marina(Burm.)Merr.)belongs to the family *** is a close relative of cultivated Vigna species such as adzuki bean(***),cowpea(***),mung bean(***),and blackgram(***),and is distributed throughout the *** its ability to tolerate salt stress,beach pea has great potential to contribute salt-tolerance genes for developing salt-tolerant cultivars in cultivated Vigna ***,it is still underutilized in Vigna breeding programs.A draft genome sequence of beach pea was generated using a high-throughput next-generation sequencing platform,yielding 23.7 Gb of sequence from 79,929,868 filtered reads.A de novo genome assembly containing 68,731 scaffolds gave an N50 length of 10,272 bp and the assembled sequences totaled 365.6 Mb.A total of 35,448 SSRs,including 3574 compound SSRs,were identified and primer pairs for most of these SSRs were *** analysis identified 50,670 genes with mean coding sequence length 1042 *** analysis revealed highest sequence similarity with ***,followed by *** with the *** genome revealed 16,699 SNPs and 2253 InDels and comparison with the *** genome revealed 17,538 SNPs and 2300 *** our knowledge this is the first draft genome sequence of beach pea derived from an accession(ANBp-14-03)adapted locally in the Andaman and Nicobar Islands of *** draft genome sequence may facilitate the genetic enhancement in cultivated Vigna species.

Simulation of Runoff and Sediment Yield for a Himalayan Watershed Using SWAT Model

Watershed is considered to be the ideal unit for management of the natural resources. Extraction of water-shed parameters using Remote Sensing and Geographical Information System (GIS) and use of mathematical models is the current trend for hydrologic evaluation of watersheds. The Soil and Water Assessment Tool (SWAT) having an interface with ArcView GIS software (AVSWAT2000/X) was selected for the estimation of runoff and sediment yield from an area of Suni to kasol, an intermediate watershed of Satluj river, located in Western Himalayan region. The model was calibrated for the years 1993 & 1994 and validated with the observed runoff and sediment yield for the years 1995, 1996 and 1997. The performance of the model was evaluated using statistical and graphical methods to assess the capability of the model in simulating the run-off and sediment yield from the study area. The coefficient of determination (R2) for the daily and monthly runoff was obtained as 0.53 and 0.90 respectively for the calibration period and 0.33 and 0.62 respectively for the validation period. The R2 value in estimating the daily and monthly sediment yield during calibration was computed as 0.33 and 0.38 respectively. The R2 for daily and monthly sediment yield values for 1995 to 1997 was observed to be 0.26 and 0.47.

Growth, photosynthesis and nitrogen metabolism in soybean varieties after exclusion of the UV-B and UV-A/B components of solar radiation

A field experiment was conducted to study the impact of the exclusion of the solar UV components on growth, photosynthesis and nitrogen metabolism in soybean(Glycine max)varieties Pk-472, Pusa-24, JS 71-05, JS-335, NRC-7 and kalitur. The plants were grown in specially designed UV exclusion chambers wrapped with filters to exclude UV-B or UV-A/B and transmitted all UV. Exclusion of UV significantly enhanced the growth of the aerial parts as well as the growth of the below ground parts in all of the six soybean *** reductase activity(NRA) was significantly reduced, whereas leghemoglobin(Lb)content, total soluble protein, net photosynthesis(Pn) and α-tocopherol content were enhanced after UV exclusion. The exclusion of solar UV-A/B enhanced all parameters to a larger extent than the exclusion of solar UV-B in four of the six varieties of soybean except for NRC-7 and kalitur. These two varieties responded more to UV-B exclusion compared to UV-A/B exclusion. A significant inverse correlation between the NRA and the number of nodules per plant was observed. The extent of response in all parameters was greater in Pk-472 and JS71-05 than that in kalitur and JS-335 after UV exclusion. The exclusion of UV augmented the growth of nodules, Lb content and α-tocopherol levels and conferred higher rates of Pnto support better growth of nodules. Control plants(+ UV-A/B) seemed to fulfill their N demand through the assimilation of NO-3resulting in lower symbiotic nitrogen fixation and higher NR activity.

Handling higher order time series forecasting approach in intuitionistic fuzzy environment
