Circulating levels of vitamin D and colorectal adenoma: A case-control study and a meta-analysis

AIM: To examine the association between circulating 25-hydroxyvitamin D [25(OH)D] levels and colorectal adenoma in a case-control study and a meta-analysis. METHODS: We conducted a matched case-control study(112 cases and 112 matched controls) and combined 15 studies, including our study, in a metaanalysis. The study-specific odds ratios(ORs) and 95% confidence intervals(CIs) were pooled using a randomeffects model. In total, 5454 colorectal adenomas and 6656 controls were included in the meta-analysis. RESULTS: In a meta-analysis including 14 previous studies and our study, we observed a significant inverse association between circulating 25(OH)D levels and colorectal adenoma(OR = 0.68; 95%CI: 0.54-0.82) when comparing the highest category with the lowest category. Stratification by adenoma location(proximal or distal adenoma) showed similar estimates. When we stratified by study region, the ORs(95%CIs) were 0.70(0.52-0.88) in the US and 0.66(0.34-0.97) in Asia. CONCLUSION: These data suggest an inverse association between circulating 25(OH) Dlevels and colorectal adenoma in both Western and Asian populations.

BACKGROUND New prognostic factors have been reported in patients with metastatic or recurrent gastric cancer(MRGC),necessitating modifications to the previous prognostic *** To develop a new model,MRGC patients who received fluoropyrimidines/platinum doublet chemotherapy between 2008 and 2015 were *** A total of 1883 patients was divided into a training set(n=937)and an independent validation set(n=946).RESULTS Multivariate analysis showed that the following six factors were associated with poor overall survival(OS)in the training set:Eastern Cooperative Oncology Group performance score≥2 and bone metastasis(2 points each),peritoneal metastasis,high alkaline phosphatase level,low albumin level,and high neutrophil-lymphocyte ratio(1 point each).A prognostic model was developed by stratifying patients into good(0-1 point),moderate(2-3 points),and poor(≥4 points)risk *** the validation set,the median OS of the three risk groups was 15.8,10.1,and 5.7 mo,respectively,and those differences were significant(P<0.001).CONCLUSION We identified six factors readily measured in clinical practice that are predictive of poor prognosis in patients with *** new model is simpler than the old and more easily predicts OS.

This paper provides an overview of South Korea’s 20-year journey in adopting building information modeling(BIM) and future direction. It first discusses the six phases of BIM adoption in South Korea, starting from the use of BIM as a marketing tool to its current intelligent BIM phase. The government’s support for BIM-related research and development projects is also highlighted, with a focus on the artificail intelligence (AI)-based architectural design automation project. As the future direction, it explores the integration of AI with BIM in both local and global contexts. The paper presents AIpowered architectural design methods, including AI-powered early architectural design generation and architectural *** to AI-based early architectural design generation, architectural detailing is an unexplored research topic. This paper introduces two AI-and BIM-based architectural detailing methods, being developed at Yonsei University:namely,BIM library transplant and Natural language-based Architectural Detailing through Interaction with AI (NADIA). These methods demonstrate how AI-enhanced BIM can enable architects to interactively develop building details using a language model as a conversational AI and a knowledge base, and a BIM authoring tool as a design platform, in the near future.

Prognostic value of inflammation-based markers in patients with pancreatic cancer administered gemcitabine and erlotinib

AIM: To evaluate the value of systemic inflammationbased markers as prognostic factors for advanced pancreatic cancer(PC). METHODS: Data from 82 patients who underwent combination chemotherapy with gemcitabine and erlotinib for PC from 2011 to 2014 were collected retrospectively. Data that included the neutrophil-to-lymphocyte ratio(NLR), the platelet-to-lymphocyte ratio, and the C-reactive protein(CRP)-to-albumin(CRP/Alb) ratio were analyzed. Kaplan-Meier curves, and univariate and multivariate Cox proportional hazards regression analyses were used to identify the prognostic factors associated with progression-free survival(PFS) and overall survival(OS). RESULTS: The univariate analysis demonstrated the prognostic value of the NLR(P = 0.049) and the CRP/Alb ratio(P = 0.047) in relation to PFS, and a positiverelationship between an increase in inflammation-based markers and a poor prognosis in relation to OS. The multivariate analysis determined that an increased NLR(hazard ratio = 2.76, 95%CI: 1.33-5.75, P = 0.007) is an independent prognostic factor for poor OS. There was no association between the PLR and the patients' prognoses in those who had received chemotherapy that comprised gemcitabine and erlotinib in combination. The Kaplan-Meier method and the log-rank test determined significantly worse outcomes in relation to PFS and OS in patients with an NLR > 5 or a CRP/Alb ratio > ***: Systemic inflammation-based markers, including increases in the NLR and the CRP/Alb ratio, may be useful for predicting PC prognoses.

Three hundred thirty-six Ross 308 male broiler chicks were used in a 21-d study to explore performance and gut function when treated with a proton pump inhibitor(PPI;0 or 89 mg/kg)in a 2×2 factorial arrangement with a xylanase(Xyl;0 or 0.1 g/kg)to determine if the beneficial activity of arabinoxylan(AX)depolymerisation,through arabinoxylo-oligosaccharides(AXOS)production,starts in the upper gastrointestinal *** with the PPI started from d 14,and by d 21 animal performance had deteriorated(Pdole,p-cresol,ammonia and the ratio of total volatile fatty acid(VFA)to butyric acid were increased with PPI(P<0.05),indicating enhanced protein *** activity in the digesta were greatest in the caeca,especially when Xyl was supplemented(P<0.001).The concentration of total soluble AX was greater in the gizzard and ileal digesta with Xyl supplementation(P<0.05),supporting the depolymerisation action of xylanase even under acidic *** data suggest xylanase may function in the gizzard even though pH is not optimal for activity and emphasises the importance of chlorohydric acid secretions in ensuring overall optimum gut *** depolymerisation benefits animal performance although it is still unknown how the AXOS produced with xylanase supplementation in the upper gastrointestinal tract influence the microbial populations and overall gut functionality.
