BACKGrOUND: selective inhibition of cyclooxygenase-2 (COX-2) may be associat ed with an increased r.sk of thrombotic events, but only limited long-term data have been available for analysis. We report on the cardiovascular outcomes asso ciated with the use of the selective COX-2 inhibitor rofecoxib in a long-term, multicenter, randomized,placebo-controlled, double-blind trial designed to de termine the effect of three years of treatment with rofecoxib on the r.sk of rec urrent neoplastic polyps of the large bowel in patients with a history of colore ctal ***: A total of 2586 patients with a history of colorectal ade nomas underwent randomization: 1287 were assigned to receive 25 mg of rofecoxib daily, and 1299 to receive placebo. All investigator-reported serious adverse e vents that repr.sented potential thrombotic cardiovascular events were adjudicat ed in a blinded fashion by an external committee. r.sULTs: A total of 46 patient s in the rofecoxib group had a confirmed thrombotic event during 3059 patient-y ears of follow-up (1.50 events per 100 patient-years), as compared with 26 pat ients in the placebo group during 3327 patient-years of follow-up(0.78 event p er 100 patient-years); the corr.sponding relative r.sk was 1.92 (95 percent con fidence interval, 1.19 to 3.11; P=0.008). The increased relative r.sk became apparent after 18 m onths of treatment; during the first 18 months, the event rates were similar in the two *** r.sults primarily reflect a greater number of myocardial infa rctions and ischemic cerebrovascular event s in the rofecoxib group. There was earlier.separation (at approximately five months) between groups in the incidence of nonadjudicated investigator-reported congestive heart failure, pulmonary edema, or cardiac failure (hazar d ratio for the compar.son of the rofecoxib group with the placebo group, 4.61 ; 95 percent confidence interval, 1.50 to 18.83). Acute effects of Helicobacter pylori extracts on gastric mucosal blood flow in the mouse

AIM： To investigate the mechanisms underlying the reduction in gastric blood flow induced by a luminal water extract of Hellcobacter pylori （HPE）. METHODs： The stomachs of isoflurane-anesthetized mice were exteriorized, and the mucosal surface exposed. Blood flow was measured with the laserDoppler technique, and systemic arterial blood pr.ssure monitored. C57BL/6 mice were exposed to water extract produced from Hpylori strain 88-23. To investigate the role of a nerveor iNOs-mediated pathway, we used intraluminal lidocaine and iNOs-/- mice. Blood flow r.sponse to the endogenous nitric oxide synthase inhibitor asymmetric dimethyl arginine （ADMA） was also assessed. r.sULTs： In wild-type mice, HPE decreased mucosal blood flow by approximately 30%. This reduction was abolished in iNOs-deficient mice, and by pre-treatment with lidocaine. Luminally applied ADMA r.sulted in reduction in blood flow similar to that observed in wildtype mice exposed to HPE. CONCLUsION： A H py/ori water extract reduces gastric mucosal blood flow acutely through iNOs- and nerve-mediated pathways.

OBJECTIVE: We sought to evaluate whether there are unique character.stics associated with irritable bowel syndrome (IBs)-within a population that has chronic pelvic pain. METHODs: This cr.ss-sectional study of new referral patients attending a pelvic pain clinic between 1993 and 2000 (N = 987) evaluated character.stics associated with IBs at entry to the clinic. The character.stics that we evaluated included demographic character.stics, clinical diagnoses, history of abuse, depr.ssion, pain, and prior abdominal surgeries. r.sULTs: Thirty-five percent of chronic pelvic pain patients had IBs defined by rome I criteria. Age 40 years or older (odds ratio [Or] = 1.98, 95%confidence interval [CI]: 1.27, 3.11), muscular back pain (Or = 5.37, 95%CI: 0.98, 29.29), symptom Checklist-90 global index score in top quartile (Or = 1.77, 95%CI: 1.09, 2.86), depr.ssion (Or = 1.93, 95%CI: 124, 3.01), 6 or more pain sites (Or = 1.67, 95%CI: 1.01, 2.78), and history of adult physical abuse (Or=1.51, 95%CI: 1.01, 2.26) were associated with IBs in the final reduced multivariable model. CONCLUsION: specific character.stics distinguish women with IBs suggesting that IBs and chronic pelvic pain are not simply manifestations of the same disorder. Our findings could help physicians attempt to effectively treat women with IBs and chronic pelvic pain. Physicians could diagnose and treat IBs in conjunction with treatment for chronic pelvic pain.

screening pre-bariatric surgery patients for esophageal disease with esophageal capsule endoscopy

AIM:To determine if esophageal capsule endoscopy(ECE)is an adequate diagnostic alternative to esophagogastroduodenoscopy(EGD)in pre-bariatric surgery ***:We conducted a pr.spective pilot study to assess the diagnostic accuracy of ECE(PillCam EsO2,Given Imaging)vs conventional EGD in pre-bariatric surgery *** who were scheduled for bariatric surgery and referred for pre-operative EGD were pr.spectively *** patients underwent ECE followed by standard *** experienced gastroenterologists blinded to the patient’s history and the findings of the EGD reviewed the ECE and documented their *** gold standard was the findings on ***:Ten patients with an average body mass index of 50 kg/m2were enrolled and completed the *** identified 11 of 14(79%)positive esophageal/gastroesophageal junction(GEJ)findings and 14of 17(82%)combined esophageal and gastric findings identified on ***’s exact test was used to compare the findings and no significant difference was found between ECE and EGD(P=0.64 for esophageal/GEJ and P=0.66 for combined esophageal and gastric findings r.spectively).Of the positive esophageal/GEJ findings,ECE failed to identify the following:hiatal hernia in two patients,mild esophagitis in two patients,and mild schatzki ring in two *** was able to identify the entire esophagus in 100%,gastric cardia in0%,gastric body in 100%,gastric antrum in 70%,pylor.s in 60%,and duodenum in 0%.CONCLUsION:There were no significant differences in the likelihood of identifying a positive finding using ECE compared with EGD in preoperative evaluation of bariatric patients.

Effect of Carboxyamidotriazole Orotate, a Modulator of Calcium-Dependent signaling Pathways, on Advanced solid Tumors

Pre-clinical studies suggest carboxyamidotriazole orotate (CTO) demonstrates anti-tumor activity through modulation of multiple tyr.sine kinase signaling pathways and interactions with the tumor microenvironment. We determined the safety and tolerability, pharmacokinetic profile, maximum tolerated dose, and recommended Phase II dose of CTO monotherapy in patients with advanced solid tumors. In this first-in-human Phase I clinical trial, eligible patients with advanced solid tumors were enrolled to receive a once-daily dose of CTO following a standard 3 + 3 Phase I design (starting at 50 mg/m2/day) with dose escalations of 30% - 100%. Dose limiting toxicity (DLT) was defined in the first cycle of treatment. Measurable disease and r.sponse were defined by rECIsT version 1.1. Forty-four patients were evaluable for.safety. CTO-related grade 3 toxicities included diarrhea (2.5%), fatigue (5.0%), lymphopenia (2.5%) and transient creatine phosphokinase (CPK) elevation (2.5%). There were no grade 4 or 5 toxicities. steady state plasma levels of CAI (CTO metabolite) were achieved by day 12 with a half life estimate of 55 hr. Although no objective r.sponse rates were observed, nine patients with rapidly progr.ssive and treatment-refractory tumors achieved stable disease (sD) durable for up to 14 months. The maximum tolerated dose for CTO alone was 427 mg/m2/day. The dose-limiting toxicity was grade 3 fatigue. CTO is orally bioavailable, safe, well tolerated and produces disease stabilization in a broad range of heavily treated refractory tumors. Combination trials of CTO with other antineoplastic agents are ongoing.

Purpose:The purpose of this study was to determine the best wound infection p rophylaxis in pediatric acute ***:From 1969 to 1995 inclusive,453 consecutive pediatric patients at the same children’s hospital had an app endix with acute inflammation(acute appendicitis)removed by the same staff sur geon and his *** stump was not inverted,and chromic catgut was used t *** intraperitoneal antibiotics,irrigation,or drains were used,and the skin closure was with silk sutur.s initially and then with staples since 198 *** infants and children were divided into 6 consecutive groups of 52 to 96 p atients,with each group lasting 2 to 5 *** wound treatment groups were a s follows:no treatment,drain or pack,drain or pack plus antibiotic powder,an tibiotic powder,preoperative intravenous antibiotic plus antibiotic powder,and preoperative intravenous *** wound Penr.se drain,one half-inch ga uze pack,and/or antibiotic powder(ampicillin,1977-1981;cefoxitin,1982-199 5)were all placed in the subcutaneous ***:There were a total of 50(11%)wound infections(pus)that occurred between 4 and 40 days when no antibi otic powder was used and 2 to 14 days with antibiotic *** all 6 groups of patients,no organism was grown in most(80%)infections and Escherichia coli was the second commonest(12%).The serous ooze,which occurred only with the u se of antibiotic powder(8%),was seen between 6 and 18 days,and no organism w as ever ***:The patients with preoperative(or intraoperative)intravenous antibiotics(cefoxitin)pluswound antibiotic powder(cefoxitin)ha d the lowest infection rate(2.5%).When this group was compared with the basel ine group 1(no treatment),it was the only group in which wound treatment made a significant difference(P =0.003).
