Long-Term Outcomes of Systemic-to-Pulmonary Artery Shunt in Patients with Functional Single Ventricle and Heterotaxy Syndrome

Objectives: We sought to determine the long-term outcomes and mortality-associated factors after systemic-topulmonaryartery shunt (SPS) in patients with heterotaxy syndrome. Methods: We retrospectively analyzed allpatients with a functional single ventricle and heterotaxy syndrome who underwent SPS at our center from January2001 to April 2022. Results: This study involved 84 patients. Restrictive pulmonary blood flow requiringearly modulation was presented in 34 (40%) patients. Compared with patients without restrictive pulmonaryblood flow (N = 50 [60%]), the postnatal survival of these 34 patients was significantly lower at 10 years (log rank:p = 0.04), but the statistical significance disappeared at 20 years (log rank: p = 0.18). Among 31 patients whounderwent SPS, 11 (35%) had extracardiac total anomalous pulmonary venous connection (TAPVC). The survivalrate after SPS was 80% at 10 years. Cox regression analysis showed that extracardiac TAPVC (hazard ratio6.44, 95% confidence interval 1.23–33.7, p = 0.03) and pulmonary venous obstruction (PVO) at TAPVC repair(hazard ratio 11.2, 95% confidence interval 2.13–58.5, p = 0.004) were significantly associated with death. In25 patients who underwent bidirectional cavopulmonary shunt (BCPS), surgical interventions on the pulmonaryartery (PA) were performed after SPS in 7 of 9 patients with PA coarctation, 3 of 4 with non-confluent PAs, and4 of 12 with normal PAs. At SPS, primary central PA plasty was performed in three patients with PA coarctationand 2 with non-confluent PAs. There was no significant difference in the PA index before BCPS between patientswith and without primary central PA plasty (p = 0.49). Among 20 patients who underwent total cavopulmonaryconnection (TCPC), adverse events occurred in 7 (35%) patients, including death in 1 (5%), intervention for pulmonaryarteriovenous malformation (PAVM) in 3 (15%), and surgical intervention for PVO in 3 (15%). Precocious Puberty in a 5-Year-Old Girl with a Giant Hypothalamic Hamartoma Discovered Perinatally: Case Report

Background: Hypothalamic hamartoma is a rare non-neoplastic lesion, typically identified in early childhood during investigations for precocious puberty and/or gelastic seizures. However, cases of diagnosis even earlier or during fetal development have been documented. Case Presentation: A newborn girl was diagnosed with hydrocephalus during pregnancy. An MRI revealed a large oval hypothalamic process, which suggested a hypothalamic hamartoma. At the age of 2, she experienced alternating crying and laughing seizures, followed by a decrease in visual acuity. Due to involvement of the optic pathways, surgery was not performed and she underwent antiepileptic medicines and gamma knife radiotherapy. At the age of 5 years and 3 months, she presented with breast development and laboratory tests confirmed central precocious puberty. Quarterly injections of GnRH agonists have since been administered with favorable results. Conclusion: Early-diagnosed hypothalamic hamartomas require close monitoring, by an experienced multidisciplinary, to promptly detect and treat potential complications, especially precocious puberty, and prevent any undesirable impact on final height.

The present study comparing actual data and simulated parameters was carried out using the PVsyst software for a 5.94 KWp grid-connected photovoltaic system,consisting of three types of mono-Si(2.04 KWp),poly-Si(2.04 KWp)and amorphous-Si(1.86 KWp)photovoltaic panels and a weather station.The research is based on data collected for one year(2020)of energy production Simulations were performed using long-term meteorological data generated by NASA and on-site measurement.A comparison of evaluated monthly and annually performance elements has also been considered:Eac,PR,FC.As shown by the results,each photovoltaic technology has certain advantages in different weather conditions.The uncertainties of the PVSyst software used to estimate the output of the photovoltaic installation have also been critically examined.

Physicians’knowledge about radiation protection of patients during prescription of CT scan procedures in Morocco

Objective:To evaluate the knowledge and practices of Moroccan physicians in terms of radioprotection of patients when prescribing computed tomography(CT)procedures.Methods:This is a cross-sectional study in which a questionnaire with 23 multiple-choice questions was sent to prescribers of CT examinations such as radiotherapists(RMs),other medical specialists(OMSs),general practitioners(GPs)and residents/interns(R/Is).The first eight questions asked about the demographics of the participants,while the remaining questions asked about knowledge of ionizing radiation examinations,doses received,relative risks,and patient radiation safety training.Results:A total of 223 physicians participated in this survey.Radiation therapists,considered as the reference group,had a better knowledge of irradiating and non-irradiating imaging compared to the other groups(P=0.003).Thus,67%of the reference group declared to take into account the number of scans performed by the patient during the last year,unlike the other groups(P=0.002).Furthermore,the knowledge of the different groups about the risks related to exposure to ionizing radiation was globally low(2%)(P=0.73).Regardless of their specialties and seniority,only 12%of the participants informed the patient at the time of prescription about the risks of X-rays.Finally,only 21%of the participants declared having had training in radiation protection,with no significant differences between the subgroups(P=0.832).Conclusions:The results obtained are similar to those reported in previous studies.They show that Moroccan prescribers have a low level of knowledge of the risks associated with CT examinations.Training on patient radiation protection should be included in the initial curriculum of interns and the continuing professional development of physicians should be reinforced.

Long-term outcomes of postgastrectomy syndrome after total laparoscopic distal gastrectomy using the augmented rectangle technique

BACKGROUND For total laparoscopic distal gastrectomies for gastric cancer,the reconstruction method is critical to the clinical outcome of the procedure.However,which reconstruction technique is optimal remains controversial.We originally reported the augmented rectangle technique(ART)as a reconstruction option for total laparoscopic Billroth I reconstructions.Still,little is known about its effect on long-term outcomes,specifically the incidence of postgastrectomy syndrome and its impact on quality of life.AIM To analyze postgastrectomy syndrome and quality of life after ART using the Postgastrectomy Syndrome Assessment Scale-37(PGSAS-37)questionnaire.METHODS At Juntendo University,a total of 94 patients who underwent ART for Billroth I reconstruction with total laparoscopic distal gastrectomies for gastric cancer between July 2016 and March 2020 completed the PGSAS-37 questionnaire.Multidimensional analysis was performed,comparing those 94 ART cases from our institution(ART group)to 909 distal gastrectomy cases with a Billroth I reconstruction from other Japanese institutions who also completed the PGSAS-37 as part of a larger national database(PGSAS group).RESULTS Patients in the ART group had significantly better total symptom scores in all the symptom subscales(i.e.,esophageal reflux,abdominal pain,meal-related distress,indigestion,diarrhea,constipation,and dumping).The loss of body weight was marginally greater for those in the ART group than in the PGSAS group(-9.3%vs-7.9%,P=0.054).The ART group scored significantly lower in their dissatisfaction of ongoing symptoms,during meals,and with daily life.CONCLUSION ART for Billroth I reconstruction provided beneficial long-term results for postgastrectomy syndrome and quality of life in patients undergoing total laparoscopic distal gastrectomies for gastric cancer.

Experiences of patients with HIV/AIDS receiving mid- and long-term care in Japan:A qualitative study

Purpose: In the era of antiretroviral treatment (ART), treatment of HIV has become more manageable, and most patients with HIV benefit from long-term therapy in Japan. The aim of this qualitative study was to examine the experiences of patients with HIV/AIDS receiving mid-and long-term care in Japan. Methods: A qualitative study was performed using semi-structured interviews at an outpatient clinic of a university hospital in the Greater Tokyo area. A total of 31 Japanese patients with HIV who had been receiving treatment for at least a year were interviewed in Japanese. Data from these interviews were analyzed by'content analysis' (Krippendorff, 1980). Results: The data were organized into the following seven themes:"feelings toward diagnosis of HIV infection";"perceptions and behavior after diagnosis of HIV";"attitudes toward HIV therapy";"fear and hope for the future";"feelings toward professional support";"life changes after HIV therapy";and"struggles in relationships with others". Conclusions: Some participants accepted themselves as HIV-positive, some did not when they were diagnosis. This difference of reaction had effect on attitudes toward HIV therapy. But all participants continued mid- and long-treatments owing to feeling well and receiving support from professionals. Additionally, it found that patients were felt stigma of HIV-positive similarly to another Asian countries. Therefore, we should provide the entire Japanese society with knowledge about HIV/AIDS and be early and constant intervention by professional teams after being infected.

Heavy Ion Beam Probe Measurement in Turbulence Diagnostic Simulator

A numerical measurement module simulating a heavy ion beam probe was developed, and numerical measurements of electrostatic potential and density fluctuations are carried out for 3-D turbulent data generated by a global simulation of drift-interchange mode turbulence in helical plasmas. The deviation between measured and local values is estimated. It is found that the characteristic structures can be detected in spite of the screening effect due to the finite spatial resolution.

Numerical Simulation and Measurement of Air Temperature and Relative Humidity in an Olive Cuttings Tunnel Greenhouse

Understanding the environment of olive tree cuttings is a key factor in improving these plants’ rooting rate and survival. This study aims to develop a three-dimensional (3-D) Computational Fluid Dynamics (CFD) model for numerically assessing air temperature and relative humidity in an olive cuttings greenhouse under Mediterranean climatic conditions. The results are deduced from a steady-state simulation performed with recorded boundary conditions at 10:00 am, 12:00 pm, 02:00 pm, 04:00 pm, and 06:00 pm at different observation points. The calculations were validated using experimental data. Expression of the P2X_2 receptor in different classes of ileum myenteric neurons in the female obese ob/ob mouse

AIM:To examine whether the ob/ob mouse model of obesity is accompanied by enteric nervous system ab-normalities such as altered motility METHODS:The study examined the distribution of the P2X 2 receptor (P2X 2 R) in myenteric neurons of female ob/ob mice. Specifically, we used immunohistochemistry to analyze the co-expression of the P2X 2 R with neuronal nitric oxide synthase (nNOS), choline acetyltrans-ferase (ChAT), and calretinin (CalR) in neurons of the small intestine myenteric plexus in ob/ob and control female mice In these sections, we used scanning confocal microscopy to analyze the co-localization of these markers as well as the neuronal density (cm 2 ) and area profile (μm2) of P2X 2 R-positive neurons In addition, enteric neurons were labeled using the nicotinamide adenine dinucleotide (NA H) diaphorase method and analyzed with light microscopy as an alternate means by which to analyze neuronal density and areaRESULTS:In the present study, we observed a 29 6% increase in the body weight of the ob/ob animals (OG) compared to the control group (CG) In addition, the average small intestine area was increased by approxi-mately 29 6% in the OG compared to the CG Immu-noreactivity (IR) for the P2X 2 R, nNOS, ChAT and CalR was detectable in the myenteric plexus, as well as in the smooth muscle, in both groups This IR appeared to be mainly cytoplasmic and was also associated with the cell membrane of the myenteric plexus neurons, where it outlined the neuronal cell bodies and their processes P2X 2 R-IR was observed to co-localize 100% with that for nNOS, ChAT and CalR in neurons of both groups In the ob/ob group, however, we observed that the neuronal density (neuron/cm 2 ) of P2X 2 R-IR cells was in-creased by 62% compared to CG, while that of NOS-IR and ChAT-IR neurons was reduced by 49% and 57%, respectively, compared to control mice The neuronal density of CalR-IR neurons was not different between the groups Morphometric studies further demonstrated that the cell body profile area (μm2) of nNOS-IR, ChAT-I

“汉语热”燃起了摩洛哥人学习汉语的热情。同时也促进和开启了中摩关系的新时代。2016年,在摩洛哥卡萨布兰卡市哈桑二世大学艾因·乔克人文科学院,设立了中文专业。虽然摩洛哥汉语教学和中文专业发展比较良好,但还是存在很多问题和阻碍,特别是成立不久的中文专业,汉语教学的多个环节中的问题都凸显出来了。因此本文将通过问卷调查和访谈的形式,来了解目前摩洛哥哈桑二世大学汉语教学的情况,从多个角度分析汉语教学中存在的问题和不足,分析其原因,问题导向提出教学建议。观察的角度有教师、学生、教学资源、教学方法等多个方面。发现的主要问题和建议有:(1)师资短缺,特别是固定的有丰富经验的汉语教师非常缺失,建议培养汉语教师,签署长期合同,减少教师流动性,并考虑两国教师搭配教学,取长补短。(2)学生差异大,入学时汉语水平参差不齐,建议提高入学门槛,要求学生学前学习预备知识,根据学生水平不同分班。(3)教材更换频繁,建议固定教材或自建本土教材,为学生量身定做,教学效果更佳。(4)无听力课程和专门的口语课,调整教学方案和设置,从“听、说、读、写”多个方面培养学生的语言能力。(5)班级人数太多,建议控制人数,每班不宜超过30人。(6)教学方法方面,从摩洛哥人的性格特点出发,多一些互动和交流,活动项目应该多一些,提高兴趣是学习最大的动力。(7)教学管理方面较松散,建议加强管理,不迟到,按时完成作业,严格要求。本论文分五个章节研究了哈桑二世大学中文专业汉语教学的情况。第一章,主要是论述了本文选题的意义、相关前沿研究、研究内容和摩洛哥汉语教学的情况;第二章,分六节详细介绍了哈桑二世大学中文专业的基本情况;第三章,通过问卷调查分析了汉语教学中存在的问题和不足;第四章,结合前两章的内容,现状和调查问卷结果,提出建议和解决方案;第五章,对全文进行总结。
