Etiologies of tropical acute febrile illness in West Pahang, malaysia: A prospective observational study

Objective: To determine the etiologies of tropical acute febrile illness(TAFI) in West Pahang, malaysia and to investigate morbidity and mortality factors in relation to TAFI. methods: A multicenter prospective cohort study was conducted between January and June 2016 in six district hospitals throughout the western part of Pahang State in Peninsular malaysia. A total of 336 patients answered a standardized questionnaire and blood samples were collected for laboratory confirmation of infectious etiology. Descriptive analysis and logistic regression were performed to identify factors associated with TAFI. Results: A total of 336 patients were included. The patients were mainly malays(70.2%), males(61.3%), aged(44.6±17.4) years, with more than half(58.9%) presenting with gastrointestinal symptoms. The majority were diagnosed with dengue(35.7%) while malaria(4.5%) was the least frequent. The in-hospital mortality due to TAFI was 9.2%. Patients with meliodosis had five times higher mortality [Adjusted OR: 5.002, 95% CI:(1.233, 20.286)]. Patients with comorbidities such as cardiovascular symptoms(Pment therapy initiation(Pmortality in all TAFI. Conclusions: The etiology of TAFI in the western Pahang includes dengue, leptospirosis, malaria and melioidosis, which carry the highest risk of in-hospital mortality. The presence of cardiovascular symptoms may be used to assess the disease severity in TAFI, but more studies are needed in the future.

Obstetric recto-vaginal fistula: What technique to use?

Objective: The recto-vaginal fistula is an abnormal communication between the previous wall of the rectum and the posterior face of the vagina through the recto-vaginal septum. They are often due to the obstetric traumas and they can be of post-surgical origin. methods: We report five cases of patients with obstetric recto-vaginal fistula and all of the patients have surgical intervention with a resection of the fistulous route and rectal wall. Results: The patient did not present any recidivating and the evolution is satisfactory without complications or recidivating. Conclusion: The treatment of recto-vaginal fistula is often difficult exposed to recurrence;and the therapeutic indications must be adapted to every type of fistula by taking into account its size, its location and its etiology.

The Psycho-Social Impact of Urinary Incontinence on the Quality of Life among Kuwaiti Women with Type 2 Diabetes mellitus

This study was conducted to determine the psycho-social impact of urinary incontinence (UI) on the quality of life of 250 Kuwaiti women with type 2 diabetic mellitus (Dm). A survey method, using a 33-item 5-point Likert scale Arabic questionnaire adapted from the King’s Health Questionnaire (KHQ), was employed for data collection from February to may, 2014 while all the participants were receiving treatment for UI at a specialized urology center in Kuwait. Participants with UI and a co-morbidity of type 2 Dm and obesity were 20 to 65 years old. Results showed the following variables were statistically significant for frequency of urine leak: Age was (m>χm>2 = 36.877, df = 3, P ≤ 0.000). Parity showed nulliparous women reported less urine leak compared to parous women: Chi-square was (m>χm>2 = 24.83, df = 12, P ≤ 0.016). Type 2 Dm for more than 3 years duration had the highest incidence of several leaks per day. BmI of above 25 kg/m2 caused daily urine leak: Chi-square (m>χm>2 = 17.912, df = 9, P ≤ 0.036). Participants’ self reports of good general health were those who leaked urine either 2 - 3 times weekly or occasionally. Finally, the impact of incontinence on their lifestyle was reported as extreme by 128 (51.2%), and 6 (2.4%) reported little or no impact on their quality of life: Chi-square was (m>χm>2 = 52.392, df = 18, P ≤ 0.000). In conclusion, this study showed a clear correlation between UI and reduced quality of life. midwives are well positioned to correct the myth that UI is an inevitable byproduct of childbearing. midwives should explain to all pregnant women how childbirth can be a risk factor and provide anticipatory guidance by teaching preventive measures like pelvic floor exercises before and after delivery. Family members should assist sufferers in coping with their challenges by dispelling any form of stigmatization, joining them in practicing pelvic floor exercises, and encouraging,

The tumor desmoplastic small round cell in women is rare. Ovarian its location is exceptional. We report a patient of 20 years, the originalNorth Africawho consulted for an increase in abdominal volume with impaired general condition. Pelvic ultrasonography and computed tomography were in favor of multiple peritoneal masses with ascites. After surgical exploration, the histological and immunohistochemical data for diagnosis small round cell desmoplastic tumor of ovarian the patient was a candidate for poly chemotherapy but she was died a month later. Diagnosis relies on histological and immunohistochemical data. The demonstration of a nonrandom translocation t (11;22) (p13;q12) is specific for the disease. The management is multidisciplinary and combining surgery, a poly aggressive chemotherapy and radiotherapy. The prognosis remains poor.

Diagnostic value of a panel of tumor markers as a part of a diagnostic work-up for ascites of unknown etiology

Background & objectives: Data regarding tumor marker usefulness in diagnosing ascites of unknown etiology and determining its malignant nature are conflicting. We aim to assess the diagnostic value of ascitic and serum tumor markers in ascites of unknown etiology and to evaluate their usefulness besides other laboratory tests in a diagnostic work-up in those patients. Design & setting: A prospective case-control study conducted at Assiut University hospital and oncology institute. Patients & methods: Three groups were included;Group I: 41 patients with ascites of unknown etiology Group II: 7 Patients with TB ascites and Group III:14 patients with cirrhotic ascites. We assessed the CEA, CEA mRNA, CA15-3, CA19.9, CA125, AFP and PSA in serum and ascetic fluid. A diagnostic work-up for group I included: IPD test, ultrasound, CT, ascetic fluid cytology, SAAG, Laparotomy and biopsy. Results: Ascetic fluid and serum levels of CA15-3 and CA125 were significantly increased in group I and were significantly increased in histopathologically proved malignant ascites compared to TB and cirrhotic ascites. In group I, CA 125 was significantly higher in ascites than serum. With the exception of PSA, all tumor markers significantly correlated in serum and ascetic fluid. No significant difference in the level of ascetic CEA messenger RNA was detected between the 3 groups. Cytology had 53% sensitivity, 94% specificity and CA 125 & CA15-3 had 81% sensitivity and 75% specificity in detection of malignant ascites repectively. Laparotomy and biopsy: diagnosed malignnancy in 53.3% and TB in 13.3%. Conclusions: A diagnostic work-up including SAAG, tumor markers in the serum and ascetic fluid may help in adjunct with ascetic fluid cytology, laparotomy and biopsy, imag- ing and other laboratory tests in diagnosing ascites of unknown etiology.

For several decades, tamoxifen has proven effective in reducing recurrence and mortality for early and metastatic breast cancer with positive estrogen receptors, and has been proven to be beneficial in preventing breast cancer in high-risk women. However, tamoxifen has been associated with an increased risk of endometrial cancer by its agonist effects on the endometrium in postmenopausal women. The purpose of this article, through an exhaustive summary of literature, is to provide a better understanding of this relationship and its impact on managed care on patients.

The aim of this study was to test the impact of Rhizophagus irregularis, an arbuscular mycorrhizal fungus (AmF), on durum wheat growth and soil microbial functionalities in a field inoculation trial conducted in morocco. The results showed that i) the R. irregularis inoculum effectively improved wheat growth, ii) wheat growth promotion depended on the soil mycorrhizal infectivity and iii) functional abilities of soil microflora depended on AmF inoculation. This study confirms that field AmF inoculation can be proposed as an effective agronomic practice in wheat production and as a sustainable cultural practice to manage soil biofunctioning. To fully promote inoculation practices, a better knowledge of AmF ecology has to be acquired to better adapt AmF inoculation to environmental conditions, and thus warrant better yields and agricultural sustainability.

Aim: The aim of the current study is to determine: (1) the prevalence of extended-spectrum β-lactamase-producing K. pneumoniae (ESBL-Kp) isolated from clinical samples and a hospital environment in Hassan II Hospital (Settat, morocco);(2) the associated risk factors of ESBL-Kp infections;(3) the link between clinical and environmental isolates. methods: During the study period (April 2010 to march 2011), all patients infected and hospital environment sites contaminated by K. pneumoniae were considered as the potential study population and environmental site. The clinical data were collected to identify risk factors for ESBL carriage of K. pneumoniae infection. Screening of ESBL-and carbapenemase-producing isolates was performed by using a double-disk synergy test and the modified Hodge test, respectively. ESBL-Kp isolates were tested for the presence of genes encoding β-lactamases and were investigated by PCR. The clonal relationship between ESBL-producing isolates was analysed by ERIC- and REP-PCR method. Results: The overall prevalence of ESBL-Kp among clinical and environmental K. pneumoniae isolates was 35.13% (13/37) and 4.04% (4/99), respectively. The main risk factors for carrying ESBL-Kp were renal disease (46.15%), recent surgery (53.84%), previous hospitalisation (76.92%), and the presence of many invasive devices (53.84%). All ESBL isolates were multidrug resistant. The blaCTX-m group1and blaSHV (70.58% for each) were the most prevalent followed by blaTEm (52.94%). Thirteen strains expressed at least two bla genes. One isolate was positive in the modified Hodge test and was a blaOXA-48 producer. ERIC and Rep-PCR methods revealed an epidemic clonal dissemination of these isolates. Conclusion: The emergence of OXA-48 carbapenemase, endemic clonal dissemination and multi-drug resistance of ESBL-Kp isolates in our institution is highly alarming.

The Social media and Technology Addiction and Its Associated Factors among University Students in Lebanon Using the media and Technology Usage and Attitudes Scale (mTUAS)

The contemporary developments of Information and Communication Technologies usage have fueled a popular concern. The young people are experiencing technology addiction behavior due to excessive media and technology usage. A valid instrument is required to detect individuals at high risk of social media and technology addiction. Objective: The objectives of this study are to assess prevalence of the technology addiction using the media and Technology Usage and Attitudes Scale (mTUAS) and to investigate the impacts of online social networking on health and academic performance of young university students in Lebanon. methods: During the academic year 2017-2018, a cross-sectional study which enrolled 705 young university students was conducted. Data was collected using mTUAS questionnaire. The translation and cultural adaptation of the mTUAS followed a standardized protocol. The psychometric properties were then evaluated. We used multivariate regression analyses to determine which characteristics were associated with the technology addiction among students. Results: 705 filled out the questionnaire aged 17 to 35 years (mean = 20.42;SD = 2.41), 74% of whom were females. The internal consistency was high for all subscales and ranged from 0.54 - 0.87. The mTUAS yielded statistically significant scores for subgroups (known group validity). males’ students had a higher risk of addiction to social media and technology than females. The use of media was viewed common among the smokers (OR = 1.76;95% CI = 1.10 - 2.82, p = 0.02) and drinkers of alcohol (OR = 1.80;95% CI = 1.04 - 3.11, p = 0.04). Students that used e-mailing, smartphone, and TV viewing had a high academic performance level. Females and public university students had negative attitudes toward technology.
