Efficacy and safety of modified medium-chain triglyceride ketogenic diet in patients with drug-resistant epilepsy

Background Medium-chain triglyceride ketogenic diet(MCTKD)is previously less commonly used in *** study was aimed to assess the efficacy and safety of the modified MCTKD in the treatment of drug-resistant epilepsy in Chinese *** Patients with drug-resistant epilepsy were enrolled to receive treatment with modified MCTKD in Guangdong sanjiu Brain Hospital during December 2020 and September *** modified MCTKD contained fat that provided 50–70%of the total energy,as well as proteins and carbohydrates that provided 20–30%and 20%of energy,*** fat component was composed of 20–30%medium-chain triglycerides(MCTs)and 30–40%long-chain *** efficacy and safety of the diet were assessed at 1,3 and 6 *** A total of 123 patients aged 2.5 to 65 years,were included in this *** response rates at 1,3 and 6 months were 49.6%,43.1%,and 30.9%,*** seizure freedom rates at 1,3 and 6 months were 12.2%,10.6%,and 6.5%,*** retention rates at 1,3 and 6 months were 98.4%,65.0%and 33.3%*** effects occurred in 21.14%of patients,which were predominantly gastrointestinal symptoms such as abdominal pain,diarrhea,vomiting,and constipation,and most of them resolved after dietary adjustments.A total of 82 patients(66.7%)discontinued the treatment with the reason of refusing to eat(8.1%),poor efficacy(35.0%),poor compliance(4.9%),and inability to follow-up(9.8%).Only 4 patients(3.3%)withdrew the diet due to side *** The modified MCTKD with MCTs providing 20–30%of energy has a good safety in patients with drug-resistant epilepsy,but its effectiveness needs to be *** modifications of MCTKD with an optimal energy ratio are required to achieve a better efficacy and safety.

Objective:To analyze the factors related to vessel vasovagal reaction(VVR)in apheresis donors,establish a mathematical model for predicting the correlation factors and occurrence risk,and use the prediction model to intervene in high-risk VVR blood donors,improve the blood donation experience,and retain blood ***:A total of 316 blood donors from the Xi'an Central Blood Bank from June to September 2022 were selected to statistically analyze VVR-related factors.A BP neural network prediction model is established with relevant factors as input and DRVR risk as ***:First-time blood donors had a high risk of VVR,female risk was high,and sex difference was significant(P value<0.05).The blood pressure before donation and intergroup differences were also significant(P value<0.05).After training,the established BP neural network model has a minimum RMS error of o.116,a correlation coefficient R=0.75,and a test model accuracy of 66.7%.Conclusion:First-time blood donors,women,and relatively low blood pressure are all high-risk groups for *** BP neural network prediction model established in this paper has certain prediction accuracy and can be used as a means to evaluate the risk degree of clinical blood donors.

This work studied the thickening progression mechanism of the silica fume-oil well cement composite system at high temperatures(110-180.C) in order to provide a theoretical guidance for the rational application of silica fume in the cementing engineering. Results showed that silica fume seldom affected the thickening progression of oil well cement slurry at 110-120.C, but when temperature reached above130.C, it would aggravate the bulging degree of thickening curves and significantly extend the thickening time, meanwhile causing the abnormal “temperature-based thickening time reversal” and“dosage-based thickening time reversal” phenomena in the range of 130-160.C and 170-180.C respectively. At 130-160.C, the thickening time of oil well cement slurry was mainly associated with the generation rate of calcium hydroxide(CH) crystal. The introduced silica fume would be attracted to the cement minerals' surface that were hydrating to produce CH and agglomerate together to form an“adsorptive barrier” to hinder further hydration of the inner cement minerals. This “adsorptive barrier”effect strengthened with the rising temperature which extended the thickening time and caused the occurrence of the “temperature-based thickening time reversal” phenomenon. At 170-180.C, the pozzolanic activity of silica fume significantly enhanced and considerable amount of C-S-H was generated, thus the “temperature-based thickening time reversal” vanished and the “dosage-based thickening time reversal” was presented.

The Li-Mg-B-H composite(2LiBH_(4)+MgH_(2))has a high capacity of 11.4 wt%as a hydrogen storage ***,the slow kinetics and poor cycling stability severely restrict its practical *** this work,a layered Nb_(2)C MXene was first synthesized and then introduced to tailor the kinetics and cycling stability of the Li-Mg-B-H *** milled 2LiH+MgB_(2)composites were initially hydrogenated to obtain the 2LiBH_(4)+MgH_(2)*** 2LiBH_(4)+MgH_(2)+5wt%Nb_(2)C can release 9.0 wt%H_(2)in 30 min at 400℃,while it is only 2.7 wt%for the undoped 2LiBH_(4)+MgH_(2).The dehydrogenation activation energies of MgH_(2)and LiBH_(4)are 123 and 154 kJ·mol^(-1)respectively for the 5 wt%Nb_(2)C-doped composite,lower than the undoped composite(164 and 165 kJ·mol^(-1)).The 2LiBH_(4)+MgH_(2)+5 wt%Nb_(2)C possesses excellent cycling stability,with the reversible capacity only slightly reduced from 9.4 wt%for the 1st cycle to 9.3 wt%for the 20th ***_(2)C keeps stable in the composite and acts as an efficient catalyst for the Li-Mg-B-H *** is believed that both the layered structure and the active Nb element contribu te to the enhanced hydrogen storage performances of the Li-Mg-B-H *** work confirms that the Nb_(2)C MXene with layered stru cture has a significant enhancing impact on the Li-Mg-B-H hydrogen storage materials,which is different from the bulk NbC.
