Early insulin therapy is capable to achieve glycemic control and restoreβ-cell function in newly diagnosed type 2 diabetes(T2D),but its effect on cardiovascular outcomes in these patients remains *** this nationwide real-world study,we analyzed electronic health record data from 19 medical centers across China between 1 January 2000,and 26 May *** included 5424 eligible patients(mean age 56 years,2176 women/3248 men)who were diagnosed T2D within six months and did not have prior cardiovascular *** Cox regression models were used to estimate the associations of early insulin therapy(defined as the first-line therapy for at least two weeks in newly diagnosed T2D patients)with the incidence of major cardiovascular events including coronary heart disease(CHD),stroke,and hospitalization for heart failure(HF).During 17,158 persons years of observation,we documented 834 incident CHD cases,719 stroke cases,and 230 hospitalized cases for *** diagnosed T2D patients who received early insulin therapy,compared with those who did not receive such treatment,had 31%lower risk of incident stroke,and 28%lower risk of hospitalization for *** significant difference in the risk of CHD was *** found similar results when repeating the aforesaid analysis in a propensity-score matched population of 4578 patients and with inverse probability of treatment weighting *** findings suggest that early insulin therapy in newly diagnosed T2D may have cardiovascular benefits by reducing the risk of incident stroke and hospitalization for HF.

Identity-Preserving Adversarial Training for Robust Network Embedding

Network embedding,as an approach to learning low-dimensional representations of nodes,has been proved extremely useful in many applications,e.g.,node classification and link ***,existing network embed-ding models are vulnerable to random or adversarial perturbations,which may degrade the performance of network em-bedding when being applied to downstream *** achieve robust network embedding,researchers introduce adversari-al training to regularize the embedding learning process by training on a mixture of adversarial examples and original ***,existing methods generate adversarial examples heuristically,failing to guarantee the imperceptibility of generated adversarial examples,and thus limit the power of adversarial *** this paper,we propose a novel method Identity-Preserving Adversarial Training(IPAT)for network embedding,which generates imperceptible adversarial exam-ples with explicit identity-preserving *** formalize such identity-preserving regularization as a multi-class classification problem where each node represents a class,and we encourage each adversarial example to be discriminated as the class of its original *** experimental results on real-world datasets demonstrate that our proposed IPAT method significantly improves the robustness of network embedding models and the generalization of the learned node representations on various downstream tasks.

目的:调查健康管理专科护士培训迁移现状,并分析其影响因素,以期为健康管理专科护士培训方案的改进和完善提供参考。方法:于2021年11月至2023年11月,采用便利抽样法,选取在四川省健康管理专科护士培训基地接受专科护士培训的196名健康管理专科护士为调查对象。采用一般资料调查表、健康管理专科护士培训迁移现状调查表、一般自我效能感量表、培训迁移气氛量表进行调查。采用多元线性回归分析健康管理专科护士培训迁移的影响因素。结果:健康管理专科护士的培训迁移现状调查表得分为(21.49±1.88)分。多元线性回归分析结果显示,职务、医院等级、培训迁移气氛、自我效能感是健康管理专科护士培训迁移的影响因素(P<0.05)。结论:健康管理专科护士培训迁移处于中等偏上水平,受多种因素影响。护理管理者应根据相关影响因素,制订有针对性的培训方案,以提升其培训迁移水平,进而提高健康管理专科护士培训质量。

Adaptive Hybrid Forward Error Correction Coding Scheme for Video Transmission

This paper proposes an adaptive hybrid forward error correction(AH-FEC)coding scheme for coping with dynamic packet loss events in video and audio ***,the proposed scheme consists of a hybrid Reed-Solomon and low-density parity-check(RS-LDPC)coding system,combined with a Kalman filter-based adaptive *** hybrid RS-LDPC coding accommodates a wide range of code length requirements,employing RS coding for short codes and LDPC coding for medium-long *** delimit the short and medium-length codes by coding performance so that both codes remain in the optimal ***,a Kalman filter-based adaptive algorithm has been developed to handle dynamic alterations in a packet loss *** Kalman filter estimates packet loss rate utilizing observation data and system models,and then we establish the redundancy decision module through receiver *** a result,the lost packets can be perfectly recovered by the receiver based on the redundant *** results show that the proposed method enhances the decoding performance significantly under the same redundancy and channel packet loss.

Efficacy of budesonide/formoterol inhalation powder in treating viral pneumonia in children

BACKGROUND Respiratory viruses are increasingly detected in children with communityacquired *** strategies to limit antibiotic use in children with viral pneumonia are *** To explore clinical efficacy of budesonide/formoterol inhalation powder for viral pneumonia in children and its impact on cellular immunity and inflammatory factor *** A total of 60 children with viral pneumonia were recruited:30 receiving budesonide/formoterol inhalation powder and 30 conventional symptomatic *** measures included peripheral blood levels of inflammatory cytokines,CD4^(+),CD8^(+),Th1,Th2,Th17 and Treg,clinical efficacy,and incidence of adverse *** Compared with the control group,the observation group showed a significant reduction in interleukin-6 and high-sensitivity C-reactive protein levels after *** with the control group,the observation group showed a significant increase in CD4^(+)/CD8^(+)and Th1/Th2 levels,and a decrease in Th17/Treg levels after *** total effective rates in the observation group and the control group were 93.75%and 85.00%,respectively,which was a significant difference(P=0.003).CONCLUSION Budesonide/formoterol inhalation powder significantly improved therapeutic efficacy for viral pneumonia in *** mechanism of action may be related to downregulation of the inflammatory response and improved cellular immune function.

基于改进YOLOv5s-face的Face5系列人脸检测算法

针对人脸检测中小尺度人脸和遮挡人脸的漏检问题,提出了一种基于改进YOLOv5s-face(you only look once version 5 small-face)的Face5系列人脸检测算法Face5S(face5 small)和Face5M(face5 medium)。使用马赛克(mosaic)和图像混合(mixup)数据增强方法,提升算法在复杂场景下检测人脸的泛化性和稳定性;通过改进C3的网络结构和引入可变形卷积(DCNv2)降低算法的参数量,提高算法提取特征的灵活性;通过引入特征的内容感知重组上采样算子(CARAFE),提高多尺度人脸的检测性能;引入损失函数WIoUV3(wise intersection over union version 3),提升算法的小尺度人脸检测性能。实验结果表明,在WIDER FACE验证集上,相较于YOLOv5s-face算法,Face5S算法的平均mAP@0.5提升了1.03%;相较于先进的人脸检测算法ASFD-D3(automatic and scalable face detector-D3)和TinaFace,Face5M算法的平均mAP@0.5分别提升了1.07%和2.11%,提出的Face5系列算法能够有效提升算法对小尺度和部分遮挡人脸的检测性能,同时具有实时性。

对话系统在自然语言处理中发挥着重要作用,具有较好的实际应用前景和许多值得研究的方向.对话策略是基于管道方法的人机对话系统的核心组件,能够根据对话状态生成响应动作,进而指导对话生成.对话策略学习常建模为(半)马尔可夫决策过程,然后通过强化学习求解.近年来,基于强化学习算法解决任务型对话策略问题的研究层出不穷,而相关综述缺乏.因此,本文对基于强化学习的任务型对话策略进行分析、归类、总结.首先,介绍分类强化学习的一般模型,并基于强化学习的分类,分析并总结现有对话策略学习的一般思路和存在问题;其次,基于不同的研究热点,包括多领域、多模态、多代理和共情对话策略,深度剖析新近研究的理论模型、研究进展和存在的问题;接着,针对对话策略的相关研究,包括用户模拟器、对话策略评估、对话策略平台与数据集以及大语言模型与对话策略等进行介绍;针对现有研究的不足,本文从5种不同的角度分析了对话策略的未来研究方向;最后,对全文进行总结与展望.本文不仅从强化学习分类上概述任务型对话策略,而且从应用的角度分类任务型对话策略,全方面、多角度地综述了任务型对话策略,为未来的任务型对话策略的研究提供启示.

滑剪破坏型下盘边坡是矿山工程中经常出现的一种顺层边坡,具有突发性强、破坏范围广的特点。如何快速、准确地确定开挖塑性扰动区并评估其稳定性对边坡安全控制至关重要。本文首先通过数值模拟揭示了该类边坡的破坏模式和特征。然后,根据其破坏模式,建立了描述开挖扰动区的数学方程。最后,提出了一种计算下盘边坡开挖扰动区和稳定性的力学截断新方法(MTM)。主要结果有,MTM方法考虑岩体单轴抗压强度,计算过程无需一般极限平衡方法需要的多次循环迭代,计算过程简便。MTM方法可以准确地表征扰动区,计算得到的极限边坡高度、扰动厚度和安全系数与数值模拟结果较吻合,且评价结果比数值模拟更安全。扰动区的最大厚度决定了锚索锚固端的深度,分区是为了指明加固的重点范围。该方法为下盘边坡的工程施工和监测提供指导。
