Background: Maternal obesity is reported to be associated with increased incidence of gestational diabetes mellitus and hypertension. These cause failure of labour induction, leading to higher incidence of cesarean section (CS). The aim of this study was to assess which reduces CS rate, labor induction at 39 weeks or leaving women for spontaneous labor onset till 41 weeks. Methodology: A randomized controlled trial was conducted in Ain Shams Maternity Hospital in Egypt from 2016 to 2018. Study population consisted of 200 term primigravida pregnant obese women delivered in Ain Shams Maternity Hospital. They were divided into two groups: Group A: induction of labor at 39 + 0 weeks (n = 100) by vaginal administration of 25 μg misoprostol (PGE1) every 6 hours for 5 doses;Group B: waiting spontaneous labor onset till 41 + 0 weeks (n = 100), and if no spontaneous labor occurred at 41 weeks, induction was performed in the same way. Results: Induction (Group A) vs. waiting spontaneous labor (Group B) showed the followings, which were significant: CS: 22% vs 39%, p = 0.009;maternal birth injury: 4% vs 12%, p = 0.037;non-assisted vaginal deliveries: 93.6% vs 78.7%, p = 0.034;APGAR scores at 1 & 5 min: 7.6 ± 0.8 vs 7.3 ± 1.1 p = 0.038, 8.4 ± 1.0 vs 8.1 ± 1.3 p = 0.040, respectively;birth weight;3.3 ± 0.1 vs 3.5 ± 0.2 kg, p y lower in women with induction of labor at 39 weeks than those waiting for spontaneous labor onset till 41 weeks in obese Egyptian pregnant women.

Objective:This study is a prospective randomized, double-blind, placebo-controlled study to evaluate the effect of calcium and magnesium (Ca/Mg) infusion on amelioration of oxaliplatin neuropathy, the dose-limiting toxicity. Methods:Sixty patients with resected colorectal carcinoma (CRC) planned to receive adjuvant oxaliplatin-containing regimen were randomly assigned to two arms; Arm A:patients received Ca/Mg were given as 1 gm Ca gluconate and 1 gm MgSO 4 in 250 mL of intravenous (IV) solution over 30 min pre and post oxaliplatin infusion, and Arm B:patients received 250 mL of IV solution without Ca/Mg over 30 min pre and post oxaliplatin infusion. Primary outcome was to assess percentage of patients with oxaliplatin-induced neurotoxicity. Neurotoxicity was assessed according to the National Cancer Institute Common Terminology Criteria for Adverse Events (NCI-CTCAE) version 3.0. Results:Sixty patients in both arms were assessed, 30 with Ca/Mg infusion and 30 without. Patients developed neurotoxicity in arm A were significantly lower than that in arm B after the end of treatment; 7 (23.3%) and 14 (46.6%) respectively (P y lower duration of neuropathy in months (8 ± 2.5 vs 18 ± 3) respectively (P y significant reduction of peripheral neuropathy (PN) in patients with CRC receiving oxaliplatin in the adjuvant settings.

Background: Poor postoperative pain control leads to longer postoperative care, longer hospital stay and decreased patient overall satisfaction. Aim: To compare the efficacy and safety of bilateral ultrasound-guided quadratus lumborum block versus lumbar epidural block on the management of postoperative pain following major lower abdominal cancer surgery. Methods: The study was a double-blinded, and randomized study, conducted in South Egypt Cancer Institute, Assiut University, Egypt. It included cancer patients scheduled for major lower abdominal cancer surgery in the period from 2019 to 2020. They were divided into two groups: Group Ι received pre-emptive ultrasound-guided Quadratus Lumborum Block (QLB) with 25 mL of 0.25% bupivacaine on each side of the abdominal wall before induction of General Anesthesia (GA), and Group II received pre-emptive lumbar epidural block with 15 mL of 0.25% bupivacaine before induction of GA. VAS score, and time of the first analgesic request and postoperative total analgesic consumption were evaluated. Results: Sixty patients were included in our study. VAS score at rest was comparable between both studied groups in the first 6 h. At 8 and 10 h, Group II had a significantly higher VAS score at rest (P ely). Meanwhile, at 12 h, patients in Group I had a significantly higher VAS score (P = 0.026). Mean time of the first request for rescue analgesia was significantly prolonged in Group I (13.27 ± 2.38 hrs.) compared to Group II (10.20 ± 1.42 hrs.) Biomass gasification is a well-developed technology with the potential to convert agricultural residues to value-added products. The availability of on-farm gasifiers that can handle low-density agricultural wastes such as soybean residue, an underutilized feedstock, is limited. Therefore, the goal of this research was to install and assess an allothermal, externally heated, auger gasifier capable of converting agricultural wastes to combustible gas for on-farm grain drying. The system was used to convert soybean residues under different reactor temperature, i.e., 700°C, 750°C, 800°C, and 850°C. The results showed that increasing the reactor temperature from 700°C to 850°C increased the producer gas molar fractions of H2, CO, and CH4, from 1.1% to 1.5%, from 15.0% to 23.8%, and from 5.1% to 7.7%, respectively. The higher heating value of the producer gas reached 6.3 MJ/m3 at reactor temperature of 850°C. Specific gas y.eld increased from 0.32 to 0.58 m3/kgbiomass while char and particulate y.eld decreased from 41.7% to 33.6% by increasing the reactor temperature from 700°C to 850°C. Maximum carbon sequestration achieved, in the form of biochar-carbon, was 32% of the raw feedstock carbon. Gasification of collectable soybean residues from 1 acre would be sufficient to dry 1132 kg of soybean seeds (the average y.eld from one acre)

The approach utilized in the design of achromatic periods formed of two layers a high-index and a low-index is developed by combining the first two periods together. These two periods are therefore reduced to three layers a central one, an upper layer and a lower layer. Both the upper and lower layers are matched to the central at two different wavelengths. This forms the so called a combined achromatic period or the basic unit. Multilayers showing low transmittance bands are then synthesized of this basic unit. Parameters used in the synthesis of such multilayers are pointed out and their control role is investigated.

We present a case of a patient with progressive unilateral sensorineural hearing loss and tinnitus with internal auditory canal enhancement on magnetic resonance imaging (MRI) secondary to isolated cochlear neuritis from varicella reactivation. MRI following antiviral treatment showed resolution of enhancement. Varicella reactivation is commonly seen in the form of Ramsay Hunt syndrome, which is known to produce abnormal MRI enhancement from facial and vestibulocochlear neuritis; however, its characteristic clinical signs aid the diagnosis. This case is unique in that the only manifestation of varicella infection was unilateral hearing loss. This case outlines the importance of maintaining a broad differential diagnosis in the evaluation of unilateral hearing loss as well as recognizing the limited specificity of MRI.

提出一种基于马鞍面的新型索杆张力结构拓扑,结构整体形态更加稳定.进一步对形态分析中的迭代找力方法进行优化改进,在保证计算精度的前提下提高计算效率.应用改进的迭代找力方法对提出的新型马鞍形索杆张力结构进行了预应力设计.对跨度为36 m的马鞍形索杆张力结构进行静动力性能分析,计算结果显示结构在0.8 kg/m^(2)面荷载作用下,最大节点位移小于跨度的1/250,最大索应力小于索抗拉强度的50%.研究结果表明:所提出的马鞍形索杆张力结构在拓扑构型上具有独特的优势,以及较好的力学性能,能应用在实际工程中.

The study aims to find out the changes of climate change parameters for Mysore district. To analyse the variations in climatic parameters like Rainfall and Temperature, data were used for a period of 1986-2016, and Humidity data were used for a period of 2000-2016 to study the nature of climate change in the region. The findings of the study show that the climate variability and climate change for Mysore district, the monthly Rainfall is increased and monsoon Rainfall shows a decrease in trend. And the monthly, summer, monsoon minimum Temperature shows that decrease. Generally, the monthly maximum Temperature has decreased, but in case of summer and monsoon seasons increased respectively, and the monthly Relative Humidity has increased (positive). There is a decrease in Relative Humidity from January to April, and increase in the Relative Humidity during Monsoon season for Mysore District. The Annual average of Relative Humidity shows that it is increased for Mysore District.
