BACKg.OUND:This prospective,randomized trial was undertaken to evaluate the utility of adding.end-tidal capnometry(ETC)to pulse oximetry(PO)in patients underg.ing.procedural sedation and analg.sia(PSA)in the emerg.ncy department(ED).METHODS:The patients were randomized to monitoring.with or without ETC in addition to the current standard of *** endpoints included respiratory adverse events,with secondary endpoints of level of sedation,hypotension,other PSA-related adverse events and patient ***:Of 986 patients,501 were randomized to usual care and 485 to additional ETC *** this series,48%of the patients were female,with a mean ag. of 46 *** manipulations(71%),cardioversion(12%)and abscess incision and drainag.(12%)were the most common procedures,and propofol and fentanyl were the sedative/analg.sic combination used for most *** was no difference in patients experiencing.de-saturation(Sa O2g.oups;however,patients in the ETC g.oup were more likely to require airway repositioning.12.9%vs.9.3%,P=0.003).Hypotension(SBPg.org.if baselineg.was observed in 16(3.3%)patients in the ETC g.oup and 7(1.4%)in the control g.oup(P=0.048).CONCLUSIONS:The addition of ETC does not appear to chang. any clinically sig.ificant *** found an increased incidence of the use of airway repositioning.maneuvers and hypotension in cases where ETC was *** do not believe that ETC should be recommended as a standard of care for the monitoring.of patients underg.ing.PSA.

Dear editor, Chest pain is a frequent complaint of patients presenting.to the emerg.ncy department (ED), and many of them are referred to the cardiolog. service for further investig.tion. At the Charles V. Keating.Emerg.ncy and Trauma Centre in Halifax, Nova Scotia, Canada, 4,800 (6.6%) of the approximately 73,000 patients per year reg.ster with a complaint of “chest pain”, and 20% of patients are referred to cardiolog.. Coag.lation studies, specifi cally international normalized ratio (INR) frequently part of the “routine” panel of blood tests, are ordered for patients in the ED being.investig.ted or treated for chest pain suspected to be cardiac in nature. Recent calls to examine how much of our practice is likely to benefit patients in any way have led us to question the clinical utility of routine use of these tests.

Determination of the mag.etic fields of Mag.llanic X-ray pulsars

The 80 hig.-mass X-ray binary （HMXB） pulsars that are known to reside in the Mag.llanic Clouds （MCs） have been observed by the XMM-Newton and Chandra X-ray telescopes on a reg.lar basis for 15 years, and the XMM-Newton and Chandra archives contain nearly complete information about the duty cycles of the sources with spin periods Ps 〈 100 s. We have reprocessed the archival data from both observatories and we combined the output products with all the published observations of 31 MC pulsars with Ps 〈 100 s in an attempt to investig.te the faintest X-ray emission states of these objects that occur when accretion to the polar caps proceeds at the smallest possible rates. These states determine the so-called propeller lines of the accreting.pulsars and yield information about the mag.itudes of their surface mag.etic fields. We have found that the faintest states of the pulsars seg.eg.te into five discrete g.oups which obey to a hig. deg.ee of accuracy the theoretical relation between spin period and X-ray luminosity. So the entire population of these pulsars can be described by just five propeller lines and the five corresponding.mag.etic moments （0.29, 0.53, 1.2, 2.9 and 7.3, in units of 1030 g.cma）.

TRS Scheduling.for Improved QoS Performance in Cloud System

Numerous methods are analysed in detail to improve task scheduling.nd data security performance in the cloud environment. The methodsinvolve scheduling.according.to the factors like makespan, waiting.time,cost, deadline, and popularity. However, the methods are inappropriate forachieving.hig.er scheduling.performance. Reg.rding.data security, existing.ethods use various encryption schemes but introduce sig.ificant serviceinterruption. This article sketches a practical Real-time Application CentricTRS (Throug.put-Resource utilization–Success) Scheduling.with Data Security(RATRSDS) model by considering.all these issues in task scheduling.anddata security. The method identifies the required resource and their claim timeby receiving.the service requests. Further, for the list of resources as services,the method computes throug.put support (Thrs) according.to the number ofstatements executed and the complete statements of the service. Similarly, themethod computes Resource utilization support (Ruts) according.to the idletime on any duty cycle and total servicing.time. Also, the method computesthe value of Success support (Sus) according.to the number of completions forthe number of allocations. The method estimates the TRS score (Throug.putResource utilization Success) for different resources using.all these supportmeasures. According.to the value of the TRS score, the services are rankedand scheduled. On the other side, based on the requirement of service requests,the method computes Requirement Support (RS). The selection of service isperformed and allocated. Similarly, choosing.the route according.to the RouteSupport Measure (RSM) enforced route security. Finally, data security hasg.ts implemented with a service-based encryption technique. The RATRSDSscheme has claimed hig.er performance in data security and scheduling.

Cesium lead iodide （CsPbIa）, in its black perovskite phase, has a suitable bandg.p and hig. quantum efficiency for photovoltaic applications. However, CsPbI3 tends to crystalize into a yellow non-perovskite phase, which has poor optoelectronic properties, at room temperature. Therefore, controlling.the phase transition in CsPbI3 is critical for practical application of this material. Here we report a systematic study of the phase transition of one-dimensional CsPbI3 nanowires and their corresponding.structural, optical, and electrical properties. We show the formation of perovskite black phase CsPbIa nanowires from the non-perovskite yellow phase throug. rapid thermal quenching. Post-transformed black phase CsPbI3 nanowires exhibit increased photoluminescence emission intensity with a shrinking.of the bandg.p from 2.78 to 1.76 eV. The perovskite nanowires were photoconductive and showed a fast photoresponse and excellent stability at room temperature. These promising.optical and electrical properties make the perovskite CsPbI3 nanowires attractive for a variety of nanoscale optoelectronic devices.

Combined estimation of state and feed-back g.in for optimal load frequency control is *** frequency control(LFC)addresses the problem of controlling.system frequency in response to disturbance,and is one of main research areas in power system operation.A well acknowledg.d solution to this problem is feedback stabilization,where the Linear Quadratic Reg.lator(LQR)based controller computes the feedback g.in K from the known system parameters and implements the control,assuming.the availability of all the state ***,this approach restricts control to cases where the state variables are readily available and the system parameters are ***,by estimating.the states continuously from available measurements of some of the states,it can accommodate dynamic chang.s in the system *** paper proposes the technique of aug.enting.the state variables with controller *** introduces a non-linearity to the aug.ented system and thereby the estimation is performed using.an Extended Kalman *** results in producing.controller g.ins that are capable of controlling.the system in response to chang.s in load demand,system parameter variation and measurement noise.

Emerg.ncy department procedural sedation for primary electrical cardioversion — a comparison with procedural sedations for other reasons

BACKg.OUND: Atrial fibrillation(AF) is the most common arrhythmia treated in the emerg.ncy department(ED), with primary electrical cardioversion(PEC) the preferred method of rhythm control. Anecdotally, patients underg.ing.ED procedural sedation(EDPS) for PEC differ from those requiring.EDPS for other procedures: they are at hig.er risk of adverse events, and require fewer drug. and lower doses. We attempt to verify this using.an EDPS reg.stry at a Canadian, tertiary care teaching.***: This is a retrospective review of patients that underwent EDPS for the period of June 2006 to September 2014. We compared demog.aphics, medication use and intra-procedural adverse events between those receiving.EDPS for PEC for AF compared to that for other indications. We report the asssociation between AEs and predictors using.log.stic ***: A total of 4 867 patients were included, 714 for PEC for AF and 4 153 for other indications. PEC patients were more likely male(58.5% vs. 47.1%), older(59.5 years vs. 48.1 years), and less likely to be ASA I(46.6% vs. 69.0%). PEC patients received smaller doses of propofol and less likely to receive adjuvant analg.sic therapy(11.5% vs. 78.2%). PEC patients were more likely to experience hypotension(27.6% vs. 16.5%) but respiratory AEs(apnea, hypoxia and airway intervention) were not ***: EDPS for PEC differs from that conducted for other purposes: patients tend to be less healthy, receive smaller doses of medication and more likely to suffer hypotension without an increase in respiratory AEs. These factors should be considered when performing.EDPS.
