BACKGROUND Idiopathic inflammatory myopathies(IIM)are systemic autoimmune disorders such as dermatomyositis(DM),polymyositis(PM),inclusion body myopathy,and autoimmune necrotizing myopathy that,similar to osteoarthritis,affect quality of life and activities of daily ***,these patients are often burdened with chronic pain and disability;however,the outcomes and risk of total hip arthroplasty(tHA)in this patient population remain *** to evaluate 90-d complications and costs in patients with these *** A retrospective case control study was designed by accessing data from the Medicare dataset available on the PearlDiver *** with IIM,here,those with DM and PM were matched based on possible confounding variables to a cohort without these diseases and with the same 10-year risk of mortality as defined by the Charlson Comorbidity Index Score(CCI).Univariate and multivariate analysis were performed to evaluate complications and t-tests to evaluate 90-d Medicare reimbursements as markers of costs after *** the total sample was 1090 patients with each cohort comprised of *** were 74.9%of the *** mean CCI was 5.89(SD 2.11).those with IIM had increased rates of pneumonia[odds ratio(OR)1.45,Ptoma risks(OR 0.58,P=0.00).90-d costs were on average$1411 greater for those with IIM yet not significantly different(P=0.034).CONCLUSION Patients with IIM have an increased 90-d rate of pneumonia and pulmonary embolism concomitant with a decreased hematoma rate consistent with their procoagulatory *** attention to increased resource utilization in these patients is also warranted.

Biosynthetic gene clusters(BGCs)are regions of a genome where genes involved in a biosynthetic pathway are in *** origin and evolution of plant BGCs as well as their role in specialized metabolism remain largely *** this study,we have assembled a chromosome-scale genome of Japanese catnip(Schizonepeta tenuifolia)and discovered a BGC that contains multiple copies of genes involved in four adjacent steps in the biosynthesis of p-menthane *** BGC has an unprecedented bipartite structure,with mirrored biosynthetic regions separated by 260 *** bipartite BGC includes identical copies of a gene encoding an old yellow enzyme,a type of flavin-dependent *** vitro assays and virus-induced gene silencing revealed that this gene encodes the missing isopiperitenone *** enzyme evolved from a completely different enzyme family to isopiperitenone reductase from closely related Mentha spp.,indicating convergent evolution of this pathway *** analysis revealed that this bipartite BGC has emerged uniquely in the *** lineage and through insertion of pathway genes into a region rich in monoterpene *** cluster gained its bipartite structure via an inverted *** discovered bipartite BGC for p-menthane biosynthesis in *** has similarities to the recently described duplicated p-menthane biosynthesis gene pairs in the Mentha longifolia genome,providing an example of the convergent evolution of gene *** work expands our understanding of plant BGCs with respect to both form and evolution,and highlights the power of BGCs for gene discovery in plant biosynthetic pathways.

Season of the year influences infection rates following total hip arthroplasty

AIM to research the influence of season of the year on periprosthetic joint *** We conducted a retrospective review of the entire Medicare files from 2005 to 2014. Seasons were classified as spring, summer, fall or winter. Regional variations were accounted for by dividing patients into four geographic regions as per the United States Census Bureau(Northeast, Midwest, West and South). Acute postoperative infection and deep periprosthetic infections within 90 d after surgery were tracked. RESULtS In all regions, winter had the highest incidence of periprosthetic infections(mean 0.98%, SD 0.1%) and was significantly higher than other seasons in the Midwest, South and West(P t not the Northeast(P = 0.358). Acute postoperative infection rates were more frequent in the summer and were significantly affected by season of the year in the *** Season of the year is a risk factor for periprosthetic joint infection following total hip arthroplasty(tHA). Understanding the influence of season on outcomes following tHA is essential when risk-stratifying patients to optimize outcomes and reduce episode of care costs.

Excitotoxicity refers to toxicity caused by abnormal concentrations of glutamate in the synaptic cleft that may lead to neuronal death. Since its description, the phenomenon of glutamatergic excitotoxicity has been implicated in the physiopathology of a wide range of neurological and psychiatric disorders, from acute brain damage such as traumatic brain injury, ischemia as well as chronic condi- tions like epilepsy, depression and neurodegenerative pathologies such as Huntington＇s, Parkinson＇s and Alzheimer＇s diseases. Exces- sive stimulation of glutamatergic receptors, mainly N-methyl-D-as- partate （NMDA） receptors （NMDAR）, can have numerous adverse effects on the cell viability, including increased nitric oxide release （NO）, activation of proteases, increased production of reactive oxygen （ROS） and nitrogen （RNS） species and massive influx of calcium ions （Ca2＋）, resulting in cell death. thus, the use of strategies that modulate the excitotoxic cell damage represents a perspective for the treatment of diseases such as Parkinson＇s and Alzheimer＇s diseases, ischemia, traumatic brain injury （tBI） and seizures.

theoretical mean wave resistance and regional division of the energy of single-layer flow over topogra-phy is studied at the near-resonant region in the weakly nonlinear, long wave limit. the theoretical meanwave resistance is determined in terms of the 1st and 2nd conservation laws of the fKdV equation. It isproved by the asymptotic mean method that the theoretical mean wave resistance depends only on the intensityand moving velocity of the topography. the theoretical results of this paper are in good agreement withnumerical calculations. Comparisons between the theoretical and numerical results showed that the theoryof the present paper holds for any small compact topography.

the study aimed to assess the clinical, laboratory, and blood gas analysis of horses undergoing Marcha training and the effects of voluntary ingestion of hydroelectrolytic and energy replenishers after exercise. Eight horses of both genders aged between 5 and 10 years, were included in the study. the exercise consisted of a 10-min warm-up followed by 45 min uninterrupted Marcha on a flat dirt track in the morning. After exercise, the horses received one of the following treatments: Drinking water (control group);Hydroelectrolytic and energy replenisher containing sodium chloride, potassium chloride, calcium acetate, magnesium chloride, sodium citrate, dextrose, maltodextrin, and sucrose in three different concentrations (Replenishers A, B, and C). the horses were distributed across the four treatments in a 4 × 4 Latin Squares design using a Split-plot system with 48-hr intervals. Clinical and laboratory evaluations were conducted at four time points: t0 - 5 min before exercise;t1 - up to 5 min after exercise;t2 - 2 hr after starting treatment;and t4 - 4 hr after beginning treatment. Concentrations of urea, creatinine, lactate, phosphorus, and ionized calcium significantly changed after exercise. An increase in blood pH and a decrease in chloride concentrations were observed when replenishers B and C were offered after exercise. the replacements were ingested spontaneously by the animals in a volume greater than that of the control group (water). Replacement B was the most ingested by the animals, demonstrating its greatest potential.

A practical strategy to subcutaneous administered in-situ gelling co-delivery system of arsenic and retinoic acid for the treatment of acute promyelocytic leukemia

Arsenic trioxide(AtO) combined with all trans retinoic acid(AtRA) is the first choice for the treatment of low and medium risk acute promyelocytic leukemia(APL). Clinical studies reported that the combination of AtO and AtRA could achieve a significant curative effect. However, the retinoic acid syndrome, serious drug resistance and the short half-life in vivo which lead to frequent and large dose administration limit the application of AtRA. In addition, the preparations of arsenic are conventional injections and tablets in clinic, which has poor patients’ compliance caused by frequent long-term administration and serious side effects. In order to overcome the above limitations, a phospholipid phase separation gel(PPSG) loaded with AtO and AtRA was developed. AtO + AtRA-PPSG(AAP), as a biodegradable sustained-release delivery system, was the first achievement of co-delivery of hydrophilic AtO and lipophilic AtRA with high drug loading which is the main problem in the application of nano preparation. the prepared PPSG displayed high safety and biocompatibility. the drug in PPSG was released slowly and continuously in vivo and in vitro for up to 10 d, which could reduce the side effects caused by the fluctuation of blood drug concentration and solve the problem of the long treatment cycle and frequent administration. In vivo pharmacokinetics depicted that PPSG could improve the bioavailability, decrease the peak concentration, and prolong the t 1/2 of AtO and AtRA. Particularly, AAP significantly inhibited the tumor volume, extended the survival period of tumor-bearing mice, and promoted the differentiation of APL cells into normal cells. therefore, AtO + AtRA-PPSG not only could co-load hydrophilic AtO and lipophilic AtRA according to the clinical dosage, but also possessed the sustained-release and long-acting treatment effect which was expected to reduce administration time and ameliorate compliance of patients. thus, it had great potential for clinical transforma
