Background Ni-kshay Poshan Yojana(NPY)is a direct benefit transfer scheme of the Government of India introduced in 2018 to support the additional nutritional requirements of persons with TB(PwTB).Our recent nationwide evaluation of implementation and utilization of NPY using programmatic data of PwTB from nine randomly selected Indian states,reported a 70%coverage and high median delay in benefit *** undertook a qualitative study between January and July 2023,to understand the detailed implementation process of NPY and explore the enablers and barriers to effective implementation and utilization of the NPY *** We followed a grounded theory approach to inductively develop theoretical explanations for social phenomena through data generated from multiple *** conducted 36 in-depth interviews of national,district and field-level staff of the National Tuberculosis Elimination Programme(NTEP)and NPY beneficiaries from 30 districts across nine states of India,selected using theoretical *** analytical framework developed through inductive coding of a set of six interviews,guided the coding of the subsequent *** and themes emerged through constant comparison and the data collection continued until theoretical *** Stakeholders perceived NPY as a beneficial *** political commitment from the state administration,mainstreaming of NTEP work with the district public healthcare delivery system,availability of good geographic and internet connectivity and state-specific grievance redressal mechanisms and innovations were identified as enablers of ***,the complex,multi-level benefit approval process,difficulties in accessing banking services,perceived inadequacy of benefits and overworked human resources in the NTEP were identified as barriers to implementation and *** The optimal utilization of NPY is enabled by strong political commitment and challenged by its length

Customer churn poses a significant challenge for the banking and finance industry in the United States, directly affecting profitability and market share. This study conducts a comprehensive comparative analysis of machine learning models for customer churn prediction, focusing on the U.S. context. The research evaluates the performance of logistic regression, random forest, and neural networks using industry-specific datasets, considering the economic impact and practical implications of the findings. The exploratory data analysis reveals unique patterns and trends in the U.S. banking and finance industry, such as the age distribution of customers and the prevalence of dormant accounts. The study incorporates macroeconomic factors to capture the potential influence of external conditions on customer churn behavior. The findings highlight the importance of leveraging advanced machine learning techniques and comprehensive customer data to develop effective churn prevention strategies in the U.S. context. By accurately predicting customer churn, financial institutions can proactively identify at-risk customers, implement targeted retention strategies, and optimize resource allocation. Blockchain-Based System for Secure and Efficient Cross-Border Remittances: A Potential Alternative to SWIFT

This paper proposes a blockchain-based system as a secure, efficient, and cost-effective alternative to SWIFT for cross-border remittances. The current SWIFT system faces challenges, including slow settlement times, high transaction costs, and vulnerability to fraud. Leveraging blockchain technology’s decentralized, transparent, and immutable nature, the proposed system aims to address these limitations. Key features include modular architecture, implementation of microservices, and advanced cryptographic protocols. The system incorporates Proof of Stake consensus with BLS signatures, smart contract execution with dynamic pricing, and a decentralized oracle network for currency conversion. A sophisticated risk-based authentication system utilizes Bayesian networks and machine learning for enhanced security. Mathematical models are presented for critical components, including transaction validation, currency conversion, and regulatory compliance. Simulations demonstrate potential improvements in transaction speed and costs. However, challenges such as regulatory hurdles, user adoption, scalability, and integration with legacy systems must be addressed. The paper provides a comparative analysis between the proposed blockchain system and SWIFT, highlighting advantages in transaction speed, costs, and security. Mitigation strategies are proposed for key challenges. Recommendations are made for further research into scaling solutions, regulatory frameworks, and user-centric designs. The adoption of blockchain-based remittances could significantly impact the financial sector, potentially disrupting traditional models and promoting financial inclusion in underserved markets. However, successful implementation will require collaboration between blockchain innovators, financial institutions, and regulators to create an enabling environment for this transformative system.

A prospective cohort of men with localized prostate cancer on active surveillance protocol in Hong Kong,China:what did we learn?

This study aimed to report the outcomes of active surveillance(AS)in the management of low-risk prostate cancer(PCa).It recruited87 men who were prospectively followed up according to the Prostate Cancer Research International Active Surveillance(PRIAS)protocol with local adaptation at SH Ho Urology Centre,prince of Wales Hospital,Hong Kong,*** investigated the predictorsof disease progression and found that baseline prostate-specific antigen density(PSAD)and the presence of the highest ProstateImaging-Reporting and Data System(PI-RADS)score 5 lesion on magnetic resonance imaging(MRI)are significantly correlatedwith disease ***,men with PSAD>0.2 ng ml^(−2)or PI-RADS 4 or 5 lesions had significantly worse upgradingfree survival compared to those with PSAD≤0.2 ng ml−2 and PI-RADS 2 or 3 *** study concludes that AS is a safe andeffective management strategy for selected patients to defer radical treatment and that most disease progression can be detectedafter the first repeated *** combination of PSAD>0.2 ng ml^(−2)and PI-RADS 4 or 5 lesions may serve as a useful predictorof early disease progression and provide a guide to optimize follow-up protocols for men in different risk groups.

Cost effectiveness of nonpharmacological prevention programs for diabetes:A systematic review of trial-based studies

Trial-based economic value of prevention programs for diabetes is *** aimed to review the cost-effectiveness of nonpharmacological interventions to prevent type-2 diabetes mellitus(T2DM)for high-risk *** electronic databases were searched up to March *** assessing both the cost and health outcomes of nonpharmacological interventions for people at high-risk of T2DM were *** quality of the study was assessed by the Consolidated Health Economic Evaluation Reporting Standards 2022 *** primary outcome for synthesis was incremental cost-effectiveness ratios(ICER)for quality-adjusted life years(QALYs),and costs were standardized in 2022 US *** synthesis was performed,considering different types and delivery methods of *** studies included five based on the US diabetes prevention program(DPP),six on non-DPP-based lifestyle interventions,four on health education,and one on screening plus lifestyle *** with usual care,lifestyle interventions showed higher potential of cost-effectiveness than educational *** lifestyle interventions,DPP-based programs were less cost-effective(median of ICERs:$27,077/QALY)than non-DPP-based programs(median of ICERs:$1395/QALY)from healthcare perspectives,but with larger decreases in diabetes ***,the cost-effectiveness of interventions was more possibly realized through the combination of different delivery *** interventions to prevent T2DM in high-risk populations are both cost-effective and feasible in various ***,economic evidence from low-and middle-income countries is still lacking,and interventions delivered by trained laypersons and combined with peer support sessions or mobile technologies could be potentially a cost-effective solution in such settings with limited resources.

Internet of things (IoT) has been significantly raised owing to thedevelopment of broadband access network, machine learning (ML), big dataanalytics (BDA), cloud computing (CC), and so on. The development of IoTtechnologies has resulted in a massive quantity of data due to the existenceof several people linking through distinct physical components, indicatingthe status of the CC environment. In the IoT, load scheduling is realistictechnique in distinct data center to guarantee the network suitability by fallingthe computer hardware and software catastrophe and with right utilize ofresource. The ideal load balancer improves many factors of Quality of Service(QoS) like resource performance, scalability, response time, error tolerance,and efficiency. The scholar is assumed as load scheduling a vital problem inIoT environment. There are many techniques accessible to load scheduling inIoT environments. With this motivation, this paper presents an improved deerhunting optimization algorithm with Type II fuzzy logic (IDHOA-T2F) modelfor load scheduling in IoT environment. The goal of the IDHOA-T2F is todiminish the energy utilization of integrated circuit of IoT node and enhancethe load scheduling in IoT environments. The IDHOA technique is derivedby integrating the concepts of Nelder Mead (NM) with the DHOA. Theproposed model also synthesized the T2L based on fuzzy logic (FL) systemsto counterbalance the load distribution. The proposed model finds usefulto improve the efficiency of IoT system. For validating the enhanced loadscheduling performance of the IDHOA-T2F technique, a series of simulationstake place to highlight the improved performance. The experimental outcomesdemonstrate the capable outcome of the IDHOA-T2F technique over therecent techniques.

Twentieth Century Wanzams among the Asante People of Ghana: A Historical Study of the Facts on Male Circumcision

This paper studies the history of circumcision amongst religious and traditional societies and the arguments on the medical or non-medical essence of the practice. The paper further highlights amongst the Asante people of Ghana, a practice argued to have been introduced by Islamic Wanzams who are severally referred to as Muslim Circumcision Surgeons. Often circumcision amongst communities in West, Southern Africa, Asia and other Aboriginal groups has been spurred on by customs and traditions [1]. Yet the case of Asante was different. Hence attention has been paid to the history of circumcision amongst the people of Asante. More significantly, the health implications associated with circumcision surgery in Asante have been highlighted. Possible benefits from the practice of circumcision and the potential dangers associated with it, especially the operations of the Asante circumcision surgeons (Wanzams) have been amply scrutinised in the narrative. In the conclusion, attention is paid to some of the scientific arguments for male circumcision. The study is purely qualitative relying on documentary and non-documentary sources. Some of the documentary sources have been gleaned from journal articles, news papers and books etc. The non-documentary data have been sourced from interviews. Both the documentary and non-documentary sources have been thematically pieced together to form a social history of medicine narrative which has potential ramifications on the essence of male circumcision by Wanzams, as well as the need for further research, training and dialogue in the practice in Ghana and Asante in particular.

Analysis of Farmers Adoption Behaviour of CRIG Recommended Technologies as a Package: The Case of Some Self Help Cocoa Farmer Associations in the Eastern Region of Ghana

Adoption of recommended technologies as a package is the prelude to increase cocoa productivity per unit area. This is due to the interactive benefits of individual technologies which have been recommended by the Cocoa Research Institute of Ghana (CRIG). However, many surveys among cocoa farmers have reported low adoption of technologies resulting in low productivity of cocoa with an average of 450 kg/ha among small holder cocoa farmers in Ghana. The current study investigates the adoption behavior of some cocoa farmers belonging to some self help farmer associations in the Eastern Region who are being monitored by CRIG since 2011. Primary data was collected from 131 respondents using questionnaires to interview farmers between 2013 and 2015. The results showed that adoption of recommended cocoa technologies as a package was still low. Respondents, however, identified a number of challenges including high cost of inputs, lack of finance and access to credit, high cost of labour and old age as some factors hampering composite adoption of the full CRIG technologies. It is recommended that small-scale cocoa farmers need a sustainable and convenient microfinance that can motivate and help them afford the cost associated with the full package of technologies. Farmers’ savings culture should also be nurtured to enhance their financial capabilities and investment in the cocoa farm.

Seroprevalence and Risk Factors of Syphilis Infection among Antiretroviral Therapy Naive HIV Patients at the Cape Coast Teaching Hospital, Ghana

Diagnosis of sexually transmitted infections is very important considering the spread of HIV and the extensive use of highly active antiretroviral therapy worldwide. This will assist in planning of treatment schedule in controlling these infections. The study therefore aimed at determining the prevalence of syphilis in HIV positive antiretroviral therapy naive patients in Cape Coast and the associated risk factors involved in infection. A cross-sectional study was carried out using initial HIV rapid and confirmation tests, and then Venereal Disease Research Laboratory test with the Ultra Rapid Test Kits for syphilis. Demographic data, risky sexual behaviours capable of co-transmission of both HIV and Syphilis, were also collected through the use of questionnaires. In all, 150 HIV positive antiretroviral naive subjects were studied and 15 (10%) were positive for VDRL test, with females (6.00%) and males (4.00%), who were mainly within the age group of 20 - 39 years. A significant number of males (p = 0.019) and females (p = 0.015) participants were not smoking with a fewer number of the females (p = 0.002) having multiple sexual partners. Also a smaller number of those who were infected with the bacteria (p = 0.004) did not support the control of sexually transmitted infection (p = 0.022). The result showed that co-infection of Syphilis in HIV positive antiretroviral therapy naive patients persists in the Cape Coast Metropolis, which is an indication of prominence of STIs that require further study on a larger scale to ascertain the extent of co-infection and to formulate policy for treatment to help minimize the rate of infection.
