BACKGROUND The impact of age on outcomes in cardiogenic shock(CS)is poorly described in the pre-hospital *** assessed the impact of age on outcomes of patients treated by emergency medical services(EmS).mETHODS This population-based cohort study included consecutive adult patients with CS transported to hospital by *** linked patients were stratified into tertiles by age(18-63,64-77,and>77 years).Predictors of 30-day mortality were assessed through regression *** primary outcome was 30-day all-cause *** A total of 3523 patients with CS were successfully linked to state health *** average age was 68±16 years and 1398(40%)were *** patients were more likely to have comorbidities including pre-existing coronary artery disease,hypertension,dyslipidemia,diabetes mellitus,and cerebrovascular *** incidence of CS was significantly greater with increasing age(incidence rate per 100,000 person years 6.47[95%CI:6.1-6.8]in age 18-63 years,34.34[32.4-36.4]in age 64-77 years,74.87[70.6-79.3]in age>77 years,Pmortality with increasing age *** adjustment,compared to the lowest age tertile,patients aged>77 years had increased risk of 30-day mortality(adjusted hazard ratio=2.26[95%CI:1.96-2.60]).Older patients were less likely to receive inpatient coronary *** Older patients with EmS-treated CS have significantly higher rates of short-term *** reduced rates of invasive interventions in older patients underscore the need for further development of systems of care to improve outcomes for this patient group.

This paper shows that one-dimensional(1-D) [and three-dimensional(3-D) computational fluid dynamics(CFD)] simulations can replace the state-of-the-art usage of pseudo-homogeneous dispersion or back mixing *** is based on standardized lab-scale cell experiments for the determination of droplet rise,breakage,coalescence and mass transfer parameters in addition to a limited number of additional mini-plant experiments with original ***,the hydrodynamic parameters can also be derived using more sophisticated 3-D CFD *** 1-D modeling served as a basis to replace pilot-plant experiments in any column *** combination of 3-D CFD simulations with droplet population balance models(DPBm)increased the accuracy of the hydrodynamic simulations and gave information about the local droplet *** high computational costs can be reduced by open source CFD codes when using a flexible mesh *** combined simulations using a three way coupled CFD/DPBm/mass-transfer solver pave the way for a safer design of industrial-sized columns,where no correlations are available.

Application of metaheuristic Algorithms for Optimizing Longitudinal Square Porous Fins

The objectives of this study involve the optimization of longitudinal porous fins of square cross-section using metaheuristic algorithms.A generalized nonlinear ordinary differential equation is derived using Darcy and Fourier’s laws in the energy balance around a control volume and is solved numerically using RFK 45 *** temperature of the base surface is higher than the fin surface,and the fin tip is kept adiabatic or cooled by convection heat *** other pertinent parameters include Rayleigh number(100≤Ra≤10^(4)),Darcy number,(10^(−4)≤Da≤10^(−2)),relative thermal conductivity ratio of solid phase to fluid(1000≤kr≤8000),Nusselt number(10≤Nu≤100),porosity(0.1≤φ≤0.9).The impacts of these parameters on the entropy generation rate are investigated and optimized using metaheuristic *** computer science,metaheuristic algorithms are one of the most widely used techniques for optimization *** this research,three metaheuristic algorithms,including the firefly algorithm(FFA),particle swarm algorithm(PSO),and hybrid algorithm(FFAPSO)are employed to examine the performance of square *** is demonstrated that FFA-PSO takes fewer iterations and less computational time to converge compared to other algorithms.

Central retinal artery resistive index and optical coherence tomography in assessment of glaucoma progression

AIm: To assess the relation between central retinal artery(CRA) resistive index(RI) and retinal nerve fiber thickness measured by optical coherence tomography(OCT) in assessment of disease progress in cases of open angle ***: Twenty-three patients with diagnosed open angle glaucoma were included in this study. They were examined by colored duplex ultrasonography of CRA with estimation of RI of CRA and ophthalmic artery(OA)with estimation of CRA/OA RI ratio as well as OCT measurement of the average retinal nerve fiber layer(RNFL) thickness in order to assess the disease ***: There was strong inverse relation between the increased RI in CRA as well as the increased CRA/OA RI ratio and the decrease in average RNFL thickness in cases of open angle ***: Assessment of CRA RI can indirectly assess the vascular changes associated with glaucoma and can assess the degree of retinal atrophy helping in evaluating prognosis thus guiding the choice of treatment.

Effect of Cowshed Design and Cooling Strategy on Welfare and Productivity of Dairy Cows

Nine cowsheds were used, the differences between each cowshed are orientation (east-west or north-south), cooling (with or without), and height (3 m, 5 m or 8 m height). Dry-bulb temperature, relative humidity, dew point, shaded area, air velocity, and maximum temperature were measured beneath each shed. The temperature-humidity index (THI) and the shading efficiency were calculated for each cowshed. The trials were conducted using 180 lactating Holstein Friesian cows; twenty cows housed beneath each shade structure. measurements performed on cows were respiration rate, skin temperature, and rectal temperature; moreover, the milk produced by each cow was recorded, and the feed intake. High air velocities were recorded under high shade structures which enhance the aeration; consequently, maximum temperatures, and THI recorded beneath high sheds were less than those recorded beneath low sheds; also, skin temperatures, and respiration rates for cows housed under high sheds were less than those measured for animals housed under low sheds(differences were found to be significant). Thereby, increasing shed height enhances dairy cows’ microclimate under Egyptian conditions (hot climate); consequently, the milk production increases. The best orientation is the east-west orientation, which is preferable for hot climates. The exploitation of cooling has significant effect on microclimate, cooling enhances the ambient environment and provides comfortable zone for dairy cows within which cows reach high production levels.

Resistin is not an appropriate biochemical marker to predict severity of acute pancreatitis:A case-controlled study

AIm:To assess levels of serum resistin upon hospital admission as a predictor of acute pancreatitis(AP) ***:AP is both a common and serious disease,with severe cases resulting in a high mortality *** predictive inflammatory markers have been used clinically to assess *** prospective study collected data from 102 patients who were diagnosed with an initial acute biliary pancreatitis between march 2010 and February *** of body mass index(BmI) and waist circumference(WC) were obtained and serum resistin levels were analyzed at the time of hospital admission using enzyme-linked immunosorbent ***,resistin levels were measured from a control group after matching gender,BmI and ***:A total of 102 patients(60 females and 42 males) were diagnosed with acute gallstone-induced *** mean age was 45 years,and mean BmI value was 30.5 kg/m2(Obese,class?Ⅰ).Twentytwo patients(21.6%) had severe AP,while eighty-eight patients had mild pancreatitis(78.4%).Our results showed that BmI significantly correlated with pancreatitis severity(P = 0.007).Serum resistin did not correlate with BmI,weight or ***,serum resistin was significantly higher in patients with AP compared to control subjects(P mean resistin values upon admission were 17.5 ng/mL in the severe acute biliary pancreatitis group and 16.82 ng/mL in the mild AP group(P = 0.188),indicating that resistin is not an appropriate predictive marker of clinical ***:We demonstrate that obesity is a risk factor for developing severe ***,although there is a correlation between serum resistin levels and AP at the time of hospital admission,resistin does not adequately serve as a predictive marker of clinical severity.
