Ai.： To assess the objecti.e and subjecti.e results of a two-stage procedure for management of keratoconus： clear lensectomy wi.h aspheri.al i.traocular lens（i.L） i.plantati.n followed by wave front-gui.ed photorefracti.e keratotomy（WFG-PRK）.METHODS： Thi. prospecti.e i.terventi.nal non-randomi.ed study i.cluded pati.nts aged 35 years old or more wi.h grade i.and i. stable keratoconus, a clear vi.ual axi., mi.i.al corneal thi.kness（MCT） 420 μm or more and average keratometri. readi.g（K） less than 54 di.pter（D）. Refracti.n of all selected eyes should be-8.00 D sphere or more wi.h less than-6.00 D cyli.der and could be corrected two li.es or more wi.h spectacles or contact lenses. All studi.d eyes underwent a two-stage approach treatment： fi.st refracti.e lens exchange and aspheri.al i.L i.plantati.n followed, after at least 3 mo, by WFGPRK. Pre and postoperati.e complete ophthalmologi.al exami.ati.n were performed. Topographi.al, vi.ual and aberrometri. results were recorded and evaluated duri.g 6 mo follow up peri.d. Moreover, pati.nt sati.facti.n and other subjecti.e outcomes were also ***： The 13 eyes of 11 pati.nts di.gnosed wi.h stable keratoconus and aged from 39 to 49 y（42.4±6.2 y） were enrolled i. the study. At baseli.e, 8 eyes had grade i.and 5 eyes had grade i. keratoconus. The mani.est sphere was-10.3±4.2 D（ranged from-8.0 to-14.0 D） and the mani.est cyli.der was-4.2±1.2 D（ranged from-1.75 to-5.50 D）. After the two-stage procedure, sphere and cyli.der reduced si.ni.i.antly to-0.43±0.22 D and-1.3±0.72 D respecti.ely（P〈0.001）. There was also a hi.hly si.ni.i.ant i.provement i. the mean uncorrected di.tance vi.ual acui.y（UDVA） from logMAR 1.41±0.49 preoperati.ely to 0.51±0.16 postoperati.ely（P〈0.001） and the mean corrected di.tance vi.ual acui.y（CDVA） from 0.76±0.24 preoperati.ely to 0.49±0.13 after the operati.n（P〈0.001）. Ai. of Work: The ai. of our work was early cytologi.al and laboratory detecti.n of any ocular surface mali.nant changes i. xeroderma pi.mentosa. Pati.nts and Methods: Ei.hteen cases wi.h xeroderma pi.mentosa were i.cluded i. thi. study. All cases were subjected to full ophthalmolog-i.al exami.ati.n, conjuncti.al bi.psy from any abnormal lesi.n, polymerase chai. reacti.n assay and i.pressi.n cytology. Results: All cases were boys;the mean age was 11 years old. The mai. ocular presentati.n was dryness and abnormal epi.ulbar masses i. di.ferent locati.ns. Polymerase chai. reacti.n represents preci.i.ati.n of mali.nant cells whi.h was confi.med by i.pressi.n cytology. Conclusi.n: PCR and i.pressi.n cytology are benefi.i.l i.vesti.ati.ns for detecti.n of any mali.nant changes i. xeroderma pi.mentosa.

Accelerated versus standard corneal cross li.ki.g i. the treatment of ectasi. post refracti.e surgery and penetrati.g keratoplasty: a medi.m term randomi.ed tri.l

Ai.: To compare the cli.i.al outcomes of the standard corneal cross li.ki.g(CXL) and the accelerated CXL i. pati.nts wi.h progressi.e corneal ectasi. post refracti.e surgery and penetrati.g ***: Totally 120 eyes of 83 pati.nts scheduled to recei.e ei.her standard CXL(3 m W/cm^2 for a peri.d of 30 mi.) or accelerated CXL(18 m W/cm2 for a peri.d of 5 mi.). The mai. outcomes for compari.on were the change i.: maxi.um-K readi.g(K-max), mani.est refracti.e spheri.al equi.alent(SE), central corneal thi.kness(CCT), and the best corrected di.tance vi.ual acui.y(CDVA).RESULTS: One hundred and eleven eyes completed the study. The mai. outcome measurement was the K-max readi.g. Both group showed si.ni.i.ant i.provement i. the value postoperati.ely at 6 and 12 mo. The mean change i. the standard group was 1.21±0.11 D and i. the accelerated group was 0.90±0.05 D at the end of 12 mo postoperati.ely, wi.h no stati.ti.ally si.ni.i.ant di.ference between the 2 groups. Si.i.arly, CDVA i.proved si.ni.i.antly from thei. preoperati.e value i. the standard group by 2.98±0.11 letters, and i. the accelerated group by 2.20±0.06 letters, wi.h no stati.ti.ally si.ni.i.ant di.ference between the two groups. Both of the SE, and CCT showed no stati.ti.ally si.ni.i.ant di.ference at the end of follow up peri.d i. each ***: Both standard CXL and accelerated CXL are safe and effecti.e treatment i. halti.g ectasi. after corneal refracti.e surgery. The accelerated CXL results are comparable to the standard CXL wi.h short ti.e exposure of the cornea to ultravi.let i.radi.ti.n, leadi.g to reduced operati.n ti.e, reduced operati.e ocular di.comfort, and corneal haze.
