Two-stage procedure in the management of selected cases of keratoconus: clear lens extraction with aspherical IOL implantation followed by WFG-PRK

AIM： To assess the objective and subjective results of a two-stage procedure for management of keratoconus： clear lensectomy with aspherical intraocular lens（IOL） implantation followed by wave front-guided photorefractive keratotomy（WFG-PRK）.METHODS： This prospective interventional non-randomized study included patients aged 35 years old or more with grade I and II stable keratoconus, a clear visual axis, minimal corneal thickness（MCT） 420 μm or more and average keratometric reading（K） less than 54 diopter（D）. Refraction of all selected eyes should be-8.00 D sphere or more with less than-6.00 D cylinder and could be corrected two lines or more with spectacles or contact lenses. All studied eyes underwent a two-stage approach treatment： first refractive lens exchange and aspherical IOL implantation followed, after at least 3 mo, by WFGPRK. Pre and postoperative complete ophthalmological examination were performed. Topographical, visual and aberrometric results were recorded and evaluated during 6 mo follow up period. Moreover, patient satisfaction and other subjective outcomes were also ***： The 13 eyes of 11 patients diagnosed with stable keratoconus and aged from 39 to 49 y（42.4±6.2 y） were enrolled in the study. At baseline, 8 eyes had grade I and 5 eyes had grade II keratoconus. The manifest sphere was-10.3±4.2 D（ranged from-8.0 to-14.0 D） and the manifest cylinder was-4.2±1.2 D（ranged from-1.75 to-5.50 D）. After the two-stage procedure, sphere and cylinder reduced significantly to-0.43±0.22 D and-1.3±0.72 D respectively（P〈0.001）. There was also a highly significant improvement in the mean uncorrected distance visual acuity（UDVA） from logMAR 1.41±0.49 preoperatively to 0.51±0.16 postoperatively（P〈0.001） and the mean corrected distance visual acuity（CDVA） from 0.76±0.24 preoperatively to 0.49±0.13 after the operation（P〈0.001）. Accelerated versus standard corneal cross linking in the treatment of ectasia post refractive surgery and penetrating keratoplasty: a medium term randomized trial

AIM: To compare the clinical outcomes of the standard corneal cross linking(CXL) and the accelerated CXL in patients with progressive corneal ectasia post refractive surgery and penetrating ***: Totally 120 eyes of 83 patients scheduled to receive either standard CXL(3 m W/cm^2 for a period of 30 min) or accelerated CXL(18 m W/cm2 for a period of 5 min). The main outcomes for comparison were the change in: maximum-K reading(K-max), manifest refractive spherical equivalent(SE), central corneal thickness(CCT), and the best corrected distance visual acuity(CDVA).RESULTS: One hundred and eleven eyes completed the study. The main outcome measurement was the K-max reading. Both group showed significant improvement in the value postoperatively at 6 and 12 mo. The mean change in the standard group was 1.21±0.11 D and in the accelerated group was 0.90±0.05 D at the end of 12 mo postoperatively, with no statistically significant difference between the 2 groups. Similarly, CDVA improved significantly from their preoperative value in the standard group by 2.98±0.11 letters, and in the accelerated group by 2.20±0.06 letters, with no statistically significant difference between the two groups. Both of the SE, and CCT showed no statistically significant difference at the end of follow up period in each ***: Both standard CXL and accelerated CXL are safe and effective treatment in halting ectasia after corneal refractive surgery. The accelerated CXL results are comparable to the standard CXL with short time exposure of the cornea to ultraviolet irradiation, leading to reduced operation time, reduced operative ocular discomfort, and corneal haze.

Natural Convection Flow and Heat Transfer Enhancement of a Nanofluid past a Truncated Cone with Magnetic Field Effect

A nonsimilarity analysis is performed to investigate the laminar, free convection boundary layer flow over a permeable isothermal truncated cone in the presence of a transverse magnetic field effect. A suitable set of dimensionless variables is used and non-similar equations governing the problem are obtained. Fourth order Runge-Kutta with shooting technique is employed for the numerical solution of the obtained equations. Different water-based nanofluids containing Cu, Ag, CuO, Al2O3, and TiO2 are taken into consideration. The effects of pertinent parameters such as the solid volume fraction of nanoparticles, and magnetic field parameter have been investigated. Furthermore, different models of nanofluid based on different formulas for thermal conductivity and dynamic viscosity on the flow and heat transfer characteristics are discussed. Various comparisons with previously published work for the case of a vertical plate are performed and the results are found to be in excellent agreement.

Concomitant Boost Radiotherapy after Conservative Breast Surgery in Early Breast Cancer

Background: Radiation therapy after breast conserving surgery is a standard part of treatment for invasive breast cancer. Based on radiobiological models, it was found that shorter radiation schedules offered the promise of equivalent local control to standard radiation therapy by giving larger doses per fraction in shorter period of time. Methods: This study included 36 female patients with operable invasive stage I-II breast cancer. These patients underwent microscopic wide local excision of the primary tumor and lymph node dissection. They received adjuvant radiotherapy. The radiation dose was 40 Gy total dose in 15 fractions for whole breast and additional dose of 9 Gy in three consecutive fractions was delivered to tumour cavity simultaneously. Results: Mean age was 52 years (range: 30 - 67);most patients were of stage II disease and Grade II was the most common one. Invasive ductal carcinoma was reported in 94.4% and 72.2% of patients were hormone receptor positive. After median follow-up of 52 months, all patients were alive and ipsilateral local recurrence was reported in 1 case only. Grade IV radiation toxicity was not observed;moist desquamation was the most common acute reaction (61%) with grade III in 5.5% followed by dry desquamation in 55.6% of grade I only. Grade I erythema was recorded in 41.7% and grade II in 11%. Fibrosis was the most frequent late reaction (44.3%) with grade II in 11% followed by telengectesia then pigmentation (41.7%, 33.3% respectively). Conclusion: The regimen used in this study appears promising with acceptable acute toxicities and convenient for our patients and has the advantage of economic use of radiation facilities. However, larger number of patients and longer period of follow-up are needed for further evaluation.

Groundwater Conditions and the Geoenvironmental Impacts of the Recent Development in the South Eastern Part of the Western Desert of Egypt

The area to the southeast of the Western Desert of Egypt has been subjected to considerable development activities over the last few years. The development includes the cultivation of about 2260 km2 of the desert lands “the well-known Toshka Project”. The hydrogeological conditions of the area are subjected to detailed investigation based upon the construction of the water table maps, hydrologeologic cross-sections, pumping tests, aquifer geometry, and recharge-discharge relationship. The study revealed that the Quaternary and the Nubia sediments are the main water bearing layers in the area. The Quaternary aquifer is of limited potential and made of mixed sand with clay deposit ranges in thickness between 5 to 10 m. The Nubia aquifer is the oldest sedimentary formation and the main groundwater resources in the area. It is represented by multilayered of sand and silt exists generally under artesian conditions. It is composed of three water bearing horizons partially separated by two confining horizons and extends in thickness ranges between 70 and 230 meters. The thickness increases away from the high dam lake. The analysis of pumping tests of the aquifer indicated that its potentiality is increasing north of the High Dam Lake (HDL) whereas it decreases in the other direction. This is due to high hydraulic conductivity and aquifer thickness in the area northeast of Khor Toshka and at west of Garf Hussein. The hydraulic conductivity of the aquifer ranges between 12.73 and 0.9 m/day. The review of the changes in groundwater levels in the area showed that there is a drop in ranges between 1 and 14 meters in the last few years indicating that the extraction from the groundwater is much more higher that the replacement rate. Also, the analysis of the fluctuation of water levels of the HDL and the groundwater level indicated that the influence of water on groundwater level in the area is observed only at a distance less than 10 km from the lake shore line. Application of Salt Whey from Egyptian Ras Cheese in Processed Cheese Making

The objective of this study was to evaluate characteristics of processed cheese made from salt Ras cheese whey and young Cheddar containing exopolysaccharide producing cultures (EPS) as compared with those in cheese made from EPS-negative cheeses. Processed cheeses were manufactured using young (1 day) or 1-month-old EPS-positive or EPSnegative Cheddar cheese and replacing the added water by Ras cheese whey or ultrafiltrated (UF) whey. Moisture and fat of processed cheese were standardized to 54% and 23%, respectively. Exopolysaccharide-positive processed cheese was softer, less chewy and gummy than the EPS-negative cheeses. The hardness and chewiness were lower in processed cheeses made from 1-month-old Cheddar than in the corresponding cheeses made from 1-day-old Cheddar cheese. Sensory scores for texture of EPS positive processed cheeses were higher than those of the EPS-negative cheeses. Generally, processed cheese made from replacing all added water with whey received higher scores in flavor and overall than did the corresponding cheeses made without whey. The obtained results are showing promise for application of salt whey in processed cheese making.

Unsteady MHD Mixed Convection Flow with Slip of a Nanofluid in the Stagnation Region of an Impulsively Rotating Sphere with Effects of Thermal Radiation and Convective Boundary Conditions

Unsteady magnetohydrodynamic mixed convection flow of an electrically conducting nanofluid in a stagnation region of a rotating sphere is studied numerically in the present article. Slip and convective boundary conditions are imposed to surface of the sphere and the thermal radiation effects are taken into account. The nanofluid is simulated using Buongiorno’s nanofluid model and the nanofluid particle fraction on the boundary is considered to be passively rather than actively controlled. Non-similar solutions are applied on the governing equations and the MATLAB function bvp4c is used to solve the resulting system. Effects of the key-parameters such as slip parameter, Biot number, radiation parameter, rotation parameter, Lewis number and Brownian motion parameter on the fluid flow, temperature and nanoparticle volume fraction characteristics are examined. Details of the numerical solution and a comprehensive discussion with the physical meaning for the obtained results are performed. The results indicated that the increase in slip parameter enhances the velocity profiles, while it decreases the temperature distributions. Also, the increase in either slip parameter or Biot number causes an improvement in the rate of heat transfer.

Sarcoidosis is a non caseating granulomatous disease of unknown etiology. Many factors may be attributed to the occurrence of that granuloma including the chemical factors. We present this case aiming to highlight Synthol injection which is chemical substance frequently used by athletics to increase their muscle size as it can trigger the occurrence of musculocutanous sarcoidosis. In conclusion Synthol should be added to the chemical list which can induce musculocutanous sarcoidosis.
