Efficacy and safety of botulinum toxin in treatment of anismus: A systematic review

AiM: To evaluate the efficacy and safety of botulinum toxin type A(BTX-A) in the management of patients with anismus. METHODS: An organized search of published literature was conducted using electronic databases including: Pub Med/MEDLiNE, and Cochrane Central Register of Control ed Trials, also an internet-based search using "Google Scholar" service was conducted. Both comparative and observational studies were included. We excluded irrelevant articles, editorials, case reports, reviews, and meta-analyses. The studies that followed the patients less than 6 mo were excluded. Variables collected were demographic data of the patients, technique of BTX-A injection and number of sessions, short-term and longterm clinical improvement, post-injection changes in electromyography(EMG), defecography, manometry, and balloon expulsion test, and complications recorded after BTX-A ***: Seven studies comprising 189 patients were included in the review. The median age of the patients was 41.2 years and female-to-male ratio was 1.3:1. The median dose of BTX-A injected per procedure was 100 iU(range, 20-100 iU). Lateral injection was done in five trails and combined lateral and posterior injections in two trials. Three studies used endorectal ultrasonographyguided technique, one study used EMG-guided technique,whereas the remaining three studies used manual palpation with the index finger. The median percentage of patients who reported initial improvement of symptoms was 77.4%(range 37.5%-86.7%), this percentage declined to a median of 46%(range 25%-100%) at 4 mo after injection of BTX-A. Rates of improvement evaluated by balloon expulsion test, EMG, and defecography ranged between(37.5%-80%),(54%-86.7%), and(25%-86.6%), respectively. Fourteen(7.4%) patients developed complications after injection of BTX-A. Complication rates across the studies ranged from 0% to 22.6%. Characterization of Radiation Prepared Copolymer and Studies of Their Biodegradability

Blends of biodegradable copolymer based on starch, poly lactic acid (PLA), poly vinyl alcohol (PVA) and natural rubber (NR) have been prepared. Gamma radiation induced synthesis and modification of polymer hydrogel was studied. The polymer blends have been chemically surface modified by glycerol. The modified polymer blends have been investigated for swelling ratio, tensile strength and 9 scanning electron microscopy. The swelling ratio of polymer blends increased significantly after surface modification with glycerol. The swelling of polymer was decreased as a function of (NR) content in polymer blends. The gel fraction (PVA-starch-PLA) and (PVA-starch-NR) blends increased by increasing the radiation doses (kGy) to reach the maximum amount of (~99%) and (~88.2%), respectively. Addition of 2.5% (PLA) led to greater increase of the swelling ratio than 10% (NR) to blends and the maximum swelling was found at dose (5 kGy). At concentration of glycerol (5.0% w/w), tensile strength decreased and elongation at break % increased. The polymers degrading microorganisms were isolated from soil samples. The degradation ability of the microbial isolates for each polymeric material was tested on agar plates. Among these isolates, the most efficient degrader isolates for prepared blends in MSM shaking flasks were selected and the degradation was confirmed by scanning electron microscopy.

The Effectiveness of intraoperative Frozen Section Analysis of Safety Margins in Breast Conserving Surgery and the Role of Surgeon in Decreasing the Rate of Positive Margins

introduction: Nowadays the more accepted surgical option for treating early breast cancer is breast conserving surgery. The main challenge in this type of surgery is to get free safety margins without need of second surgical operation, so many breast surgeons have started to depend on intraoperative frozen sections to ensure free safety margins. Aim of work: To assess our policy that we prefer to depend on intraoperative frozen section analysis to get free safety margin in breast conserving surgery from the 1st surgery, and its oncologic outcome. Patients and Methods: This is a retrospective study conducted in Oncology Center—Mansoura University (OCMU), where the data of 219 patients with breast cancer, who were managed by breast conserving surgery with intraoperative frozen section analysis of the safety margins, was analyzed. Results: The intraoperative frozen section analysis of safety margin was negative from the start in 183 (83.6%) patients, while it was positive in 36 patients (16.4%). intraoperative decision of margin re-excision was applied for 29 patients (13.2%) in order to reach negative margin, modified radical mastectomy was offered for 4 patients (1.8%), while nipple sparing mastectomy with immediate breast reconstruction using latissimus dorsi flap was offered for 3 patients (1.4%). The postoperative paraffin results were typical with intraoperative frozen section analysis results in 216 patients (98.6%) and different results were obtained in only 3 patients (1.4%) who were managed by modified radical mastectomy in a second operation. Only 4 patients had local recurrence (1.8%) during the period of follow-up duration which was ranged from 1 to 86 months with mean ± SD (22.3 ± 14.1). Conclusion: The intraoperative frozen section analysis of safety margins in breast conserving surgery has very high-rate typical results with the paraffin section analysis and it is very helpful in decreasing the rate of second surgical operation in cases of infiltrated m
