Urinary Continence after Orthotopic Detubularized ileocaecal Pouch Versus ileal Neobladder:Does it Differ?

Objective:To compare both techniques of detubularized ileocaecal and ileal neobladder post radical cystectomy with regards to functional results in the form of continence(day and night)with the use of urodynamics(for assessment of voiding volume and pouch capacity).Methods:This is a prospective randomized clinical trial that was done at the National Cancer institute,Cairo University from August 2016 and January ***-two patients presented with muscle invasive urinary bladder cancer and no history of urinary incontinence or urethral stricture were included in this *** patients underwent preoperative cystoscopic *** cystectomy was done,and patients were divided into two groups according to the type of *** A included 31 patients with orthotopoic detubularized ileal neobladder and group B included 31 patients with orthotopic detubularized ileocaecal *** studies were done to 27 patients in group A and 27 patients in group B at just 6 and 12 months ***:The mean age in group A was 59.2±8.0 while in group B was 57.8±*** each group,we compared day and night continence in a period of 3 and 6 *** in group A,day continence after 3 months was 81.5% which improved to 96.3% after 6 months postoperatively with significant P-value(0.025).Night continence after 3 months was fair in 66.7%,unsatisfactory in 33.3% and 0% for good state which improved to 70% for fair state,18% unsatisfactory and 11% for good state after 6 months with significant P-value(0.035)while in group B,day continence after 3 months and 6 months postoperatively was 100% and night continence after 3 months was good in 0.0%,fair in 78.6% and unsatisfactory in 21.4% which improved to 10.7% for good state,85.7% for fair state and 3.6% for unsatisfactory after 6 months with significant P-value(0.005).Conclusion:There are no significant differences between detubularized ileocaecal and ileal neobladder in relation to contin

BACKGROUND The current coronavirus disease 2019(COViD-19)pandemic has affected routine endoscopy service across the gastroenterology *** led to the suspension of service provision for elective *** To assess the potential barriers for resuming the endoscopy service in *** A national online survey,four domains,was disseminated over a period of 4 wk in August *** primary outcome of the survey was to determine the impact of the COViD-19 pandemic on the endoscopy service and barriers to the full resumption of a disabled center(s).RESULTS A hundred and thirteen Egyptian endoscopy centers participated in the *** waiting list was increased by≥50% in 44.9% of areas with clusters of COViD-19 cases(n=49)and in 35.5% of areas with sporadic cases(n=62).Thirty nine(34.8%)centers suffered from staff shortage,which was considered a barrier against service resumption by 86.4% of centers in per-protocol *** multivariate analysis,the burden of cases in the unit locality,staff shortage/recovery and the availability of separate designated rooms for COViD-19 cases could markedly affect the resumption of endoscopy practice(P=0.029,ively)and Odd’s ratio(0.15,1.8 and 0.16,respectively).CONCLUSiON The COViD-19 pandemic has led to restrictions in endoscopic *** staff shortage/recovery and the availability of COViD-19 designed rooms are the most important barriers against *** working hours and dividing endoscopy staff into teams may help to overcome the current situation.

Finding water resources for the desert plants is one of the important research areas since it enables saving water *** inermis plant was noticed to keep 5%of moisture in its rhizosphere zone;therefore,this study aimed to identify this source of *** endophytic bacteria were isolated from the root of *** and *** specific respiration rate was *** faecalis showed the highest specific respiration *** is a facultative chemoautotrophic hydrogen-fixing bacterium that utilizes the hydrogen gas as energy source and the water is produced as an end *** source of hydrogen gas for this bacterium is not only from air and soil gases but also from the hydrogen-producing bacteria such as Enterobacter spp.,which was among the isolated *** hydrogenases synthesizing genes(HoxC,HypA and HypB)were detected in most of the isolated bacteria and roots of four wild plants,out from 18 wild plant samples,epically the grains of the wild wheat *** result suggests that the hydrogen-fixing and hydrogen-producing bacteria transfer from the root through the plant to inhabit the grain/*** can help the grain/seeds to germinate in drought environment.

Role of Vascular Endothelial Growth Factor-C during Stem Cell Therapy Using Autologous Bone Marrow Mononuclear Cells in Patients with Lower Limb Lymphedema

introduction: Vascular endothelial growth factor-C (VEGF-C) is the primary lymphangiogenic factor that stimulates lymphangiogenesis by signaling via specific receptor, vascular endothelial growth factor receptor 3 (VEGFR3). This study was conducted to evaluate the change in the level of VEGF-C before and after autologous bone marrow mononuclear cell transplantation for treatment of Lower limb lymphedema. Patient and methods: Forty patients with lower limb lymphedema were divided into two groups. Group i included 20 patients with chronic lower limb lymphedema who underwent autologous bone marrow mononuclear cell transplantation. Group ii included 20 patients with chronic lower limb lymphedema who were exposed only to compression therapy as a control group. VEGF-C level in the diseased limbs was measured in both groups at the beginning of the study then 3 and 6 months respectively. Results: Group i included 20 patients, 8 patients were male (40%) and 12 patients were females (60%) with mean age 29.5 ± 12.15 while group ii included 20, 10 patients were male (50%) and 10 patients were females (50%) with mean age 39.5 ± 11.5. in group i, the specimens were taken at 3 and 6 months after transplantation showed a marked decrease in the VEGF-C level with statistically significant p value, 0.02 and 0.001 respectively. in group ii the level of VEGF-C after compression therapy alone at 3 and 6 months interval showed fluctuation with statistically non-significant p value, 0.64 and 0.55 respectively. Conclusion: VEGF-C is essential for regulation of lymphangiogenesis. The level of VEGF-C was found elevated in patients with lymphedema and decrease after autologous mononuclear bone marrow cells, however these results were statically non-significant.

Brand versus Generic Rosuvastatin in Egyptian Patients with Hyperlipidemia;Cost-Minimization Analysis

Background: Serum level of cholesterol is one of the most vital risk factors for cardiovascular diseases (CVD). Statins are highly effective drugs for reducing serum cholesterol;hence, preventing coronary heart disease (CHD). Rosuvastatin (Crestor) is one of the most potent and widely prescribed statins. Even though generic statins have been approved based on their bioequivalence with brand-name drugs, there remains considerable concern as regards their effectiveness and safety. Most clinicians and patients welcome the generic drug decreased costs;however, it is indispensable for them that effectiveness and safety are not compromised. Thus, the rationale intended for this study is to compare brand rosuvastatin and generic rosuvastatin as regard their economic impact using a cost-minimization analysis. Methods: This cost-minimization model estimates potential impact of rosuvastatin brand versus generic on the healthcare resource utilization for one-year frame from the payer perspective. The model conforms to real practice of management of hyperlipidemia in Egypt and was validated by experts. Results: The drug costs in the rosuvastatin brand group were 3,155,250 EGP while in the generic group were 2,299,030 EGP. The costs of CVD events in the rosuvastatin brand group were 5,863,558 EGP, while in the generic group were 6,810,180 EGP. The total costs in the rosuvastatin brand group were 9,018,808 EGP, while in the generic group were 9,109,210 EGP with a difference of −100,047 EGP. Conclusions: in conclusion, the real cost of generic treatment is more than that of the brand statin when taking into consideration the cardiovascular events.

Nonspecific shoulder pain is common presentation among elderly patients and patients with a history of cancer. The major concern of shoulder pain in cancer patients is the development of metastases. MRi of the shoulder is the most commonly used imaging modality for assessment of suspected soft tissue pathologies associated with the shoulder pain. in this study, we try to examine the role of shoulder MRi in elderly patients with a history of cancer presenting with nonspecific shoulder complaints for assessment of metastases.

Totally Endoscopic (Thoracoscopic and Laparoscopic) Radical Esophagectomy with Gastric Tube Reconstruction through a Small Neck incision: An Early Experience with Thirty Egyptian Patients

Background: Minimally invasive esophagectomy nowadays is replacing the classic open technique. Additional studies are needed to confirm its safety and efficacy. Methods: thirty patients with esophageal carcinoma were enrolled in this study. Patients were evaluated preoperatively and they underwent thoracoscopic and laparoscopic procedures for assessment of resectability. Resectablepatients underwent radical esophagectomy with gastric tube reconstruction through a four-cm neck incision. Results: 17 patients were operable and 13 patients were inoperable. The mean operative time for the whole procedure was 5.97 ± 1.66 hours. The mean blood loss was 250 ± 138.07 cc. The mean overall hospital stay was 17.47 ± 5.49 daysdays. Common postoperative complications included pneumonia (13.3%) pleural effusion (6.7%), cervical anastomotic leakage (10%), and wound infection (13.3%). One patient died in the early postoperative period. Conclusions: we conclude that totallyendoscopic (thoracoscopic and laparoscopic) esophagectomy is feasible and relatively safe technique. Beside its efficacy as an assessment tool, total esophagectomy and lymphadenectomy could be performed in the same time.

involvement of Male Youth into Accidents in Upper Egypt: Pattern and Risk Analysis

Background: Accidents pose a serious threat to health and economy in Egypt. Objectives: To identify and analyze the epidemiological factors associated with different types of accidents among adult males in Upper Egypt. Methods: A sample of 500 Egyptian males 20 - 28 years of age attending for health care at Quena University Hospital (QUH) or affiliated health institutions in South Egyptian governorates of Luxor and Red Sea was studied. The demographic, socioeconomic, and habitual data, as well as health status, medical history, and history of engagement into accidents for the recruits all were evaluated. Results: Accidents occurrences accounted up to 23/500 (4.6%) of the studied group. Road traffic accidents (RTAs) represented the highest percentage (30.4%), followed by street fights (21.7%) and animal bites (13.1%). Occupational accidents, electrical injuries, and near-drowning, all were equally encountered (8.7%), whereas falls and burns were least prevalent (4.3% each). Drug abuse, manual work, obesity, and having epilepsy were significant risk factors for accident injuries (OR = 6.35, 95% Ci: 1.64 - 24.58, OR = 3.36, 95% Ci: 1.22 - 9.73, OR = 4.81, 95% Ci: 1.50 - 15.46, OR = 45.33, 95% Ci: 3.94 - 518.01, respectively). Conclusions: Engagement of young Upper Egyptian males into accidents is alarming. Most of the accident mechanisms encountered are preventable. Enforcement of standard occupational, traffic and public safety laws is needed to minimize the burden of accidents upon the public health and economy in Upper Egypt.

Objective:Early treatment of acute ischemia of the heart by performing immediate percutaneous coronary intervention(PCi)to restore blood fl ow in patients with the clinical presentation of an acute coronary syndrome and more specifi cally with ST-elevation myocardial infarction(STEMi)can save *** study aims to identify the mean time(door to balloon time and fi rst contact to balloon time)to primary PCi for STEMi patients and to assess the percentage of primary PCi and its success rate in ***:A registry study of patients presenting to cardiac centers in Egypt was designed,where patients’basic characteristics,the treatment strategy,and the door to balloon time and the fi rst contact to balloon time were ***:One thousand six hundred fi fty STEMi patients with a mean age of 57 years were included in the *** transfer for primary PCi was the most used treatment strategy,representing 74.6%of all treatment strategies *** door to balloon time and the fi rst contact to balloon time were 50 and 60 minutes,respectively,with a primary PCi success rate of 65.1%.Conclusion:The registry study results showed a marked improvement by implementation of the best treatment strategy with respect to the time factor to achieve a better outcome for STEMi patients in Egypt.

Epilepsy is a chronic and the fourth most common neurological disorder which affects people of all age groups. Recently research and awareness on epilepsy-related deaths have rapidly grown over the past two decades. Many previous studies are attributed to the guidelines that apprise health care professionals in handling these deaths, but there is a relative scarcity of information accessible for clinicians and pharmacists who are responsible for manufacturing or preparing the extemporaneous anti-epileptic suspensions in the hospitals. Mostly in partial seizures, phenytoin is one of the first-choice drugs. In Saudi Arabian hospitals, the extemporaneous preparation of phenytoin suspension is common, but the hot climatic weather in Saudi Arabia possesses stability problems that should be tackled as the prepared suspension should pass all the stability tests to ensure uniform dosage of the extemporaneous formulation. In the current study, the commercial capsules were used to prepare the oral phenytoin sodium extemporaneous suspension. The physical, chemical and microbiological stability of phenytoin sodium suspension is analyzed at various temperatures.
