Background: Increasing awareness level and health education can reduce the burden of Chronic Kidney Disease (CKD) at community base. Therefore, the aim of the present study was to assess the level of knowledge towards CKD manifestations in Saudi Arabia. Methodology: This is a descriptive prospective study including 900 apparently health Saudi volunteers, their ages ranging from 18 to 65 years old with a mean age of 34 years. A purposeful questionnaire was deliberated and used for obtaining data about the CKD. Results: Out of 900 contributors, 268 (29.8%) were males and 632 (70.2%) were females, giving males:females ratio of 1.00:2.36. About 102/900 (11.3%) of the participants (all of them were females) were found to believe that CKD has no specific symptoms. Conclusion: The awareness towards CKD manifestation is relatively low, which necessitates the important of community based intervention including health educational programs.

The purpose of this article is to report that some anti-A1 that react at 37°C are IgG antibodies and are clinically significant, as they can cause the destruction of a proportion of A1 cells in vivo following the transfusion of red blood cells. Therefore, when a serum of an individual contains anti-A1, further testing of the plasma against group A1, A2, and O by referencing red blood cells and individual cells against anti-A1 lectin (Dolichos biflorus lectin) must be performed. Confirming the specificity of anti-A1 by referencing red blood cells is also important in selecting the appropriate blood for transfusion. ABO antibodies are naturally occurring and activate the complement cascade, making them more likely to cause severe transfusion reactions compared to antibodies to other RBC antigens.

Background and Objective: The liver is responsible for metabolism and detoxification of the most of components that enter the body. Once the liver became injured, its efficient treatment with famous chemical drugs was limited. Therefore, interest concerned the use of alternative medicines for the treatment of hepatic disease has been arisen. The present study was aimed to investigate the therapeutic effect of the two types of stem cells against chromosomal aberrations in bone marrow (BM) cells of rats treated with carbon tetrachloride (CCl4). Design and Method: BM-derived mesenchymal stem cells (MSCs) were isolated and propagated in culture for 2 weeks and were characterized morphologically. Human umbilical cord blood (UCB) cells were obtained after full-term caesarean delivery from healthy donors. Low-density mononuclear cells were separated over Ficoll-Paque, and CD34+ hematopoietic cells were isolated using a magnetic cell sorter. Rats were divided into 4 groups: control, CCl4, CCl4 plus MSC, and CCl4 plus CD34+. Liver tissue was examined histopathologically for all groups. Results: The results of the present study indicated a significant increase (p < 0.05) in the number of metaphases with different types of chromosomal aberrations in CCl4 group. Treatment of the animals with BM-MSCs and UCB-CD34+ cells improved both genotoxicity and histopathological changes induced by CCl4.

Patients Returning to Dialysis after Failed Kidney Transplant: How Do They Fare? A Gulf Perspective

An increasing number of failed transplant patients returning to dialysis (FTRD) have been observed with reported worse survival compared to transplant-naive dialysis (TxN) patients. This study aimed to assess outcomes of FTRD vs. matched TxN controls in a Gulf region multi-center trial of 800 HD patients. Similar mortality was seen, likely due to earlier start and better HD adequacy in FTRD. Younger age, less diabetes and living donor transplantation in majority with 27% graft nephrectomy (Nx) might also confer benefits. Subgroup analysis of Nx patients showed more hospitalizations and prior rejection episodes with lower graft survival. The deaths, however, oc-curred only in nonNx group and are likely explained by older age, longer duration on HD, more prevalence of diabetes and CAD. FTRD showed similar survival to TxN. Early intensive HD might account for the benefit. Whether Nx confers advantage is unclear because of the small sample size.

Objective:Despite resection with curative intent,a majority of patients with gastric cancer will develop disease *** adjuvant chemo-radiotherapy increase the curability of surgery,prevent local recurrence and improve ***:Between December 2005 and February 2010,33 patients were eligible for the study,17 patients were randomly assigned for chemo-radiotherapy (GI) and 16 patients with surgery alone (GII).Patients in GI received chemotherapy (fluorouracil,425 mg/m 2/day,and leucovorin,20 mg/m 2/day,for 5 days) was initiated on day 1 and was followed by chemo-radiotherapy beginning 28 days after the start of the initial cycle of ***-radiotherapy consisted of 4500 cGy of radiation at 180 cGy/day,five days/week for five weeks,with fluorouracil (400 mg/m 2/day) and leucovorin (20 mg/m 2/day) on the first four and the last four days of *** month after the completion of radiotherapy,two five-day cycles of fluorouracil (425 mg/m 2/day) plus leucovorin (20 mg/m 2/day) were given one month ***:Grade 3 gastrointestinal toxicity was more common (23.2%) while grade 3 hematological toxicity were (5.8%).Both 3-year survival (53%) and disease free survival (41%) rates were higher in GI than in GII in which they were 43.7% and 31% *** rate was higher in GII (56.3%) than in GI (35.3%).Conclusion:The present study revealed that chemo-radiotherapy after gastric resection in patients with gastric adenocarcinoma improves survival and relapse rates with manageable ***,studies with larger number of patients are recommended to confirm our results.

Background: In comparison to other forms of chronic liver diseases, cirrhosis is generally poorly studied in sub Saharan Africa. In Chad, more particularly, no data are available despite the burden of liver diseases considered as the first cause of hospitalizations in the country. Methods: We conducted a retrospective analysis of 268 patients with liver cirrhosis attending care at the University Reference Hospital between 2007 and 2016. Results: This series of liver cirrhoses was characterized by a weak mal predominance (M:F = 1.7). The age of onset occurs significantly earlier in women than in men (40.6 ± 12.0 vs. 44.4 ± 13.4, p = 0.0171). The principal risk factor was persistent infection with hepatitis B virus (49% of cases) followed distantly by infection with hepatitis C virus (13%) and excessive alcohol consumption (10%). Men were more frequently carrying HBV surface antigen than women (65.6% vs 35.9% p = 0.0019). HBV-associated liver cirrhosis was overall more severe than diseases from other causes. A large proportion of cirrhosis (30%), observed primarily in women (48.1% vs 24.1%, p = 0.0036), was considered are cryptogenic. Conclusions: The etiological spectrum of liver cirrhosis remains to be properly defined in Chad. This lack of knowledge prevents the implementation of an efficient policy of prevention. A significant effort should be secured to characterize hitherto neglected infectious, lifestyle or genetic risk factors responsible of this form of terminal disease and improve subsequently liver health of local populations.

Background: Neonatal sepsis is a global health problem that mainly affects low- and middle-income countries. We have previously shown that early neonatal mortality is high at the Ho Teaching Hospital (HTH) of Ghana. We sought to determine the prevalence of neonatal sepsis, sepsis-related mortality, and bacterial species patterns in neonatal and young infant sepsis in this hospital. Methods: A hospital-based study was conducted in the hospital’s neonatal intensive care unit (NICU) from March to June 2018. Blood samples from 96 babies clinically diagnosed with or at risk of sepsis were cultured using the BACTEC 9050® machine. Clinical data including gravida, parity and antibiotic medication before delivery of mother and delivery type, gestation, birth weight and antibiotic medication status were collected for analysis. MALDI-TOF MS identified bacterial isolates, and their identities were confirmed via tuf gene sequence typing. The data were analyzed using GraphPad Prism 8.0.2. Results: Blood cultures were positive in 28 of the babies, with 14 and 12 representing early-onset and late-onset neonatal sepsis, respectively, and two cases of unknown sepsis type. Institutional Review Board Compliance with National Regulatory Body: A Successful Experience from Saudi Arabia

Background: The Institutional Review Board (IRB) or Research Ethics Committees (RECs) is established to review the research proposals and ensure that participants’ ethical standards, scientific merit, and human rights are protected. Purpose: The authors report the experience of the REC at Qassim Region, Saudi Arabia over 10 years period. Methods: All proposals submitted to Qassim REC during the period 2008-2017 were studied using a 30 items data collection form based on The National Committee of Bioethics Regulations. Data extracted included;principal investigator characteristics, numbers of proposals reviewed, applications completeness, approval decision status, reported ethical issues, classification of the ethical review, and committee review duration. The structure, workload, and review process of Qassim REC were addressed redundant. Results: During 10 years, Qassim Research Ethics Committee (QREC) witnessed a progressive increase in the number of submitted proposals, from 9 to 149 proposals. Out of 508 submitted applications, 439 (86.4%) proposals were eligible for ethical review. Of these, 50 (11.4%) proposals were incomplete due to nonresponse of the principal investigators to the QREC comments. The final decision was made for 389 (88.6%) completed proposals. The approval rate was 85.4%, while the rejection rate was only 1.1%. The median time taken for ethical review was 13 days. Proposals that underwent full board review had a long review duration (Median: 19 days) in comparison to the expedited review (Median: 10 days). Incomplete Committee requirements, unclear research methodology, or possible ethical violation opportunities were the main reasons for delayed decisions. Conclusion: The workload of the Qassim ethics committee is high and growing progressively. However, the process indicators as per National Bioethics Committee rules were satisfactory. Background:There is a growing interest in postgraduate nursing education in Saudi Arabia,including the availability of Master’s and PhD degrees in ***,various specialized studies in nursing have emerged,such as the Master’s in emergency and disaster ***,only a limited number of studies have been conducted in Saudi Arabia to explore the factors that can have a positive or negative impact on a Master’s program from the students’***,no studies have investigated the motivation and barriers encountered by registered nurses when it comes to enrolling in a Master’s program in emergency and disaster nursing at Taif ***:A qualitative descriptive approach was employed for this study.A total of 12 postgraduate nurses who had successfully obtained a Master’s degree in emergency and disaster nursing from Taif University participated in the ***:Two themes emerged from the *** first theme centered around the motivation to enroll in the Master’s degree program,which encompassed the desire to improve knowledge and skills,enhance self-learning abilities,and achieve financial *** second theme focused on the barriers faced when enrolling in the Master’s program,which included challenging admission criteria and requirements,high program costs,and difficulties in obtaining ***:This study identified several motivations and barriers related to the enrollment in a Master’s program in emergency and disaster nursing at Taif ***,it is recommended that efforts be made to increase financial support and facilitate fellowships to support students in their pursuit of this program.
