The Diagnostic and Prognostic Value of Serum Procalcitonin among Ventilator Associated Pneumonia Patients

Ventilator-associated pneumonia (VAP) is a complication in as many as 28% of patients who receive mechanical ventilation. Studies have consistently shown that a delay in diagnosis and treatment increases the mortality risk. The aim of this work was to clarify the role of the serum procalcitonin (PCT) in the diagnosis and the prognosis of ventilator associated pneumonia. Methods: Forty two VAP patients, 20 non VAP-ICU (on mechanical ventilation) admitted patients and 20 healthy control subjects of similar age and sex were included in the study. PCT levels in serum samples were measured in all subjects. Results: There was a highly statistically significant difference (p value 0.001) between VAP patients on one side and non VAP-ICU patients and healthy control subjects on the other side regarding the mean values of PCT. Also, the mean values of PCT were statistically significantly higher (p 0.001) among died VAP group than the survivor VAP group. There was a statistically positive correlation (p = 0.449), CRIP (R = 0.403) and SOFA (R = 0.437)) and initial PCT serum levels. Conclusions: This study found that the increased PCT serum level is an important diagnostic tool for VAP and the PCT serum levels can predict the outcome of VAP patients. We recommend other larger studies to augment our findings.

Spatial Risk Analysis of Water Borne Diseases (Bovine Leptospirosis) in the Rive Nile State-Sudan

Geographical Information Science (GIS) technologies have been used increasingly for ecology and epidemiology of water-borne diseases, providing approach for animal health issues. This study was set up to investigate the geographical distribution of Bovine that was affected by Leptospira hardijo, in River Nile state, on October 2012. Locations of targeted cattle were delimited using GPS. Fifty three (53) of blood samples were collected, and screened in the laboratory for Leptospira hardijo specific antibodies using indirect elISA. 15.09% had evidence of infection as determined by the presence of anti-leptospira antibodies. It was inferred that no incidences were recorded in 45 locations out of the 53 selected locations in the state. Leptospirosis risk area for transmission was mapped using 5 km buffer distance. Animals’ movements routes were mapped with their contacts area and positive samples locations, hafair locations where animals contacts were mapped. This study demonstrated the value of GIS and GPS in disease mapping for animals’ health, and this might help veterinary authorities to implement strategic interventions for animal disease control.

Characterization of Egyptian Manganese Ores for Production of High Carbon Ferromanganese

This work aims at studying the reactivity of Egyptian manganese ores to be used in the production of ferromanganese alloys in submerged electric arc furnace. Ores with different manganese content (high-medium and low) were selected and characterized by X-Ray Fluorescence (XRF), X-Ray Diffraction (XRD) and Scanning electron Microscope (SEM). The main mineralogical compositions in the three ores are pyrolusite (MnO2) and hematite (Fe2O3). Porosity of selected Mn ores was determined. The reactivity of the different ores was carried out through pre-reduction of the selected ores using thermobalance at 900°C and 1100°C and mixture of CO and CO2 gases. The reduction process was done until steady weight. The reduced ores were examined using XRD and SEM. The results showed that pyrolusite in high and medium ores are converted completely to MnO at 1100°C. However, the ore with low manganese content was converted to MnO and Mn3O4. Consequently, it is clear from the results that Mn ores with high and medium MnO2 content are more reactive than those with low MnO2. Therefore, high MnO2 content Mn ores are preferable to get good economic impact during the production of high carbon ferromanganese.

ANFIS-based Sensor Fusion System of Sit-to-stand for elderly People Assistive Device Protocols

This paper describes the analysis and design of an assistive device for elderly people under development at the EgyptJapan University of Science and Technology（E-JUST） named E-JUST assistive device（EJAD）.Several experiments were carried out using a motion capture system（VICON） and inertial sensors to identify the human posture during the sit-to-stand *** EJAD uses only two inertial measurement units（IMUs） fused through an adaptive neuro-fuzzy inference systems（ANFIS） algorithm to imitate the real motion of the *** EJAD consists of two main parts,a robot arm and an active *** robot arm is a 2-degree-of-freedom（2-DOF） planar *** addition,a back support with a passive joint is used to support the patient s *** IMUs on the leg and trunk of the patient are used to compensate for and adapt to the EJAD system motion depending on the obtained patient *** ANFIS algorithm is used to train the fuzzy system that converts the IMUs signals to the right posture of the patient.A control scheme is proposed to control the system motion based on practical measurements taken from the experiments.A computer simulation showed a relatively good performance of the EJAD in assisting the patient.

3-Bromopyruvate(3BP) is a new, promising anticancer alkylating agent with several notable functions. In addition to inhibiting key glycolysis enzymes including hexokinase II and lactate dehydrogenase(LDH), 3BP also selectively inhibits mitochondrial oxidative phosphorylation, angiogenesis, and energy production in cancer cells. Moreover, 3BP induces hydrogen peroxide generation in cancer cells(oxidative stress effect) and competes with the LDH substrates pyruvate and lactate. There is only one published human clinical study showing that 3BP was effective in treating fibrolamellar hepatocellular carcinoma. LDH is a good measure for tumor evaluation and predicts the outcome of treatment better than the presence of a residual tumor mass. According to the Warburg effect, LDH is responsible for lactate synthesis, which facilitates cancer cell survival, progression, aggressiveness, metastasis, and angiogenesis. Lactate produced through LDH activity fuels aerobic cell populations inside tumors via metabolic symbiosis. In melanoma, the most deadly skin cancer, 3BP induced necrotic cell death in sensitive cells, whereas high glutathione(GSH) content made other melanoma cells resistant to 3BP. Concurrent use of a GSH depletor with 3BP killed resistant melanoma cells. Survival of melanoma patients was inversely associated with high serum LDH levels, which was reported to be highly predictive of melanoma treatment in randomized clinical trials. Here, we report a 28-year-old man presented with stage IV metastatic melanoma affecting the back, left pleura, and lung. The disease caused total destruction of the left lung and a high serum LDH level(4,283 U/L). After ethics committee approval and written patient consent, the patient received 3BP intravenous infusions(1-2.2 mg/kg), but the anticancer effect was minimal as indicated by a high serum LDH level. Azoniaallene salts as versatile building blocks in the synthesis of antibacterial and antifungal heterocyclic compounds

Substituted 2-azoniaallene salts 1 are strong bifunctional electrophiles, undergo cyclization reactions furnish many series of heterocyclic compounds, where reacted with p-tolyl urea, phenyl thiourea and thiosemicarbazone derivatives to afford triazinium salts, and converted to corresponding free bases 3, 5, 7 under treatment with Na2CO3. While triazole derivatives 8 and 9 were obtained by the reaction 2-azoniaallene salts 1 with benzohydrazide and phenyl hydrazine, respectively. Benzoxazinium salts 10 and 11 were acquired when asymmetric 2-azoniaallene salt reacted in (1:1) ratio with p-cresol and 3-methyl-1-phenyl-5-pyrazolone, respectively. The reaction of 2-azoniaallene salt with malononitrile furnished the primidinium salt 12 which underwent neutralization with Na2CO3 followed by heterocyclization with hydrazine hydrate afforded the bicyclic compound 3-aminopyrazolo[3,4-d]pyrimidine 14, which is highly reactive for nucleophilic addition to phenyl isothiocyanate to furnish thiourea derivative 15. Moreover, 14 undergo condensation with aldehydes to give imine derivatives 16a,b. All free base compounds were screened for their antimicrobial activities.

Contrast-Enhanced CT-Guided Core Biopsy of Retroperitoneal Masses

Objective: To evaluate the efficacy, feasibility and safety of contrast-enhanced CT-guided core biopsy of retroperitoneal masses. Materials and Methods: Contrast-enhanced CT-guided biopsy of retroperitoneal masses was performed in 26 patients. Histopathological diagnosis was obtained and accuracy was calculated. Two blinded radiologists determined feasibility of biopsy procedures on routine CT-guidance, and compared accessibility of target lesions using contrast-enhanced and non-contrast-enhanced CT. Results: Satisfactory samples were obtained in all 26 patients and pathological diagnosis was made in 24. Accuracy was 95.5%. Routine CT-guided biopsy punctures were not satisfaction in 4/26 patients, and contrast-enhanced CT scans rendered the target lesions more accessible in 16 patients. Conclusion: Contrast-enhanced CT-guided biopsy increases the feasibility of biopsy of retroperitoneal masses.

Sustainable Bio-Conversion of Rice Straw Waste into High Quality Organic Fertilizer

The aim of this study is to investigate the effect of different additives including biochar, effective micro-organisms (EM), animal manure and commercial microbial inoculants on the bioconversion of rice straw. Different compost piles were constructed, and each contained 50 kg of rice straw and mixture of natural rocks to enrich the compost nutritional value. The physical, chemical and biological parameters indicating the decomposition of organic material, maturation and quality of the organic fertilizer product were investigated during the composting process. A rapid increase in compost temperature was obtained in inoculated piles. All piles reached maturation after around 42 days. All analysis of the properties of the final compost products indicated that it was in the range of the matured level and can be used as organic fertilizer without limitation. The highest decomposition rate and highest organic fertilizer quality were obtained in the pile inoculated with EM and 10% biochar compared to other treatments.

Predictive value of serum lactate dehydrogenase in diagnosis of septic shock in critical pediatric patients:A cross-sectional study

Objectives:To determine the predictive value of lactate dehydrogenase(LDH)in the diagnosis of septic shock and its association with other prognostic scores in critical pediatric ***:A cross-sectional study was performed at Children’s Hospital of Cairo University between June 2019 and December 2019.A total of 200 pediatric patients were divided into the septic shock group[100 critically ill patients with septic shock from the pediatric intensive care unit(PICU)]and the control group(100 patients with only sepsis).LDH was determined in the first 24 hours of *** sensitivity and specificity of LDH in diagnosis of septic shock were assessed;the levels of related indicators of patients with different etiologies were compared;correlations between LDH,Paediatric Index of MortalityⅡ,and Pediatric Sequential Organ Failure Assessment(pSOFA)were ***:Median LDH was 512μL(406.50-663.00)in the septic shock group and was significantly higher than that[190μL(160.00-264.50)]in the control group(Pelation was found between pSOFA and LDH(r=0.503,P<0.001).Conclusions:LDH could be a potential inflammatory marker in the diagnosis of septic shock and is valuable for PICU admission decisions.

Nutritional Quality of Biscuit Supplemented with Wheat Bran and Date Palm Fruits (<i>Phoenix dactylifera</i>L.)

Fruits of date palm (Phoenix dactylifera L.) are consumed throughout the world and are a vital component of the diet in most Arabian countries. The effect of partial replacement of wheat flour with a 1:1 mixture of wheat bran and date palm fruit powder on rheological properties of the biscuit dough was studied. Levels of mixture used were 10%, 20%, 30% and 40%. Dough rheology (farinograph, extensograph properties) and biscuit quality (physical properties, colour, sensory evaluation) were assessed. Results obtained indicated that water absorption gradually increased by increasing the levels of wheat bran: palm date powder, meanwhile mixing tolerance index decreased. Dough stability, which indicates the dough strength, also found to decrease by increasing mixture of wheat bran and date powder (1:1). Furthermore, dough development time gradually increased by increasing the mixture at all levels. It was 6.04 min at level of 40% compared to the control (2.5 min). Extensograph results showed that dough energy and dough resistance to extension and proportional number also increased compared with control sample. The proportional number (R/E) ratios increased largely from 3.20 to 5.27 at the level of 40.0%. Ash and fibre contents gradually increased as the mixture levels increased. However, mineral content progressively increased by increasing the level of date powder in the mixture. Incorporation of wheat bran and date powder mixture decreased the spread of the biscuits from 55.66 to 52.82 mm without change in the thickness of the biscuits. Organoleptic properties revealed that quality of biscuits was acceptable at mixture level of 30%.
