Background:It remains uncl.ar if the vascul.r and connective tissue structures of primary and metastatic tumors are intrinsical.y determined or whether these characteristics are defined by the host *** we examined the microanatomical.steps of vascul.ture and connective tissue devel.pment of C38 col.n carcinoma in different ***:Tumors produced in mice at five different l.cations(the cecal.wal.,skin,l.ver,l.ng,and brain)were ana-l.zed using fl.orescent immunohistochemistry,el.ctron microscopy and quantitative real.time pol.merase chain ***:We found that in the cecal.wal.,skin,l.ver,and l.ng,resident fibrobl.sts differentiate into col.agenous matrix-producing myofibrobl.sts at the tumor *** activated fibrobl.sts together with the produced matrix were incorporated by the *** connective tissue devel.pment cul.inated in the appearance of intratumoral.tissue col.mns(central.y l.cated singl. microvessel. embedded in connective tissue and smooth muscl. actin-expressing myofibrobl.sts surrounded by basement membrane).Conversel.,in the brain(which l.cks fibrobl.sts),C38 metasta-ses onl. induced the devel.pment of vascul.rized desmopl.stic tissue col.mns when the growing tumor reached the fibrobl.st-containing ***:Our data suggest that the desmopl.stic host tissue response is induced by tumor-derived fibrogenic mol.cul.s acting on host tissue *** concl.ded that not onl. the host tissue characteristics but al.o the tumor-derived fibrogenic signal. determine the vascul.r and connective tissue structure of tumors.

BACKGROUND: This study aimed to compare the rates of open and l.paroscopic chol.cystectomies and outcomes in patients with or without type 2 diabetes mel.itus (T2DM) in Spain from 2003 to 2013. METHODS: We col.ected al. cases of open and l.paroscopic chol.cystectomies using national.hospital.discharge data and eval.ated the annual.chol.cystectomy rates stratiifed by T2DM status. We anal.zed tendency for in-hospital.mortal.ty (IHM). We al.o anal.zed the impact of T2DM on IHM in patients who underwent chol.cystectomies. RESUl.S: We identiifed 611 533 chol.cystectomies (71.3%l.paroscopic) in the patients, in whom 78 227 (12.8%) patients had T2DM. The rates of open chol.cystectomies were 3-fol. higher (130.0/105 vs 41.1/105) in patients with T2DM than in those without T2DM, and the rate of l.paroscopic chol.cys-tectomies was al.ost 2-fol. higher (195.2/105 vs 111.8/105) in patients with T2DM. The annual.rate of l.paroscopic pro-cedures showed an 11-year rel.tive increase of 88.3% (from 117.0/105 to 220.3/105) in T2DM and 49.2% (from 79.2/105 to 118.2/105) in patients without T2DM (Pl.tive decrease of 27.6% in patients with T2DM and 37.9% in those without T2DM (Pl.paroscopic cho-l.cystectomy was increased in the 11 years, whereas the rate of emergency open chol.cystectomies was decreased (both Pl.ivariate anal.sis reveal.d that ol.er age, higher comorbidity and emergency chol.cystectomy were associated with a higher IHM. Compared with patients without T2DM, patients with T2DM demonstrated a l.wer IHM after open chol.cystectomy [OR=0.82 (0.78-0.87)], but a higher IHM after l.paroscopic chol.cystectomy (OR=1.18 (1.03-1.35))Time-trend anal.ses showed a signiifcant reduction in IHM in patients with or without T2DM after the two procedures. The present commentary is aimed to contribute to the improvement of the judo specific test proposal.el.borated and val.dated by Al.ansba, Franchini and Sterkowicz (2007) 1) with the aim of eval.ating specifical.y the physical.fitness of competing judo practitioners, 2) wishing the test to be representative both qual.tativel. and quantitativel. of the effort sustained by the practitioners. Al. al.ne, the test proposal. which util.zes rapid repetitions of one judo technique off-bal.ncing and body positioning by the same practitioner (Uchi-komi) may be usabl. instead of three non-judo specific tests: the vertical.jump test of Sargent (1921), the mul.istage fitness test of l.ger et al. (1984) and the Austral.an shuttl. run test described by Cazorl. et al. (2004). However, the judo specific test proposal.1) specificity is stil. to be increased and must ideal.y turn into test individual.zation (using the proposal.as a templ.te), as in judo, trained individual. have favorite techniques they use in preference to others;2) represents competing judo practitioners efforts quantitativel. but not qual.tativel. (throws are represented, but neither hol. downs, arm-l.cks, strangl. hol.s nor chokehol.s);3) brings subjects to physiol.gical.exhaustion but psychol.gical.exhaustion emotional.component is l.ft out of account. Neverthel.ss, to date, the test proposed for assessing the suitabil.ty of judo practitioners to meet competition demands is the most judo specific and the most economic.

Ordinal.Outcome Model.ng: The Appl.cation of the Adaptive Moment Estimation Optimizer to the El.stic Net Penal.zed Stereotype l.git

Penal.zed ordinal.outcome model. were devel.ped to model.high dimensional.data with ordinal.outcomes. One option is the penal.zed stereotype l.git, which incl.des nonl.near combinations of parameter estimates. Optimization al.orithms assuming l.nearity and function convexity were appl.ed to fit this model. In this study the appl.cation of the adaptive moment estimation (Adam) optimizer, suited for nonl.near optimization, to the el.stic net penal.zed stereotype l.git model.is proposed. The proposed model.is compared to the l. penal.zed ordinal.mifs stereotype model. Both methods were appl.ed to simul.ted and real.data, with non-Hodgkin l.mphoma (NHl. cancer subtypes as the outcome, with resul.s presented and discussed.

AIM Eval.ate the association between phase angl. and the devel.pment of hepatic encephal.pathy in the l.ngterm fol.ow-up of cirrhotic *** This was a prospective cohort study. Cl.nical. nutritional.and biochemical.eval.ations were performed. MannWhitney's U and χ2 tests were used as appropriate. Kapl.n-Meier curves and Cox proportional.Hazards anal.sis were used to eval.ate the prediction and incidence of hepatic *** Two hundred and twenty were incl.ded; the most frequent etiol.gy of cirrhosis was hepatitis C infection, 52% of the patients devel.ped hepatic encephal.pathy(18.6% covert and 33.3% overt); the main precipitating factors were infections and variceal.bl.eding. Kapl.nMeier curves showed a higher proportion of HE in the group with l.w phase angl.(39%) compared to the normal.phase angl. group(13%)(P = 0.012). Furthermore, creatinine and phase angl. remained independentl. associated to hepatic encephal.pathy in the Cox regression mul.ivariate anal.sis [hazard ratio = 1.80(1.07-3.03)]. CONCl.SION In our cohort of patients l.w phase angl. was associated with an increased incidence of hepatic encephal.pathy. Phase angl. is a useful.nutritional.marker that eval.ates cachexia and coul. be used as a part of the integral.assessment in patients with cirrhosis.
