Spinal accessory neuropathy in patients with chronic neck pain

AIM: To assess the presence of spinal accessory neuropathy in patients with chronic neck ***: Patients with pain either regional or focal in the neck or shoulders for at least 6 mo(chronic neck pain) were recruited randomly from the Rheumatology and Rehabilitation Outpatient Clinic at the Faculty of Medicine-Suez Canal University. Two groups were compared: 30 patients with chronic neck pain with mean age(36.97 ± 12.45 years) and 10 apparently healthy controls. Trapezius muscle examination including inspection and range of motion both active and passive was performed. A full clinical neurological examination was carried out to exclude peripheral neuropathy and motor neuron disease. According to the subject's type of work, cases were categorized into labor-intensive and non-labor intensive tasks. A nerve conduction study(NCS) was performed on spinal accessory nerves at both sides for all patients and controls. Parameters including latencies and amplitudes of compound motor action potential(CMAP) were compared with the chronicity of neck pain using the neck disability score. This cross sectional study was carried in the Rheumatology and Rehabilitation Department, at Suez Canal University Hospital, Ismailia, ***: Physical examination revealed that 80% of cases had spinal trapezius muscle spasm. Restricted neck motion was present in 16.6% of cases. No one suffered from muscle wasting or weakness. Pain was bilateral in 18 patients(60%), localized to the right side in six patients(20%) and localized to the left side in six patients(20%). The causes of neck pain in the patients studied were nonspecific, due to physical stresses, cervical spondylosis and mild cervical disc herniation. Mean disease duration in patients with labor-intensive tasks was(3.9 ± 2.1 years), which was longer than that in patients with non-labor intensive tasks(3.1 ± 1.9 years); however, this difference was statistically insignificant. This work was undertaken to establish a new experimental model of hepatic fibrosis by gamma irradiation and CCl4 and to study the hepatoprotective effect of Reishi Mushroom (RM) against hepatic fibrosis induced in that model. Our results revealed that oral co-administration of 110 mg/kg RM by gavage to fibrotic rats offered an obvious hepatic protection as assured by the significant decrement in ALT and AST, HP content, MDA and NO levels with elevation of the antioxidant enzymes activities. The levels of TGF-β, TNF-α, HO-1 and type-1 collagen and their m-RNA expression were markedly declined as compared with those of fibrotic rats. Microscopical examination revealed that the exposure of rats to radiation aggravated the effect of CCl4 causing extensive collagen deposition and marked pseudolobulation of the hepatic parenchyma indicative of bridging fibrosis. While, oral co-administration of RM obviously improved the state of steatosis and apparently suppressed hepatic fibrogenesis.

Background： Drug-infection interaction should be considered in drug prescribing, particularly if potentiated byco-occuring drug-drug interactions. The effects of Candida and e-coli infections separately, and fluconazole administration weretested on cyclosporine blood level in rabbits. Methods： Three study designs were carried out, crossoversingle-dose-fluconazole-cyclosporine study testing fluconazole-cyclosporine interaction, multi-dosecandida-fluconazole-cyclosporine and multi-dose escherichia coli-cyclosporine study designs involving each rabbit acting as itscontrol; cyclosporine was given daily in both studies. Candida and e-coli infections were inoculated on day 5. In theMulti-Candida-Fluc-CyA, fluconazole-cyclosporine interaction was also considered, and flueonazole was administered daily fromday 9-18. In all three studies, Cyclosporine trough levels and serum creatinine were measured by CMIA and enzymatic assayrespectively, pre, during, post-infection and after fluconazole administration in the Candida study. Results： Both infections resulted insignificant rise in cyclosporine trough level （p = 0.018） and （p = 0.005） in fungal and bacterial studies, respectively. The median risein CyA level reached 52% （range 9-426%） in the Candida infection and reached 60 - 47.5% （mean） with the e-coli *** also increased mean cyclosporine trough levels in the single-dose study by 70.3 - 45.3 and the concomitant rise inthemulti-dose study reached 76% （median, range 22-665%）. Conclusions： Cyclosporine trough levels increased during Candida ande-coli infections and also during fluconazole administration in the single and multi-dose studies. Fluconazole exerted an additiveeffect to the Candida inhibitory effect. Type of infection and inoculum size affected CyA levels differently. Monitoring cyclosporinelevel during episodes of infection as well as in therapy regimen involving interacting drugs is advisable.

Incremental value of magnetic resonance neurography of Lumbosacral plexus over non-contributory lumbar spine magnetic resonance imaging in radiculopathy: A prospective study

AIM: To test the incremental value of 3T magnetic resonance neurography(MRN) in a series of unilateral radiculopathy patients with non-contributory magnetic resonance imaging(MRI).METHODS: Ten subjects(3 men,7 women; mean age54 year and range 22-74 year) with unilateral lumbar radiculopathy and with previous non-contributory lumbar spine MRI underwent lumbosacral(LS) plexus MRN over a period of one year. Lumbar spine MRI performed as part of the MRN LS protocol as well as bilateral L4-S1 nerves,sciatic,femoral and lateral femoral cutaneous nerves were evaluated in each subject for neuropathy findings on both anatomic(nerve signal,course and caliber alterations) and diffusion tensor imaging(DTI)tensor maps(nerve signal and caliber alterations).Minimum fractional anisotropy(FA) and mean apparent diffusion coeffcient(ADC) of L4-S2 nerve roots,sciatic and femoral nerves were ***: All anatomic studies and 80% of DTI imaging received a good-excellent imaging quality grading. In a blinded evaluation,all 10 examinations demonstrated neural and/or neuromuscular abnormality corresponding to the site of radiculopathy. A number of contributory neuropathy findings including double crush syndrome were observed. On DTI tensor maps,nerve signal and caliber alterations were more conspicuous. Although individual differences were observed among neuropathic appearing nerve(lower FA and increased ADC) as compared to its contralateral counterpart,there were no significant mean differences on statistical comparison of LS plexus nerves,femoral and sciatic nerves(P > 0.05).CONCLUSION: MRN of LS plexus is useful modality for the evaluation of patients with non-contributory MRI of lumbar spine as it can incrementally delineate the etiology and provide direct objective and non-invasive evidence of neuromuscular pathology.

The Impact of the FreeStyle Libre^TM Flash Glucose Monitoring System on Glycemic Control in Patients with Diabetes;Observational Multicenter 15-Months Study

Objectives: The study was to determine the impact of using the FreeStyle LibreTM flash glucose monitoring system on glycemic control and the rate of events due to diabetes in people with diabetes from different types and age groups. Methods: a retrospective cohort chart review study was carried out at three centers in the Taif region in the Kingdom of Saudi Arabia: The study was approved by an accredited centralized institutional review board. Paper or electronic medical records were included for individuals of any age with diabetes (type 1, type 2, gestational diabetes) managed with diet, insulin therapy, or/and oral antihyperglycemic medication and/or non-insulin injection therapy. The primary outcome measure was the laboratory HbA1c level as well as reduction. Secondary outcome measures were frequency of severe hypoglycemia, admission to hospital or ER visit related to diabetes complications, and severe hyperglycemia (DKA or HHS). Results: Data was analyzed from 1695 patients. The average HbA1c before using the flash glucose monitoring system was 9.60% ± 1.44% and 3 months HbA1c after using the FreeStyle LibreTM flash glucose monitoring system was 8.70% + 1.45% for a difference of -0.90% ([95% CI -0.92: -0.88];p 65 years, (p-values Conclusion: The benefits of using the FreeStyle LibreTM flash glucose monitoring system are self-evident in reducing HbA1c and events due to hyperglycemia or hypoglycemia.

AIM To assess the feasibility and parental acceptance of diagnostic microlaryngobronchoscopy(MLB) as day case *** A prospective study was performed over a 26 moperiod at a tertiary paediatric ENT centre. Patients were selected in clinic using set criteria. All MLBs were performed using a standardised anaesthetic protocol and patients monitored post-operatively. Six weeks following surgery, parents underwent questionnaire *** Ninety-four out of 101 MLBs was successfully performed as day case surgery over the set period. Seven patients required an overnight stay for further observation. Fiftyseven parents took part in the questionnaire of which 68.4% were highly satisfied with same day discharge. CONCLUSION MLB is feasible, safe and acceptable as day case surgery in carefully selected patients.

Evaluation of Pear Varieties for Physiochemical Traits Grown under Climatic Conditions of Soon Valley, Pakistan

The study carried out aimed at characterizing the pear cultivars and to explore the specific cultivar most suitable to be commercially grown in Soon valley region. Nine pear varieties (Leconte, Bagugosha, Bartlet pear, Concord, Pear selection-1, Pear Red, Pear White, Kashmiri Nashpati and Kashmiri Nakh) were tested for their physiochemical quality attributes. The observations made during the study revealed that variety Bagugosha scored maximum (7.95, 8.10) in taste and flavor respectively. The same variety produced maximum fruit size (5081 mm2), fruit weight (205 g), soluble solid contents (15%) and total sugars (9.56%). However, fruit yield per plant was maximum (98.80 kg) in Bartlet pear variety and Bagugosha produced fruitweight of 60.20 kg. Titratable acidity was determined maximum (0.44%) in Kashmiri Nakh. Number of seeds were maximum (8.20) in Bartlet Pear as well as in Bgugosha. The exploration of this research study revealed that the Bagugosha is the prime quality cultivar to be suggested for commercial cultivation in Soon valley compared to all nine evaluated varieties.

Bolhasan forest region with 5725 hectare area lies in east north of the *** region belongs to natural forests in *** regard of the fact that the area is mainly arrested by valuable species of Amygdalus Scopartia,it is imperative to consider principled exploitation,development and revival *** identify ecological needs to provide questionnaire Delphi method were 64 and natural resources experts and university *** this study,37 criteria defined 27 criteria were selected as the definitive *** evaluation of the conditions in the region was fulfilled by weighing the criteria with Analysis Hierarchy Process（AHP） in the Expert Choice soft *** this research,evaluation of the criteria,by AHP and geographic data setting was followed by display of ecological power status by means of such method and transfer of the inputs on the *** of the research indicate that about 599 hectare [35.59 percent] of Bolhasan forest region is in high,471 hectare [27.97 percent] in good and 614 hectare [26.44 percent] in weak situation.

Mosul Dam is located on the River Tigris about 60 km northwest Mosul in Iraq. It is the biggest dam where its storage capacity reaches 11.11 billion cubic meters at normal operational level (330 m. above sea level). The dam was constructed on alternating beds of karistified limestone, gypsum and marl. This dam suffered from water seepage under its foundation since its operation in 1986. Grouting operations were implemented since that time to overcome this problem. This seepage is believed to be due to dissolution of gypsum beds under the foundation, which was not carefully considered by the designers. It was recommended by the international board of experts that the water level should be kept at or below 319 m.a.s.l. to minimize damages in case of the failure of the dam. ISIS occupied the dam site on 8 August 2014 and it was seized back from the hands of ISIS on the 16th of the same month. They did plenty of damage despite the short period they occupied the area. After that, the Iraqi Ministry of Water Resources rebuilt the damaged parts and used new grouting and maintenance program. Now, the dam looks very safe at 319 m water level at its reservoir. In addition, the impounding was raised 325 m.a.s.l. for few days and nothing abnormal was noticed.
