Background Wilms tumor (WT) is the most common childhood kidney cancer worldwide, yet its incidence and clinical behavior vary according to race and access to adequate healthcare resources. To guide and streamline therapy in the war-torn and resource-constrained city of Baghdad, Iraq, we conducted a first-ever molecular analysis of 20 WT specimens to characterize the biological features of this lethal disease within this challenged population. Methods Next-generation sequencing of ten target genes associated with WT development and treatment resistance (WT1, CTNNB1, WTX, IGF2, CITED1, sIX2, p53, N-MYC, CRABP2, and TOP2A) was completed. Immunohistochemistry was performed for 6 marker proteins of WT (WT1, CTNNB1, NCAM, CITED1, sIX2, and p53). Patient outcomes were compiled. Results Mutations were detected in previously described WT 'hot spots' (e.g., WT1 and CTNNB1) as well as novel loci that may be unique to the Iraqi population. Immunohistochemistry showed expression domains most typical of blastemal-predominant WT. Remarkably, despite the challenges facing families and care providers, only one child, with combined WT1 and CTNNB1 mutations, was confirmed dead from disease. Median clinical follow-up was 40.5 months (range 6–78 months). Conclusions These data suggest that WT biology within a population of Iraqi children manifests features both similar to and unique from disease variants in other regions of the world. These observations will help to risk stratify WT patients living in this difficult environment to more or less intensive therapies and to focus treatment on cell-specific targets.

Purpose: To find out the incidence of intraocular lens (IOL)-optic opacificat ion in a suspect IOL. Methods: Retrospective review of patients’notes 3-4 year s postimplant. Results: In all, 56 IOLs showed signs of opacification, resulting in reduced vision out of 181 lenses traced. Conclusion: Certain hydrophilic acr ylic IOLs like the model in question (sC60B-0UV) show opacification of the IOL, which may impair the vision of pseudophakic patients. However, not all lenses o f this variety exhibit this cloudiness. Factors affecting this phenomenon are no t yet completely clear but may include changes in the UV absorbant material inco rporated in the lens substance itself. We report a series of patients who had re ceived this lens and experienced loss of vision due to opacification of the IOL. In a follow-up ranging from 1 to 4 years postoperatively, a higher incidence o f postoperative opacification of this lens emerges, after comparison with existing literature. IOL exchange in such patients is the only option. However, this mode of treatment should not be offered lightly as results may be guarded at best even in the face of a technica lly satisfactory procedure.

Cystic fibrosis(CF)as the commonest inherited disorder in Caucasians can affect around 1 in every 2,500 live *** is due to the CF transmembrane conductance regulator(CFTR)dysfunction,located mainly at the apical surfaces of epithelial cells in the airways,pancreas,intestine,kidneys,sweat gland,and male reproductive tract,in which CFTR is vital for the transepithelial fluid *** malfunction may result in life-threatening disorders[1].The digestive problems are resulted in the forming abnormally thick mucus because of failure in chloride and water *** the gastrointestinal tract,such thick and sticky mucus can clog the ducts preventing digestive enzymes to enter the *** insufficiency remains a big concern in CF patients and it accrues owing to the CFTR mutation which leads to the production of abnormally thick and sticky mucus affecting the food digestion and absorption,especially,fat and fat-soluble vitamins,and prohibiting digestive enzymes to be entered the ***,foods are not digested or absorbed in good order,resulting in mal-digestion and *** complications are consist of exocrine pancreatic dysfunction,intestinal inflammation,enteric circular muscle impairment,bacterial overgrowth,and impaired bicarbonate and bile acid release[2].

Background: There are associations between the severity of periodontal diseases and some predisposing factors, such as age, race, and income rate. Therefore, the present study was designed to evaluate the extra-oral digital panoramic radiographic interpretation and clinical findings of stage IV periodontitis among some samples concerning saudi/non-saudi nationality. Material and Methods: The study comprised a random sample of 298 residents in saudi Arabia referred to the internship program clinics at the College of Dentistry, King Khalid University. They were according to the age of participants as follows: youth (15 - 24 years), adults (25 - 64 years), and seniors (65 years and over) within two groups according to nationality (saudi and non-saudi). Clinical periodontal examination and radiographic extra oral digital panoramic radiographs were done, and then the periodontal parameters, the percentage of bone loss, and the pattern of bone loss were recorded. The data was collected, and the statistical analysis was performed with ANOVA test, Tukey’s test, Chi-square test, and T-test and considered a P-value at (p Results: The samples included 62 (20%) youth participants with mean of age (21.806 ± 3.390), 182 (57%) adults’ participants with mean age (42.802 ± 9.094), and 54 (23%) seniors participants with a mean age (69.074 ± 6.810). There were highly statistically significant differences in the comparison between the participants’ ages (P 0.05). Conclusion: At the end of this study, we concluded that there are alterations in clinical and radiographic and clinical findings of stage IV periodontitis with the alteration of participants’ ages and nationality.

Background. Autoimmune bullous diseases (ABDs) are a rare but significant grou p of dermatoses that pose great challenges to the treating dermatologist. Most e pidemiological studies have focused on a single ABD. Few surveys have been carri ed out to describe the whole spectrum of ABDs in a region, and no such studies a re available from the Arabian Peninsula. Objectives. To determine the clinico-e pidemiological features of various ABDs in Kuwait, and to compare the results wi th those reported elsewhere. Methods. A total of 128 cases of ABDs were studied over a span of 11.5 years. The diagnosis in all cases was confirmed by histopath ology, and direct and indirect immunofluorescence (IMF). The diagnosis of variou s subepidermal ABDs was further confirmed by indirect IMF on salt-split skin (s ss) and that of pemphigus by desmoglein 1and3enzyme-linked immunosorbent assay (ELIsA). Results. Eighty seven per cent of patients were of Arab ethnicity. Pemp higus was observed to be the commonest ABD (47%), followed by pemphigoid (22%) , pemphigoid gestationis (PG)(19%), linear IgA bullous disease (LABD) (7%), li chen planus pemphigoides (LPP) (3%), and epidermolysis bullosa acquisita (EBA) (2.3%). The minimum estimated incidence in the local population was 4.6, 2.14, 1.83, 0.69, 0.30, and 0.23 cases per million per year, respectively. Pemphigus p atients were observed to have a younger age of onset (36.50 ±11.36 years)than r eported elsewhere. BP, although the second commonest ABD, was less prevalent tha n in Europe and singapore, and BP patients were observed to have a striking fema le predominance (85%). The prevalence of PG was much higher than that reported elsewhere. LABD was the fourth commonest ABD, and 89%of patients were children. Conclusions. The study suggests that similar surveys from different regions wou ld expand our understanding of ABD.
