Prevalence and factors of work-related musculoskeletal disorders among hand surgeons

BACKGROUND The nature of tasks required by hand surgeons require both forceful and repetitive maneuvers,thus subjecting these surgeons to the risk of musculoskeletal(mSK)injuries during their years in practice.AIm To assess the prevalence,characteristics and impact of mSK disorders among hand surgeons.mETHODS A modified version of the physical discomfort survey was sent to surgeons who were members of the American Society for Surgery of the Hand via e-mail.The collected data were analyzed using descriptive statistics,one-way analysis of variance,and Fisher’s exact test.P values ofmSK injury,of which the most common diagnoses were lateral elbow epicondylitis(18.7%),low back pain(17.1%)and carpal tunnel syndrome(15.6%).Among those that reported an injury,73.1%required treatment and 29.2%needed time off work as a direct result of their injury.The number of work-related injuries incurred by a surgeon increased significantly with increasing age(PmSK injuries sustained by Hand surgeons with a high incidence.These results should increase awareness on this aspect and fuel future studies directed at preventing these types of work-related injuries,thus minimizing the financial and psychological burden on these surgeons and the healthcare system.

Detecting hate speech automatically in social media forensics has emerged as a highly challenging task due tothe complex nature of language used in such platforms. Currently, several methods exist for classifying hatespeech, but they still suffer from ambiguity when differentiating between hateful and offensive content and theyalso lack accuracy. The work suggested in this paper uses a combination of the Whale Optimization Algorithm(WOA) and Particle Swarm Optimization (PSO) to adjust the weights of two multi-Layer Perceptron (mLPs)for neutrosophic sets classification. During the training process of the mLP, the WOA is employed to exploreand determine the optimal set of weights. The PSO algorithm adjusts the weights to optimize the performanceof the mLP as fine-tuning. Additionally, in this approach, two separate mLP models are employed. One mLPis dedicated to predicting degrees of truth membership, while the other mLP focuses on predicting degrees offalse membership. The difference between these memberships quantifies uncertainty, indicating the degree ofindeterminacy in predictions. The experimental results indicate the superior performance of our model comparedto previous work when evaluated on the Davidson dataset.

BACKGROUND Psychological assessment after intensive care unit(ICU)discharge is increasingly used to assess patients'cognitive and psychological well-being.However,few studies have examined those who recovered from coronavirus disease 2019(COVID-19).There is a paucity of data from the middle East assessing the post-ICU discharge mental health status of patients who had COVID-19.AIm To evaluate anxiety and depression among patients who had severe COVID-19.mETHODS This is a prospective single-center follow-up questionnaire-based study of adults who were admitted to the ICU or under ICU consultation for>24 h for COVID-19.Eligible patients were contacted via telephone.The patient’s anxiety and depression six months after ICU discharge were assessed using the Hospital Anxiety and Depression Scale(HADS).The primary outcome was the mean HADS score.The secondary outcomes were risk factors of anxiety and/or depression.RESULTS Patients who were admitted to the ICU because of COVID-19 were screened(n=518).Of these,48 completed the questionnaires.The mean age was 56.3±17.2 years.Thirty patients(62.5%)were male.The main comorbidities were endocrine(n=24,50%)and cardiovascular(n=21,43.8%)diseases.The mean overall HADS score for anxiety and depression at 6 months post-ICU discharge was 11.4(SD±8.5).A HADS score of>7 for anxiety and depression was detected in 15 patients(30%)and 18 patients(36%),respectively.Results from the multivariable ordered logistic regression demonstrated that vasopressor use was associated with the development of anxiety and depression[odds ratio(OR)39.06,95% confidence interval:1.309-1165.8;Pmonths after ICU discharge,30% of patients who had COVID-19 demonstrated a HADS score that confirmed anxiety and depression.To compare the psychological status of patients following an ICU admission(with vs without COVID-19),further studies are warranted.

BACKGROUND Orthopedic oncology surgeons commonly engage in prolonged and complex surgical procedures.These types of surgeries increase the risk of physical and psychological stressors,which may in turn make these physicians prone to workrelated occupational injuries.AIm The aim of this study was to explore in orthopedic oncologists,the prevalence of work-related physical injuries and psychological disturbances.mETHODS A modified version of the physical discomfort survey was developed to assess occupational injuries among orthopedic oncology surgeon members of the musculoskeletal Tumor Society,the Canadian Orthopedic Oncology Society and European musculoskeletal Oncology Societies.The survey was sent by email,and it explored musculoskeletal complaints,psychological disturbances,treatment required for these complaints and the requirement of time off work.RESULTS A total of 67 surgeon responses were collected.A high number of orthopedic oncologists(84%)reported an occupational injury.Low back pain(39%)was the most prevalent musculoskeletal condition,followed by lumbar disk herniation(16%),shoulder tendinitis(15%)and lateral epicondylitis(13%).Of the cohort,46%required surgery and 31%required time off work due to their injury.Thirty-three respondents reported a psychological disorder.Burnout(27%),anxiety(20%)and insomnia(20%)were the most commonly reported.Time required off work due to injury was associated with old age and years in practice.CONCLUSION Orthopedic oncology surgeons report a high prevalence of work-related disorders.Lower back related injury and burnout were the most reported disorders.Improving operative room ergonomics and prevention of stress related to the work environment should be areas to explore in upcoming research.

The increased adoption of Internet of medical Things (IomT) technologies has resulted in the widespread use ofBody Area Networks (BANs) in medical and non-medical domains. However, the performance of IEEE 802.15.4-based BANs is impacted by challenges related to heterogeneous data traffic requirements among nodes, includingcontention during finite backoff periods, association delays, and traffic channel access through clear channelassessment (CCA) algorithms. These challenges lead to increased packet collisions, queuing delays, retransmissions,and the neglect of critical traffic, thereby hindering performance indicators such as throughput, packet deliveryratio, packet drop rate, and packet delay. Therefore, we propose Dynamic Next Backoff Period and Clear ChannelAssessment (DNBP-CCA) schemes to address these issues. The DNBP-CCA schemes leverage a combination ofthe Dynamic Next Backoff Period (DNBP) scheme and the Dynamic Next Clear Channel Assessment (DNCCA)scheme. The DNBP scheme employs a fuzzy Takagi, Sugeno, and Kang (TSK) model’s inference system toquantitatively analyze backoff exponent, channel clearance, collision ratio, and data rate as input parameters. Onthe other hand, the DNCCA scheme dynamically adapts the CCA process based on requested data transmission tothe coordinator, considering input parameters such as buffer status ratio and acknowledgement ratio. As a result,simulations demonstrate that our proposed schemes are better than some existing representative approaches andenhance data transmission, reduce node collisions, improve average throughput, and packet delivery ratio, anddecrease average packet drop rate and packet delay.

BACKGROUND Work-related injuries have gained recent attention,especially in the orthopaedic literature.As upper extremity orthopaedic surgical tasks require repetitive and constant maneuvers,these surgeons can be at increased risk of acquiring workrelated musculoskeletal(mSK)disorders during their years in practice.AIm To assess the prevalence,characteristics and impact of mSK disorders among upper extremity orthopaedic surgeons.mETHODS A modified version of the physical discomfort survey was sent to surgeons who were members of the American Shoulder and Elbow Surgeons and the Canadian shoulder and elbow society via e-mail.The collected data were analyzed using descriptive statistics,one-way analysis of variance,and Fisher's exact test.P values ofmales and the majority were younger than 55 years old(65.5%).A work-related mSK injury was reported by 89.4% of respondents,of which the most common diagnoses were low back pain(26.1%)and lateral elbow epicondylitis(18.3%).Among those that reported an injury,82.7%required treatment and 26% required time off work as a direct result of their injury.The need to undergo treatment due to the injury was associated with increased number of injuries(Pmoreover,surgeons were more likely to require time off work when they had been in practice for>21 years(PmSK injuries,with more than one quarter of surgeons reported requiring time off work due to an mSK injury.The high incidence of these disorders may place a financial and psychological burden on surgeons and affect their ability to provide patient care.Awareness of operative ergonomics,irrespective of surgical specialty may help to decrease or possibly prevent the occurrence of these disorders.

Side lobe level reduction(SLL)of antenna arrays significantly enhances the signal-to-interference ratio and improves the quality of service(QOS)in recent and future wireless communication systems starting from 5G up to 7G.Furthermore,it improves the array gain and directivity,increasing the detection range and angular resolution of radar systems.This study proposes two highly efficient SLL reduction techniques.These techniques are based on the hybridization between either the single convolution or the double convolution algorithms and the genetic algorithm(GA)to develop the Conv/GA andDConv/GA,respectively.The convolution process determines the element’s excitations while the GA optimizes the element spacing.For m elements linear antenna array(LAA),the convolution of the excitation coefficients vector by itself provides a new vector of excitations of length N=(2m−1).This new vector is divided into three different sets of excitations including the odd excitations,even excitations,and middle excitations of lengths m,m−1,andm,respectively.When the same element spacing as the original LAA is used,it is noticed that the odd and even excitations provide a much lower SLL than that of the LAA but with amuch wider half-power beamwidth(HPBW).While the middle excitations give the same HPBWas the original LAA with a relatively higher SLL.Tomitigate the increased HPBWof the odd and even excitations,the element spacing is optimized using the GA.Thereby,the synthesized arrays have the same HPBW as the original LAA with a two-fold reduction in the SLL.Furthermore,for extreme SLL reduction,the DConv/GA is introduced.In this technique,the same procedure of the aforementioned Conv/GA technique is performed on the resultant even and odd excitation vectors.It provides a relatively wider HPBWthan the original LAA with about quad-fold reduction in the SLL.

Computational calculations using density functional theory(DFT) were performed for the first time using the full potential linearized augmented plane wave plus local orbital method(FP-LAPW + LO) to determine the structural, elastic, electronic and magnetic properties of europium-based cubic perovskites EuYO_(3)(Y=Cr, mn, Fe). The exchange correlation potentials of GGA along with some analytical methods were adopted for the computation of structural and elastic properties. moreover, the GGA + U formalism was also added for obtaining more precise electronic and magnetic properties, particularly to address the Eu-4f and Y-3d orientations in the spin-polarized double cell symmetry. The observed lattice parameters of these compounds are consistent with experiment. The observed bulk moduli predict that EuCrO_(3) is harder and less compressible than EumnO_(3) and EuFeO_(3). The calculated tolerance factors of these compounds are within the cubic symmetry range. Our computed critical radius of EuCrO_(3) shows that EuCrO_(3) has a larger migration energy. Based on their elastic properties, these compounds are ductile in nature. We also computed the thermal properties of these compounds. The band structures and density of states show that these compounds are metallic in character. The lowest ground state energy and magnetic moments of these compounds expose their ferromagnetic nature. The metallic nature and strong ferromagnetism of these compounds make them promising applicants for application in spintronic.

BACKGROUND Patients with diabetes mellitus(Dm)are predisposed to an increased risk of infection signifying the importance of vaccination to protect against its potentially severe complications.The Centers for Disease Control and Prevention/Advisory Committee on Immunization Practices(CDC/ACIP)issued immunization recommendations to protect this patient population.AIm To assess the adherence of patients with Dm to the CDC/ACIP immunization recommendations in Saudi Arabia and to identify the factors associated with the vaccine adherence rate.mETHODS An observational retrospective study conducted in 2023 was used to collect data on the vaccination records from 13 diabetes care centers in Saudi Arabia with 1000 eligible patients in phase I with data collected through chart review and 709 patients in phase II through online survey.RESULTS Among participants,10.01%(n=71)had never received any vaccine,while 85.89%(n=609)received at least one dose of the coronavirus disease 2019(COVID-19)vaccine,and 34.83%(n=247)had received the annual influenza vaccine.Only 2.96%(n=21),2.11%(n=15),and 1.12%(n=8)received herpes zoster,tetanus,diphtheria,and pertussis(Tdap),and human papillomavirus(HPV)vaccines,respectively.For patients with Dm in Saudi Arabia,the rate of vaccination for annual influenza and COVID-19 vaccines was higher compared to other vaccinations such as herpes zoster,Tdap,pneumococcal,and HPV.Factors such as vaccine recommendations provided by family physicians or specialists,site of care,income level,Dm-related hospitalization history,residency site,hemoglobin A1c(HbA1c)level,and health sector type can significantly influence the vaccination rate in patients with Dm.Among non-vaccinated patients with Dm,the most reported barriers were lack of knowledge and fear of side effects.This signifies the need for large-scale research in this area to identify additional factors that might facilitate adherence to CDC/ACIP vaccine recommendations in patients with Dm.CONCLUSION In Saudi Arabia,patients with Dm showed higher vacci
