Nursing Care in Patient with Advanced Cancer: The Experience of the Adult Oncology Unit of Lomé

Background: Cancer patients suffer physical, psychological, spiritual, and social pains, especially in the advanced stage. Nurses spend more time with patients than any other healthcare team member. This study aimed to assess nurses’ behavior and care experiences in patients with advanced cancer and explore patients’ perceptions of nursing care. Methods: A cross-sectional study was conducted with eight nurses and thirty patients with advanced cancer hospitalized in the oncology unit at Sylvanus Olympio Teaching Hospital of Lomé from July to August 2020. Results: The mean age of nurses was 34.3 years ranging from 23 to 48 years. There were five men (62.5%) and three women (37.5%). The mean duration of working in oncology nursing of all was less than two years. Only one nurse has training in palliative care. Stress (100%), sadness (100%), and fear (50%) were the most frequently expressed feeling of nurses. The frequently expressed difficulties were the lack of training in palliative care (87.5%), insufficiency of nursing staff (75%), and helplessness in front of the patient’s distress (75%). Among the thirty patients, were 22 women (72.7%) and 8 men (27.3%). The needs expressed by the patients were psychological support (n = 11;36.7%), pain relief (n = 10;33.3%), and moral support (n = 9;30%). Most of the patients (73.3%) affirmed that nurses did not inform them well about their disease. Three (10%) were very satisfied with the care provided, 23 patients (76.7%) were satisfied and 4 (13.3%) were unsatisfied. Conclusion: This study revealed that nursing care in Togolese patients with cancer faces many difficulties and there is a need for providing specialized oncology nursing.

The novel coronavirus, saRS-CoV-2, has the potential to cause natural ventilation systems in hospital environments to be rendered inadequate, not only for workers but also for people who transit through these environments even for a limited duration. Studies in of the fields of geosciences and engineering,when combined with appropriate technologies, allow for the possibility of reducing the impacts of the saRS-CoV-2 virus in the environment, including those of hospitals which are critical centers for healthcare. In this work, we build parametric 3D models to assess the possible circulation of the saRS-CoV-2 virus in the natural ventilation system of a hospital built to care infected patients during the COVID-19 pandemic. Building Information Modeling(BIM) was performed, generating 3D models of hospital environments utilizing Revit software for Autodesk CFD 2021. The evaluation considered dimensional analyses of 0°, 45°, 90° and 180°. The analysis of natural ventilation patterns on both internal and external surfaces and the distribution of windows in relation to the displacement dynamics of the saRS-CoV-2 virus through the air were considered. The results showed that in the external area of the hospital, the wind speed reached velocities up to 2.1 m/s when entering the building through open windows. In contact with the furniture, this value decreased to 0.78 m/s. In some internal isolation wards that house patients with COVID-19, areas that should be equipped with negative room pressure, air velocity was null. Our study provides insights into the possibility of saRS-CoV-2 contamination in internal hospital environments as well as external areas surrounding hospitals, both of which encounter high pedestrian traffic in cities worldwide.

Comparison of Tongue and Lip Trills with Phonation of the Sustained Vowel /<i>ε</i>/ Regarding the Periodicity of the Electroglottographic Waveform and the Amplitude of the Electroglottographic Signal

Aim: The aim is to compare the vocal fold vibration seen during lip and tongue trills with that seen during phonation of the sustained vowel /ε/, in terms of the periodicity of the EGG waveform and the amplitude of the EGG signal, in professional voice users. Study design: This was a quasi-experimental study. Methods: We used electroglottography (EGG) to compare the vocal fold vibration seen during tongue and lip trills with that seen during phonation of the sustained vowel /ε/, in terms of the EGG waveform periodicity and signal amplitude, in 10 classically trained, professional singers. The participants produced the sustained vowel /ε/ and performed tongue and lip trills at the same frequency and intensity. The periodicity of the waveform and the amplitude of the signal were visually analyzed by three blinded, experienced readers. To confirm the visual analysis results, we measured the jitter and shimmer of the signal and the frequency of variation in vocal fold vibration during the trill exercises. Results: The EGG waveform was classified as periodic for the sustained vowel phonation task and as quasi-periodic for the trill exercises, the vibration pattern repeating at approximately 24 Hz. Conclusion: The vibration of the vocal folds was modified according to the supraglottic movement in trills exercises.

Spatial Distribution of Internal Temperatures in a LGR （Light Green Roof） for Brazilian Tropical Weather

This article aims to assess the spatial distribution of the IST (internal surface temperatures) in the ceiling and DBT (dry bulb temperatures) of a LGR (light green roof) in a test cell. Cover systems known as green roofs have the potential to retain rainwater and help reduce runoff. However, the characteristic considered in this work is the insulation capacity of this kind of coverage. To evaluate the spatial distribution of temperatures in an environment with light green roof, we proposed a new method for acquisition of series of climatological data and temperatures according to spatial and temporal approaches of dynamic climatology. Climatological data were provided by an automatic weather station and temperatures were collected in a test cell with light green roof. The spatial distribution of surface temperatures and internal air temperature (DBT) are based on the concepts of a climatic episode and typical experimental day from the study of the dynamic climatology. The results led to the conclusion that the light green roof has a balanced spatial distribution of the IST and of the internal air temperature (DBT), i. e., without substantial variations over the day. The new methodology also showed the importance of specifying the location of the sensors and automatic weather station in experimental studies on the thermal behaviour of buildings.

The planning and implementation of large-scale urban development projects(UDPs)tend to arouse expectations of increasing investments to urban and social ***,these urban interventions have been less democratic and more elite-driven priorities,resulting in socio-spatial segregation in the *** the case of the Rio de Janeiro/RJ,the urban interventions planned for the years following the 2016 Olympic Games have not yet occurred in the *** this sense,it is necessary to understand the impacts of this mega-event’s intervention when the legacy does not attend the expectations of improvements in the ***,the analyses took into account official documents and the perception of residents close to the areas that received interventions for the Olympic Games,regarding the changes that occurred in these *** is evident the importance of discussing the subject to later outline guidelines or strategies that aim to minimise significant impacts generated by this insertion in the urban environment.

目的:研究牛末梢血中单核细胞由来的巨噬细胞在受到LPS刺激后细胞表面Toll样受体表达的变化。方法:试验采取3头日本黑牛外周血,进行分离得到外周血单核细胞,并用Repcell进行巨噬细胞的分离培养,7天后用脂多糖(lipopolysaccharide,LPS)刺激细胞24小时后,通过RT-PCR测定巨噬细胞表面Toll样受体mRNA的表达以及细胞因子mRNA的表达。结果:在LPS刺激24小时后,IL-6、TNF-α和IL-1βmRNA的表达量显著升高(P<0.05),与此同时IL-8和IL-12p40 mRNA的表达量相对提高;TLR1和TLR10的转录物显著下降(P<0.05),然而TLR2、4和6保持稳定。结论:巨噬细胞在受到病原刺激后通过提高IL-6、TNF-α和IL-1βmRNA的表达量,从而提高了天然免疫的作用。该研究为探讨巨噬细胞在无然免疫中的作用奠定了基础。
