strong>Background:strong> Neoadjuvant chemotherapy is used in gynecologic malignancies as primary treatment for advanced disease and has demonstrated improvement in survival rates. However, the data for use in endometrial cancer is limited. Case: This is a case of stage IV endometrial undifferentiated carcinoma with complete response to four cycles of neoadjuvant chemotherapyspan>span>span> span>span>(carboplatin/paclitaxel) and no evidence of disease during the interval debulking surgery (robotic-assisted total laparoscopic hysterectomy with bilateral salpingoophorectomy, pelvic and paraaortic lymph node dissection, and cystoscopy). Conclusion: Complete response to neoadjuvant chemotherapy prior to interval debulking surgery is only reported in a limited number of cases, and even less common in advanced stage high-grade endometrial cancers. The use of neoadjuvant chemotherapy may reduce tumor burden, increase opportunities for optimal debulking, and have an impact on survival. Further research is needed to understand the potential for neoadjuvant chemotherapy in these patients.span>span>span>

COVID-19 mortality and its risk factors:A single-center observational study

Objectives:To determine COVID-19 mortality and its risk factors in hospitalized patients at of a tertiary care center in north ***:A retrospective observational study was conducted of patients who were hospitalized from May 2020 to January *** in-hospital mortality was assessed,and demographic variables and comorbidities between COVID-19 deaths and survivors were ***:A total of 24000 patients were admitted during the study period,among which 17000 had shown positive results of the RT-PCR test for *** total mortality was 329 patients(1.37%),among which 232(70.52%)succumbed due to COVID-19,and 97(29.48%)died due to other *** mean age of the patients was(64.09依16.99)*** mean age was significantly higher in COVID-19 related deaths[(67.63±13.78)years]as compared to that of the survivors[(60.52±19.5)years](Psurvivors,there were more males(72.41%v.s.61.5%)and less females(27.59%v.s.38.5%)in COVID-19 related deaths(P=0.001).Comorbidities such as hypertension,diabetes mellitus,and chronic kidney disease showed a significant correlation with COVID-19 mortality with an adjusted odds ratio of 2.389(95%CI:1.465-2.982),3.891(95%CI:2.059-5.392),and 6.358(95%CI:5.675-10.564),***:Elderly males with comorbidities have higher risk for mortality related to *** vaccination drive is rightfully prioritised to serve the high-risk category first.

Bariatric surgery in old age:a comparative study of laparoscopic Roux-en-Y gastric bypass and sleeve gastrectomy in an Asia centre of excellence

Bariatric surgery has been proved to be the safest and efficient procedure in treating morbid obese patients,but data is still lacking in the elderly *** aim of our study was to compare the safety and efficacy of laparoscopic Roux-en-Y gastric bypass（LRYGB） and sleeve gastrectomy（LsG） in patients aged more than 55 *** performed2 a retrospective review of a prospectively collected *** patients with body mass index（BMI） §32 kg/mand aged more than 55 years undergoing LRYGB or LsG in BMI surgery Centre,E-Da Hospital between January 2008 and December 2011 with at least one year of follow up were included for the ***,peri-operative data,weight loss and surgical complications were all recorded and *** age and BMI of these 68 patients22（22 males and 46 female） were 58.8 years（55-79 years） and 39.5 kg/m（32.00-60.40 kg/m）.LRYGB was performed in 44 patients and LsG in 24 *** two groups were comparable in their preoperative BMI,American society of Anaesthesia（AsA） score and gender *** patients were significantly older than patients receiving *** proportion of type 2 diabetes preoperatively was significantly higher in LRYGB patients as compared to LsG patients（88.63% vs.50%; P,0.01）.The prevalence of other co-morbidities was similar and comparable2 between the *** BMI in the LRYGB and LsG groups at the end of 1 year were 28.8 kg/mand228.2 kg/m,respectively,and there was no statistically significant difference in mean percentage of excess weight loss（%EWL） at 1 *** percentage of resolution of diabetes was significantly higher in LRYGB（69.2%） as compared to LsG（33.3%）.On the other hand,there was no statistical difference in the percentage of resolution of hypertension,hyperlipidemia and fatty liver *** overall morbidity and re-operation rate was higher in LRYGB *** morbidly elderly patients,both surgeries achieved good weight loss and resolution of *** is s

Baastrup's disease: The kissing spine

A 67-year-old male presented with a gradually progressive low back pain of 2 years duration. The patient was leading a retired life and there was no history of chronic fever or significant trauma. There was no radiation of pain or any features suggestive of claudication. There was no history of any comorbidity. The pain was aggravated with extension of the spine and relieved with flexion. There was no swelling or neurological deficit, but muscle spasm was present. Radiographs of the spine revealed degenerative changes in the lumbosacral spine, along with articulation of spinous processes at in lumbar spine at all levels level suggestive of Baastrup's disease, commonly known as "kissing spine". Routine blood investigations were within normal limits. The patient was managed conservatively. He was given a week's course of analgesics and muscle relaxants and then started on spinal flexion exercises, with significant improvement being noted at 6 months follow up.

Laparoscopic management of a two staged gall bladder torsion

Gall bladder torsion(GBT) is a relatively uncommon entity and rarely diagnosed preoperatively. A constant factor in all occurrences of GBT is a freely mobile gall bladder due to congenital or acquired anomalies. GBT is commonly observed in elderly white females. We report a 77-year-old, Caucasian lady who was originally diagnosed as gall bladder perforation but was eventually found with a two staged torsion of the gall bladder with twisting of the Riedel's lobe(part of tongue like projection of liver segment 4A). This together, has not been reported in literature, to the best of our knowledge. We performed laparoscopic cholecystectomy and she had an uneventful postoperative period. GBT may create a diagnostic dilemma in the context of acute cholecystitis. Timely diagnosis and intervention is necessary, with extra care while operating as the anatomy is generally distorted. The fundus first approach can be useful due to altered anatomy in the region of Calot's triangle. Laparoscopic cholecystectomy has the benefit of early recovery.

Background and aim:Double-balloon enteroscopy(DBE)and single-balloon enteroscopy(sBE)are new techniques capable of providing deep *** of individual studies comparing these techniques have not been able to identify a superior *** aim was to systematically pool all available studies to compare the efficacy and safety of DBE with sBE for evaluation of the small ***:Databases were searched,including PubMed,Embase,and the Cochrane Central Register of Controlled *** main outcome measures were complete small-bowel visualization,diagnostic yield,therapeutic yield,and complication *** analysis was performed using Review Manager(RevMan version 5.2).Meta-analysis was performed using fixed-effect or random-effect methods,depending on the absence or presence of significant *** used the v2 and I2 test to assess heterogeneity between *** were expressed as risk ratios(RR)or mean differences with 95%confidence intervals(CI).Results:Four prospective,randomized,controlled trials with a total of 375 patients were *** was superior to sBE for visualization of the entire small bowel[pooled RR=0.37(95%CI:0.19–0.73;P=0.004)].DBE and sBE were similar in ability to provide diagnosis[pooled RR=0.95(95%CI:0.77–1.17;P=0.62)].There was no significant difference between DBE and sBE in therapeutic yield[pooled RR=0.78(95%CI:0.59–1.04;P=0.09)]and complication rate[pooled RR=1.08(95%CI:0.28–4.22);P=0.91].Conclusions:DBE was superior to sBE with regard to complete small bowel *** was similar to sBE with regard to diagnostic yield,ability to provide treatment and complication rate,but these results should be interpreted with caution as they is based on very few studies and the overall quality of the evidence was rated as low tomoderate,due to the small sample size.
