Analysis of Maternal Mortality at the Maradi Maternal and Child Health Center/Niger: About 379 Cases

Introduction: Pregnancy, childbirth, and their consequences continue to be the leading cause of death, illness, and disability among women of reproductive age in developing countries. In Niger, maternal deaths account for about 39% of all deaths of women aged 15-49. Our aim was to identify the factors linked to maternal mortality in order to contribute to its reduction. Method: this is a descriptive retrospective study of 379 cases of maternal deaths collected at the Maradi CsME from January 1, 2018, to December 31, ***: Our study’s maternal mortality ratio is 2645.72 per 100,000 live births. The mean age of our patients was 27.46 years, with extremes ranging from 15 to 47 years. The 20-24 age group was the most represented, with 88 cases (23.2%). Housewives were 361 (95.3%), and 334 patients (88.1%) were uneducated. There were 261 of them from rural areas, i.e. 69%. The mean parity in our study was 3.46, with extremes ranging from 0 to 15. Multiparous patients 117 cases (30.9%) represented the most significant proportion. Only 42 patients (11.08%) had performed four or more antenatal consultations. Direct obstetric causes accounted for 56.73% of the causes of maternal death. Anemia was the leading cause of maternal death (31.66%). Nearly half of the patients had died before 24 hours after admission, i.e. 49.86%. Conclusion: Maternal mortality remains a significant challenge for developing countries, given its magnitude and timid decline. The fight to reduce maternal mortality must be the priority of our countries’ health policies.

Functional Capacity of Patients with Venous Thromboembolic Disease six to Twelve Months after Treatment

Background: Venous thromboembolism (VTE) is among the leading causes of hospital-related disability-adjusted life years lost. We aimed to determine the prevalence and determinants of functional capacity impairment six to twelve months after an acute VTE event. Methods: This was a cross-sectional study conducted between January and April 2021 in two referral hospitals of Yaoundé, including consenting adult patients admitted to these hospitals six to twelve months ago for VTE. We excluded dead patients and those with any comorbidity or symptoms limiting physical activity. The functional outcome was assessed with the six-minute walk test. Functional capacity impairment was defined as walking distance lower than the expected value. Results: We included 27 cases in this study with a mean age of 53.2 ± 14.4 years. The prevalence of functional capacity impairment was 29.6% (95% CI: 14.8 - 48.1). Factors associated with poor functional outcome were obesity (OR: 59.5;95% CI: 4.6 - 767.2;p - 207.4;p = 0.017), massive PE (OR: 30;95% CI: 2.5 - 354;p = 0.004), and poor adherence to treatment (OR: 30.3;95% CI: 2.5 - 333.3;p = 0.004). Conclusion: Functional capacity impairment is common in the medium-term after VTE and factors associated with this poor outcome are obesity, the severity of the VTE, and poor adherence to treatment.

Estimation of the Direct Cost of Management of Venous Thromboembolism in Three Reference Hospitals in the City of Yaoundé: A Retrospective study over a Three-Year Period

Background: Venous thromboembolism (VTE) is a major public health problem due to its increasing frequency, mortality and management cost. This cost may require major financial efforts from patients, especially in developing countries like ours where less than 7% of the population has health insurance. This study aimed to estimate the direct cost of managing VTE in three reference hospitals in Yaoundé. Methods: This was a cross-sectional retrospective study over a three-year period (from January 1st 2018 to December 31 2020) carried out in the Cardiology departments of the Central and General Hospitals, and the Emergency Centre of the city of Yaoundé. All patients managed during the study period for deep vein thrombosis and pulmonary embolism confirmed by venous ultrasound coupled with Doppler and computed tomography pulmonary angiography respectively were included. For each patient, we collected sociodemographic and clinical data as well as data on the cost of consultation, hospital stay, workups and medications. These data were analysed using sPss version 23.0. Results: A total of 92 patient’s records were analysed. The median age was 60 years [48 - 68] with a sex ratio of 0.53. The median direct cost of management of venous thromboembolism was 766,375 CFAF [536,455 - 1,029,745] or $1415 UsD. Management of pulmonary embolism associated with deep vein thrombosis was more costly than isolated pulmonary embolism or deep vein thrombosis. Factors influencing the direct cost of management of venous thromboembolism were: hospital structure (p = 0.015), health insurance (p 0.001), type of pulmonary embolism (p = 0.021), and length of hospital stay (p = 0.001). Conclusion: Management of VTE is a major financial burden for our patients and this burden is influenced by the hospital structure, health insurance, type of pulmonary embolism and length of hospital stay.

Anthelmintic activity of saba senegalensis(***.) Pichon (Apocynaceae) extract against adult worms and eggs of Haemonchus contortus

Objective: To evaluate the anthelmintic property of saba senegalensis(***) Pichon(Apocynaceae)(s. senegalensis) on Haemonchus contortus that is traditionally used in Burkina Faso for its gastrointestinal parasites ***: The lyophilized aqueous decoction of leaves of s. senegalensis at concentrations of 0.10, 1.00, 3.00, 10.00 and 15.00 mg/m L was used on eggs and adult worms of Haemonchus contortus collected from gastrointestinal tract of small ***: The LC_(50) on adult worms was 6.79 mg/m L and 3.25 mg/m L for the leaves of s. senegalensis and the levamisole(reference drug), respectively. Inhibition of hatching assay showed a concentration-dependent manner with an inhibition of 93.63% at the concentration of 15.00 mg/m L of s. ***: These results indicate that the aqueous extract of s. senegalensis possesses an anthelmintic property and may justify its use in traditional medicine for the treatment of gastrointestinal parasites.

The selection of Dry Port Location by Analytic Network Process Model: A Case study of Dosso-Niger

The aim of this paper is to select the best location for the construction of a dry port in Niger which is a land locked country (LLC). Niger is located in the sahel and has a land area of 1,267,000 square kilometers [1], with the closest port being port of Cotonou in Benin. The transport corridor from Niamey to Cotonou is approximately 1036 km long [2]. It is estimated that this corridor carries about 40 percent of Niger’s overseas trade traffic [3]. In this work, the Analytic Network Process (ANP) model is used to determine the optimal location of the dry port, among three major cities: Niamey (capital city), Dosso and Gaya. From the application of this selection model, Dosso was selected as the best location for the location of the dry port, while Gaya and Niamey were placed second and third respectively. The results obtained in this work strongly confirm the decision of the government of Niger to construct a dry port in Dosso, a project that commenced in 2010 and is still in progress.

Covariant Prolongation structure, Conservation Laws and soliton solutions of the Gross-Pitaevskii Equation in the Bose-Einstein Condensate

In this paper, we investigate the Gross-Pitaevskii (GP) equation which describes the propagation of an electron plasma wave packet with a large wavelength and small amplitude in a medium with a parabolic density and constant interactional damping by the Covariant Prolongation structure Theory. As a result, we obtain general forms of Lax-Pair representations. In addition, some hidden structural symmetries that govern the dynamics of the GP equation such as sL(2,R), sL(2,C), Virasoro algebra, sU(1,1) and sU(2) are unearthed. Using the Riccati form of the linear eigenvalue problem, infinite number of conservation laws of the GP equation is explicitly constructed and the exact analytical soliton solutions are obtained by employing the simple and straightforward Hirota’s bilinear method.

Preventive Effects of Ananas comosus Juice on Obesity Risk Factors in Female Wistar Rats

The present study aim was to assess the preventive effects of Ananas comosus juice consumption on the risk factors of obesity in female Wistar rats. 108 rats were tested for 90 days. After randomization, they were shared out into six groups including four experimental groups (GTc, GPlp, GEns and GBrs) and two control groups (src="Edit_***" width="15" height="3" align="right" alt="" />src="Edit_***" width="80" height="22" alt="" />). The control group consumed 1.20 ml of distilled water, the experimental rats received in oral, 1.20 ml of drinks made from the different parts of Ananas comosus fruit. This study aimed to provide relevant knowledge about the dynamics of the hydrological parameters in the river-estuary continuum of the Wouri-Nkam river estuary for a sustainable management program. The hydrological parameters were recorded in eleven stations spanned out on the basisspan> of population density and human activities. Water quality parameters (Temperature, salinity, dissolved oxygen, pH, Total dissolved solutes, Redox potential and conductivity) were collected in subsurface water using a multiple parameter. Surface currents and morphometric (depth and width) parameters were recorded using a drifter, sonar depth and GPS. The field measurements took placed between 18/05/2019 to 08/09/2020 and were divided into six (06) cruises. The data were later subjected to an analysis of variance (ANOVA) and Principle Component Analysis using XLSTAT 2017 (2.7 version) software. Results obtained revealed that, the water quality parameters were spatially more stable nospan>tspan>signficant at (df = 9, p span> span>0.05) with a relatively low temperature (25.5span>°C - 27°C) during the wet period. The revaluation of medicinal plants in the veterinary pharmacopoeia is essential for the development of livestock in Burkina Faso. In order to justify the use of C. sesamoides and s. hermonthica in the treatment of gastrointestinal parasitosis of small ruminants, a quantification of phenolic compounds as well as antioxidant activity and acute toxicité evaluation of both plants were performed. Acute toxicity was evaluated by administering a single dose of 2000 mg/kg body weight of aqueous extract of both plants to mice. The highest total polyphenol content for C. sesamo?des was obtained in the ethyl acetate fraction with 47.236 ± 2.57 mgEAG/100mg while that of s. hermonthica was 74.871 ± 2.57 mgEAG/100mg obtained with the n-butanol Fraction. The dichloromethane extract of s. hermonthica obtained the highest total flavonoid content with 7.31 ± 0.48 mgEQ/100 while the highest total flavonoid content of C. sesamo?des was obtained with ethyl acetate fraction with 5.4273 ± 0.35 mgEQ/100mg. The aqueous extract of C. sesamo?des obtained the highest content of condensed tannins with 3.028 ± 0.13 mgEAT/100mg. Both plant’s extracts did not show any signs of toxicity in NMRI mice after administration of the dose of 2000 mg/kg. The antioxidant activity by DPPH, FRAP and ABTs methods was good compared to those of Quercetin and Trolox taken as standard.

Background and Objectives: Cerebro-vascular accident or stroke constitutes a major challenge in sub-saharan Africa. In Cameroon, basic epidemiologic data are not routinely available. Aims: The aim of this study was to determine the type, the associated risk factors, time to admission, the clinical presentation and the case fatality of stroke at the Douala General Hospital (DGH) in Cameroon. Methods: A cross-sectional study was performed from January 1, 2010 to December 31, 2012 at the neurology and intensive care units of the DGH. All patients above 15 years of age with a diagnosis of established stroke were enrolled. For each patient, socio-demographic, clinical and paraclinical data were recorded as well as the duration of hospitalization and the case fatality. Results: In all, 325 patients were enrolled with males constituting 68.1% and general mean age of 58.66 ± 13.6 years. The mean initial consultation delay was 47.36 ± 18.48 hours. The majors cerebro-vascular risk factors were hypertension (81.15%), chronic alcohol consumption (28.3%), diabetes mellitus (20.61%), obesity (18.15%), cigarette smoking (16%), dyslipidemia (8.9%) and atrial fibrillation (3.07%). Ischemic stroke accounted for 52% of cases while 48% were hemorrhagic. The mean duration of hospitalization was 8.58 ± 6.35 days with a case fatality rate of 26.8%. septic conditions appeared to be the leading cause of death accounting for 35.6% of cases. Conclusion: stroke in the DGH is associated with a high case fatality rate and hypertension remains the number one risk factor. There is a clear and urgent need for public health authorities to reinforce measures for the control of modifiable stroke risk factors.
