护士在唇腭裂患儿中应用奶瓶喂养技术现状的调查研究

目的调查护士对有喂养困难的唇腭裂患儿所采用的临床奶瓶喂养技术现状。方法采用质性描述性研究设计。于2021年12月至2022年1月,向日本1109所设有产科、新生儿科或小儿牙科病房的医院分别发放匿名问卷。参与者是具有5年以上唇腭裂患儿护理工作经验的护士。问卷包括喂养技术4个方面的开放式问题,包括:奶瓶喂养前的准备、乳头插入方法、吸吮帮助和停止奶瓶喂养的标准。根据意义相似性对资料进行分类和分析。结果共获得410份有效问卷。资料分析结果显示,奶瓶喂养前的准备工作分为7个类别(如,改善患儿的口腔运动、保持患儿平稳呼吸),27个子类别;在乳头插入方法中,分为4个类别(如,用乳头在口腔内形成负压封闭裂口、插入乳头时不接触裂口),11个子类别;在吸吮帮助中,分为5个类别(如,促进觉醒、口腔产生负压),13个子类别;在停止奶瓶喂养的标准中,分为5个类别(如,觉醒水平降低、生命体征变差),16个子类别。96.0%护士希望学习唇腭裂患儿的奶瓶喂养技术。结论本研究确定了多项用于唇腭裂喂养困难患儿的奶瓶喂养技术。然而,这些技术之间存在相互矛盾,方法的有效性也未得到评估。未来需要进行干预研究,以确定各项技术的益处或潜在的危害。

＜正＞Obstruction of the distal duodenum is a rare condition, the aetiological factors being pseudoobstruction, henlatoma, neoplasm, extrinsic compression by the superior mesenteric artery, and so on. In this study, we report on six cases of distal duodenal obstruction developed after surgery for cancer of the left colon and rectum and discuss on the underlying causative favors.

AIM To evaluate the diagnostic performance of computed tomography(CT) volumetry for discriminating the fibrosis stage in patients with nonalcoholic fatty liver disease(NAFLD).METHODs A total of 38 NAFLD patients were enrolled. On the basis of CT imaging, the volumes of total, left lateral segment(LLs), left medial segment, caudate lobe, and right lobe(RL) of the liver were calculated with a dedicated liver application. The relationship between the volume percentage of each area and fibrosis stage was analyzed using spearman's rank correlation coefficient. A receiver operating characteristic(ROC) curve analysis was performed to determine the accuracy of CT volumetry for discriminating fibrosis *** The volume percentages of the caudate lobe and the LLs significantly increased with the fibrosis stage(r = 0.815, P spectively). Contrarily, the volume percentage of the RL significantly decreased with fibrosis stage(r =-0.563, P st diagnostic accuracy for staging fibrosis, and the area under the ROC curve values for discriminating fibrosis stage were as follows: ≥ F1, 0.896; ≥ F2, 0.929; ≥ F3, 0.955; and ≥ F4, 0.923. The best cut-off for advanced fibrosis(F3-F4) was 4.789%, 85.7% sensitivity and 94.1% *** The volume percentage of the caudate lobe calculated by CT volumetry is a useful diagnostic parameter for staging fibrosis in NAFLD patients.

The differential diagnosis between intracystic hemorrhage and cystadenocarcinoma of the liver is often difficult even with the use of various imaging modalities. A 73-year-old woman was admitted to our hospital with the complaint of right upper quadrant pain. Ultrasonography (Us) demonstrated a heterogeneous echogenic cyst measuring 11 cm × 8 cm in size in s2 of the liver, indicated intracystic hemorrhage of simple liver cyst or cystadenocarcinoma, but the differential diagnosis was considerably difficult. Levovist (schering, Berlin, Germany) Us revealed no enhancement of the intracystic structures, suggesting a clot in the case of intracystic hemorrhage. An operation was performed and the resected lesion showed a solitary benign liver cyst, measuring 5.5 cm × 4.7 cm × 8.5 cm containing a large blood clot. The patient had an uneventful recovery after the surgery. Levovist Us may play an important role in discrimination between intracystic hemorrhage of simple hepatic cysts and cystadenocarcinoma of the liver.

传统的寿命理论将滚动接触疲劳（RCF）寿命定义为：由最大接触压力（P）和总的应力循环数（N）决定。但是，分析寿命试验轴承的结果可知，RCF寿命、最大接触压力和总的循环数之问基本无关。进一步研究后发现滑动速度是影响轴承RCF寿命的其它因素。在特殊试验条件下发现，当PV值接触压力和滑动速度之积高于临界值时就会产生特殊剥落类型。白色组织引起的剥落出现在PV值最大的点上。虽然增加最大接触压力是一种传统的方法，但增大沟道曲率是防止白色组织形成的一种有效措施。在轴承材料添加铬是另外一种有效的措施。

AIM:To test the hypothesis that the shape and length of Barrett's epithelium are associated with prevalence of erosive ***:A total study population comprised 869 patients who underwent endoscopy during a health checkup at our *** presence and extent of Barrett's epithelium were diagnosed based on the Prague C & M *** originally classified cases of Barrett's epithelium into two types based on its shape,namely,flamelike and lotus-like Barrett's epithelium,and into two groups based on its length,its C extent shape and length of Barrett's epithelium with erosive esophagitis was ***:Barrett's epithelium was diagnosed in 374 cases(43%).Most of these were diagnosed as shortsegment Barrett's *** prevalence of erosive esophagitis was significantly higher in subjects with flame-like than lotus-like Barrett's epithelium,and in those with a C extent of ≥ 2 cm than s-like Barrett's epithelium,and Barrett's epithelium with a longer segment were more strongly associated with erosive esophagitis.

Alzheimer’s Disease (AD) shows cognitive dysfunction as core symptoms and Behavioral and Psychological symptoms of Dementia (BPsD). since acetylcholine nerve system derived from septum is collapsed in the AD patients, we have used Olfactory Bulbectomized (OBX) mice whose cholinergic system is largely impaired in the septum. Recently, Yokukansankachimpihange (YKH), a traditional Japanese Kampo medicine has used for BPsD in addition to improve cognitive dysfunction in AD patients. However the essential components for cognition and BPsD improvement and their mechanism are largely unknown. In present study, we found that Atractylenolide III (Aen-III), one of the components of YKH, improved cognitive deficits and depression in the OBX mice. OBX mice were orally administered with Aen-III (1.0 and 3.0 mg/kg) and YKH extracts daily for 18 days. Like YKH extracts, the Aen-III treatments ameliorated cognitive deficits and depression-like behavior observed in OBX mice. Importantly, Aen-III administration significantly restored the decreases in Ca2+/calmodulin-dependent protein kinase II (CaMKII) autophosphorylation and phosphorylation of Ca2+/calmodulin-dependent protein kinase IV (CaMKIV) and cyclic AMP response element binding protein (CREB). The restoration of CaMKII and CaMKIV signaling is closely related to the increased BDNF levels. Furthermore, ATP reduction in OBX mice was rescued by Aen-III (3.0 mg/kg) and YKH (1000 mg/kg) treatment. In summary, Aen-III as a component of YKH ameliorates cognitive dysfunctions and depression via restoring CaMKII and CaMKIV signaling.
