Brain aging is typically associated with a significant decline in cognitive *** risk factors (VRF) and subsequent atherosclerosis (As) play a major role in this *** resilience reflects the brain’s ability to withstand external perturbations,but the relationship of brain resilience with cognition during the aging process remains ***,we investigated how brain topological resilience(BTR) is associated with cognitive performance in the face of aging and vascular risk *** used data from two cross-ethnicity community cohorts,PolyvasculaR Evaluation for Cognitive Impairment and Vascular Events (PRECIsE,n=2220) and sydney Memory and Ageing study (MAs,n=246).We conducted an attack simulation on brain structural networks based on k-shell decomposition and node degree *** was defined based on changes in the size of the largest subgroup of the network during the simulation ***,we explored the negative correlations of BTR with age,VRF,and As,and its positive correlation with cognitive ***,using structural equation modeling(sEM),we constructed path models to analyze the directional dependencies among these variables,demonstrating that aging,As,and VRF affect cognition by disrupting *** results also indicated the specificity of this metric,independent of brain ***,these findings underscore the supportive role of BTR on cognition during aging and highlight its potential application as an imaging marker for objective assessment of brain cognitive performance.

The soft X-ray Imager(sXI)is part of the scientific payload of the solar wind Magnetosphere Ionosphere Link Explorer(sMILE)*** is a joint science mission between the European space Agency(EsA)and the Chinese Academy of sciences(CAs)and is due for launch in *** is a compact X-ray telescope with a wide field-of-view(FOV)capable of encompassing large portions of Earth’s magnetosphere from the vantage point of the sMILE *** is sensitive to the soft X-rays produced by the solar Wind Charge eXchange(sWCX)process produced when heavy ions of solar wind origin interact with neutral particles in Earth’s *** provides a mechanism for boundary detection within the magnetosphere,such as the position of Earth’s magnetopause,because the solar wind heavy ions have a very low density in regions of closed magnetic field *** sensitivity of the sXI is such that it can potentially track movements of the magnetopause on timescales of a few minutes and the orbit of sMILE will enable such movements to be tracked for segments lasting many *** is led by the University of Leicester in the United Kingdom(UK)with collaborating organisations on hardware,software and science support within the UK,Europe,China and the United states.

An adaptive meshing technique and solution method is proposed in which a two-dimensional body-fitted multi-block mesh is locally adjusted to arbitrarily embedded boundaries that are not necessarily aligned with the *** only does this scheme allow for rapid and robust mesh generation involving complex embedded boundaries,it also enables the solution of unsteady flow problems involving bodies and interfaces moving relative to the flow *** scheme has been implemented within a block-based adaptive mesh refinement(AMR)numerical framework which can ease computational expense while maintaining a detailed representation of the embedded boundary and providing an accurate resolution of the spatial characteristics of the fluid *** body motion and evolving motion due to physical processes are considered.A block-based AMR level set method is used to deal with evolving embedded *** results for various test problems are presented to verify the validity of the scheme as well as demonstrate the capabilities of the approach for predicting complex two-dimensional inviscid and laminar fluid flows.

Effect of omeprazole and domperidone on adult asthmatics with gastroesophageal reflux

AIM： To study the effect of combined omeprazole（Ome） and domperidone（Dom） therapy on asthma symptoms and pulmonary function in asthmatics with gastroesoph- ageal reflux. METHODs： We selected 198 asthmatics with gastro- esophageal reflux diagnosed by 24-h esophageal pH moni- toring to receive Ome 20 mg twice daily and Dom 10 mg three times daily or placebo for 16 wk （1：1 double-blind randomization）. spirometry was done at baseline and after 16 wk of treatment. The primary outcome measures were： mean daily daytime and nighttime asthma symptom scores. Mean daily reflux symptom scores, albuterol use as rescue medication （number of puffs）, daytime and nighttime peak expiratory flow rate （PEFR）, postbronchodilator forced expiratory volume in 1 second （FEVl） and postbronchodilator forced vital capacity （FVC） were secondary outcome measures. REsULTs： Comparison of mean change from baseline between antireflux therapy and placebo groups revealed significant reduction in daytime asthma symptom score （17.4% vs 8.9 %）, nighttime asthma symptom score （19.6% vs 5.4%）, reflux symptom score （8.7% vs 1.6%） and rescue medication use （23.2% vs 3.1%） after antireflux therapy compared to mean change in placebo group （P 〈 0.001）. There was significant improvement in morning PEFR （7.9% vs 0.2%）, evening PEFR （9.8% vs 0.5%）, FEW （11.1% vs 3.78%） and FVC （9.3%vs 1.52%） in the antireflux therapy group compared to placebo on comparng the mean change from baseline after 16 wk （P 〈 0.01） CONCLUsION： Combined therapy with Ome and Dom in adult asthmatics with gastroesophageal reflux may be beneficial by reducing asthma symptoms, rescuing medi- cation use, and improving pulmonary function.

BACKGROUND Roux-en-Y gastric bypass(RYGB) is the most commonly performed surgical procedure used to treat obesity worldwide. Despite satisfactory results in terms of weight loss, over time many patients experience weight regain. There are many factors that contribute to weight regain after RYGB, including the diameter of the gastric-jejunal anastomosis(GJA). One of the most commonly performed endoscopic procedures for weight regain after RYGB is argon plasma coagulation(APC). We report a case of hematemesis after outlet revision with APC. We highlight several treatment modalities that can be used to treat this *** sUMMARY A 45-year-old female with a history of weight regain after RYGB was referred for possible endoscopic treatment for weight regain. On endoscopic evaluation, the diameter of the GJA was 22 mm. Due to the dilated GJA, treatment with APC was performed. several months later she reported a return of poor satiety and an increased appetite. A repeat endoscopy was then performed. The GJA was approximately 15 mm and was incompetent. APC was performed. One day post procedure she had four episodes of hematemesis. An endoscopy was performed and a large ulcer with a visible arterial vessel was visualized at the *** was attempted using a Coagrasper and after initial contact with the vessel, the vessel started oozing. Due to fibrosis and the depth of ulceration in the area, clips and repeat APC could not be used. Therefore, an attempt to inject epinephrine injection was made. However, persistent oozing was noted. As a result, hemostatic powder was applied to the region of the bleeding ***, no more bleeding was observed. On follow-up, the patient remained hemodynamically stable and a second look endoscopy was not performed.
