Esophageal motility abnormalities in gastroesophageal reflux disease

Esophageal motility abnormalities are among the main factors implicated in the pathogenesis of gastroesophageal reflux disease. The recent introduction in clinical and research practice of novel esophageal testing has markedly improved our understanding of the mechanisms contributing to the development of gastroesophageal reflux disease, allowing a better management of patients with this disorder. In this context, the present article intends to provide an overview of the current literature about esophageal motility dysfunctions in patients with gastroesophageal reflux disease. Esophageal manometry, by recording intraluminal pressure, represents the gold standard to diagnose esophagealmotility abnormalities. In particular, using novel techniques, such as high resolution manometry with or without concurrent intraluminal impedance monitoring, transient lower esophageal sphincter (LEs) relaxations, hypotensive LEs, ineffective esophageal peristalsis and bolus transit abnormalities have been better defined and strongly implicated in gastroesophageal reflux disease development. Overall, recent findings suggest that esophageal motility abnormalities are increasingly prevalent with increasing severity of reflux disease, from nonerosive reflux disease to erosive reflux disease and Barrett's esophagus. Characterizing esophageal dysmotility among different subgroups of patients with reflux disease may represent a fundamental approach to properly diagnose these patients and, thus, to set up the best therapeutic management. Currently, surgery represents the only reliable way to restore the esophagogastric junction integrity and to reduce transient LEs relaxations that are considered to be the predominant mechanism by which gastric contents can enter the esophagus. On that ground, more in depth future studies assessing the pathogenetic role of dysmotility in patients with reflux disease are warranted.

BACKGROUND Ankle fractures are common lesions of the lower *** 40%of ankle fractures affect the posterior malleolus(PM).Historically,PM osteosynthesis was recommended when PM size in X-ray images was greater than 25%of the ***,computed tomography(CT)has been gaining traction in the preoperative evaluation of ankle *** To elucidate the similarity in dimensions and to correlate PM size in X-ray images with the articular surface of the affected tibial plafond in the axial view on CT(AXCT)of a PM *** Eighty-one patients(mean age:39.4±13.5 years)were evaluated(54.3%were male).Two independent examiners measured PM size in profile X-ray images(PMXR)and sagittal CT(sAGCT)*** correlation of the measurements between the examiners and the difference in the PM fragment sizes between the two images were ***,the PM size in PMXR was compared with the surface of the tibial plafond involved in the fracture in AXCT according to the Haraguchi *** The correlation rates between the examiners were 0.93 and 0.94 for PMXR and sAGCT,respectively(Ps were 2.12%larger in sAGCT than in PMXR(P=0.018).In PMXR,there were 56 casesses≥25%.When PMXR wassponded to 10.13%of the tibial *** PMXR was≥25%,AXCT was 24.52%(Pssification,fracture types I and II had similar PMXR measurements that were greater than those of type *** analyzing AXCT,a significant difference was found between the three types,with II>I>III(PsION PM fractures show different sizes using X-ray or CT *** showed a larger PM in the sagittal plane and allowed the visualization of the real dimensions of the tibial plafond surface.

AIM:To examine the links between quality of sleep and the severity of intestinal symptoms in irritable bow-el syndrome(IBs).METHODs:One hundred and forty-two outpatients(110female,32 male)who met the Rome Ⅲ criteria for IBs with no psychiatric comorbidity were consecutively en-rolled in this *** on age,body mass index(BMI),and a set of life-habit variables were recorded,and IBs symptoms and sleep quality were evaluated using the questionnaires IBs symptom severity score(IBs-sss)and Pittsburgh sleep Quality Index(PsQI).The associa-tion between severity of IBs and sleep disturbances was evaluated by comparing the global IBs-sss and PsQI score(Pearson's correlation and Fisher's exact test)and then analyzing the individual items of the IBs-sss and PsQI questionnaires by a unitary bowel-sleep model based on item response theory(IRT).REsULTs:IBs-sss ranged from mild to severe(120-470).The global PsQI score ranged from 1 to 17(median 5),and 60 patients were found to be poor sleepers(PsQI>5).The correlation between the global IBs-sss and PsQI score indicated a weak association(r=0.2 and 95% CI:-0.03 to 0.35,Ps stronger using our unitary ***,the IBs and sleep disturbances severities,estimated as latent variables,resulted significantly high intra-subject cor-relation(posterior mean of r=0.45 and 95% CI:0.17 to 0.70,Ps between patient features(age,sex,BMI,daily coffee and alcohol intake)and IBs and sleep disturbances were also ana-lyzed through our unitary *** was a signif icant regressor,with patients≤50 years old showing more severe bowel disturbances(posterior mean=-0.38,Pss severe sleep disturbances(posterior mean=0.49,Ps correlated with a lower sever-ity of bowel disturbances(posterior mean=-0.31,Psex(female)and daily alcohol intake(modest)were correlated with less severe sleep ***:The unitary bowel-sleep mod

Esophageal complications caused by gastroesophageal reflux disease(GERD)include reflux esophagitis and Barrett’s esophagus(BE).BE is a premalignant condition with an increased risk of developing esophageal adeno-carcinoma(EAC).The carcinogenic sequence may progress through several steps,from normal esophageal mucosa through BE to EAC.A recent advent of functional esophageal testing(particularly multichannel intraluminal impedance and pH monitoring)has helped to improve our knowledge about GERD pathophysiology,including its *** findings(when properly confirmed)might help to predict BE neoplastic *** the last few decades,the incidence of EAC has continued to rise in Western ***,only a minority of BE patients develop EAC,opening the debate regarding the cost-effectiveness of current screening/surveillance ***,major efforts in clinical and research practice are focused on new methods for optimal risk assessment that can stratify BE patients at low or high risk of developing EAC,which should improve the cost effectiveness of screening/surveillance programs and consequently significantly affect health-care ***,the area of BE therapeutic management is rapidly *** eradication therapies have been shown to be effective,and new therapeutic options for BE and EAC have *** aim of the present review article is to highlight the status of screening/surveillance programs and the current progress of BE ***,we discuss the recent introduction of novel esophageal pathophysiological exams that have improved the knowledge of the mechanisms linking GERD to BE.

Particulate matter with an aerodynamic diameter below 2.5 μm (PM2.5), present in polluted air, has been associated with a large spectrum of health impairments, mainly because of its deep deposition into the lungs. Araraquara City (southeast Brazil) is surrounded by sugar-cane plantations, which are burned to facilitate the harvesting;this process causes environmental pollution due to the large amounts of soot that are released into the atmosphere. In this work, the elemental composition of PM2.5 was studied in two scenarios, namely in sugar-cane harvesting (HV) and in non-harvesting (NHV) seasons. The sampling strategy included one campaign in each season. PM2.5 was collected using a dichotomous sampler (10 L·min-1, 24 h) with PTFE filters. Information concerning the bulk elemental concentration was provided by energy-dispersive X-ray fluorescence. Enrichment factor analysis indicated that s, Cl, K, Cr, Ni, Cu, Zn, As, Cd and Pb were highly enriched relative to their crustal ratios (to Al). Principal component analysis was used to get some insight about the sources of the elements. Principal component 1 (PC1) explained 30.5% of data variance. The elements that had high loading (>0.7) were: s, Cr, As, and Pb;these are associated with combustion of fossil fuels. In principal component 2 (PC2), Cl, Cu, Zn, and Cd showed high loadings;these elements are associated with biomass burning. The Ni concentration found is three times larger than the threshold of risk for lung cancer, as recommended by the World Health Organization.

Heteroepitaxial growth is a process of profound fundamental importance as well as an avenue to realize nanostructures such as Ge/si quantum dots(QDs),with appealing properties for applications in opto-and ***,controlling the Ge/si QD size,shape,and composition remains a major obstacle to their practical ***,Ge nanostructures on si(111)were investigated in situ and in real-time by low energy electron microscopy(LEEM),enabling the observation of the transition from wetting layer formation to 3D island growth and *** island size,shape,and distribution depend strongly on the growth *** the deposition temperature increases,the islands become larger and sparser,consistent with Brownian nucleation and capture *** 550◦C,two distinct Ge/si nanostructures are formed with bright and dark appearances that correspond to flat,atoll-like and tall,faceted islands,*** annealing,the faceted islands increase in size at the expense of the flat ones,indicating that the faceted islands are thermodynamically more *** contrast,triangular islands with uniform morphology are obtained from deposition at 600◦C,suggesting that the growth more closely follows the ideal *** annealing,the islands formed at 600◦C initially show no change in morphology and size and then rupture simultaneously,signaling a homogeneous chemical potential of the *** observations reveal the role of dynamics and energetics in the evolution of Ge/si QDs,which can serve as a step towards the precise control over the Ge nanostructure size,shape,composition,and distribution on si(111).

The influences of life habits on the cardiovascularsystem may have important implications for publichealth, as cardiovascular diseases are among the lead-ing causes of shorter life expectancy worldwide. A linkbetween excessive ethyl alcohol(ethanol) consumptionand arterial hypertension was first suggested early lastcentury. since then, this proposition has received con-siderable attention. support for the concept of ethanolas a cause of hypertension derives from several epi-demiologic studies demonstrating that in the generalpopulation, increased blood pressure is significantlycorrelated with ethanol consumption. Although thelink between ethanol consumption and hypertension iswell established, the mechanism through which etha-nol increases blood pressure remains elusive. Possiblemechanisms underlying ethanol-induced hypertensionwere proposed based on clinical and experimental ob-servations. These mechanisms include an increase insympathetic nervous system activity, stimulation of therenin-angiotensin-aldosterone system, an increase of intracellular Ca2+ in vascular smooth muscle, increased oxidative stress and endothelial dysfunction. The pres-ent report reviews the relationship between ethanol intake and hypertension and highlights some mecha-nisms underlying this response. These issues are of interest for the public health, as ethanol consumption contributes to blood pressure elevation in the population.
