sleep and well-being have been intricately linked,and sleep hygiene is paramount for developing mental well-being and resilience.Although widespread,sleep disorders require elaborate polysomnography laboratory and patient-stay with sleep in unfamiliar environments.Current technologies have allowed various devices to diagnose sleep disorders at home.However,these devices are in various validation stages,with many already receiving approvals from competent authorities.This has captured vast patient-related physiologic data for advanced analytics using artificial intelligence through machine and deep learning applications.This is expected to be integrated with patients’Electronic Health Records and provide individualized prescriptive therapy for sleep disorders in the future.

Central venous catheter insertion in the internal jugular vein(IJV)is frequently performed in acute care settings,facilitated by its easy availability and increased use of ultrasound in healthcare settings.Despite the increased safety profile and insertion convenience,it has complications.Herein,we aim to inform readers about the existing literature on the plethora of complications with potentially disastrous consequences for patients undergoing IJV cannulation.

knee osteoarthritis is a degenerative disorder of the knee,which leads to joint pain,stiffness,and inactivity and significantly affects the quality of life.With an increased prevalence of obesity and greater life expectancies,total knee arthroplasty(TkA)is now one of the major arthroplasty surgeries performed for knee osteoarthritis.When enhanced recovery after surgery(ERAs)was introduced in TkA,clinical outcomes were enhanced and the economic burden on the healthcare system was reduced.ERAs is an evidence-based scientific protocol aimed at ameliorating the surgical stress response.ERAs aims to enhance the recovery phase,which encompasses multidisciplinary strategies at every step of perioperative care,including the rehabilitation phase.Implementation of ERAs in TkA aids in reducing the length of hospital stay,improving pain management,reducing perioperative complications,and enhancing patient satisfaction.Multidisciplinary collaboration,integrating the expertise of anesthesiologists,orthopedic surgeons,nursing personnel,and other healthcare professionals,is the cornerstone of ERAs in patients undergoing TkA.

Laparoscopic management of remnant gall bladder with stones: Lessons from a tertiary care centre's experience

Objective:Laparoscopic cholecystectomy is currently the gold standard for treating symptomatic gallstone disease.Despite its success,approximately 10%of patients may experience persistent biliary symptoms,leading to the post-cholecystectomy syndrome.A remnant gallbladder with cystic duct or bile duct stones is one of the causes of this syndrome.The objective of this study was to shed light on the clinical manifestations,evaluation,therapeutic strategies,and outcomes associated with laparoscopic management of symptomatic remnant gallbladders.Methods:This was a retrospective study,conducted over a five-year period(January 2017 to December 2022)at Apollo Hospitals in south India.All patients who underwent laparoscopic completion cholecystectomy for a remnant gall bladder were included.The following data were collected:patient demographics,symptoms,preoperative investigations,intraoperative details and post operative outcomes.Results:In total,36 patients were included and analysed.The majority of patients were male(25,69.4%),with a mean age of 50.7±12.1 years.The most common presentation was pain in the upper abdomen or right upper quadrant region(24,66.7%).The laparoscopic approach was attempted in all patients,with a success rate of 94.4%.Two patients required conversion to open surgery.Cholecystoenteric fistula to the colon was observed in one patient.Choledocholithiasis was observed in 7 patients(19.4%),and stone clearance was successfully achieved using endoscopic retrograde cholangiopancreatography in all patients preoperatively.Conclusion:Incomplete gall bladder removal either intentionally or unintentionally leaves a remnant gall bladder that is at risk for stone formation and infection.Patients who have this clinical entity with symptoms require a redo or complete cholecystectomy,a complex procedure associated with certain risks.This study highlights the feasibility and safety of laparoscopic completion cholecystectomy for the management of remnant gallbladder with cystic duct or bile duct stones.

Pain in chronic pancreatitis(CP)is difficult to manage.Many patients suffer from inadequate pain relief,completely incapacitating them in their daily activities.Historically,despite their well-known adverse effects,opioids have been the pillar of treatment regimens in painful CP.The management is now gradually evolving with a better understanding of the underlying pathophysiology of CP-related pain.Clinicians should follow a holistic approach to the management of CPassociated pain,which must involve lifestyle changes that are coupled with analgesic medications and other pain-relieving interventions.Furthermore,there is no easy cure for vanquishing CP-associated pain.Each patient must be evaluated on a case-by-case basis by a multidisciplinary team to decide which treatment option is best suited for that individual.

This paper presents the simulation results of DC,small-signal and noise properties of GaP based Double Drift Region( DDR) Impact Avalanche Transit Time( IMPATT) diodes. In simulation study we have considered the flat DDR structures of IMPATT diode based on GaP,GaAs,si and GaN( wurtzite,wz) material. The diodes are designed to operate at the millimeter window frequencies of 94 GHz and 220 GHz. The simulation results of these diodes reveal GaP is a promising material for IMPATT applications based on DDR structure with high break down voltage( V_B) as compared to si and GaAs IMPATTs. It is also encouraging to worth note GaP base IMPATT diode shows a better output power density of 4. 9 × 10~9 W/m^2 as compared to si and GaAs based IMPATT diode. But IMPATT diode based on GaN( wz) displays large values of break down voltage,efficiency and power density as compared to si,GaAs and GaP IMPATTs.

Realising the potential of Magnesium(Mg),several globally leading ventures have invested in the Mg industry,but their relatively poor corrosion resistance is a never ending saga till date.The corrosion and bio-corrosion behaviour of Mg has gained research attention and still remains a hot topic in the application of automobile,aerospace and biomedical industries.The intrinsic high electrochemical nature of Mg limits their utilization in diverse application.This scenario has prompted the development of Mg composites with an aim to achieve superior corrosion and bio-corrosion resistance.The present review enlightens the influence of grain size(Gs),secondary phase,texture,type of matrix and reinforcement on the corrosion and bio-corrosion behaviour of Mg composites.Firstly,the corrosion and bio-corrosion behaviour of Mg composites manufactured by primary and secondary processing routes are elucidated.secondly,the comprehensive corrosion and bio-corrosion mechanisms of these Mg composites are proposed.Thirdly,the individual role of Gs,texture and corrosive medium on corrosion and bio-corrosion behaviour of Mg composites are clarified and revealed.The challenges encountered,unanswered issues in this field are explained in detail and accordingly the scope for future research is framed.The review is presented from basic concrete background to advanced corrosion mechanisms with an aim of creating interest among the readers like students,researchers and industry experts from various research backgrounds.Indeed,the corrosion and bio-corrosion behaviour of Mg composites are critically reviewed for the first time to:(i)contribute to the body of knowledge,(ii)foster research and development,(iii)make breakthrough,and(iv)create life changing innovations in the field of Mg composite corrosion.

A Recommender system(Rs)is a crucial part of several firms,particularly those involved in e-commerce.In conventional Rs,a user may only offer a single rating for an item-that is insufficient to perceive consumer preferences.Nowadays,businesses in industries like e-learning and tourism enable customers to rate a product using a variety of factors to comprehend customers’preferences.On the other hand,the collaborative filtering(CF)algorithm utilizing AutoEncoder(AE)is seen to be effective in identifying user-interested items.However,the cost of these computations increases nonlinearly as the number of items and users increases.To triumph over the issues,a novel expanded stacked autoencoder(EsAE)with kernel Fuzzy C-Means Clustering(kFCM)technique is proposed with two phases.In the first phase of offline,the sparse multicriteria rating matrix is smoothened to a complete matrix by predicting the users’intact rating by the EsAE approach and users are clustered using the kFCM approach.In the next phase of online,the top-N recommendation prediction is made by the EsAE approach involving only the most similar user from multiple clusters.Hence the EsAE_kFCM model upgrades the prediction accuracy of 98.2%in Top-N recommendation with a minimized recommendation generation time.An experimental check on the Yahoo!Movies(YM)movie dataset and TripAdvisor(TA)travel dataset confirmed that the EsAE_kFCM model constantly outperforms conventional Rs algorithms on a variety of assessment measures.

kLnF_(4):Yb^(3+),Er^(3+)/Ho^(3+)(Ln=Y,Yb,Gd)phosphors were prepared by solid-state reaction technique.For each of the Yb^(3+)/Er^(3+)and Yb^(3+)/Ho^(3+)co-doped phosphors independently,phase confirmation and morphological examination were accomplished using the X-ray diffraction technique and scanning electron microscope analysis.X-ray photoelectron spectroscopy analysis shows the charge state and existence of lanthanides and dopants.Upconversion luminescence emissions of green and red colour were detected for the synthesized phosphors.The changes in the upconverted emission intensities and colour output pertaining to the change of the lanthanide host matrix along with dopants were investigated for the prepared phosphors.In the upconversion luminescence,erbium-doped samples exhibit a predominance of red emission while holmium-doped samples display a prevalence of green emission.The rare-earth Er/Ho doped kYbF4shows strong red emission compared to other hosts.kYF_(4):Yb,Er/Ho exhibits relatively significant green upconve rsion emissions,while Yb,Er-doped kGdF_(4)host displays a 10-fold increase in green/red emission ratio with an increase in the input power of 20-780 mW.Downshifting photoluminescence studies and decay curve analysis were carried out using UV light excitation for the developed phosphors.A higher lifetime of 2.2 ms was measured for the downshifting red emission in the Ho doped kYbF_(4)sample.This study suggests a potential for colour tunability in the upconversion and downshifting emissions through host modifications to the fluoride phosphor.
