Goals: To assess epidemiologic features and predictive factors of mortality of acute upper gastrointestinal bleeding (UGIB). study: During a 6-month period, a prospective population based study including all the UGB occurring in a geographic area of 3 million people was conducted. Data from cirrhotic patients were compared with those of noncirrhotic patients. Results: A total of 2,133 UGIB were recorded, 21.9%in cirrhotic patients (n = 468). Endoscopic hemostasis was performed in 46.5%and 8.3%in cirrhotic and noncirrhotic patients, respectively (P spitalization was 23.5%in cirrhotic patients and 11.2%in noncirrhotic patients (P six independent predictive factors of mortality were observed in both patient groups: a prothrombin level less than 40%, an UGIB occurring in inpatients, a concomitant digestive carcinoma, a hematemesis revealing the UGIB, a recent use of steroid drugs, and age over 60 years. Four other predictive factors of mortality were also identified in noncirrhotic patients. Conclusions: Although epidemiologic features, clinical course, management, and prognosis of UGIB were quite different in cirrhotic and noncirrhotic patients, the majority of predictive factors of mortality were the same in both patient groups. These data underline the major role of debilitated status and hepatic failure in the prognosis of UGIB in cirrhotic patients.

Molecular and Chemical Analyses of Cyanobacterial Blooms in Tropical Lagoons from southeast Brazil

The genetic diversity and the potential toxicity of bloom-forming cyanobacteria were studied in four lagoons located in the state of sao Paulo (Campinas, Limeira and Piracicaba cities). Bloom samples were collected on the water surface and cyanobacterial communities were evaluated using DGGE fingerprinting and 16s rDNA clone library. The amplification of genes encoding secondary metabolites such as microcystin (mcy), anatoxin (ana), cylindrospermopsin (cyr), saxitoxin (sxt), cyanopeptolin (mcn) and aeruginosin (aer) was performed and their production analyzed by LC-Ms. The comparison of DGGE banding pattern among the different water samples suggested that some operational taxonomic units (OTUs) in these locations were predominant over others. The 16s rDNA clone libraries sequences matched with nine different known cyanobacterial genera available in NCBI, identified as Anabaena, Brasilonema, Cylindrospermopsis, Limnococcus, Microcystis, Nostoc, Pseudanabaena, synechococcus and Woronichinia. The lagoons EsALQ2, Taquaral and Limeira had more than 80% of the cyanobacterial community assigned to the genus Microcystis. Genes encoding aeruginosin, cyanopeptolin and microcystin synthetases and saxitoxin synthase were amplified, and LC-Ms/Ms confirmed the production of aeruginosin, cyanopeptolin and microcystin. Rapid and sensitive methods for the detection of these secondary metabolites, especially toxins, using chemical and molecular tools together, can be used for a faster diagnostic of toxic cyanobacterial blooms.

The results of a study on Brassica carinata A. Braun, genotypes CT 204, in semi-marginal areas of southern Italy are shown in this paper. The research was carried out in three sites of Apulia region, Alberobello-Murgia of Bari（s1）, Troia-sub Apennine Dauno, Foggia（s2）, Monteroni- Area of salento, Lecce（s3） and one site of Basilicata region, Matera- Hill Materana（s4）. The B. carinata in all trial areas was submitted to different doses of nitrogen fertilization, no nitrogen supply, low doses of 50 and 60 kg N ha-1 and high doses of 100 and 120 kg N ha-1. The s2 and s4 sites were subjected to minimum and conventional tillage and in the s3 site, two sowing densities of 50 and 100 plant m2 were used. The site with the highest seed yield was s4 site（3.2 t ha-1） under conventional tillage in interaction with organic fertilizer and with or without mineral nitrogen addition. In the s1 site with the application of 100 kg N ha-1, although the marginality of the site due to shallow soil layer, seed yield was 1.8 t ha-1 and there was the highest percentage of oil content（41%）. In the s1 site, the effects of nitrogen fertilization were more noticeable which determined increases in seed yield and oil content proportional to the increases in N doses. The response of B. carinata to the two soil tillage methods was different in the two sites, minimum tillage increased seed yield in s2 while conventional tillage determined an increase in seed yield in s3.

Introduction: D-Aspartate is an endogenous amino acid involved in LH and testosterone release in humans. In this study we investigate the impact of nutritional supplementation of sodium D-aspartate on the improvement of sperm quality in sub-fertile patients and the rate of pregnancies that occurred with their partners. Materials and Methods: A group of 30 patients affected by oligo-asthenozoospermia and a group of 30 patients affected by asthenozoospermia were treated with a daily dose of sodium D-aspartate for 90 days. After which, the change in spermatozoa concentration and their motility and the pregnancies that occurred with their partners were recorded. Results: We found that the supplementation of D-aspartate significantly increased the concentration and the motility of spermatozoa. In oligo- asthenozoospermic patients the increase of sperm concentration was found to be 2.0-fold, P Conclusions:Treatment of sub-fertile patients with sodium D-aspartate improved the number and the motility of the spermatozoa and consequently improved the rate of pregnancies of their partners.

May the assessment of baseline mucosal molecular pattern predict the development of gluten related disorders among microscopic enteritis?

AIM To evaluate mucosal baseline m RNA expression of tissue transglutaminase 2(t TG2), interferon gamma(IFNγ), toll-like receptor 2(TLR2) and Myeloid Differentiation factor 88(MyD 88) in patients with microscopic enteritis(ME).METHODs We retrospectively enrolled 89 patients with ME of different etiology, which was defined within a 2-year mean period of follow-up. Baseline histological examination was performed on Hematoxylin-Eosin stained sections and CD3 lymphocyte immunohistochemistry was used for intraepithelial lymphocyte count(IELs). ME was defined according to the criteria of Bucharest Consensus Conference. For each patient, formalin embedded biopsy samples of the duodenum referred to the period of ME diagnosis were retrieved. Real-time polymerase chain reaction(RT-PCR) was used to detect the amount of mR NA coding for tT G2, IFNγ, TLR2 and My D88, and the quantity was expressed as fold change compared to controls. Control group was represented by duodenal normal specimens from 15 healthy subjects undergoing endoscopy for functional symptoms. Comparisons among continuous variables were performed by One way analysis of variance(ANOVA) and Bonferroni’s test. The χ~2 test was used for categorical variables. Pearson’s test was used to evaluate correlations. Receiver operating curves were drawn for all four markers to estimate sensitivity and specificity in discriminating the development of CD and *** After a period of follow up of 21.7 ± 11.7 mo, the following diagnoses were achieved: gluten related disorders in 48 subjects(31 CD; 17 Gs) and non-gluten related ones in 41(29 Irritable Bowel syndrome- IBs; 12 Others). CD patients had the highest tT G2 levels(8.3 ± 4.5). The ANOVA plus Bonferroni analysis showed that CD > Other ME > Gs = IBs > negative controls. A cut off value of 2.258 was able to discriminate between CD and Gs with a sensitivity of 52.94% and a specificity of 87.1%. Additionally, CD patients had the highest IFNγ levels(8.5 ± 4.1). Neurotrophic and metabotrophic potential of nerve growth factor and brain-derived neurotrophic factor: Linking cardiometabolic and neuropsychiatric diseases

One of biggest recent achievements of neurobiology is the study on neurotrophic factors. The neurotrophins are exciting examples of these factors. They belong to a family of proteins consisting of nerve growth factor(NGF), brain-derived neurotrophic factor(BDNF), neurotrophin-3(NT-3), NT-4/5, NT-6, and NT-7. Today, NGF and BDNF are well recognized to mediate a dizzying number of trophobiological effects, ranging from neurotrophic through immunotrophic and epitheliotrophic to metabotrophic effects. These are implicated in the pathogenesis of various diseases. In the same vein, recent studies in adipobiology reveal that this tissue is the body's largest endocrine and paracrine organ producing multiple signaling proteins collectively termed adipokines, with NGF and BDNF being also produced from adipose tissue. Altogether, neurobiology and adipobiology contribute to the improvement of our knowledge on diseases beyond obesity such as cardiometabolic(atherosclerosis, type 2 diabetes, and metabolic syndrome) and neuropsychiatric(e.g., Alzheimer's disease and depression) diseases. The present review updates evidence for(1) neurotrophic and metabotrophic potentials of NGF and BDNF linking the pathogenesis of these diseases, and(2) NGF- and BDNF-mediated effects in ampakines, NMDA receptor antagonists, antidepressants, selective deacetylase inhibitors, statins, peroxisome proliferator-activated receptor gamma agonists, and purinergic P2X3 receptor up-regulation. This may help to construct a novel paradigm in the field of translational pharmacology of neuro-metabotrophins, particularly NGF and BDNF.
