Neuroligins（NLs） are postsynaptic cell-adhesion molecules essential for normal synapse *** in neuroligin-4（NL4）（gene symbol:NLGN4） have been reported in some patients with autism spectrum disorder（AsD）and other neurodevelopmental ***,the low frequency of NL4 mutations and the limited information about the affected patients and the functional consequences of their mutations cast doubt on the causal role of NLA mutations in these ***,we describe two brothers with classical AsD who carry a single amino-acid substitution in NL4（R87W）.This substitution was absent from the brothers’ asymptomatic parents,suggesting that it arose in the maternal germ line.R87 is conserved in all NL isoforms,and the R87W substitution is not observed in control *** the protein level,the R87W substitution impaired glycosylation processing of NL4 expressed in HEK293 and COs cells,destabilized NL4,caused NL4 retention in the endoplasmic reticulum in non-neuronal cells and neurons,and blocked NLA transport to the cell *** a result,the R87Wsubstitution inactivated the synapse-formation activity of NLA and abolished the functional effect ofNLA on synapse *** together,these observations suggest that a point mutation in NLA can cause AsD by a loss-of-function mechanism.

Alzheimer’s disease (AD) is a brain disorder that eventually causes memory loss and the ability to perform simple cognitive functions;research efforts within pharmaceuticals and other medical treatments have minimal impact on the disease. Our preliminary biological studies showed that Repeated Electromagnetic Field stimulation (REFMs) applying an EM frequency of 64 MHz and a specific absorption rate (sAR) of 0.4 - 0.9 W/kg decrease the level of amyloid-β peptides (Aβ), which is the most likely etiology of AD. This study emphasizes uniform E/H field and sAR distribution with adequate penetration depth penetration through multiple human head layers driven with low input power for safety treatments. In this work, we performed numerical modeling and computer simulations of a portable Meander Line antenna (MLA) to achieve the required EMF parameters to treat AD. The MLA device features a low cost, small size, wide bandwidth, and the ability to integrate into a portable system. This study utilized a High-Frequency simulation system (HFss) in the design of the MLA with the desired characteristics suited for AD treatment in humans. The team designed a 24-turn antenna with a 60 cm length and 25 cm width and achieved the required resonant frequency of 64 MHz. Here we used two numerical human head phantoms to test the antenna, the MIDA and spherical head phantom with six and seven tissue layers, respectively. The antenna was fed from a 50-Watt input source to obtain the sAR of 0.6 W/kg requirement in the center of the simulated brain tissue layer. We found that the E/H field and sAR distribution produced was not homogeneous;there were areas of high sAR values close to the antenna transmitter, also areas of low sAR value far away from the antenna. This paper details the antenna parameters, the scattering parameters response, the efficiency response, and the E and H field distribution;we presented the computer simulation results and discussed future work for a practical model.

Introduction: The exposure to shift works, and especially to night shift, goes against the circadian rythmicity of the social man, which brings about a multitude of disruptive effects on health. In a hospital environment, such night shift is necessary so as to ensure the continuity of care. sleep and vigilance disorders related to this work mode are frequent and often pose adaptation problems. The objective of this study is to detect the harmful effects of night shift on sleep, vigilance and the quality of life of nurses in Farhat Hached Teaching Hospital in sousse. Material and Methods: It is a cross-sectional study carried out among two groups of nurses: one group performing a fixed night work and the other one working permanently during the day at Farhat Hached Teaching Hospital in sousse. Data collection is mainly based on validated questionnaires evaluating sleep quality (Pittsburg sleep quality index), daytime sleepiness (Epworth sleepiness scale), sleep typology (Horne and Ostberg questionnaire), and the quality of life (sF-12). An objective evaluation of vigilance was realized by the study of reaction time and error rates to different tasks of “super lab” software. Results: 50 night and 50 day nurses have been colligated. Both groups were age- and gender-matched. sleep quality mean index was significantly higher among night nurses in comparison with day nurses (10.2 ± 5.9 versus 6.76 ± 5.16). After multivariate analysis, sleep quality was significantly correlated to age, department, schedule and the choice of schedule. Night shift was not related to daytime sleepiness. The objective analysis of vigilance showed that the reaction time to different tasks exploring the attentional mechanisms, as well as errors rate, were significantly more important among the night group. The mental score of the quality of life was significantly higher among day nurses. However, such difference was not significant with regard to physical score. Conclusion: The findings of our s

EMF Antenna Exposure on a Multilayer Human Head simulation for Alzheimer Disease Treatments

In this paper, we follow up with our preliminary biological studies that showed that Repeated electromagnetic field stimulation (REMFs) decreased the toxic amyloid-beta (Aβ) levels, which is considered to be the cause of Alzheimer’s disease (AD). The REMFs parameters of these exposures were a frequency of 64 MHz and a specific absorption rate (sAR) of 0.4 to 0.9 W/Kg in primary human neuronal cultures. In this work, an electromagnetic field (EMF) model was simulated using high-frequency simulation system (HFss/EMPro) software. Our goal was to achieve the EM parameters (EMF Frequency and sAR) required to decrease the toxic Aβ levels in our biological studies in a simulated human head. The simulations performed here will potentially lead to the successful development of an exposure system to treat Alzheimer’s disease patients. A popular VFH (very high frequency) patch microstrip antenna system was considered in the study. The selection was based on simple and easy construction and appropriateness to the VHF applications. The evaluation of the sAR and temperature distribution on the various head layers, including skin, fat, dura, the cerebrospinal (CsF), and grey matter, brain tissues, were determined for efficacy sAR and safety temperature increase on a simulated human head. Based on a current pulse of 1 A peak current fed to the antenna feeder, a maximum sAR of 0.6 W/Kg was achieved. A range of 0.4 to 0.6 sAR was observed over the various layers of the simulated human head. The initial design of the antenna indicated an antenna size in the order of 1 m in length and width, suggesting a stationary practical model for AD therapy. Future direction is given for wearable antenna and exposure system, featuring high efficiency and patient comfort.

Buerger’s Disease and Fitness for Work in a Flight Personnel: A Case Report

Buerger’s disease, also called thromboangiitis obliterans (TAO), is an inflammatory disease affecting small and medium size blood vessels of the limbs. several risk factors are strongly associated with this disease. In fact, heavy smokers represent 99% of patients diagnosed with Buerger’s disease and smoking cessation can slow down the progression of the pathology. Ulcers and gangrene are very common complications and TAO may also cause a vasculitis as well as ischemic damage to the distal part of the limbs that can eventually lead to limb amputation. Here, we report the case of a 43 years old patient, a flight personnel working in a Tunisian airline company who consults for a lesion of the left toe that appeared 2 weeks ago. He is a chronic inveterate smoker with 36 pack-years. The diagnosis of TAO was made based on the clinical presentation and the results of the Doppler Ultrasound of the left toe. This disease has a major impact on the quality of life due to the risk of necrotic ulcers and amputation. Navigation is a profession that requires a high level of physical and mental fitness. According to the international and Tunisian standards, the applicant or the holder of medical certificate must not have a cardiovascular condition that may affect the safety of the exercise.

AIM To evaluate the association between C-reactive protein(CRP) and radiological evidence of lower respiratory tract infection(LRTI) in *** All patients aged less than 4 years who presented with suspected lower respiratory tract infection,who received a peri-presentation chest radiograph and CRP blood measurement over an 18-mo period were included in the ***,gender,source of referral,CRP,white cell count,neutrophil count along with the patients' symptoms and radiologist's report were *** Three hundred and eleven patients met the *** chest radiographs were more common in patients with elevated CRP levels(P signs of LRTI were identified in 73.7% of chest radiographs when a patient had a CRP level between 50-99 mg/*** levels were a better predictor of positive chest radiograph findings for those aged greater than I year compared to those 1 year or *** CRP may be used in patients with suspected LRTI diagnosis to select those who are likely to have positive findings on chest radiograph,thus reducing unnecessary chest radiographs.
