Background:Informed consent has medicolegal implications and is imperative in presentday medical *** and Objectives:The objectives of this study were to estimate the awareness regarding informed consent in interns and to determine the effectiveness of sessions in interns’orientation programs about informed *** and Methods:Pretest-Posttest study was conducted with a validated questionnaire distributed through Google Forms to interns attending the orientation program at our *** were recorded on the Likert scale(−2 to+2).Paired t-test was used to compare the mean pretest and posttest *** number of interns with a positive score(+1,+2)for every question in pretest and posttest were also ***:One hundred and seventyone out of 232 interns who attended the program submitted both pretest and posttest *** the pretest,12 interns scoreds scored more than 75%.A significant increase in posttest responses was seen in the mean score(Pstions related to consent in research,radiological investigations,regarding the need for consent in the additional nonlifesaving *** was an increase in the number of interns having a positive score(+1,+2)for all the questions in posttest as compared to pretest,but a statistically significant increase was seen in questions related to consent in ultrasound and need for consent in the additional nonlifesaving ***:A wellstructured interns’orientation program is effective in improving awareness regarding *** gaps in knowledge regarding various aspects of consent need to be emphasized.

Design of a Proportional-Integral-Derivative Controller for an Automatic Generation Control of Multi-area Power Thermal systems Using Firefly Algorithm

Essentially, it is significant to supply the consumer with reliable and sufficient power. since, power quality is measured by the consistency in frequency and power flow between control areas. Thus, in a power system operation and control,automatic generation control(AGC) plays a crucial role. In this paper, multi-area(Five areas: area 1, area 2, area 3, area 4 and area 5) reheat thermal power systems are considered with proportional-integral-derivative(PID) controller as a supplementary controller. Each area in the investigated power system is equipped with appropriate governor unit, turbine with reheater unit, generator and speed regulator unit. The PID controller parameters are optimized by considering nature bio-inspired firefly algorithm(FFA). The experimental results demonstrated the comparison of the proposed system performance(FFA-PID)with optimized PID controller based genetic algorithm(GAPID) and particle swarm optimization(PsO) technique(PsOPID) for the same investigated power system. The results proved the efficiency of employing the integral time absolute error(ITAE) cost function with one percent step load perturbation(1 % sLP) in area 1. The proposed system based FFA achieved the least settling time compared to using the GA or the PsO algorithms, while, it attained good results with respect to the peak overshoot/undershoot. In addition, the FFA performance is improved with the increased number of iterations which outperformed the other optimization algorithms based controller.

Objective:To identify the larvicidal activities of silver nano particles synthesised with Rhizophora mucronata（***） leaf extract against the larvae of Aedes aegypti（***） and Culex quinquefasciatus（***）.Methods:In vitro larvicidal activities such as LCsub>50sub> and LCsub>90sub> were assessed for the *** and *** larval ***, characterisation such as UV,XRD,FTIR and AFM analysis were carried out for the synthesised silver nano ***:The LCsub>50sub> value of the synthesised silver nano particle was identified as 0.585 and 0.891 mgg/L for *** and *** larvae respectively. Further,the LGsub>90sub> values are also identified as 2.615 and 6.291 mg/L for *** and Cx. quinquefasciatus species *** synthesised silver nanoparticles have maximum absorption at 420 nm with the average size of 60-95 *** XRD data showed 20 intense values with various degrees such as 37.10°,47.66°,63.97°and 70.01°.The FTIR data showed prominent peaks in（3 426.89,2 925.49,2 869.56,2 346.95,1 631.49,1 031.73,669.18 and 455.12） different ***:The biosynthesis of silver nanoparticles with leaf aqueous extract of R. mucronata provides potential source for the larvicidal activity against mosquito borne diseases.

Effect of various pesticides (insecticides, fungicides and herbicides) has been studied on growth and efficiency of symbiotic properties of 3 fast growing Rhizobium sp. under green house conditions. The results revealed adverse effects on plant growth and nitrogen fixing capacity as measured by dry weight and total nitrogen content of plants infected with pesticide treated Rhizobium. Of the pesticides tested, herbicides were found to be more effective on the above parameters than the insecticides and fungicides.

Comparative studies of Different Organs of Nyctanthes arbortristis in Modulation of Cytokines in Murine Model of Arthritis

Objective To study the modulation effect of pro- and anti-inflammatory cytokines following long term use of water soluble ethanol extracts from different organs of Nyctanthes arbortristis （NAT） in mouse model of arthritis. Methods Arthritis was induced in mice by two injections of Freund＇s complete adjuvant on days 0 and 12 in the sub-planter surface of the right hind paw. Results Injection of adjuvant resulted in a maximum primary edema of the footpad with erythema, and edema and distortion of joints of the right hind paw after 24-48 hours. second injection of FCA led to the formation of secondary swellings persisting more than four weeks that spread onto the other hind limb but to a lesser extent. Histological analysis of the ankle on day 47 showed marked evidence of cartilage destruction in association with pannus formation and moderate bone resorption. Proinfiammatory cytokine levels in the inflamed joint homogenate were elevated on days 2, 14, and 47. Oral administration of leaf and fruit extracts in arthritic mice reduced joint homogenate levels of tumor necrosis factor-α, interleukin-1β, and interleukin-6 on days 2, 14, and 47 in comparison to untreated arthritic mice. Intedeukin-10 level was elevated in the inflamed joint on days 2, 14, and 47 in comparisons to untreated arthritic mice. Confusion Evidence of lesser inflammation of the footpad and joint and associated histological observation support the therapeutic benefit of leaf and fruit extracts from Nyctanthes arbortristis. This study helps in understanding the mechanism of anti-inflammatory action of Nyctanthes arbortristis in the light of pro- and anti-inflammatory cytokine balance.

Dysphagia after vertical sleeve gastrectomy: Evaluation of risk factors and assessment of endoscopic intervention

AIM To evaluate the risks of medical conditions, evaluate gastric sleeve narrowing, and assess hydrostatic balloon dilatation to treat dysphagia after vertical sleeve gastrectomy(VsG).METHODs VsG is being performed more frequently worldwide as a treatment for medically-complicated obesity, and dysphagia is common post-operatively. We hypothesize that post-operative dysphagia is related to underlying medical conditions or narrowing of the gastric sleeve. This is a retrospective, single institution study of consecutive patients who underwent sleeve gastrectomy from 2013 to 2015. Patients with previous bariatric procedures were excluded. Narrowing of a gastric sleeve includes: inability to pass a 9.6 mm gastroscope due to stenosis or sharp angulation or spiral hindering its *** Of 400 consecutive patients, 352 are included; the prevalence of dysphagia is 22.7%; 33 patients(9.3%) have narrowing of the sleeve with 25(7.1%) having sharp angulation or a spiral while 8(2.3%) have a stenosis. All 33 patients underwent balloon dilatation of the gastric sleeve and dysphagia resolved in 13 patients(39%); 10 patients(30%) noted resolution of dysphagia after two additional dilatations. In a multivariate model, medical conditions associated with post-operative dysphagia include diabetes mellitus, symptoms of esophageal reflux, a low whole blood thiamine level, hypothyroidism, use of non-steroidal anti-inflammatory drugs, and use of *** Narrowing of the gastric sleeve and gastric sleeve stenosis are common after VsG. Endoscopic balloon dilatations of the gastric sleeve resolves dysphagia in 69% of patients.

Management of hepatitis C virus infection in HIV/HCV co-infected patients: Clinical review

Nearly one fourth of individuals with human immunodeficiency virus (HIV) infection have hepatitis C virus (HCV) infection in the Us and Western Europe. With the availability of highly active antiretroviral therapy and the consequent reduction in opportunistic infections, resulting in the prolongation of the life span of HIV-infected patients, HCV co-infection has emerged as a signif icant factor influencing the survival of HIV patients. Patients with HIV/HCV co-infection have a faster rate of fibrosis progression resulting in more frequent occurrences of cirrhosis, end-stage liver disease, and hepatocellular carcinoma. However, the mechanism of interaction between the two viruses is not completely understood. The treatment for HCV in co-infected patients is similar to that of HCV monoinfection; i.e., a combination of pegylated interferon and ribavirin. The presence of any barriers to antiHCV therapy should be identified and eliminated in order to recruit all eligible patients. The response to treatment in co-infected patients is inferior compared to the response in patients with HCV mono-infection. The sustained virologic response rate is only 38% for genotype-1 and 75% for genotype-2 and -3 infections. Liver transplantation is no longer considered a contraindication for end-stage liver disease in coinfected patients. However, the 5 year survival rate is lower in co-infected patients compared to patients with HCV mono-infection (33% vs 72%, P = 0.07). A better understanding of liver disease in co-infected patients is needed to derive new strategies for improving outcome and survival.
