Aim: To correlate the microcalcifications' characteristics, such as morphology and elemental compositions, with the occurrence of bone metastatic lesions at 5 years from diagnosis. Methods: In this retrospective study, we enrolled 70 patients from which we collected one breast biopsy each. From each biopsy, paraffin serial sections were obtained to perform histological classifications, immunohistochemical analyses and Energy Dispersive X-ray evaluation. Results: Microcalcifications analysis showed a significant association between the presence of calcium crystals made of magnesium substituted hydroxyapatite and the development of bone metastasis from 5 years from diagnosis. No significant association was observed by evaluation the morphological appearance of microcalcifications. Immunohistochemical analysis displayed a significant association between the expression of bone morphogenetic proteins 2 and pentraxin-3, two osteoblast induction factors, and the formation of bone metastatic lesions. Conclusion: Results here reported highlighted the possible use of breast microcalcifications as a negative prognostic marker of bone metastatic diseases. In particular, the association between elemental composition of breast microcalcifications and the formation of bone lesions can lay the foundation for the development of new in vivo diagnostic tools based on the analysis of microcalcifications and capable to predict the formation of bone metastasis.

Huge Recalcitrant Warts Treated with Pulse Dye Laser after Conventional Therapies and Others Destructive Physical Modalities Failed

Cutaneous infections with the human papilloma virus, despite being almost always asymptomatic, can induce slow-growing lesions knowed as warts that can remain subclinical for long periods of time. There are several therapeutical options namely keratolytics, retinoids, imiquimod, cryotherapy, electrosurgery, lasers and classic surgery, but none has antiviral effects. We describe the case of a patient with large warts located on the dorsal surface of the right hand, treated with cryotherapy, CO2 laser and topical imiquimod having either no clinical response or quick relapse. Treatment with PDL was started and after 13 sessions, the patient was without lesions, with a good cosmetic result and so far without relapse. This case report is relevant to us and deserves to be published because of the size of the lesions and the great response to PDL despite all the failures with the other treatments.

AIM:To assess linear endoscopic ultrasound (L-EUs) and magnetic resonance imaging (MRI) in biliary tract dilation and suspect small ampullary ***:L-EUs and MRI data were compared in 24 patients with small ampullary tumors;all with subsequent histological *** were collected prospectively and the accuracy of detection,histological characterization and N staging were assessed retrospectivelyusing the results of surgical or endoscopic treatment as a ***:A suspicion of ampullary tumor was present in 75% of MRI and all L-EUs examinations,with 80% agreement between EUs and histological findings at ***,L-EUs and histological TN staging at surgery showed moderate agreement (κ=0.54).CONCLUsION:L-EUs could be a useful adjunct as a diagnostic tool in the evaluation of patients with sus-pected ampullary tumors.

Carbon sequestration capability of Fagus sylvatica forests developing in the Majella National Park (Central Apennines,Italy)

Terrestrial ecosystems represent a major sink for atmospheric carbon(C) and temperate forests play an important role in global C cycling, contributing to lower atmospheric carbon dioxide(CO2) concentration through photosynthesis. The Intergovernmental Panel of Climate Change highlights that the forestry sector has great potential to decrease atmospheric CO2concentration compared to other sectoral mitigation activities. The aim of this study was to evaluate CO2sequestration(CO2s)capability of Fag us sylvatica(beech) growing in the Orfento Valley within Majella National Park(Abruzzo,Italy). We compared F. sylvatica areas subjected to thinning(one high-forest and one coppice) and no-management areas(two high-forests and two coppices). The results show a mean CO2s of 44.3 ± 2.6 Mg CO2ha-1a-1,corresponding to 12.1 ± 0.7 Mg C ha-1a-1the no-managed areas having a 28% higher value than the managed areas. The results highlight that thinning that allows seed regeneration can support traditional management practices such as civic use in some areas while no management should be carried out in the reserve in order to give priority to the objective of conservation and naturalistic improvement of the forest heritage.

A simple treatment plan for manual therapists is presented based on current evidence-based literature, it is designed to lessen chronic pain and inflammation in the Irritable Bowel syndrome (IBs). A chronic continuous or intermittent gastrointestinal tract dysfunction, IBs appears due to dysregulation of brain-gut-microbe communication. An overview of its management using Osteopathic Manipulative Treatment (OMT) is described. In IBs OMT focuses on the nervous and circulatory systems, spine, viscera, thoracic and pelvic diaphragms in order to restore homeostatic balance, normalize autonomic activity in the intestine, promote lymphatic flow and address somatic dysfunction. Lymphatic and venous congestion is treated by the Lymphatic Pump Techniques and stimulation of Chapman’s Reflex Points. The food itself, food allergies and intolerance could contribute to symptom onset or even cause IBs. Furthermore the “microbiota” greatly impacts on the bi-directional brain-gut axis communication. This paper also provides appropriate dietary modifications for patients with IBs.

sustainable development is a common goal for all countries since the concept was adopted at the 1992 Rio Conference. European Union (EU) became a world leader in most of indicators despite successive enlargement of country members with newcomers often facing basic environmental problems. In 1986, Portugal became a member of the European Economic Community (EEC), and immediately began to reflect its environmental policies. Over the last 30 years, the support of EU funds and the implementation of environmental public policies in Portugal, have been essential for the country’s sustainable development. As a result, Portugal’s standards started to be closer to those of the EU and, in some cases even exceeded it. The present article addresses, for the first time, a long series of 30 years (1987-2017), in a retrospective analysis of a set of indicators, in order to characterize the evolution of Portugal’s environmental sustainability, its relationship to the national public policies, and in the context of the EU. The progress of the indicators towards specific environmental sustainability targets is also evaluated. Moreover, a set of key indicators that also evaluate the goals of the 2030 Agenda for sustainable Development are identified. The results show that the evolution of Portugal’s environmental sustainability in the last 30 years was remarkable. The water standards, air quality, urban waste, and energy sectors showed high improvement. However, the evolution in the sectors such as land use, and environmental risks, particularly forest fires, were negative. On the other hand, for some indicators, such as renewable energies or bathing water quality, Portugal performed better than the EU-28. Environmental performance shows the positive influence of EU environment policies and structural support, changing dramatically the quality of the environment in Portugal, from a negative situation to a success case study in a generation period.
