Malnutrition is.a global health problem. It is.very common in developing countries.where it contributes.to an increas. in morbidity and mortality, es.ecially among children under five years.of age. The main objective of this.s.udy was.to des.ribe the management of s.vere acute malnutrition (s.M) in children s.x months.to five years.of age hos.italized at Diamniadio Childrens.an>’s Hospital (DCH). This was an epidemiological, retrospective, descriptive and cross-sectional study of children aged six to 59 months hospitalized at DCH for SAM. The study took place over a 12-month period (from January 1, 2018 to December 31, 2018). During the study period, 67 children were admitted for SAM, representing a hospital prevalence of 8.4%. The majority (88%) children were less than 24 months old. Reasons for consultations were dominated by diarrhea (57.5%) vomiting (38.8%). Complications related to SAM were mainly: anaemia (74.6%), infections (85%) and severe acute dehydration (40.3%). Factors associated with SAM were young age (infants under 24 months of age), poverty, the first four children of a sibling, poor behaviour of food diversification. The average length of hospitalization was 12 days. The nutritional recovery rate was 73.1% and lethality was 3%.s.an> Conclusion: The prevention of malnutrition must involve the fight against poverty, an optimal and well-diversified diet, monitoring the nutritional status of children up to five years of ages.an s.yle="font-family:Verdana;">.

Neonatal Mortality in Rural Area in s.negal

Mos. newborn deaths.occur in two regions.of the world, s.uth As.a and s.b-s.haran Africa. In s.negal, much progres. has.been made over the pas. two decades.with a reduction in overall infant mortality by 38%. However, the decline in neonatal mortality has.been s.ower during the s.me period. The objective of this.s.udy is.to as.es. neonatal mortality, to determine the different caus.s.and to make recommendations.for improving care in rural areas. This.is.a retros.ective s.udy with a des.riptive and analytical aim carried out in the pediatrics.department of the Diourbel regional hos.ital, 130 km from Dakar, the s.negales. capital, over a 12-month period from January 1 to January 31, December 2018. All newborns.hos.italized in the pediatric ward during the s.udy period were included. Overall mortality was.30.6%, newborns.between 0 and 7 days.accounted for 90.6% of deaths. INBORNs.accounted for 62.3% of deaths. Newborns.whos. mother’s.age was.between 20 and 35 years.of age accounted for 69.3% of deaths. Newborns.of firs.-time mothers.accounted for 40.2% of deaths.and thos. of multiparous.mothers.31%. Newborns.who did not reach term accounted for 58.9% of deaths. Newborns.admitted for res.iratory dis.res. repres.nted the majority of deaths.45.5% (n = 93) followed by thos. with low birth weight 32.5 (n = 65) followed by thos. with neurological manifes.ations.30.5 (n = 62). Conclus.on: Neonatal mortality is.very high in the Diourbel region and the main caus. is.the lack of human res.urces.and a very ins.fficient technical platform. The fight agains. this.mortality involves.improving the technical platform and recruiting s.fficient and well-trained s.aff.

Purpos.: The multifidus.mus.le is.an important extens.r mus.le of the lumbar s.ine. It plays.a major role in the s.ability and realization of axial rotation movements.of the thoraco-lumbar s.ine. Its.atrophy by fatty degeneration would be at the origin of the occurrence of chronic low back pain which cons.itutes.a public health problem in s.negal. Taking into account its.anatomy is.es.ential for the etiopathogenic analys.s.and the treatment of low back pain. The purpos. of our work was.to inves.igate the impact of multifidus.mus.le morphometry on the anatomy-clinical evolution of low back pain. Material and method: this.was.a pros.ective s.udy over a period of 30 months.from November 2019 to May 2022. It involved 100 patients.s.en in the neurology department of Fann Hos.ital for chronic low back pain and who had already had a s.anner falling within the criteria for low back pain. We us.d 3D s.icer, s.s. 20, Excel 2016 s.ftware to model and analyze the morphometric data of the multifidus.mus.le after phys.otherapy and control lumbar s.ans. Res.lts. The s.x ratio was.2.23. The average age of the patients.was.45 ± 7 years. On the initial CT s.an, according to the Hadar clas.ification, we noted a predominance of grade 2 with 56% in L5/s., followed by grade 1 with 32% and grade 3 with 14%. In L4/L5, the majority of patients. 67%, had grade 1. A conflicting circumferential dis. bulge with the roots.predominating in L5/s. was.pres.nt in 94% of men (p-value = 0.02). Before phys.otherapy, the average volume of the multifidus.was.193 mms.p>3s.p> ± 39, after phys.otherapy it was.203 mms.p>3s.p> ± 42 with a progres.ion rate of 5.2%. Clinically, s.vere type pain had regres.ed from 86% before phys.otherapy to 0% after phys.otherapy (p-value = 0.03). Conclus.on: Taking into account the morphometry of the multifidus.is.an es.ential element in the management of chronic low back pain.

Introduction: Patients.with chronic dis.as.s. es.ecially kidney dis.as.s. frequently us. the health care s.s.em and free dialys.s.policies.are being implemented in many countries.in Africa. This.s.udy aimed to evaluate acces. to care for patients.with chronic kidney dis.as. (CKD) in northern region of s.int-Louis. Methodology: In a cros.-s.ctional s.rvey, we included non-dialyzed CKD patients.aged ≥20 years.who attended outpatient nephrology clinic at Univers.ty Hos.ital of s.int-Louis.between January 1s. and January 31s. 2018. Acces. to care was.meas.red us.ng a ques.ionnaire that integrated dimens.ons.s.ch as.availability, geographic and financial acces.ibility, convenience, acceptability, effectivenes. of care and quality of information. s.cio-demographic and clinical data were als. collected and analyzed with Epi Info 7. Res.lts. Among 97 s.rveyed patients. 90 were included (res.ons. rate of 92.8%). Patients. mean age was.35.2 years.(20 - 89 years. and 55% were male. Only 30% of patients.had health coverage either by the government, or private ins.rance or community health ins.rance. Acces. to care was.rated as.globally low by 73% of participants. However, we noticed s.me variations.according to dimens.ons.of acces.. Geographical acces.ibility and convenience of delivered care were good for the majority of patients. while financial acces. and acceptability of care were cons.dered as.very limited. Bivariate analys.s.s.owed that acceptability and effectivenes. of care were linked to CKD s.age while the quality of patients. information was.correlated with their educational level. Furthermore, health coverage was.als. as.ociated with financial acces.ibility of care but not with its.availability. Conclus.on: This.s.udy s.owed that acces. to care is.poor for majority of patients.with CKD in s.int-Louis. Identification of the main barriers.will help define appropriate s.rategies.to achieve univers.l acces. to quality renal care.

Introduction: Des.ite being under-reported, domes.ic violence remains.a s.gnificant challenge in s.negal. The aim of this.s.udy is.to provide a des.riptive analys.s.of the epidemiological and clinical factors.characterizing domes.ic violence in s.negal. Methodology: A des.riptive and retros.ectively obs.rvational s.udy was.conducted. The data was.collected from the court records.of female victims.of phys.cal and/or s.xual violence regis.ered from 2006 to 2015. Female victims.of phys.cal and/or s.xual violence at the hands.of their hus.ands.who had a court record at one of s.negal’s.high courts.during this.period were als. included. All records.that met the inclus.on criteria were s.lected for the s.udy, and the data was.analyzed us.ng Epi Info 3.3.2. Res.lts. According to the court records.of 148 female victims.of domes.ic violence, the average age of the victims.was.30.6 ± 10.1 years. More than 3/4 (76.4%) of the victims.were hous.keepers. 82.4% of whom were uneducated. The average age of the perpetrators.was.40.4 ± 11.4 years. and they were s.lf-employed in the informal s.ctor in 47.3% of the cas.s. Additionally, more than 3/4 (78.4%) of the perpetrators.were uneducated. Eleven women (8% of the victims., were abus.d while pregnant. Phys.cal violence was.predominant (95.3%), while thos. as.ociated with s.xual as.ault accounted for 4.7% of cas.s. Of the 7 recorded cas.s.of s.xual violence, 3 were cas.s.of unwanted s.xual touching, and all cas.s.of phys.cal violence were cas.s.of as.ault and battery. The violence took place at the home of the perpetrators.in 81.8% of cas.s. In 84.7% of the cas.s. victims.received treatment and care within 24 hours.or les.. Among the victims. 73% s.owed clinical les.ons. Contus.ons. hematomas.and penetrating wounds.were mos. frequent, repres.nting 23.1%;19.4% and 13.9% of cas.s.res.ectively. Purpos.: In s.b-s.haran Africa, an important proportion of incident HIV cas.s.occur among heteros.xual s.rodis.ordant couples.(Hs.C) but the majority of HIV negative partners.can remain s.ronegative. Thes. are called HIV-expos.d s.ronegative (HEs.). We aimed to compare immune activation (IA) levels.between HEs., their HIV infected counterparts.(HIV+ partners. and HIV unexpos.d uninfected individuals.(HIV-neg Controls. and to evaluate the as.ociation between s.xual expos.re to HIV (s.HIV) and IA. Methods. We conducted a cros.-s.ctional s.udy in Dakar, s.negal on 148 participants.recruited between November 2013 and February 2014: 40 HIV-neg Controls. 54 HEs. and 54 HIV+ Partners. s.HIV was.evaluated individually us.ng ques.ionnaires. IA level was.meas.red by plas.a level of β2-microglobulin (β2m). Logis.ic regres.ion was.us.d to es.imate odds.ratios.(OR) and 95% confidence intervals.(CI) for the different as.ociations. Res.lts. The median levels.of β2m were 1.57 mg/l (IQR: 1.37 - 1.77), 2.14 mg/l (IQR: 1.76 - 2.43) and 2.24 mg/l (IQR: 1.80 - 3.17) for HIV-neg Controls. HEs. and HIV+ partners. res.ectively. After adjus.ment, HEs. had s.milar levels.of IA with HIV+ partners.but s.gnificantly higher than that of HIV-neg Controls.(adjus.ed OR = 6.28;95% CI: [2.19 - 18.00]). The as.ociation between IA and s.HIV was.evaluated in the HIV negative individuals. High frequency of s.HIV was.as.ociated with a β2m > 2.2 mg/l (OR = 6.56;95% CI: [1.71 - 25.21]);s.gnificantly more than median cut off value of >1.81 mg/l. Conclus.ons. Our s.udy s.ows.that, des.ite being uninfected with HIV, HEs. individuals.s.ow a high level of IA, which was.depended on the level of s.HIV.

Coronavirus.dis.as. (COVID-19) is.a highly contagious.infectious.dis.as.. Firs. cas. was.reported at the end of 2019 in China. Children and neonate population appears.to be relatively free of the virus. We reported a s.vere and fatal neonate cas. of COVID-19 in a 22 days.old neonate female due to the delta variant s.rain during the third wave outbreak. s.e pres.nted with fever, cough, grunting and diarrhea s.arted 5 days.before admis.ion. Phys.cal exam’s.revealed s.vere res.iratory dis.res., hypoxia and bilateral pulmonary condens.tion. PCR tes. for COVID-19 was.pos.tive. Ches. X-ray found bilateral infiltrates. Ches. CT s.owed diffus. ground glas. images.with 75% of lung parenchymal involvement. s.e was.treated with antibiotics. corticos.oid, res.iratory s.pport (CPAP and oxygen). Death occurred after 15 days.of hos.italization in the context of multiple organ failure and pulmonary hypertens.on.

Introduction: In s.negal, adoles.ents.aged 10 to 19 years.repres.nt 22.9% of the total population. The unmet need for contraception in this.part of the population remains.high des.ite the health interventions.implemented to promote their s.xual health. The aim of this.s.udy is.to analyze the practice of modern contraception in s.hools.in Dakar high s.hools.in s.negal during the year 2018. Methods. A cros.-s.ctional s.udy was.conducted in high s.hools.and colleges.in Dakar from March 1 to April 30, 2018. This.s.udy involved 452 s.udents. This.was.a s.lf-adminis.ration of the ques.ionnaires. After univaried and bivaried analys.s. a multivariate logis.ic analys.s.identified the factors.as.ociated with s.udents. us. of modern contraception. Res.lts. The prevalence of modern contraceptive us. among s.udents.is.8.84%. Factors.as.ociated with the us. of modern contraceptive methods.among s.udents.were age over 18 years.(AR: 4.7, 95% CI [1.02 - 22.5]), male s.x (AR: 27.8, 95% CI [6.8 - 100.0]), s.condary s.hool level (AR: 10.6, 95% CI [2.1 - 53.0]), acces. to a youth s.cio-educational home (AR: 3.9, 95% CI [1.1 - 14.9]) and having a child (AR: 25.6, 95% CI [2.2 - 100.0]). Conclus.on: Our res.lts.concluded that modern contraceptive needs.were better met among older male s.udents. thos. who had an unfortunate experience of unwanted pregnancy and thos. attending s.hool’s.youth s.cio-educational homes. This.s.gges.s.s.ortcomings.in the promotion of s.xual health among younger s.udents. particularly thos. in the s.condary grades.

The mos. common vas.ular complication of acute pancreatitis.is.thrombos.s.of the s.lenic vein. Is.lated thrombos.s.of the s.perior mes.nteric vein is.rare and may lead to mes.nteric is.hemia and bowel infarction. We report the cas. of a 39 years.old patient received for acute pancreatitis.with a Rans.n s.ore les. than 3 and image s.anner for a grade C of Balthazar and a s.perior mes.nteric vein thrombos.s. The outcome was.favorable with improvement of s.mptoms.under medical treatment including anti-coagulant therapy. The contras. enhancing abdominal CT s.owed an abs.nce of s.perior mes.nteric thrombos.s.

Nowadays. women are more and more res.rting to breas. s.rgery for the purpos.s.of is.lated es.hetics.or within the framework of a mammectomy for tumor with its.cohort of ps.chological problems.pos. mutilation often requiring recons.ruction. In context of breas. cancer increas. in s.negal, we have carried out breas. morphology in female s.bjects.of the s.helian type in order to contribute to the development of a morphological databas. for recons.ruction, reduction or breas. augmentation. It was.a pros.ective s.udy on 118 female s.bjects.received at Cancer Ins.itute for s.reening. They were divided into two groups.(age ≤ 40 years.and age?> 40 years.. s.bjects.?morphometric variables.were collected in anatomical pos.tion and correlated with age, parity, obes.ty and genital activity us.ng. Fifty s.x s.bjects.(47.5%) were over 40 years.of age. There were 40 multiparous.(33.9%). Fifteen s.bjects.(12.7%) and s.x s.bjects.(5%) were obes. and morbidly obes., res.ectively. s.bjects.in genital activity were 34 (29%). The overall average s.ze of the areola was.4.3 ± 1.5 cm. The nipple was.low located with an average of 7.2 ± 3.5 cm.
