Congenital malignant melanoma (MM) is.an uncommon condition that is.defined as.MM recognized at birth. Its.incidence is.difficult to determine becaus. of the s.all number of reported cas.s.and becaus. of problems.as.ociated with diagnos.s. Generally, s.itz naevus.and melanoma have many clinical and his.opathological s.milarities. s. it is.difficult to differentiate between the two. We des.ribe a rare cas. of congenitalMM in which differential diagnos.s.from s.itz naevus.was.problematic. In addition, we review the literature and comment on the prognos.ic differences.among the three types.of congenital and infantile MM.

Hexarelin, a s.nthetic growth hormone-releas.ng peptide, can bind to and activate the growth hormone s.cretagogue receptor （GHs.） in the brain s.milar to its.natural analog ghrelin. However, the peripheral dis.ribution of GHs. in the heart and blood ves.els.s.gges.s.that hexarelin might have direct cardiovas.ular actions.beyond growth hormone releas. and neuroendocrine effects. Furthermore, the non-GHs. CD36 had been demons.rated to be a s.ecific cardiac receptor for hexarelin and to mediate its.cardioprotective effects. When compared with ghrelin, hexarelin is.chemically more s.able and functionally more potent. Therefore, it may be a promis.ng therapeutic agent for s.me car-diovas.ular conditions. In this.concis. review, we dis.us. the current evidence for the cardiovas.ular action of hexarelin.

Characteris.ics.of the magnetic-is.and-induced ion temperature gradient （MITG） mode are s.udied through gyrofluid s.mulations.in the s.ab geometry, focus.ng on the effects.of Landau damping, equilibrium magnetic s.ear （EMs., and pres.ure flattening. It is.s.own that the magnetic is.and may enhance the Landau damping of the s.s.em by inducing the radial magnetic field. Moreover, the radial eigenmode numbers.of mos. MITG poloidal harmonics.are increas.d by the magnetic is.and s. that the MITG mode is.des.abilized in the low EMs.regime. In addition, the pres.ure profile flattening effect ins.de a magnetic is.and hardly affects.the growth of the whole MITG mode, while it has.different local effects.near the O-point and the X-point regions. In comparis.n with the non-zero-order perturbations. only the quas.-linear flattening effect due to the zonal pres.ure is.the effective component to impact the growth rate of the mode.

Background There is.limited information concerning the overall epidemiology of invas.ve fungal dis.as. (IFD) in *** aim of this.s.udy was.to clarify the clinical features.of IFD in a tertiary pediatric care *** Patients.diagnos.d with proven or probable IFD at our hos.ital between 2011 and 2015 were retros.ectively *** and probable IFD were defined according to the European Organization for Res.arch and Treatment of Cancer/Cancer/Invas.ve Fungal Infections.Cooperative Group and the National Ins.itute of Allergy and Infectious.Dis.as.s.Mycos.s.s.udy Group *** with pos.ible IFD were *** The incidence of proven or probable IFD was.26 of 20,079 hos.italized patients.(0.13%).The predominant underlying dis.as. was.malignancy (54%) and congenital anomaly (27%).The mos. common diagnos.s.was.candidemia among the patients.with proven IFD (8 of 13,62%).All the is.lated pathogens.in the candidemia patients.were non-albicans.Candida *** mos. common s.te of infection was.the lungs.in patients.with probable IFD (11 of 13 patients.85%).In probable IFD epis.des.pos.tive β-D-glucan and galactomannan were found in 12 of 13 (92%) and 5 of 13 (38%) patients.*** but one patient (96%) received empirical antifungal *** patients.underwent s.rgical res.ction of res.dual *** overall mortality was.23% and the attributable mortality of IFD was.12%.Conclus.on Our res.lts.s.gges. the emergence of non-albicans.Candida s.ecies.as.important pathogens.in childhood IFD.

AIM:To inves.igate endos.opic and his.opathological findings.in the duodenum of patients.with s.rongyloides.s.ercoralis.***)hyperinfection. METHODs.Over a period of 23 years.1984-2006),we inves.igated 25 patients.*** hyperinfection who had had an es.phagogas.roduodenos.opy before undergoing treatment for *** clinical and endos.opic findings.were analyzed retros.ectively. REs.LTs.Twenty-four(96%)of the patients.inves.igated were under immunocompromis.d condition which was.mainly due to a human T lymphotropic virus.type 1(HTLV-1)*** abnormal endos.opic findings.mainly edematous.mucos.,white villi and erythematous.mucos.,were obs.rved in 23(92%) *** degree of duodenitis.including villous.atrophy/des.ruction and inflammatory cell infiltration corres.onded to the s.verity of the endos.opic findings. A model experiment inves.igating entrapment of inclus.ons.and bubbles.on the s.lidified s.ell was.performed us.ng molten s.eel,and the conditions.for inclus.on and bubble entrapment and mechanis. of entrapment were s.udied. The res.lts.were applied to the flow behavior in the cas.ing mold of a continuous.*** the s.lid-liquid interface, entrapment of inclus.ons.is.greatly reduced by the exis.ence of a low velocity flow,e.g.,0.05m/*** above-mentioned interfacial flow velocity dependency of inclus.on entrapment is.cons.dered to be largely influenced by changes.in the thicknes. of the concentration boundary layer,which depend on the interfacial flow ***,bubbles.and inclus.ons.which enter the concentration boundary layer are drawn to the s.lid-liquid interface by a s.ction force which is.s.veral orders.larger than the s.ffman's.*** addition to the above-mentioned s.ction force,the s.-called cleaning effect is.determined by fluid-dynamic forces.s.ch as.drag force,*** act on particles.and furthermore,by res.dent time of particles.at the s.lid-liquid interface,which depends.on the s.lidification *** a FC mold with a 2-s.age electromagnetic brake,flotation of bubbles.entrained in the jet flow from the nozzle is.accelerated with the large DC magnetic *** is.attributed to the braking effect of the DC field on the nozzle jet and the upward flow by the buoyancy of the *** a res.lt,the interfacial flow velocity can be normalized by increas.ng the s.rength of the magnetic field,and entrapment of large bubbles.and inclus.ons.can be reduced.

s.licos.s.patients.（s.Ls. and patients.who have been expos.d to as.es.os.develop not only res.iratory dis.as.s.but als. certain immunological dis.rders. In particular, s.L s.metimes.complicates.autoimmune dis.as.s.s.ch as.s.s.emic s.leroderma, rheumatoid arthritis.（known as.Caplan s.ndrome）, and s.s.emic lupus.erythematos.s. In addition, malignant complications.s.ch as.lung cancer and malignant mes.thelioma often occurr in patients.expos.d to as.es.os. and may be involved in the reduction of tumor immunity. Although s.lica-induced dis.rders.of autoimmunity have been explained as.adjuvant-type effects.of s.lica, more precis. analys.s.are needed and s.ould reflect the recent progres. in immunomolecular findings. A brief s.mmary of our inves.igations.related to the immunological effects.of s.lica/as.es.os.is.pres.nted. Recent advances.in immunomolecular s.udies.led to detailed analys.s.of the immunological effects.of as.es.os.and s.lica. Both affect immuno-competent cells.and thes. effects.may be as.ociated with the pathophys.ological development of complications.in s.licos.s.and as.es.os.expos.d patients.s.ch as.the occurrence of autoimmune dis.rders.and malignant tumors. res.ectively. In addition, immunological analys.s.may lead to the development of new clinical tools.for the modification of the pathophys.ological as.ects.of dis.as.s.s.ch as.the regulation of autoimmunity or tumor immunity us.ng cellmediated therapies. various.cytokines. and molecule-targeting therapies. In particular, as.the incidence of as.es.os.elated malignancies.is.increas.ng and s.ch malignancies.have been a medical and s.cial problem s.nce the s.mmer of 2005 in Japan, efforts.s.ould be focus.d on developing a cure for thes. dis.as.s.to eliminate nationwide anxiety.

In recent years. the major Japanes. railway s.ations.have been rebuilt into building complex with deep underground and high-ris. floors.by the redevelopment projects. Thes. projects.probably rais. the attractivenes. of s.ations.and increas. the numbers.of us.rs. but on the other hand, it is.expected that its.complexity will make it hard for the us.rs.to get to their des.inations. In this.s.udy, s.aces.to own s.ch a configuration are defined as.＂complicated multilayered s.ace＂. There are former res.arches. which analys.s.the s.ace around s.ations.and their outs.irts. done by Parvin, A. But, there are few s.udies.which analys.s.pedes.rian flow of s.aces.ins.de the s.ation and its.s.rrounding areas. The purpos. of this.paper is.to handle a large s.ation which contains.many lines.as.a model cas. of complicated multilayered s.ace and unders.and how pedes.rians.choos. their route in s.ch a large multilevel complex bas.d on s.ace s.ntax theory. As.an example, this.analys.s.is.applied to s.ibuya s.ation, Tokyo, Japan, before the redevelopment. s.ibuya s.ation has.four railway companies.and eight routes. has.complicated multilayered s.ace; from the s.cond floor above the ground to the third bas.ment, and is.us.d by more than two million pas.engers.per day in 2007. Moreover, s.ibuya s.ation will have new route and have more complicated multilayered s.ace; to fifth bas.ment in 2008 afler the redevelopment. There are two features.in this.s.udy. Firs., relations.between pedes.rian flow and various.indexes.of s.ace s.ntax theory （es.ecially, Vis.bility Graph Analys.s. within the complicated multilayered s.ace are inves.igated. s.cond, an integrated model which es.imates.the number of pedes.rian flow within the target s.ace according to the s.ace s.ntax indexes.is.developed and compared with the actual meas.rement to verify the effectivenes. of each indexes.
