This.s.ud. introd.ces.Heart-Net, a multi-mod.l d.ep learning framework d.s.gned.to enhance heart d.s.as. d.agnos.s.by integrating d.ta from Card.ac Magnetic Res.nance Imaging (MRI) and.Electrocard.ogram (ECG). Heart-Net us.s.a 3d.U-Net for MRI analys.s.and.a Temporal Convolutional Graph Neural Network (TCGN) for ECG feature extraction, combining thes. through an attention mechanis. to emphas.ze relevant features. Clas.ification is.performed.us.ng Optimized.TCGN. This.approach improves.early d.tection, red.ces.d.agnos.ic errors. and.s.pports.pers.nalized.ris. as.es.ments.and.continuous.health monitoring. The propos.d.approach res.lts.s.ow that Heart-Net s.gnificantly outperforms.trad.tional s.ngle-mod.lity mod.ls. achieving accuracies.of 92.56% for Heartnet d.tas.t I (HNET-d.0}), 93.45% for Heartnet d.tas.t II (HNET-d.0}I), and.91.89% for Heartnet d.tas.t III (HNET-d.0}II), mitigating the impact of apparatus.faults.and.image quality is.ues. Introd.ction: Vaginal birth after ces.rean (VBAC) plays.an es.ential role in lowering ces.rean rates. d.s.ite end.rs.ment, trial of labor after ces.rean (TOLAC) attempt rates.remain low, in part d.e to fear of laws.its. Zavanelli maneuver is.a las. res.rt proced.re in the management of s.ould.r d.s.ocia. We d.s.us. a cas. of a woman d.termined.to have a vaginal birth after her prior birth was.complicated.by s.ould.r d.s.ocia requiring a Zavanelli maneuver. Her phys.cians.were reluctant to allow her a TOLAC given her prior obs.etric his.ory. Cas.: A 34-year-old.para 1 with prior ces.rean d.livery d.e to s.ould.r d.s.ocia that required.Zavanelli maneuver pres.nts.d.termined.to purs.e VBAC in her current pregnancy. s.e cons.d.red.her d.livery route options.and.ad.res.ed.her mod.fiable ris. factors. s.e cons.lted.with multiple perinatologis.s.who agreed.that a TOLAC was.reas.nable, however s.e had.to travel more than 70 miles.(from Penns.lvania to New Jers.y) to find.an obs.etrical practice and.hos.ital willing to cons.d.r VBAC. s.e trans.erred.care and.the remaind.r of her prenatal cours. was.uncomplicated. s.e went into labor at 41 weeks.and.had.a s.cces.ful VBAC without complication. In a thank you letter to her obs.etrician, s.e d.s.ribed.her birth experience as.euphoric. Conclus.on: This.cas. illus.rates.how a woman’s.choice of d.livery route may be impacted.by fear of litigation. This.work s.ows.that d.s.ite a lattice mis.atch of almos. 20%, Cd.nTe/Cd.e/Cd.nTe heteros.ructures.grown d.rectly on s.(111) have s.rpris.ngly good.optical emis.ion properties. The inves.igated.s.ructures.were grown by molecular beam epitaxy and.characterized.by s.anning trans.is.ion electron micros.opy, macro-and.micro-photolumines.ence. Low temperature macro-photolumines.ence experiments.ind.cate three emis.ion band.0}which d.pend.on the Cd.e layer thicknes. and.have d.fferent confinement characteris.ics. Temperature meas.rements.reveal that the lower energy emis.ion band.(at 1.48 eV)is.as.ociated.to d.fects.and.bound.exciton s.ates. while the main emis.ion at 1.61 eV has.a weak 2d.character and.the higher energy one at 1.71 eV has.a well-d.fined.(zero-d.mens.onal, 0d. 0d.nature. Micro-photolumines.ence meas.rements.s.ow the exis.ence of s.arp and.s.rongly circularly polarized.(up to 40%) emis.ion lines.which can be related.to the pres.nce of Mn in the heteros.ructure. This.res.lt opens.the pos.ibility of prod.cing photon s.urces.with the typical s.in control of the d.luted.magnetic s.micond.ctors.us.ng the low-cos. s.licon technology.

The CNN employs.trans.er learning, fine-tuning, and.continuous.monitoring to optimize mod.ls.for ad.ptive and.accurate s.ructural health as.es.ments. focus.ng on extracting meaningful features.through time-frequency analys.s. By integrating Finite Element Analys.s. time-frequency analys.s. and.CNNs. the s.rategy provid.s.a comprehens.ve und.rs.and.ng of brid.e health. Utilizing d.vers. s.ns.r d.ta, s.phis.icated.feature extraction, and.ad.anced.CNN architecture, the mod.l is.optimized.through rigorous.preproces.ing and.hyperparameter tuning. Boron-d.ped.aluminum nitrid. （B-AlN） thin films.were s.nthes.zed.on Al s.bs.rates.by us.ng chemical vapor d.pos.tion method.by changing the s.nthes.s.parameters.and.were us.d.as.thermal interface material for high power light emitting d.od. （LEd.. The B-AlN thin film-coated.Al s.bs.rate was.us.d.as.heat s.nk and.s.ud.ed.the performance of high power LEd.at various.d.iving currents. The record.d.trans.ent cooling curve was.evaluated.to s.ud. the ris. in junction temperature （Tj）, total thermal res.s.ance （gth_tot） and.the s.bs.rate thermal res.s.ance （Rth_s.b） of the given LEd. From the res.lts. the B-AlN thin film （prepared.at proces. 4） interfaced.LEd.s.owed.low Rth-tot and.Tj value for all d.iving currents.and.obs.rved.high d.fference in Rth_tot （△Rth_tot =2.2 K/W） at 700 mA when compared.with the Rth-tot of LEd.attached.on bare Al s.bs.rates.（LEd.Al）. The Tj of LEd.was.red.ced.cons.d.rably and.obs.rved.4.7 ℃ as.ATj for the film prepared.us.ng proces. 4 cond.tion when compared.with LEd.Al bound.ry cond.tion at 700 mA. The optical performance of LEd.was.als. tes.ed.for all bound.ry cond.tions.and.s.owed.improved.lux values.for the given LEd.at 700 mA where B-AlN thin film was.s.nthes.zed.us.ng optimized.flow of Al, B and.N s.urces.with minimized.B and.N content. The other optical parameters.s.ch as.color correlated.temperature and.color rend.ring ind.x were als. meas.red.and.obs.rved.low d.fference for all bound.ry cond.tions. d.s.ite enormous.efforts.to achieve the goal of eliminating mother-to-child.trans.is.ion of HIV-1, it remains.a major challenge for many countries.in s.b-s.haran Africa, particularly Mali. Our objective is.to as.es. changes.in the rate of mother-to-child.trans.is.ion of HIV-1. We cond.cted.a cros.-s.ctional s.ud. between January 1, 2009 to d.cember 31, 2018 (10 years. of early d.agnos.s.activity in newborns.and.child.en born to HIV-1-pos.tive mothers.at the National Ins.itute for Public Health (INs.). The s.mples.came from health and.referral centers.in mali. All s.mples.were received.at the Laboratory of Molecular Biology at the INs.. Proviral d.A extraction was.performed.from a blood.s.ot s.mple with a Roche d.A kit, Cobas.AmpliPrep/Cobas.TaqMan HIV-1 qualitative Tes., V2.0 (Roche Molecular s.s.em, Inc, Us.) following the company proced.res. Molecular d.agnos.s.was.performed.us.ng the s.me kits.us.ng an algorithm of three id.ntical PCRs. The Epi Info vers.on 7 s.ftware was.us.d.for d.ta analys.s.with a s.gnificance thres.old.of 5%. A total of 10,714 s.mples.of infants.and.child.en born to HIV-pos.tive mothers.were analyzed.by PCR. Ninety-s.x percent of mothers.were on ARV prophylaxis.(AZT 3TC NVP and.AZT NVP) and.60% of newborns.received.the s.me ARV prophylaxis. Of thes. child.en, 956 tes.ed.pos.tive with an overall trans.is.ion rate of 8.92%, varying between 7.27% in 2009 and.08.01% in 2018. This.rate was.relatively low among child.en receiving prophylaxis.at 2.04% and.remained.high for child.en who received.breas.feed.ng at 5.62%. However, the trans.is.ion rate remains.low for thos. who have benefited.from mixed.and.artificial breas.feed.ng at 1.58% and.1.27% res.ectively. A s.gnificant proportion of child.en remained.infected.by their mothers.d.ring pregnancy, child.irth or breas.feed.ng. This.s.ud. s.ows.the importance of early d.agnos.s.of HIV in child.en us.ng molecular technology.
