AiMTo evaluate efficacy/safety of hepatitis C virus (HCV) protease inhibitor boceprevir with pegylated interferon (PEG-iFN) alfa and weight-based ribavirin (RBV) in a phase 3 trial. METHODsA prospective, multicenter, phase 3, open-label, single-arm study of PEG-iFN alfa, weight-based RBV, and boceprevir, with a PEG-iFN/RBV lead-in phase was performed. The HCV/human immunodeficiency virus coinfected study population included treatment naïve (TN) and treatment experienced (TE) patients. Treatment duration ranged from 28 to 48 wk dependent upon response-guided criteria. All patients had HCV Genotype 1 with a viral load > 10000 iU/mL. Compensated cirrhosis was allowed. sample size was determined to establish superiority to historical (PEG-iFN plus RBV) rates in sustained viral response (sVR). REsULTsA total of 257 enrolled participants were analyzed (135 TN and 122 TE). in the TN group, 81.5% were male and 54.1% were black. in the TE group, 76.2% were male and 47.5% were white. Overall sVR12 rates (HCV RNA P = 0.002). Among the TN, sVR12 was 42.1% among whites and 27.4% among blacks (P = 0.09). CONCLUsiONThe trial met its hypothesis of improved sVR compared to historical controls but overall sVR rates were low. All-oral HCV treatments will mitigate these difficulties.

introduction: The United states government supported Ugandan government by introducing the District Health information software 2 (DHis2) in 2012 to improve HiV/AiDs surveillance. Districts have yet to fully adopt this relatively new system given a 70.2% reporting completeness achieved nationally between April-June 2013. Methods: The study examined one dependent variable of districts’ reporting completeness against four independent variables: 1) Number of client visits;2) Number of district health units;3) Number of NGOs delivering HiV/AiDs services;and 4) Regional location. The study employed cross-sectional study design which allowed researchers to compare many different variables at the same time. HiV/AiDs program data that were reported by districts into DHis2 during the period of April to June 2013 were used to assess for reporting completeness. Findings: Districts with the lowest number of client visits (under 2500) achieved the highest mean reporting completeness (81.6%), whereas a range of 2501 - 5000, or over 5001client visits recorded 72.4% and 51.7% respectively. The higher the number of client visits is, the lower the reporting completeness is (p se districts that were receiving support from only one and two NGO recorded 56.7% and 67.2% respectively. Districts supported by over three NGOs had the highest (80.6%) mean reporting completeness. Conclusion: The study results led us to recommend targeted future NGO support to districts with higher patient volume for HiV/AiDs services. strong>Background:strong> Henoch-schonlein Purpura (HsP), or igA vasculitis, is the most common vasculitis in children and is characterized by four symptoms: palpable purpura with no concurrent thrombocytopenia or coagulopathy, arthritis/ arthralgias, abdominal pain, and renal disease. HsP does occur in older patients at a much lower frequency. strong>Objective:strong> To discuss the atypical presentation of HsP in an elderly adult and the increased nephrogenic risks that are involved in this patient group. strong>Case Presentation: strong>We present the history, physical findings, laboratory results, and the clinical course of a 74-year-old Caucasian male with an atypical presentation of HsP. Our patient presented with severe gastrointestinal and cutaneous symptoms which occurred after the onset of abdominal pain. Biopsies demonstrated leukocytoclastic vasculitis in the skin and acute inflammation and hemorrhage in the gastrointestinal tract. The diagnosis was confirmed as igA vasculitis following the renal biopsy. The patient’s symptoms improved with the administration of methylprednisolone. strong>Conclusion:strong> in adults with HsP there is greater concern regarding renal involvement and a greater risk of developing end-stage renal disease compared to children. The importance of recognizing HsP in the elderly is imperative for early diagnosis and appropriate treatment to limit renal damage.

Tumors of the major and the minor duodenal papillae can be malignant or premalignant, and traditionally are treated by surgical excision. This study evaluated the safety and the outcome of endoscopic snare resection of such tumors. All patients with tumors of the major or the minor papilla treated by endoscopic snare resection over a 10-year period (1994-2003) were identified from an ERCP data base. Patients with tumors that had endoscopic features of malignancy and those proven to be cancerous by biopsy were excluded. Papillectomy was performed by electrosurgical snare resection. A pancreatic stent usually was placed before or after excision. Residual tumor was eradicated by repeated procedures. Endoscopic surveillance was at the discretion of the endoscopist. seventy snare resections were performed in 55 patients (mean age 59 years). Histop athologic diagnoses were the following: adenoma (45 patients; 7 with focal high -grade dysplasia, 6 with intraductal extension), adenocarcinoma (5), carcinoid tumor (2), gastric heterotopia (1), and normal histology (2). Fourteen patients had familial adenomatous polyposis. Of the 39 patients with isolated extraductal adenoma per cholangiogram, two underwent surgical resection because of persiste nt high-grade dysplasia, and 37 were successfully treated by endoscopic papillectomy alone. During follow-up (mean 30 months), 18 of 37 patients (49%) had no recurrence, 7 had recurrent adenoma (mean time interval to recurrence 27 months ), two died of unrelated illnesses, and 10 are awaiting follow-up. Of the 6 patients with intraductal adenoma per cholangiogram, two underwent surgical resecti on, two had intraductal photodynamic therapy, and two had endoscopic snare resec tion. intraductal tumor in the 4 latter patients was eliminated, although it recurred in one of the patients who had photodynamic therapy. This paper presents algorithmic components and corresponding software routines for extracting shoreline features from remote sensing imagery and LiDAR data. Conceptually, shoreline features are treated as boundary lines between land objects and water objects. Numerical algorithms have been identified and de-vised to segment and classify remote sensing imagery and LiDAR data into land and water pixels, to form and enhance land and water objects, and to trace and vectorize the boundaries between land and water ob-jects as shoreline features. A contouring routine is developed as an alternative method for extracting shore-line features from LiDAR data. While most of numerical algorithms are implemented using C++ program-ming language, some algorithms use available functions of ArcObjects in ArcGis. Based on VB .NET and ArcObjects programming, a graphical user’s interface has been developed to integrate and organize shoreline extraction routines into a software package. This product represents the first comprehensive software tool dedicated for extracting shorelines from remotely sensed data. Radarsat sAR image, QuickBird multispectral image, and airborne LiDAR data have been used to demonstrate how these software routines can be utilized and combined to extract shoreline features from different types of input data sources: panchromatic or single band imagery, color or multi-spectral image, and LiDAR elevation data. Our software package is freely available for the public through the internet.

背景:1994年4月纽约市所有结核菌培养阳性但未经治疗的结核病病人(n=184)。目的:检验与病人就诊延误(病人延误)和开始抗结核治疗延误(卫生保健系统延误)相关的因素。设计:医疗记录的回顾性调查及对病人的访视。结果:总延误天数的中位数是57天(4～764天),抗酸杆菌涂片阳性病人35名,涂片阴性病人79名(Pistic回归,年龄在55～64岁之间(经校正的让步比[OR_(adj]为10.6,95%的可信区间[Ci]为1.3—86.9)、母语不是英语的病人(OR_(adj)2.5,95％Ci 1.0—5.8)与病人延误天数较长相关。无家可归(OR_(adj) 7.1,95%Ci 1.05—33.5)、首次就诊未拍摄胸部X线片(OR_(adj) 2.4,95％Ci 1.0—5.4)、涂片阴性(OR_(adj) 10.2,95％Ci 4.4—23.3)、以及无咳嗽症状的病人(OR_(adj) 2.9,95％C 1.2—6.8)与卫生保健系统延误天数较长相关。结论:为了减少延误时间,应教育病人尽早就诊,并提供与病人文化背景相适宜的卫生保健和语言服务。内科医生对结核病应保持高的警惕性,并进行相应的诊断试验。
