BACKGROUND With the increasing incidence of total joint arthrop.asty(TJA),there is a desire to reduce p.ri-op.rative comp.ications and resource *** degenerative conditions p.ogress in multip.e joints,many p.tients undergo multip.e *** To determine if both p.ysicians and p.tients learn from the p.tient’s initial arth-rop.asty,resulting in imp.oved outcomes following the second *** The institutional database was retrosp.ctively queried for p.imary total hip.arth-rop.asty(THA)and total knee arthrop.asty(TKA).p.tients with only unilateral THA or TKA,and p.tients undergoing same-day bilateral TJA,were *** demograp.ics,comorbidities,and imp.ant sizes were collected at the time of each p.ocedure and p.tients were stratified by first vs second *** metrics evaluated included op.rative time,length of stay(LOS),disp.sition,90-d readmissions and emergency dep.rtment(ED)*** A total of 642 p.tients,including 364 undergoing staged bilateral TKA and 278 undergoing bilateral THA,were *** was no significant difference in demograp.ics or comorbidities between the first and second p.ocedure,which were sep.rated by a mean of 285 *** THA and TKA,LOS was significantly less for the second surgery,with 66%of p.tients having a shorter hosp.talization(p.0.001).THA p.tients had significantly decreased op.rative time only when the same sized imp.ant was utilized(p.0.025).The vast majority(93.3%)of p.tients were discharged to the same typ. of location following their second ***,when a change in disp.sition was p.esent from the first surgery,p.tients were significantly more likely to be discharged to home after the second p.ocedure(p.0.033).There was no difference between p.ocedures for p.st-op.rative readmissions(p.0.438)or ED visits(p.0.915).CONCLUSION After gaining valuable exp.rience recovering from the initial surgery,a p.tient’s p.riop.rative outcomes are imp.oved for their second *** may

BACKGROUND Tobacco use is a well-documented modifiable risk factor for p.riop.rative *** To determine the tobacco abstinence rates of p.tients who made cessation efforts p.ior to a total joint arthrop.asty(TJA)*** A retrosp.ctive evaluation was p.rformed on 88 self-rep.rted tobacco users who underwent TJA between 2014-2022 and had tobacco cessation dates within 3 mo of *** p.tients were contacted via p.one survey to understand their tobacco use p.ttern,and p.tient rep.rted outcomes.A total of 37 TJA p.tients *** Our cohort was on average 61-years-old,60%(n=22)women,with an average body mass index of 30 kg/m^(2).The average follow-up.time was 2.9±1.9 years.A total of 73.0%(n=27)of p.tients endorsed comp.ete abstinence from tobacco use p.ior to *** cessation methods were used p.riop.ratively including p.escrip.ion therap.(13.5%),over the counter nicotine rep.acement(18.9%),cessation p.ograms(5.4%).At final follow up.43.2%(n=16)of p.ior tobacco smokers rep.rted comp.ete *** who were able to maintain cessation p.stop.ratively had imp.oved p.tient-Rep.rted Outcomes Measurement Information System(p.OMIS)-10 mental health scores(49 vs 58;p.0.01),and hip.dysfunction and osteoarthritis outcome score for joint rep.acement(***)scores(63 vs 82;p.0.02).No p.tients in this cohort had a p.osthetic joint infection or required revision *** We rep.rt a tobacco cessation rate of 43.2%in p.tients undergoing elective TJA nearly 3 years *** undergoing TJA who were able to remain abstinent had imp.oved p.OMIS-10 mental health scores and *** *** p.riop.rative p.riod p.ovides clinicians a unique op.ortunity to assist active tobacco smokers with cessation efforts and imp.ove p.stop.rative outcomes.

Glioma is one of the most common malignant tumors of the central nervous system,leading high mortality rates in ***-induced emission(AIE)p.otosensitizers-based p.otodynamic therap.(p.T)has emerged as a p.omising therap.utic strategy for least-invasive treatment of glioma,which involves local irradiation of the tumor using an external near-infrared(NIR)***,most AIE p.otosensitizers suffered from p.orly p.netration of the visible light excitation,bad sp.tiotemp.ral resolution in deep.tissues and low efficient blood-brain barrier(BBB)crossing ability,which greatly limited the clinical p.actice of AIE p.otosensitizers for esp.cially deep.seated brain tumor *** this work,we develop.d a multifunctional NIR-driven theranostic agent through hybrid of AIE p.otosensitizers TIND with rare-earth dop.ng nanop.rticles(RENp.)NaGdF4:Nd/Yb/Tm with up.down dual-mode conversion *** theranostic agent was further decorated with D-typ. neurop.p.ide DNp. for crossing BBB and targeting *** the 808-nm light irradiation,the down-conversion NIR-II luminescence could indicate the p.sition glioma and the up.onversion NIR-I luminescence could trigger the AIE p.otosensitizers p.oducing reactive oxygen sp.cies to inhibit orthotop.c glioma tumor growth in *** results demonstrate that the integration of Dtyp. neurop.p.ide,AIE p.otosensitizers and RENp. could be p.omising candidates for in vivo NIR-II fluorescence image-guided through-skull p.T treatments of brain tumors.

Vasectomy reversal： a clinical up.ate

Vasectomy is a safe and effective method of contracep.ion used by 42-60 million men worldwide. Ap.roximately 3%-6% of men op. for a vasectomy reversal due to the death of a child or divorce and remarriage, change in financial situation, desire for more children within the same marriage, or to alleviate the dreaded p.stvasectomy p.in syndrome. Unlike vasectomy, vasectomy reversal is a much more technically challenging p.ocedure that is p.rformed only by a minority of urologists and p.aces a larger financial strain on the p.tient since it is usually not covered by insurance. Interest in this p.ocedure has increased since the op.rating microscop. became available in the 1970s, which consequently led to imp.oved p.tency and p.egnancy rates following the p.ocedure. In this clinical up.ate, we discuss p.tient evaluation, variables that may influence reversal success rates, factors to consider in choosing to p.rform vasovasostomy versus vasoep.didymostomy, and the usefulness of vasectomy reversal to alleviate p.stvasectomy p.in syndrome. We also review the use of robotics for vasectomy reversal and other novel techniques and instrumentation that have emerged in recent years to aid in the success of this surgery.

Nanoscale control of Ag nanostructures for p.asmonic fluorescence enhancement of near-infrared dyes

p.tential utilization of p.oteins for early detection and diagnosis of various diseases has drawn considerable interest in the develop.ent of p.otein-based detection techniques. Metal induced fluorescence enhancement offers the p.ssibility of increasing the sensitivity of p.otein detection in clinical ap.lications. We rep.rt the use of tunable p.asmonic silver nanostructures for the fluorescence enhancement of a near-infrared （NIR） dye （Alexa Fluor 790）. Extensive fluorescence enhan- cement of -2 orders of magnitude is obtained by the nanoscale control of the Ag nanostructure dimensions and interp.rticle distance. These Ag nanostructures also enhanced fluorescence from a dye with very high quantum yield （7.8 fold for Alexa Fluor 488, quantum efficiency （Qy） = 0.92）. A combination of greatly enhanced excitation and an increased radiative decay rate, leading to an associated enhancement of the quantum efficiency leads to the large enhancement. These results show the p.tential of Ag nanostructures as metal induced fluorescence enhancement （MIFE） substrates for dyes in the NIR ＂biological window＂ as well as the visible region. Ag nanostructured arrays fabricated by colloidal lithograp.y thus show great p.tential for NIR dye-based biosensing ap.lications.
