Background: Progressive insulin resistance (IR) is an important pathophysiologic mechanism of gestational diabetes mellitus (GDM). Homeostatic model assessment (HOMA) is commonly used as a parameter of the severity of insulin resistance. Aims: To determine indices of insulin resistance (IR) and β-cell function in gestational diabetes mellitus (GDM). Methods: This cross sectional study was conducted from March 2017 to September 2018 at Department of Endocrinology, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh. The study was performed with 41 GDM and equal number of pregnant women with normal glucose tolerance (NGT) diagnosed on basis of WHO criterion-2013 during 24 - 40 weeks of gestation. Serum glucose was measured by glucose oxidase method and fasting serum insulin was measured by chemiluminescent immunoassay. Equations of homeostatic model assessment (HOMA) were used to calculate insulin indices like-insulin resistance (HOMA-IR), β-cell function (HOMA-B) and insulin sensitivity (HOMA-%S). Data were analyzed and compared by statistical tests. Results: A total of eighty-two (82) subjects [41 women with GDM (age: 28.29 ± 3.79 years, BMI: 27.16 ± 4.13 kg/m2) and 41 women with NGT (age: 26.22 ± 5.13 years, BMI: 25.27 ± 3.01 kg/m2)] were included in this study. It was observed that GDM women were significantly older (p = 0.041) and had significantly higher BMI (p = 0.020) than pregnant women with NGT. The GDM group had significantly higher IR as indicated by higher fasting insulin value [GDM vs. NGT;10.19 (7.71 - 13.34) vs. 6.88 (5.88 - 8.47) μIU/ml, median (IQR);p = 0.001] and HOMA-IR [GDM vs. NGT;2.31 (1.73 - 3.15) vs. 1.42 (1.15 - 1.76), median (IQR);p β-cell secretory capacity [GDM vs. NGT;HOMA-B: 112.63 (83.52 - 143.93) vs. 128.60 (108.77 - 157.58), median (IQR);p = 0.04] and low insulin sensitivity [GDM vs. NGT;HOMA-%S: 43.29 (31.7

Background: By the dawn of this modern era of science, the prime challenge of physician is cardiovascular disease (CVD). The most important modifiable risk factors of CVDs are unhealthy diet, physical inactivity and tobacco use. The effects of unhealthy diet and physical inactivity include abnormal blood lipid, obesity and hypertension. We tried to evaluate and correlate the pattern of lipid profile in obese and non-obese hypertensive patients. Objectives: This study was conducted at medicine department of Cumilla Medical College Hospital. The principal aim was to evaluate the lipid profile in obese and non-obese adult hypertensive patients. Methodology: During this cross sectional analytical study, a total of 100 adult hypertensive patients were taken by purposive sampling. Among them 50 (group 1) patients were taken those were obese and 50 (group 2) patients taken those were non-obese according to BMI measurement on operational definition. Diagnosis of hypertension would be established with the help of ambulatory BP measurements two occasions few minutes apart. The staging of hypertension was done according to JNC7 Criteria. Morning blood samples were taken after 8 - 12 hours of fasting and lipid profiles were done on authentic laboratories. The laboratory values were interpreted according to the operational definition of dyslipidaemia. The ethical research and review committee approved the study protocol and signed informed consent was obtained from the participants. With the COVID-19 pandemic, disparities between the infection rate and death rate in different countries become a major concern. In some countries, lower mortality rate compared to others can be explained by better testing capacity and intensive care facilities. Complete SARS-CoV-2 genome sequences from different countries of the world are continually submitted to Global Initiative for Sharing All Influenza Data using Next Generation Sequencing method. A SARS-CoV-2 variant with a D 614G Mutation in the spike (S) protein has become the most dominant form in the global pandemic. There are a number of ongoing studies trying to relate this mutation with the infectivity, mortality, transmissibility of the virus and its impact on vaccine development. This review aims to accumulate the major findings from some of these studies and focus its future implication. Some studies suggested D 614G strain has increased binding capacity, it affects more cells at a faster rate, so has a high transmissibility. Patients infected with this strain were found with high viral load. But still now there is no such evidence that this strain produces more severe disease as well as increased mortality. The structural change of spike protein produced by D 614G mutation was minor and did not hamper the vaccine efficacy. Some studies showed antibodies produced against D614 strain can neutralize G614 strain and vice versa. Whenever a mutation occurs in spike protein there are always chances of affecting the infectivity, transmissibility, vaccine efficacy. Therefore, more studies are required to find out the overall effect of D 614G mutation.

Smartphone ownership among adolescents is getting common in this decade especially in Malaysia;Adolescent are strongly devoted to their smartphone and this may lead to smartphone *** have reported that smartphone addiction has become an emerging social and health problem especially among the youth in many countries however there is lack of study among adolescents in *** study aimed to examine the prevalence and factors associated with smartphone addiction among adolescents in *** was a cross-sectional study involving adolescents from 15 primary care clinics throughout the *** were assessed on their smartphone activities using the Malaysian short version of the Smartphone addiction scale(SAS-M-SV).Multiple logistic regression was used to determine the predictors of smartphone addiction among *** study was conducted among 921 adolescents with 49.6%male(n=457).The mean age of adolescents was 16.4±2.4 *** ethnicity distribution were 74.6%Malay,7.3%Chinese,4.7%Indian and 13.4%other *** prevalence of smartphone addiction was 37.1%(342/921);37.4%in male and 36.9%in *** on multiple logistic regression analysis,longer duration of smartphone use per week was associated with higher odds of smartphone addiction among adolescent(odd ratio=1.005%,95%confidence interval=1.000–1.009,p-value=0.039).Smartphone addiction is present in nearly four in ten adolescents in *** who spend longer duration in smartphone usage per week were associated with higher odds of having smartphone *** should be more alert and vigilant about this ***,parents should limit their children from spending too much of time with smartphone in order to prevent their children from getting smartphone addiction.

目的构建消化道早癌患者应用内镜黏膜下剥离术(ESD)治疗后术后出血的预测模型。方法回顾性分析2013年5月-2020年3月新疆医科大学第二附属医院内镜中心治疗的560例消化道早癌患者临床资料,并依据ESD术后是否出血分为出血组(n=25)、对照组(n=535)。采用logistic回归分析ESD术后出血的危险因素,根据筛选的危险因素构建ESD术后出血风险预测模型,并采用ROC曲线对预测模型进行评价。结果多因素logistic回归分析显示,有高血压(OR=3.59,95%CI:1.72~6.86,P=0.000)、操作时间>90 min(OR=1.67,95%CI:1.18~3.73,P=0.008)、最大病灶直径≥3 cm(OR=1.55,95%CI:1.23~3.12,P=0.002)、病灶位于直肠(OR=1.71,95%CI:1.26~3.98,P=0.012)为消化道早癌患者ESD术后出血的危险因素。ROC曲线分析显示,模型预测消化道早癌患者ESD术后出血的AUC为0.865(95%CI:0.849~0.976,P=0.000),准确率82.32%、敏感度84.00%、特异度82.24%、临界值为0.545。结论消化道早癌患者ESD术后出血与高血压、操作时间、病灶直径及病灶部位有关,预测模型可有效预测个体ESD术后出血风险。

OBJECTIVES: We aimed to investigate whether patients after tetralogy of Fallot(TOF) repair with right bundle branch block have left ventricular(LV) asynchrony and to assess the influence of ventricular asynchrony on regional and global LV function. BACKGROUND: Patients after TOF repair usually have right bundle branch block. However, no data regarding LV asynchrony in this group are available. METHODS: Twenty-five patients after TOF repair and 25 age-matched healthy control subjects were studied. The regional myocardial deformation of the interventricular septum(IVS) and the LV lateral wall were examined using tissue-Doppler-derived strain. The time interval between the onset of QRS complex and the peak strain was measured for each wall. According to the difference between LV and septum time intervals among the normal subjects, a normal range(mean±2 SD) was plotted, and TOF patients in whom the difference was beyond the normal range were considered to have LV asynchrony. The Tei index was used to assess global LV function. RESULTS: Thirteen(52%) of the examined patients after TOF repair had LV asynchrony. Patients after TOF repair with LV asynchrony had a significantly reduced regional septal systolic strain(p< 0.001) and significantly elevated Tei index(p< 0.001) compared with those without. CONCLUSIONS: Left ventricular asynchrony may exist in patients after TOF repair with right bundle branch block. This LV asynchrony is associated with a reduction of both regional and global LV function.

Hospitalizations for Osteoporosis-Related Fractures: Analysis from Eastern Saudi Arabia

Background and Objectives: Osteoporosis causes fragility fractures which increase the morbidity and mortality in the elderly. Our objective was to look at the hospital admissions due to fragility fractures and site of fractures. Methods: This is a retrospective study of a 2-year period (2017-2018), in which we evaluated all the admissions of fragility fractures at King Fahd Hospital of the University, Al Khobar, Saudi Arabia. A Fragility Fracture is a fracture which occurs in patients with low bone mass due to a fall from a standing height. The data were gathered from the Quadru Med patient care system for patients over the age of 50 years: sex, site of fracture, previous fractures, and treatment meted out. The hospitalization rate for fragility fractures was calculated by taking into consideration of all the adult admissions to the orthopaedic department. Ethical approval was obtained from the Institutional Review Board of the Hospital and the Imam abdul rahman Bin Faisal University, Dammam. Results: During the study period 187 patients with fragility fractures were admitted. The mean age was 68.55 ± 12.43 years;fracture femur was the most common with 136 (72.7%), tibia 15 (8%) and spine 14 (7.5%). At the time of discharge based on the T score of spine 50 were osteopenic and 45 were osteoporotic. Overall adult admission for the study period was 1410 patients and 13.26% were patients with fragility fractures. There were 3 (1.6%) deaths within the first 30 days in the hospital. Conclusions: Fracture femur was the most common fragility fracture seen in our study. Orthopaedic surgeons should be ready to treat all fragility fractures and treat osteoporosis appropriately so that a second fracture is prevented.
