Risk Factors for Cardiovascular Disease among Hotel Workers, Bohicon, Benin

Introduction: Non-communicable diseases (NCDs) are on the increase in several sectors of activity. The aim of the study was to determine the frequencies of cardiovascular risk factors (CVRF) and factors associated with hypertension among hotel workers in the city of Bohicon. Methods: This was a descriptive and analytical cross-sectional study conducted in the period from December 2018 to January 2019. The variables studied were sociodemographic, occupational, anthropometric and behavioral. Participant recruitment was exhaustive. A standardized questionnaire inspired by the STEPS-WHO questionnaire was used for data collection. After the descriptive analysis, a bivariate analysis was performed using Pearson’s Chi-square test or Fisher’s test at the 5% threshold. Results: A total of 101 workers, 53 (52.47%) of them male, participated in the study. The mean age was 30.60 ± 8.20 years, with extremes of 18 and 55 years. The frequencies of the main risk factors were: 68.32% sitting position > 6 hours/day;38.61% shift work;21.78% high psychological demand, 82.18% low decision latitude, 24.75% low social support, 21.78% occupational stress;7 (6.93%) harmful alcohol consumption;6.93% tobacco consumption;48.51% insufficient physical activity;98.02% insufficient fruit and vegetable consumption;59.41% overweight;14.85% HTA. Factors associated with hypertension were: sex, age and marital status. Conclusion: Occupational factors need to be taken into account when assessing CVRFs. NCD control programs are needed in all sectors of activity. A larger study is needed in the hotel sector.

Sleep Disorders among Professional Oil Tanker Drivers in Ouagadougou Burkina Faso

Introduction: Sleep disorders among professional drivers are a major road safety problem. They cause fatigue and drowsiness at the wheel, which can lead to road traffic accidents and even accidents at work. The aim of this study was to assess sleep disorders among professional drivers of oil tankers in Ouagadougou, Burkina Faso. Methods: This was a descriptive and analytical cross-sectional study carried out from 11 April 2020 to 11 September 2020 in the national hydrocarbon company’s fuel depot. All the drivers present at the time were included and accomplished a questionnaire incorporating the commonly scales using to assess sleep disorders, sleep quality and sleep apnoea. Data analysis using R 3.6.1 software enabled to perform univariate and multivariate analyses to identify associated factors. Adjusted odd ratios were used to measure the strength of association. The significance level chosen was p ≤ 0.05. Results: All the 339 respondents were male. A proportion of 33.6% suffered from insomnia, including 12.7% with moderate to severe insomnia, and 18.58% were at risk of sleep apnoea. On the Epworth scale, 26.6% of drivers showed sleep debt and 4.1% excessive daytime sleepiness. The risk factors associated with insomnia were the use of psychostimulants. A history of diabetes and insomnia were risk factors associated with sleep apnoea syndrome. Conclusion: The prevalence of sleep disturbance and insomnia among the respondents were 52.2% and 33.6% respectively. Use of psychostimulants was the identified risk factor. The prevalence of SAS was 4.72%, with diabetes and insomnia as associated factors.

Introduction: Work-related accidents are frequent and serious in the construction sector. The aim of the study was to determine the frequency and factors associated with occupational accidents on the construction site of a referral hospital in Benin. Methods: A cross-sectional study was carried out. The sample size was calculated using the Schwartz form adjusted for the number of workers on site and was 129 workers. Random sampling was used. The dependent variable was work-related accidents. The other variables were socio-demographic and occupational characteristics. Data were collected through a questionnaire survey. Medians and proportions were calculated. An association was sought using Chi-square and Fisher tests with a threshold of p Results: A total of 132 workers were included. Their median age was 30 years with an ITQ of [27 - 38];men were the most represented 126 (95.45%) with a level of education higher than or equal to high school in 101 (76.52%) and in the majority with a permanent status 85 (64.39%). Seniority of more than 5 years was observed in 92 (69.7%). Workers working more than 8 hours of overtime per week numbered 57 (43.18%). Exposure to vibrating objects was 49 (37.12%). In terms of psychosocial constraints, 82.58% had high psychological demands;79.53% low decision-making latitude;50.76% low social support. The frequency of work-related accidents was 6.82%, and the only associated factor was the type of worker (p = 0.016). On the other hand, there were 10.2% accidents among workers handling vibrating objects versus 4.98% among those not using them. With regard to psychosocial constraints, the following frequencies were recorded respectively: 6.42% among those with high psychological demand versus 8.7% among those with low psychological demand;7.62% among those with low decision-making latitude versus 3.7% among those with high decision-making latitude;8.96% among those with low social support versus 4.62% among those with high support. Massive Epistaxis Revealing a Post-Traumatic Aneurysm of the Internal Carotid Artery: A Clinical Case and Review of the Literature

Internal carotid artery (ICA) aneurysms are an unusual but serious cause of epistaxis. This epistaxis is massive and sometimes uncontrollable threatening the vital prognosis of patients. We report the case of a 16-year-old adolescent received in emergency with severe bilateral epistaxis, asthenia and grade-3 left exophthalmos. In his history, the subject had been the victim of an assault six months before consultation. He had received blows on the cephalic extremity with light but repeated epistaxis. The treatment consisted to blood products transfusion and local compression by sterile gazes. An ICA aneurysm in sphenoid sinus has been confirmed in a craniofacial CT scan coupled to vascular opacification. Although the ICA has a variable course in contact with the sphenoid sinus, massive epistaxis would be the consequence of a pronounced dehiscence of the ICA in the sphenoid sinus, particularly in a traumatic context. In front of this type of epistaxis in our context, general practitioners must be able to suspect a ruptured ICA aneurysm in the presence of exophthalmos and a notion of old or recent cranio-encephalic injury. Additionally, due to the high morbidity and mortality of this condition, a monitoring algorithm is necessary for patients with head trauma to facilitate early detection.

Feasibility of Integrating Leprosy Post-Exposure Prophylaxis with Single-Dose Rifampicin (LPEP) into Routine Leprosy Control Program in Bukedi and Teso Regions in Uganda

Background: World Health Organization recommends the implementation of contact tracing and Leprosy Post Exposure prophylaxis (LPEP) to interrupt the chain of transmission. To accelerate the uptake of this recommendation, a cross-sectional study among contacts of leprosy patients was conducted to investigate the feasibility of integrating leprosy systematic contact tracing and post-exposure prophylaxis (PEP) into the routine leprosy control program. Methods: This was a mixed methods cross-sectional study. The study was implemented in Kumi, Ngora, Serere, Soroti, Budaka and Kibuku Districts. Results: The 45 enrolled index patients (97.8% of the registered) identified a total of 135 contacts, of which 134 (99·2%) consented and were screened. Among them, one new leprosy patient was identified and started on treatment with multidrug therapy (MDT). All the eligible contacts, received the prophylactic treatment with Single Dose Rifampicin (SDR). Overall, SDR was administered to 133(98.5% of the listed contacts) with no adverse event reported. Factors associated with successful contact investigation and management included: Involvement of index patients, health care workers during the contact screening and SDR A administration, counselling of the index patients and contacts by the health care works, LPEP being administered as Directly observed Therapy (DOT) among others. Results Interpretation: The integration of leprosy post-exposure prophylaxis with administration of SDR and contact tracing is feasible, generally accepted by the patient, their contacts and health workers and can be integrated into the National Leprosy control programmes with minimal additional efforts once contact tracing has been established. Therefore, we recommend integration of administration of SDR in to the routine leprosy control program.

Distributional Considerations for Transboundary Risk Governance of Environmental Threats

Most policy analyses of both short-term and long-term disasters focus on aggregate impacts of their costs and the benefits of policy remedies. Distributional considerations relating to the costs of these disasters and the benefits of their risk management, however, are very important in many cases. This article examines two broad categories of cases in terms of distributional considerations. The first category is where transboundary considerations strongly affect risk governance, as in the case of climate change. The second relates to ordinary, short-term disasters, which include fewer, but still important, transboundary issues. Climate change policy requires information regarding the distribution of impacts and policy responses across countries because it is a global *** disasters involve transboundary considerations much less frequently, so the attention typically shifts to the distribution of benefits and costs within a jurisdiction. In both cases distributional information is needed to evaluate the equity of policies and to provide information for public participation in the policy process. This article offers modeling and policy approaches to address these issues.

Improved tabledng behavior of paracetamol in the presence of polyvinylpyrrolidone additive: Effect of mixing conditions

Monoclinic paracetamol (PA) is notorious as a poorly compactible model drug. Polyvinylpyrrolidone (PVP) is a polymer that can act as an effective binder to improve the mechanical properties of PA. It is surprising however that the role of mixing conditions on the physicochemical and mechanical properties of PA-PVP mixtures has not been reported previously. The results of this work showed that PA-PVP mixtures containing 5%(w/w) PVP prepared using high energy mixing conditions had considerably smaller particle size distributions and higher cohesivities than mixtures prepared using low energy mixing conditions. Solid-state analysis did not detect any change in the monoclinic crystalline form of PA after mixing with PVP. The following rank order of tabletability for PA-PVP mixtures was obtained according to the mixing condition: low shear-medium shear< dry high shear< wet high shear< high-speed homogenization. A higher level of hydrogen bonding was detected in the mixtures prepared via high energy mixing conditions than in those prepared using low energy mixing conditions. Mixing is therefore a critical process that needs to be optimized during the preparation of in teractive mixtures for tableting.

Animal personality is often studied within compressed periods of observation that represent narrow windows in comparison to animal *** much is known about the relations between repeatable personality traits and cross-situational behavioral plasticity,less is known about how such traits might differ across age classes or life history *** conducted a cross-sectional study of startle response duration in 3 size classes of Pagurus bernhardus,the common European hermit *** defined size classes using transitions in the preferred species of gastropod shells that accompany growth,and this change in preference is in turn associated with a transition from intertidal to subtidal *** with small-and medium-sized intertidal individuals the larger subtidal hermit crabs behaved cautiously by showing startle responses of greater duration following *** responses were also repeatable within all 3 size classes,confirming the presence of animal personality in intertidal hermit crabs and demonstrating that this pattern is retained within the largest size classes,which have undergone the transition from intertidal to subtidal ***,there was a trend for the pattern of repeatable startle response durations to increase with size class,with the highest value for repeatability and the greatest range of startle response durations being present within the large subtidal *** greater range of startle responses indicates that the longer startle response durations in some larger individuals are more likely due to developmental changes with age and habitat use than reflecting selection against the boldest individuals during earlier stages of life.
