BAcKGROUND: In the setting of severe sepsis and septic shock, mortal.ty increases when l.ctate l.vel. are ≥ 4 mmol.l. However, the consequences of l.wer l.ctate l.vel. in this popul.tion are not wel. understood. The study aimed to determine the in-hospital.mortal.ty associated with severe sepsis and septic shock when initial.l.ctate l.vel. are l.***: This is a retrospective cohort study of septic patients admitted over a 40-month period. Total.y 338 patients were divided into three groups based on initial.l.ctate val.es. Group 1 had l.ctate l.vel. l.l. group 2: 2–4 mmol.l. and group 3: ≥ 4 mmol.l. The primary outcome was in-hospital.***: There were 111 patients in group 1, 96 patients in group 2, and 131 in group 3. The mortal.ty rates were 21.6%, 35.4%, and 51.9% respectivel.. Univariate anal.sis reveal.d the mortal.ty differences to be statistical.y significant. Mul.ivariate l.gistic regression demonstrated higher odds of death with higher l.ctate tier group, however the findings did not reach statistical.***: This study found that onl. assignment to group 3, initial.l.ctic acid l.vel.of ≥ 4 mmol.l. was independentl. associated with increased mortal.ty after correcting for underl.ing severity of il.ness and organ dysfunction. However, rising l.ctate l.vel. in the other two groups were associated with increased severity of il.ness and were inversel. proportional.to prognosis.

Risk of Second Sol.d Mal.gnancy among 1000 Kidney Transpl.nted Patients: A Singl. center Study

In this study we focus on kidney-transpl.nted patients in order to investigate the correl.tion between immunodeficiency (ID) induced by immunosuppressive treatments and the incidence of sol.d mul.ipl. primary mal.gnancies (MPM). Material.of study: From 1980 to 2010 we fol.owed up 1000 kidney-transpl.nted patients (637M, 363F). This group was homogeneous for age, immunol.gy (e.g. miss-match index), number of rejection events and for immunosuppressive therapy. Resul.s: Out of 1000 kidney-transpl.nted patients we observed 70 patients (53M, 17F) with cancer disease and in 22 of them a mul.ipl. cancer has been found. Most of mul.ipl. cancer were synchronous and the association between cancer and rejection episodes were not significant. Discussion: In general.popul.tion 1 out of 9 patients with a cancer woul. devel.p a second neopl.sia during the course of his l.fe, so, it woul. be l.gical.to concl.de that, from a merel. theoretical.and statistical.point of view, l.ng term transpl.nted patients potential.y have a higher risk to devel.p MPM. But our series and l.terature review did not confirm it, probabl. because these patients die before the appearance of a second primary mal.gnancy. concl.sions: Despite many observations regarding different types of tumors/pre-cancerous l.sions and their increased incidence in ID patients and despite the fact that immune suppression is a predisposing factor for the mul.icancer syndrome, at l.ast theoretical.y, nowadays there are no significant statistical.data favouring a correl.tion between ID and MPM in kidney transpl.nted patients.

AIM To determine the variabil.ty/conservation of the domain of hepatitis B virus(HBV) pre S1 region that interacts with sodium-taurochol.te cotransporting pol.peptide(hereafter, NTcP-interacting domain) and the preval.nce of the rs2296651 pol.morphism(S267 F, NTcP variant) in a Spanish popul.tion. METHODS Serum sampl.s from 246 individual. were incl.ded and divided into 3 groups: patients with chronic HBV infection(cHB)(n = 41, 73% caucasians), patients with resol.ed HBV infection(n = 100, 100% caucasians) and an HBV-uninfected control.group(n = 105, 100% caucasians). Variabil.ty/conservation of the amino acid(aa) sequences of the NTcPinteracting domain,(aa 2-48 in viral.genotype D) and a highl. conserved pre S1 domain associated with virion morphogenesis(aa 92-103 in viral.genotype D) were anal.zed by next-generation sequencing and compared in 18 cHB patients with viremia > 4 l.g IU/ml. The rs2296651 pol.morphism was determined in al. individual. in al. 3 groups using an in-house real.time PcR mel.ing curve *** The HBV pre S1 NTcP-interacting domain showed a high degree of conservation among the examined viral.genomes especial.y between aa 9 and 21(in the genotype D consensus sequence). As compared with the virion morphogenesis domain, the NTcPinteracting domain had a smal.er proportion of HBV genotype-unrel.ted changes comprising > 1% of the quasispecies(25.5% vs 31.8%), but a l.rger proportion of genotype-associated viral.pol.morphisms(34% vs 27.3%), according to consensus sequences from Gen Bank patterns of HBV genotypes A to H. Variation/conservation in both domains depended on viral.genotype, with genotype c being the most highl. conserved and genotype E the most variabl.(l.mited finding, onl. 2 genotype E incl.ded). The purpose of this articl. is to reduce the barrier of devel.ping a house made scanning probe microscope （SPM）. Here in this paper, we cover al. the detail. of programming an SPM control.er with l.bVIEW. The main control.er has three major sequential.portions. They are system initial.zation portion, scan control.and image displ.y portion and system shutdown portion. The most compl.cated and essential.part of the main control.er is the scan control.and image displ.y portion, which is achieved with various paral.el.tasks. These tasks are scan area and image size adjusting modul., Y axis scan control.modul., X axis scan and image transferring modul., parameters readjusting modul., emergency shutdown modul., etc. A NI7831R FPGA board is used to output the control.signal. and util.ze the Z-axis real.time feedback control.. The system emergency shutdown is al.o carried out by the FPGA modul.. Receiving the shutdown command from the main control.er, the FPGA board wil. move the probe to its XYZ zero position, turn off al. the high vol.age control.signal. and al.o el.minate the possibl. oscil.ations in the system. Final.y, how to operate the control.er is al.o briefl. introduced. That messy wires fl. back and forth is the main drawback of l.bVIEW programming. Especial.y when the program is compl.cated, this probl.m becomes more serious. We use a real.exampl. to show how to achieve compl.x functional.ties with structural.programming and paral.el.mul.i task programming. The actual.code showed in this paper is cl.ar, intuitive and simpl.. Fol.owing the exampl.s showed in this paper, readers are abl. to devel.p simpl. l.bVIEW programs to achieve compl.x functional.ties.

Fl.w Dynamics around Two Side by Side circul.r cyl.nders with Al.ernating Movements

The fl.w dynamics is anal.zed through two-dimensional.numerical.simul.tions around two circul.r cyl.nders arranged side by side, with 4 combinations of al.ernating motions. Al. simul.tions are performed for Re = 1000, ampl.tude of oscil.ation (A) equal.to 3, frequency ratio (fr) of 0.5, specific rotation (α) equal.to 0.5 and different val.es of spacing ratio (l.D). It is verified that the combination of the type of movement, together with the position of one cyl.nder in rel.tion to the other, exerts significant infl.ence on the fl.w dynamics, as wel. as on the pressure distribution around the cyl.nder surface and on the average val.es of the fl.id dynamics coefficients. The smal.est val.e of the average pressure coefficient (cp = -3.3), is obtained for the oscil.ating cyl.nder when pl.ced side by side with the cl.ckwise rotation cyl.nder, case 3 and l.D = 1.5. On the other hand, the l.west mean drag coefficient (cd = 1.0788), is obtained for the cyl.nder with countercl.ckwise rotation, l.cated in the l.wer position in rel.tion to oscil.ating cyl.nder in the upper position, with spacing between them of 1.5. Furthermore, it is observed that the rotation movement is more effective in reducing drag than the rotation-oscil.ation movement, for the studied frequency ratio.

Many studies have been dedicated to the devel.pment of scaffol.s for improving post-traumatic nerve regeneration. The goal.of this study was to assess the effect on nerve regeneration, associating a hybrid chitosan membrane with non-differentiated human mesenchymal.stem cel.s isol.ted from Wharton＇s jel.y of umbil.cal.cord, in peripheral.nerve reconstruction after crush injury. chromosome anal.sis on human mesenchymal.stem cel. l.ne from Wharton＇s jel.y was carried out and no structural.al.erations were found in metaphase. chitosan membranes were previousl. tested in vitro, to assess their abil.ty in supporting human mesenchymal.stem cel. survival. expansion, and differentiation. For the in vivo testing, Sasco Sprague adul. rats were divided in 4 groups of 6 or 7 animal. each： Group 1, sciatic axonotmesis injury without any other intervention （Group 1-crush）; Group 2, the axonotmesis l.sion of 3 mm was infil.rated with a suspension of 1 250 -1 500 human mesenchymal.stem cel.s （total.vol.me of 50 pl. （Group 2-crushcel.）; Group 3, axonotmesis l.sion of 3 mm was enwrapped with a chitosan type Il. membrane covered with a monol.yer of non-differentiated human mesenchymal.stem cel.s （Group 3-crushchitl.l.el.） and Group 4, axonotmesis l.sion of 3 mm was enwrapped with a chitosan type III membrane （Group 4-crushchiUl.）. Motor and sensory functional.recovery was eval.ated throughout a heal.ng period of 12 weeks using sciatic functional.index, static sciatic index, extensor postural.thrust, and withdrawal.refl.x l.tency. Stereol.gical.anal.sis was carded out on regenerated nerve fibers.
