Unilateral mimicking bilateral BPPV- a forgotten entity? Characteristics of a largecohort of patients, comparison with posterior canal BPPV and clinical implications

Objective: Unilateral mimicking bilateral benign paroxysmal positional vertigo (umb-BPPV) was attributed to inappropriate head positioning during testing of the posterior canal. Despite its inclusion in theDiagnostic criteria for the classification of vestibular disorders of the Barany Society, the clinical characteristics and treatment responsiveness of this BPPV subtype have not been intensively ***: Records of patients with BPPV seen at a single outpatient dizziness clinic during the years 2000e2020 were reviewed. Eighty seven patients with umb-BPPV and 86 random patients with posteriorcanal BPPV (p-BPPV) were retrieved. Their demographics and BPPV characteristics were ***: Patients' and BPPV characteristics were similar in umb- and p-BPPV except for the prevalence ofmales in the umb-BPPV group. No differences were found between treatment responsiveness and recurrences in both groups. The recurrence rate of umb-BPPV was not influenced by age, gender, BPPV side,duration of symptoms or treatment responsiveness during the first ***: In accordance with our hypothesis about mixed canalo- and cupulolithiasis as the underlying mechanism of umb-BPPV, patients did not differ in characteristics and treatment responsivenessfrom p-BPPV patients. Recognition of umb-BPPV is important since inappropriate treatment can cause anunnecessary delay in therapy success.

西地那非(Sidenafil,万艾可,俗称伟哥)是一种非特异性5型磷酸二酯酶抑制剂,对环鸟苷单磷酸酯合成酶(cGMP)具有抑制作用。一氧化氮释放增多可导致cGMP水平提高。其主要临床效应是促进海绵体平滑肌松弛,进入阴茎血流量增加,有助于勃起功能障碍的缓解。较轻的副作用为头痛、潮红、鼻塞、色彩视觉缺陷和严重的心血管、脑血管副作用,均有报道,到目前为止尚未报道属于神经系统副作用的惊厥性癫痫发作。本文作者报告2例病人在服用西地那非不久,首次发生癫痫发作。

Elimination of murine and human T-cell epitopes in recombinant immunotoxin eliminates neutralizing and anti-drug antibodies in vivo

Antibodies against the toxin portion of recombinant immunotoxins （RIT） reduce their efficacy and pose a potential safety risk. To overcome this problem we mutated the very immunogenic immunotoxin SSIP to produce LMB-T20, a de-immunized RIT that has the eight human T-cell epitopes in SSIP modified or removed. To determine the effect of T-cell epitope removal in vivo we mapped the T-cell epitopes in immune-competent BALB/c mice and found that these mice recognize two epitopes. One corresponds to the human immunodominant T-cell epitope and the other to a human subdominant epitope; both were eliminated in LMB-T20. We found that mice immunized with LMB-T20 did not have T-cell activation and did not develop anti-drug antibodies （ADA）, whereas mice immunized with SSIP, showed T-cell activation, and developed ADA detected by both ELISA and drug neutralizing assays. The ability of the mice treated with LMB-T20 to respond to other antigens was not compromised. We conclude that elimination of T-cell epitopes is sufficient to prevent formation of antibodies to an immunogenic foreign protein.

There is substantial public concern about the health risks of proximity to petrochemical *** the Haifa Bay Area(HBA),which contains Israel’s densest industrial area,these concerns have been strengthened by elevated cancer mortality rates since the late *** studied the association between adolescent exposure to industrial air pollution in the HBA and adult-onset *** is a historical cohort *** study population comprised 2,187,317 subjects,using the Israeli medical corps data linked to the Israel National Cancer Registry with follow-up of up to 45 *** assessments were estimated by a spatial kriging interpolation model of SO2,serving as a marker for the dispersion of air pollution emitted from the complex during the study *** found increased crude(HR=1.23,95%CI=1.17 to 1.29)and adjusted(HR=1.16,95%CI=1.10 to 1.21)risk of cancer with increased exposure to air pollution in *** associations remained robust in analyses stratified by decade and socio-economic *** found evidence of monotonically increased risk in five of 13 cancer categories(leukemia,melanoma,female breast,central nervous system,and thyroid tumors).Our findings strengthen the hypothesis that this exposure posed a carcinogenic risk during the study period.
