Frequency of Transfusions and Risk Factor for Bleeding Risk to Guide a Blood-Sparing Program during Hip Arthroplasty in Gabon

The amount of digital audio and video documents being shot and stored in large archives is growing faster than ever before. However, lack of effective content searching methods is a major barrier that prevents people from operating audio and video databases pervasively and intelligently. This paper reviews the state-of-the-art in audio information retrieval and presents an audio searching solution that uses statistical pattern matching techniques. The two-tier speech retrieval solution proposed in this paper, named Audio Search, is composed of a fast searching component followed by an ends-free Viterbi keyword spotter combined with a histogram-based phoneme duration model. Its performances have been evaluated on the 1997 HUB4 database for Chinese broadcast news. In comparison with the results of large vocabulary automatic speech recognition based systems, the results we obtained with the Audio Search system show significant improvements for both long queries (typically 5 to 7 syllables) and out-of-vocabulary (OOV) queries.

Background: Hip arthroplasty is a surgery with a high risk of bleeding. The main objective of this study was to research risk factors for bleeding and transfusion in patients undergoing hip arthroplasty in order to guide a blood-saving program. Method: Retrospective, monocentric, descriptive and analytical study carried out from June 2011 to June 2021 at the Omar Bongo Ondimba army training hospital. The variables studied were demographic characteristics, biological variables, transfusion data, anesthetic and intraoperative data. The primary outcome was intraoperative or postoperative transfusion. A univariate and multivariate analysis was conducted to identify the factors associated with the occurrence of a transfusion. Results: Of the 276 patients included, 179 benefited from Total Hip Prosthesis (THP) and 97 from Intermediate Hip Prosthesis (IHP). Spinal anesthesia was performed in 67.4% of patients. The ASA 2 score predominated (65.9%). The transfusion incidence was 56.9% (157/276). Tranexamic acid was used in 16.3% (45/276) of patients. The average bleeding was 528 ± 405 ml. Preoperative anemia (OR = 0.78, 95% CI [0.66 - 0.91]) and total hip prosthesis (OR = 2.02 95% CI [1.11 - 3.67]) were predictors of bleeding and transfusion to be significant. The average serum hemoglobin predictive of a transfusion was 11.6 ± 1.8. ASA score and operative time were not found as risk factors for bleeding and transfusion. Conclusion: The incidence of transfusion is high. Preoperative anemia remains a major but modifiable risk factor unlike the choice of implant. The implementation of a patient blood management protocol could reduce this transfusion incidence.

Since the emergence of the novel 2019 coronavirus pandemic in December 2019(COVID-19),numerous modellers have used diverse techniques to assess the dynamics of transmission of the disease,predict its future course and determine the impact of different control *** this study,we conducted a global systematic literature review to summarize trends in the modelling techniques used for Covid-19 from January 1st,2020 to November 30th,*** further examined the accuracy and precision of predictions by comparing predicted and observed values for cumulative cases and deaths as well as uncertainties of these *** an initial 4311 peer-reviewed articles and preprints found with our defined keywords,242 were fully *** studies were done on Asian(78.93%)and European(59.09%)*** of them used compartmental models(namely SIR and SEIR)(46.1%)and statistical models(growth models and time series)(31.8%)while few used artificial intelligence(6.7%),Bayesian approach(4.7%),Network models(2.3%)and Agent-based models(1.3%).For the number of cumulative cases,the ratio of the predicted over the observed values and the ratio of the amplitude of confidence interval(CI)or credibility interval(CrI)of predictions and the central value were on average larger than 1 indicating cases of inaccurate and imprecise predictions,and large variation across *** was no clear difference among models used for these two *** 75%of predictions that provided CI or CrI,observed values fall within the 95%CI or CrI of the cumulative cases *** 3.7%of the studies predicted the cumulative number of *** 70%of the predictions,the ratio of predicted over observed cumulative deaths was less or close to ***,the Bayesian model made predictions closer to reality than classical statistical models,although these differences are only suggestive due to the small number of predictions within our dataset(9 in total).In addition,we found a significant neg

本论文的研究目的在于评估法国汝拉地区葡萄酒的生产现状,及在尊重当前中国进出口的实际操作流程的同时,该地区葡萄酒生产者如何将其产品出口至中国的可能性。目前,葡萄酒生产者在其所在地占有较高的市场份额,拥有其所处领域卓越的专业知识,良好的财务状况和出口至各国的有利条件。由此可见,他们已具备将其葡萄酒业务扩展至中国的理想前提。在初步定义产品可出口后,本文将确定目标市场选定的关键因素。鉴于当前欧洲和世界其它国家葡萄酒业的现状,世界上大多数葡萄酒生产者及汝拉葡萄酒的生产者都已决定将其产品的一部分出口至中国市场。事实上,中国市场的确具备诸多吸引葡萄酒生产者的优势。中国葡萄酒消费和进口量的逐年递增,种类繁多的法国葡萄酒在中国市场成功进驻的先例,都充分显示出汝拉葡萄酒具备在中国成功的潜力。同时,中国人也越来越希望能够不仅了解葡萄酒本身,而且了解其富含的文化底蕴。此外,中国大城市的西化进程也促进了葡萄酒消费的大众化。为了提高在中国市场的占有率和减少实施困难,本文分析和了解中国市场特征是十分必要的。须针对中国现状,瞄准其潜在的庞大消费群制定相应的销售及市场策略,最终实现向大型连锁超市或小型葡萄酒商,甚至餐厅或酒店直接销售的目标。明确在进口环节和分销渠道上的职能分配同样也是扩展中国市场根本因素。找到良好的合作伙伴及如何找到他们是一个公司在中国开展业务面临的主要挑战之一。除此之外,一项重要的任务即是分析出口产品至中国市场所涉及物流的不同环节(海关、运输、物流、保险、食品标准和包装)。

Dear Editor,The proto-oncogene MYC has a well-documented role in driving androgen receptor(AR)oncogenic functions in prostate cancer(PCa cells)[1]and was recently reported as a key activator in the regulation of AR transcription[2].Dissecting the mechanisms underlying MYC-mediated AR regulation may be crucial for better understanding PCa development and improving its *** recently demonstrated that menin,encoded by the MEN1 gene,plays an oncosuppressive role in the generation of ARlow/CD44+microinvasive prostate adenocarcinoma in mice but exerts oncogenic effects specifically in ARdependent human PCa cells,both likely by modulating AR transcription and AR targets[3].We herein wondered whether this oncogenic function of menin would occur through its interaction with other factors known to play a crucial role forAR transcription in PCa,in particular,MYC.
