The lack of data on air quality monitoring and neglected and overlooked pollutant emissions in the transportation and industrial sectors are motives for the government of Senegal to set up, in 2009, an air quality management center, the CGQA (Centre de Gestion de la Qualité de l’Air). Air quality monitoring at CGQA deals with mainly six pollutants: carbon monoxide (CO), nitrogen oxides (NOx) (with nitrogen dioxide (NO2) and nitrogen monoxide (NO)), sulfur dioxide (SO2), ground-level ozone (O3), Benzene-Toluene-Xylenes (BTX), and particulate matters (PM) with diameters less than 10 μm (PM10) and 2.5 μm (PM2.5). The concentration levels of gaseous pollutants (CO, O3, NO2 and SO2) in the city generally remain below the limit value set by the WHO (World Health Organization). Reassessment of the Resources of a Deep Aquifer System under Physical and Chemical Constraints:The Maastrichtian Aquifer

The deep and confined Maastrichtian aquifer contains considerable groundwater resources. It stretches over nearly 200,000 km2, from the northern part of Mauritania to the South of Guinea Bissau where it becomes shallow. The reservoir is composed mainly of coarse sands and sandstone interbedded with some clay units. The aquifer provides 40% of the total drinking water extracted from the different aquifers and nearly 800 wells equally distributed operate only in the top 50 m of the aquifer. Despite the importance of these resources for providing water in the rural and urban areas, the aquifer characteristics are not well defined. The present paper aims to define first the physical and chemical characteristics of the Maastrichtian aquifer. The reserve of the aquifer initially estimated at 350 billion m3, is reassessed using new data providing from cross sections realized as part of our research, through the Water Sectorial Project of the Ministry of Hydraulics. Data from oil wells and geophysical logging are used to investigate the geometry of the aquifer and the position of the fresh/salty water interface. The highest thickness of the aquifer is between 200 to 400 m and salty water is present below the fresh groundwater in the west side of the aquifer. In the Eastern side, potable water lies directly above the basement. The thickness of the aquifer increases from the west to the center, and then decreases towards the shallow basement rock in the South East. The average thickness is 250 m. Chemical data coming from pumping wells indicate high chloride (250 - 1600 mg/l) and fluoride content (1 - 5.5 mg/l). Therefore, the reassessment has to take into account the chemical aspect of the water.

Management and Maternal-Fetal Prognosis of Placental Abruption: A Retrospective Study of 130 Cases

Objective: To describe the epidemiological, clinical and prognostic factors and assess treatment of placental abruption in the obstetrics gynecology department of the Dakar Principal Hospital. Patients and Method: We carried out a retrospective observational study of 130 successive cases of placental abruption, which occurred from January 2015 to December 2017 at the Level 3 Maternity Unit of the Dakar Principal Hospital. Data were collected from non-computerized obstetric records and analyzed using Excel and Epi info software. Results: There were 130 cases of placental abruption, that is a prevalence of 1.5%. The average age of onset of placental abruption was 30 years. The history of hypertension concerned 32.3% of patients, the average gestational age of 32.5 weeks at the time of diagnosis, grade 3 of Sher was found in 48.5% of cases. The outcome of the pregnancy was a caesarean section in 79.2% of cases, the average weight of newborns was 2058 g. The management of the complications required a blood transfusion and intensive care. A haemostasis hysterectomy was performed in 6.2% of cases. Stillbirth rate was 53.7% and maternal mortality was zero. Discussion and Conclusion: Placental abruption, a severe complication of pregnancy, is associated with high perinatal morbidity and mortality linked to the severity of the clinical picture, despite an improved maternal prognosis.

Acute Pancreatitis during SARS CoV 2 Infection

Introduction: Covid-19 is a systemic disease that can spread to all systems. Among the gastrointestinal manifestations, pancreatic involvement can have a major prognostic impact. We report 3 acute pancreatitis cases occurring during Covid-19, in Dakar. Case 1: 65-year-old woman who presented with intense atypical epigastric pain. Physical examination revealed obesity, high blood pressure and abdominal tenderness. Biological tests found increase CRP (134 mg/l) and lipasemia (312 UI/l). Abdominal CT scan showed findings of Balthazar grade C acute pancreatitis. RT-PCR for SARS CoV 2 RT-PCR was positive. The outcome was favorable. Case 2: 56-year-old woman patient with history of nephroangiosclerosis who presented with dyspnea, cough, fever and moderate epigastric pain. Physical examination revealed epigastric tenderness, high blood pressure, anuria. Biological testing, noted increase CRP (96 mg/l), lipasemia (793 UI/l), creatinine (227 mg/l) and urea (3.84 g/l). Abdominal CT scan showed acute edematous pancreatitis findings. SARS CoV 2 RT PCR was positive. The outcome was favorable. Case 3: 27-year-old man who presented with physical asthenia, headache, and epigastric pain. Physical examination found epigastric tenderness. Elevated CRP level was of 102 mg/l and lipasemia level was of 427 UI/l (7N). Abdominal CT scan showed acute edematous pancreatitis findings. SARS CoV 2 RT PCR was positive. The outcome was favorable. Conclusion: Acute pancreatitis can occur during Covid-19 infection. However, the imputability to the Covid-19 disease necessitates to rule out the most common causes.

Varicocele is the first etiology of infertility in men worldwide and affects almost half of the men dealing with infertility in Senegal. Varicocele is associated with a high rate of sperm DNA fragmentation so this test is being more and more recommended especially when patients have to benefit from an ART (Assisted Reproductive Technology) procedure. The aim of our study was to assess if our patients with varicocele are concerned by higher DNA fragmentation rate in spermatozoon. Sperm samples were collected (95 cases of varicocele and 30 controls) and analyzed between January 2013 and June 2016 in a prospective study. Sperm DNA damage, characterized by the rate of DNA fragmentation was evaluated by TUNEL assay using fluorescence microscopy. There was a significant higher percentage of DNA fragmentation in patients with varicocele (p < 0.001), compared to controls. We plan to study a bigger cohort to assess this tendency and study the benefits of surgery in varicocele by evaluating DNA fragmentation after the procedure.

Electrochemical reduction is one of the most suitable methods for the treatment of highly nitrate-contaminated solutions. This work focuses on the optimization of parameters influencing the electrochemical denitrification of water by the Ti/RuO2 + IrO2 electrode. The methodological approach used consists in carrying out a series of electrolysis by scrutinizing the reaction selectivity according to the experimental conditions. For this study, the ions concentrations before and after electrolysis were determined by UV-vis absorption spectroscopy. The results of the process optimization showed that the electrochemical reduction ofis efficient at neutral pH after 120 mn of electrolysis at -100 mA. In contrast to works found in the literature, this study highlighted the process modeling that could open interesting perspectives to develop new treatment methods of polluted waters.

Cam-Clay Models Using Castem 2000<sup>©</sup>and Plaxis<sup>©</sup>for the Study of the Slope Stability of the “Corniche Ouest” of the Dakar Peninsula (Senegal, West Africa)

The stability of the natural slopes of the “Corniche Ouest” of Dakar Capital was intensively researched by previous [1] in [2] and recent studies [3]. Soils near the surface consist of weathered lateritic cuirass, silt and marl that have been intensively altered and fractured. Below these altered layers are limestones that are interbedded with other thin volcanic layers. Prior to the 1950s the area had been less urbanized. In the 1950s demand for habitations and administrative offices led to the development of the area but, no drainage system was available. In 1984 the area began moving down slope toward the ocean. Rates of movement were the fastest several months after the end of the rainy season and the slowest during the dry season. In the next years the earthflow moved as much as some meters, but in the processes the expensive homes and administrative buildings built became uninhabitable. Movement was caused by a combination of wave erosion along the coast removing some the mass resisting flow, added water due to the disposal of wastes, watering of lawns, and rainfall causing the clays to expand and weaken, and by the added weight of development on top of the flow. The first investigations, due to a lack of geotechnical information describe a stable area even though it was observed cracks and failure in habitations. The main results obtained were specially obtained by analytical techniques of slices. It is well known that the mechanical response of a solid mass of soil depends primarily on the laws of behavior which were affected to him. The Cam-Clay model is used with two comparative numerical codes such as Castem 2000?, a non commercial code and Plaxis 7.2? so as to compare the mechanical response of the slopes. The models of Cambridge are founded as simple theory and use only some soil mechanical parameters easy to determine. The results are founded highly different from a code to another, but they attest all that the system described is non stable. Otherwise

Infantile Spinal Muscular Atrophy at the Albert Royer National Children’s Hospital Center in Dakar

Introduction: Infantile spinal muscular atrophy (ISA) is an autosomal recessive disease caused by primary degeneration of cells in the anterior horn of the spinal cord, leading to muscle weakness and hypotonia. Its incidence is estimated at 1 in 6000 births worldwide. In Africa, particularly in Senegal, there are few studies interested on this pathology. We therefore deemed this study necessary, which set itself the objective of describing the diagnostic, therapeutic and progressive aspects of infantile spinal muscular atrophy at the Albert Royer National Children’s Hospital Center in Dakar (CHNEAR). Methodology: We conducted a retrospective descriptive study over a period of two (2) years from December 2020 to December 2022. Included were all hospitalized patients in whom the diagnosis of spinal muscular atrophy was made with or without genetic confirmation. The data were collected on a pre-established form then entered and analyzed with the following software: Excel 2013 and R version 4.1.3. Results: During our study period, 2100 children were hospitalized, the annual incidence was 0.76%. The average age of our patients was 9 ± 9 months with a range of 3 months to 32 months and the median was 6.5 months. The sex ratio was 7. The notion of family consanguinity was found in 62.5% of cases and the notion of ISA in the family in 25% of cases. Hypotonia and respiratory distress were found at the forefront in equal proportions (50% of cases). Electromyogram (EMG) was performed in 3 patients (37.5%). Symptomatic medical treatment was administered in 100% of patients, 04 patients had benefited from respiratory physiotherapy, i.e. 50% of cases, and genetic counseling was carried out in one patient (12.5%). The evolution was immediately favorable in 2 patients or 25% of cases, unfavorable in 75% of cases with a death rate of 50% and the average age of death was 5.5 months ± 1 with extremes ranging from 3 to 7 months. Haemorrhagic Lupus: A Diagnosis Emergency, Report about 6 Cases

Introduction: Haemorrhagic manifestations rarely represent the mode of onset of lupus and are a therapeutical and diagnosis emergency. We report 6 cases of haemorrhagic manifestations disclosing lupus. Patients and methods: It’s about a retrospective, descriptive and multicentric study ranged from 1st January 2014 to 5th January 2019. Were included in the study all cases of lupic disease revealed by hemorrhagic disorders, recorded in 3 Internal Medicine Departments in Dakar: DalalJamm, Dantec, Pikine. The diagnosis of lupus was made on the basis of the 1997 modified classification criteria of ACR for SLE. Results: We gathered 6 cases of women affected by lupic disease. The mean age was 32 years old with extremes ranged from 17 to 56 years. According to haemorrhagic disorders, the diagnosis of lupus was automatically made in 5 of the cases and it took 6 months to confirm the last one. Haemorrhagic features were ruled by purpura (3/6 cases) and nose-bleeding associated with gumbleeding (2/6 cases), epistaxis and melena (1/6 case), hypermenorrhea (1/6 case), cerebral haematoma (1/6case). Cutaneous (5/6), articular (4/6), renal (2/6) and serous (2/6) disorders specially represented the other manifestations. Biological findings showed cytopenia with 3 cases of anaemia, 1 case of leucopenia and 6 cases of thrombopenia. The average of the platelet count was at 20,000 cells/mm3 with extremes ranged from 1000 to 35,000 cells/mm3. Thrombocytopenia was related to autoimmune thrombocytopenic purpura in 3 cases, to Evans’ syndrome in 2 cases, to macrophage activation syndrome in 1 case. Immunological investigations found the presence of antinuclear antibodies (1/6), anti-dsDNA (2/6) and anti-Sm (4/6). All patients received a bolus of Solumedrol followed by an administration of Prednisone at a dosage of 1.5 mg/kg (3 cases) and 1 mg/kg to the other patients.
