Heterotopic pancreas is defined as pancreatic tissue found outside the usual anatomical location of the pancreas. It is often an incidental finding and can be found at different sites in the gastrointestinal tract. It may become clinically evident when complicated by pathological changes such as inflammation, bleeding, obstruction, and malignant transformation. We report an unusual case of a 25-year-old male patient with pain epigastrium caused by a polyp at the body of stomach along greater curvature revealed at endoscopy. Endoscopic biopsy was s/o inflammatory pathology. Wide excision of polyp was done and a histological diagnosis of heterotopic pancreas was established. The difficulty of making an accurate preoperative diagnosis is highlighted.

The study endeavors to provide statistical inference for a (1 + 1) cascade system for exponential distribution under joint effect of stress-strength attenuation factors. Estimators of reliability function are obtained using Maximum Likelihood Estimator (MLE) and Uniformly Minimum Variance Unbiased Estimator (UMVUE) of the parameters. Asymptotic distribution of the parameters is also obtained. Comparison between estimators is made using data obtained through simulation experiment.

Maximum Voluntary Ventilation (MVV), one of the components of Pulmonary Function Testing (PFT), has multiple uses. Various factors including the inspiratory muscle strength (IMS) influence its magnitude. Our aim was to quantify the IMS indirectly using an economical and non invasive bedside assessment tool, determine its association with MVV and then develop a predictive equation for MVV. 41 healthy non-athletic physical therapy students participated in the study. IMS measurement was performed with a sphygmomanometer. Average of the three net deflections in sphygmomanometer following deepest possible breaths was taken as indirect measurement of IMS in mm of Hg. MVV was measured according to ATS guidelines using a spirometer. Results from the data analysis revealed a significant correlation between IMS and MVV(r = 0.83, p < 0.001) and the coefficient of determination = 0.68. So, we developed a regression equation: Y = 1.9669(X) + 49.838 with SEE: 13.02L/min and ANOVA for the equation was (F=68.9, p < 0.001). Hence, it can be concluded that a strong correlation between the indirect IMS and MVV was established and a predictive equation to estimate MVV was developed. This equation proved to have a high predictive value with a small error of estimation. This indicates that the value of the indirect IMS measurement obtained using the sphygmomanometer can be used to estimate MVV in normal healthy individuals without the use of a conventional spirometer.

One of the significant health issues affecting women that impacts their fertility and results in serious health concerns is Polycystic ovarian syndrome(PCOS).Consequently,timely screening of polycystic ovarian syndrome can help in the process of *** a method to aid doctors in this procedure was crucial due to the difficulties in detecting this *** research aimed to determine whether it is possible to optimize the detection of PCOS utilizing Deep Learning algorithms and ***,feature selection methods that produce the most important subset of features can speed up calculation and enhance the effectiveness of *** this research,the tri-stage wrapper method is used because it reduces the computation *** proposed study for the Automatic diagnosis of PCOS contains preprocessing,data normalization,feature selection,and classification.A dataset with 39 characteristics,including metabolism,neuroimaging,hormones,and biochemical information for 541 subjects,was employed in this *** start,this research pre-processed the *** for feature selection,a tri-stage wrapper method such as Mutual Information,ReliefF,Chi-Square,and Xvariance is ***,various classification methods are tested and *** learning techniques including convolutional neural network(CNN),multi-layer perceptron(MLP),Recurrent neural network(RNN),and Bi long short-term memory(Bi-LSTM)are utilized for *** experimental finding demonstrates that with effective feature extraction process using tri stage wrapper method+CNN delivers the highest precision(97%),high accuracy(98.67%),and recall(89%)when compared with other machine learning algorithms.

Pseudoaneurysm of Ascending Aorta with Extrinsic Compression of Left Main Coronary Artery

Pseudoaneurysm of ascending aorta carries high mortality and morbidity due to adhesions and compression over surrounding structures. It occurs either after some form of injury to aorta or after infectious pathology. Spontaneous pseudoaneuryms are very rare as are the extrinsic compression of left main coronary artery. We are presenting a case with both of these rare ties. Patient was a female of middle age presented with cough and a past history of treated pulmonary tuberculosis 12 years before. During evaluation by X-ray, CECT is a large pseudoaneurysm of ascending aorta with oval defect in posterior wall above sinotubular junction. It was compressing 90% of the left main coronary artery, occluding 100% of the right pulmonary artery with formation of collaterals to it from surrounding arteries. Active tuberculosis and syphilis were ruled out. Patient underwent excision of pseudoaneurysm, replacement of ascending aorta by Dacron graft and coronary artery bypass under TRCA and CPB. There was massive intraoperative bleeding, and patient died on the fourth day due to sepsis and ARDS.

Coronavirus particles contain four main structural proteins. These are the spike (S), membrane (M), envelope (E), and nucleocapsid (N) proteins, all of which are encoded within the 3’ end of the viral genome. The S protein (~150 kDa) utilizes an N-terminal signal sequence to gain access to the ER, and is heavily N-linked glycosylated. In view of the increase in death due to COVID-19 (coronavirus disease), it is important to investigate the potential effects the coronavirus on different organs. A literature search was performed from Elsevier, Pubmed, Springer, and Hindawi, and literature is reviewed using customized search strategies. The search strategy included the following terms Cardiovascular effect, Neurological effect, and kidney. Myocardial damage is a common occurrence in patients with COVID-19 disease hospitalisation. This is characterized by a rise in troponin. Vascular endothelial damage in both small and mid-sized pulmonary vessels was noted together with Disseminated intravascular coagulation (DIC), Deep vein thrombosis (DVT), and Pulmonary embolism (PE), resulting in pulmonary infarction. Liver damage in patients with coronavirus infections might be directly caused by the viral infection of liver cells. Neuronal pathway is an important vehicle for neurotropic viruses to enter the CNS (Central Nervous system). Recent research studies show that apoptosis is implicated in a variety of ocular disorders, including glaucoma, retinitis pigmentosa, cataract development, retinoblastoma, retinal ischemia, diabetic retinopathy, and ocular murine glaucoma. The more the understanding about this new virus and its occurrence, the better the ability of people to cope with it. It’s far hoped that we will conquer COVID-19 soon with the invention of powerful vaccines, pills, and remedies.

The present study was designed to investigate the possible role ofHPA （hypothalamic-pituitary-adrenal） axis in neuropathic pain in streptozotocin induced diabetic rats. Wistar albino rats of either sex weighing 180-220 g （n = 6） were employed. Diabetes was induced by administering STZ （streptozotocin）（45 mg/kg, i.p.） once. Neuropathy was induced by the ligation of sciatic nerve in diabetic animals. A glucocorticoid receptor antagonist： ketoconazole 175 mg/kg, p.o. and glucocorticoid receptor agonist： hydrocortisone 1 mg/kg, i.p. were given. Assessment of neuropathic pain was achieved using hot plate and hot immersion tests. Tissue biochemical： lipid peroxidation, NO and glutathione were estimated on 28th day spectrophotometrically. Plasma cortisol level was also estimated using HPLC （high performance liquid chromatography）. Sciatic nerve ligation to diabetic animals caused significant increase in nociceptive responses as hyperalgesia and allodynia, tissue lipid peroxidation and NO products, and decrease in GSH （glutathione） level, in comparison to SC （saline control）. Ketoconazole administration produced decreased pain responses as well as decrease in oxidative stress and inflammatory mediators. Administration of hydrocortisone resulted in suppression of HPA axis. The activation of HPA axis in diabetic animals resulted in significant pain response. This may be due to increased corticosterone level which in turn desensitizes HPA axis and decreased nociceptive response in diabetic animals observed.

Abdominal cocoon is a rare disease of the peritoneum and the rare cause of intestinal obstruction. The etiology of this disease is largely unknown. It primarily affects young adolescent females. We report a case of abdominal cocoon in a 16-year-old girl patient who presented with recurrent episodes of subacute intestinal obstruction.
