Pancreatic Cancer: Epidemiological, Clinical and Therapeutic Aspects in Abidjan (Ivory Coast)

Aim: The aim was to study the epidemiological, clinical and therapeutic aspects of pancreatic cancers in Ivory Coast. Materials and methods: A retrospective multi-center and multidisciplinary study aimed at describing sixteen years and four months from January 1, 2000 to April 30, 2016 in several hospitals in the city of Abidjan taking charge of pancreatic cancers (hepato-gas troenterology services of the universities hospitals of Cocody and Yopougon), internal medicine departments of University Hospital of Treichville and Abidjan Military Hospital, the department of oncology of the university hospital center of Treichville, private clinics (DANGA, PISAM and GMP), the COBA center in Treichville. The diagnosis of cancer was based on either histological criteria or a set of clinical and paraclinical arguments. Patients whose files did not include imaging were excluded from this study. The following parameters were collected: age, sex, reason for consultation, physical signs, personal and family antecedents, tumor characteristics, delay in diagnosis and the nature of the treatment administered. Results: We have observed 150 cases of pancreatic cancer. The mean age of patients was 59.09 years with extremes ranging from 24 years to 88 years. Our study population consisted of 98 men and 52 women (sex ratio 1.88). Risk factors were dominated by diabetes 20.7%. Epigastralgia is the main reason for consultation (48%). The tumor was localized at the cephalic level in 84.67%. In 13 cases the diagnosis was histological and was dominated by adenocarcinoma (100%). The majority of our patients was seen at an advanced stage of the disease: stage IV (74.56%). The average time to diagnosis was 2.76 months. Fourteen of our patients (9.33%) had benefited surgery. This was curative surgery in 35.71% (n = 5) and palliative in 64.29% (n = 9). Among those who benefited from this curative surgery, three died two months after surgery and two lost to follow-up. Evaluation of Soil Quality under Cocoa Trees in the Marahoué Region (Central-Western Côte d’Ivoire)

This study, which assesses the quality of soils under cocoa trees in the Marahoué region of Côte d’Ivoire, is being carried out against a backdrop of declining crop yields due to a number of factors, including the loss of soil fertility. The aim was to identify homogeneous groups of soils in terms of fertility and propose sustainable management methods for each group. The study was carried out on twenty-four cocoa orchards belonging to the SCOOP KAPATCHIVA cooperative, spread over eight sectors. A morphological characterisation of the soils was carried out in the field, followed by physico-chemical laboratory analyses of soil samples taken in the field. Soil fertility was analyzed on the basis of these two groups of data. The results showed that soil depths were generally high, except in some high altitude areas, where soils were often enriched in ferromagnesian elements and compacted. The texture of the soils varied between loamy and sandy-loamy, influencing cocoa production. Indeed, the soils of the plots with high production were looser and without ferromagnesian concretions, while those of the plots with low production were more gravelly and compact. biological activity in the upper soil layer (0 - 30 cm) was notable with an abundance of termites and earthworms. The organic matter and nitrogen content varies considerably between sectors, directly impacting soil fertility. The soils were slightly acidic and showed significant differences in the levels of exchangeable magnesium and potassium, and not at all for phosphorus which appeared at very low doses. Chemical analysis also revealed that nutrient balance was a key factor in orchard productivity, with varying ratios of calcium, magnesium and potassium. Knowledge of these results made it possible to make recommendations for soil management with a view to improving cocoa yields in the study area.

康养产业作为一种新兴的产业形态,成为促进经济高质量发展的重要引擎。本研究以四川省为研究范围,首先构建指标体系并运用熵值法对四川省康养产业和休闲体育产业发展水平进行测度,运用耦合协调度模型对其康养产业与休闲体育产业的耦合协调度进行测度,并探讨两者之间的路径关系,以期为四川省康养产业与休闲体育产业的融合发展提供理论依据和实践指导。研究发现:一是四川省整体康养产业与休闲体育产业的协调水平不断提升,年复合增长速度4.91%;二是四川省康养产业与休闲体育产业的耦合协调度呈上升趋势,但整体较低,仅成都进入极度协调;三是存在专业型人才数量不足、产品结构性矛盾突出、融合项目协调度不高等问题。鉴于此,提出促进二者耦合协调发展的路径选择。

Craniofacial Horn Injuries: Rares Lesional Entities at the University Hospital of Bouaké(Côte d’Ivoire)

Aim: Describe the epidemiological, clinical and therapeutic particularities of craniofacial trauma by encornment. Type of study: This was a retrospective and descriptive study. Material and methods: It took place in the surgical emergency departments of Bouaké (Côte d’Ivoire) University Hospital from January 2013 to December 2017, for a period of 5 years. It focused on patients of 0 to 5 years old admitted for craniofacial trauma involving the horn of a bovine. Results: Of the 26 cases of traumatic encornment, 11 were craniofacial location (42%). There were 9 male and 2 female victims. The average age was 10.54 years with extremes of 6 months and 24 years. Patients came from rural areas of Côte d’Ivoire. Bovids were responsible for all causal encornements. The lesions involved the scalp (3 cases) and the face (8 cases). The lesions were cutaneous-musculo skeletal in 7 cases. The surgical treatment included debridement associated to a suture in all cases and associated with maxillo mandibular blockage in 3 cases and duro-cranioplasty in one case. Conclusion: In the local context, facial cranio traumatism by encornement is rare and seems to be essentially infantile, masculine and rural problem.

Congenital nephrotic syndrome (CNS) is defined as the presence of proteinuria > 50 mg/kg/24h associated with a protein concentration g/L or albuminemia 30 g/L in an infant less than 3 months old. The CNS is rare, of various clinical forms dominated by the Finnish type caused by a mutation of the NPHS1 gene located on chromosome 19. The edematous syndrome is the most common mode of discovery. We report a case discovered in an infant of 50 days admitted for ascites of great abundance. The aim of this study was to describe the main epidemiological, diagnostic, therapeutic and evolutionary aspects of this syndrome. Improving the prognosis of this condition requires advocacy with the political authorities of Cote d’Ivoire to provide Teaching Hospital for the resources needed to perform kidney transplantation.

The primary abscess of the psoas of the child is a rare affection, the pathogenesis of which is still unexplained. The positive diagnosis of this condition is difficult and relies mainly on medical imaging. We report a case of primary psoas abscess in an immunocompetent child, whose diagnosis was facilitated by the provision of abdominal radiography without preparation and abdominal ultrasound. The evolution was favorable thanks to the combination of antibiotic therapy and surgery.

A Rare Case of a Thoracic Spinal Hemangiopericytoma

Spinal hemangiopericytomas are rare tumors. To date, only 80 cases of spinal hemangiopericytomas have been reported in the literature. The postoperative evolution of this condition in rare cases can be complicated by a symptomatic compressive epidural hematoma that can be the source of serious neurological complications requiring emergency surgery. We report a rare case of intradural and extramedullary spinal hemangiopericytomas with favorable evolution after treatment in an 82 years old woman.

Objective: To describe the epidemiological profile and indications of mutilating eyeball surgery in Abidjan. Patients and methods: This is a retrospective descriptive study on the files of eviscerated, enucleated or exentered patients in the Ophthalmology Department of the University Hospital of Yopougon from January 2010 to December 2016. The parameters studied were: socio-demographic characteristics, indications of mutilating surgery, anesthetic technique, operative technique and patient evolution. Results: During the study period, 59 patients underwent surgery for mutilating eye surgery out of a total of 11,114 procedures, a frequency of 0.53%. The average age of the patients was 31 years with extremes of 3 years and 67 years. Men predominated in 62.1% of cases with a sex ratio of 1.64. Patients came from rural areas in 68.5% of cases. The main indications of the mutilating surgery were: post-inflammatory phthisis (58.6%) and tumoral affections (29.31%). Evisceration was the most common operative technique (67.2%). In 68% of cases the interventions were performed under peribulbar anesthesia. Patients with evisceration (67.2%) and enucleation (5.1%) all benefited from prosthetic equipment. Conclusion: The mutilating surgery of the eyeball is responsible for serious psychological and sociological impacts. The seriousness of these repercussions requires primary prevention through early and adequate management of eye conditions.
