Hepatic angiosarcoma presenting as acute liver failure in young adults. Report of two cases and review of literature

Background: Hepatic angiosarcoma is a rare tumor of endothelial origin that accounts for up to 2% of all primary neoplasms of the liver. It is uncommon in young adults and acute liver failure is a very unusual presentation of this disease. Case Presentation: We report the cases of two young male adults who presented with acute liver failure. one of them was diagnosed with primary hepatic angiosarcoma after transplantation based on the complete evaluation of the heaptectomy specimen;while the other was diagnosed through a liver biopsy which was done as part of the work-up for transplantation. Both patients had a rapidly worsening clinical course and died within 2 months of diagnosis. Conclusion: Hepatic angiosarcoma is a poor prognosis disease with high and early mortality. Therefore, a high level of suspicion should be present to promptly diagnose it, especially when dealing with patients with a rapidly worsening liver disease.

The main aim of this paper is to evaluate the influence of the recycled expanded polystyrene as lightweight aggregate on the mechanical properties of lightweight cement mortar when subjected to high temperatures. Various tests have been carried out on different mixtures of mortar. The water/cement mix proportion has always been the same and only the nature of the aggregates has changed, with a partial replacement of the conventional aggregate by recycled ground EPS（EPS-G） with values ranging from 10% to 30%, achieving significant results in relation to exposure to high temperatures. In this research, the samples have been subjected to different temperatures of exposure, in order to analyze the influence of the lightweight recycled arid dosage in the mechanical properties of mortars. The results of this study show the ability of mechanical response at high temperatures with light mortars EPS-G. This study shows how this new mix can be used in different building types, optimizing construction materials and reducing mortars density while transforming a residual product into an active product.

About 60% of blueberry (Vaccinium corymbosum) plantations in Argentina are located in the provinces of Entre Ríos and Buenos Aires. Alternaria tenuissima and Pestalotiopsis guepinii have been reported as pathogens of blueberry, causing leaf spots and branch cankers. The aims of this research were to estimate the prevalence of these microorganisms on leaves and fruits taken from crops located in Entre Ríos and Buenos Aires, as well as to differentiate leaf symptoms after target inoculations with each pathogen individually and in mixtures. Both fungi were present in blueberry fields from 2010 to 2013. A. tenuissima was the most prevalent pathogen, as most of the symptoms detected in the fields had been caused by this species. As a result of inoculations on cv. o’Neal, injured tissues showed symptoms before undamaged ones. Leaf symptoms caused by A. tenuissima differed from those caused by P. guepinii because of their predominant reddish color and the absence of drop-off of the central part of the lesions. When inoculated in a mixture, incubation period on leaves was intermediate between the registered for individual inoculations. The leaves showed reddishbrown spots typical of A. tenuissima and dark brown spots typical of P. guepinii, both with red margins. Blight, defoliation and canker symptoms caused in each case were undistinguishable.

Non-alcoholic fatty liver disease(NAFLD) is an emerging health concern in both developed and non-developed world, encompassing from simple steatosis to nonalcoholic steatohepatitis(NASH), cirrhosis and liver cancer. Incidence and prevalence of this disease are increasing due to the socioeconomic transition and change to harmful diet. Currently, gold standard method in NAFLD diagnosis is liver biopsy, despite complications and lack of accuracy due to sampling error. Further, pathogenesis of NAFLD is not fully understood, but is well-known that obesity, diabetes and metabolic derangements played a major role in disease development and progression. Besides, gut microbioma and host genetic and epigenetic background could explain considerable interindividual variability. Knowledge that epigenetics, heritable events not caused by changes in DNA sequence, contribute to development of diseases has been a revolution in the last few years. Recently, evidences are accumulating revealing the important role of epigenetics in NAFLD pathogenesis and in NASH genesis. Histone modifications, changes in DNA methylation and aberrant profiles or micro RNAs could boost development of NAFLD and transition into clinical relevant status. PNPLA3 genotype GG has been associated with a more progressive disease and epigenetics could modulate this effect. The impact of epigenetic on NAFLD progression could deserve further applications on therapeutic targets together with future non-invasive methods useful for the diagnosis and staging of NAFLD.

Nowadays, mobile technology makes it possible for us to realize processes in a relatively short amount of time, showing the user friendly and efficient interfaces that any person is capable of adapting to. Continuing this trend, we develop a mobile application that aids in giving an early diagnosis of Influenza and Dengue, two diseases that affect approximately 5% of the world population (in the case of Dengue) and 5% - 15% of the northern hemisphere (in the case of influenza). our application consists of an expert system based on fuzzy logic that analyzes symptoms introduced by the user and formulates a diagnosis in approximately 2 - 4 minutes. This system considerably reduced the diagnostic time, improving the recuperation process from these diseases due to their early detection.

About 130-170 million people, is estimated to be infected with the hepatitis C virus(HCV). Chronic HCV infection is one of the leading causes of liverrelated death and in many countries it is the primaryreason for having a liver transplant. The main aim of antiviral treatment is to eradicate the virus. Until a few years ago the only treatment strategy was based on the combination of pegylated interferon and ribavirin(PEG/RBV). However, in genotypes 1 and 4 the rates of viral response did not surpass 50%, reaching up to 80% in the rest. In 2011 approval was given for the first direct acting antiviral agents(DAA), boceprevir and telaprevir, for treatment of genotype 1, in combination with traditional dual therapy. This strategy managed to increase the rates of sustained viral response(SVR) in both naive patients and in retreated patients, but with greater toxicity, interactions and cost, as well as being less safe in patients with advanced disease, in whom this treatment can trigger decompensation or even death. The recent, accelerated incorporation since 2013 of new more effective DAA, with pan-genomic properties and excellent tolerance, besides increasing the rates of SVR(even up to 100%), has also created a new scenario: shorter therapies, less toxicity and regimens free of PEG/RBV. This has enabled their almost generalised applicability in all patients. However, it should be noted that most of the scientific evidence available is based on expert opinion, case-control series, cohort studies and phase 2 and 3 trials, some with a reduced number of patients and select groups. Few data are currently available about the use of these drugs in daily clinical practice, particularly in relation to the appearance of side effects and interactions with other drugs, or their use in special populations or persons with the less common genotypes. Happiness is the ultimate goal for every living human being, and this condition motivates people to stay active and want to be alive. Having this feeling also fosters the proactivity of citizens. The opposite is depression, a condition capable of significantly affecting a person’s behavior or perception. HoD Detector is a low-cost, easy to use application that aids in the diagnosis of a depressive disorder and motivates the user to seek professional help. The purpose of this study is to propose a way to measure happiness through the measurement of depression;to develop HoD Detector, evaluate the accuracy and usability of this application (with university students) for future enhancements of the system. The development of this application involved basic algorithms and the use of a database to successfully make a diagnosis and store the results of each user for further reference. The application is capable of giving a proper diagnosis with the time interval of 2 - 5 minutes. HoD Detector has the potential to become an extremely useful tool to diagnose depression and motivate users to seek professional help not only in Mexico, but also on an international scale for improving in people’s happiness condition.
