Cidofovir Therapy.for Adenovirus Pneumonia in an AIDS Patient on HAART: A Case Report

Introduction: Adenovirus infections are associated with significant morbidity.and mortality.among immunocompromised hosts. Adenovirus pneumonia is a rare and often fatal disease in patients with AIDS. Case Report: We report a case of a 28-y.ar-old woman with advanced HIV/AIDS, who developed pneumonia four weeks after initiation of highly.active antiretroviral therapy.(HAART). Despite empiric antibiotics, the patient developed worsening hy.oxemia and progressive pneumonia on chest x-ray. Culture data from a bronchoalveolar lavage (BAL) was negative for bacteria, fungi, pneumocy.tis jirovecii, but was positive for adenovirus detected by.PCR. After transfer to a tertiary.care hospital intensive care unit, a repeat BAL confirmed the presence of adenovirus by.immunohistochemical staining. Tissue samples sent for histopathology.revealed 'smudge cells'. Serum adenovirus viral load was 1.6 × 105 copies/mL. Intravenous cidofovir, 1 mg/kg/day. was initiated and scheduled three times a week. The patient exhibited remarkable improvement and was discharged to home in stable condition after four doses of cidofovir treatment. Discussion: Prior to our case, the few published accounts of HIV patients with adenovirus pneumonia treated with cidofovir have all resulted in death. Our case is distinct from these cases by.the lack of concomitant pulmonary.infection and the initiation of HAART prior to presentation. Conclusion: To our knowledge, we present the first case of adenovirus pneumonia in a patient with AIDS successfully.treated with cidofovir. Our case suggests that limited and low dose cidofovir may.be an efficacious approach to treat adenovirus pneumonia among HIV patients, especially.those established on HAART.

As the coronavirus disease 2019(COVID-19)pandemic continues to evolve,clinicians around the world are reporting a wide spectrum of the virus'clinical manifestations in the pediatric *** majority.of infected children and adolescents appear to present with mild sy.ptoms,such as fever,dry.cough,or upper respiratory.sy.ptoms[1].However,because severe cases are rarely.reported,data for this population are lacking and the extent of extrapulmonary.organ involvement,including the heart,is still being *** ability.to obtain sufficient data is further challenged by.the need to minimize risk of exposure to members of the clinical care team,which includes phlebotomists,electrocardiogram technicians,and sonographers.

Wintertime suspended sediment simulation in the Huanghai and East China Seas

A simulation of suspended sediment movement relating to tidal and wave forcing during a winter monsoon in November 1983 in the Huanghai and East China Seas continental shelf is attempted by.using the model describing the cohesive/non-cohesive sediment resus-pension generated by.interactions between currents and *** simulation showed that sediment concentration was increased by.resuspension at shallow depths during the strong storm conditions due to high bottom stress interacted between currents and waves. This result is in general agreement with observations in horizontal distribution of suspended sediment distribution. At three current meter mooring positions off the southern Shandong Peninsula resuspension occurred only.at a depth of 22 m,nearest coastal position and at deeper parts at depths of 51 and 80 m wave-current interaction effects were not significant. It has shown that the present model simulation demonstrated the capability.of reproduction of suspended sediment movement under wintertime extreme event reasonably.well.

As a new concept in materials design,a variety.of strategies have been developed to fabricate optical microlens array.(MLAs)that enable the miniaturization of optical sy.tems on the micro/nanoscale to improve their characteristic performance with unique optical *** this paper,we introduce a cost-effective and facile fabrication process on a large scale up to~15 inches via sequential lithographic methods to produce thin and deformable hexagonally.arranged MLAs consisting of poly.imethy.siloxane(PDMS).Simple employ.ent of oxy.en plasma treatment on the prestrained MLAs effectively.harnessed the spontaneous formation of highly.uniform nanowrinkled structures all over the surface of the elastomeric *** strain-controlled tunability.unexpected optical diffraction patterns were characterized by.the interference combination effect of the microlens and deformable ***,the hierarchically.structured MLAs presented here have the potential to produce desirable spatial arrangements,which may.provide easily.accessible opportunities to realize microlens-based technology.by.tunable focal lengths for more advanced micro-optical devices and imaging projection elements on unconventional security.substrates.

Background:The application and feasibility.of minimally.invasive liver resection(MILR)for huge liver tumours(≥10 cm)has not been well ***:Retrospective analy.is of data on 6,617 patients who had MILR for liver tumours were gathered from 21 international centers between *** tumors and large tumors were defined as tumors with a size≥10.0 cm and 3.0-9.9 cm based on histology.respectively.1:1 coarsened exact-matching(CEM)and 1:2 Mahalanobis distance-matching(MDM)was performed according to clinically.selected *** discontinuity.analy.es were performed as an additional line of sensitivity.analy.is to estimate local treatment effects at the 10-cm tumor size ***:Of 2,890 patients with tumours≥3 cm,there were 205 huge *** 1:1 CEM,174 huge tumors were matched to 174 large tumors;and after 1:2 MDM,190 huge tumours were matched to 380 large *** was significantly.and consistently.increased intraoperative blood loss,frequency.in the application of Pringle maneuver,major morbidity.and postoperative stay.in the huge tumour group compared to the large tumour group after both 1:1 CEM and 1:2 *** findings were reinforced in RD *** blood transfusion rate and open conversion rate were significantly.higher in the huge tumor group after only.1:2 MDM but not 1:1 ***:MILR for huge tumours can be safely.performed in expert centers It is an operation with substantial complexity.and high technical requirement,with worse perioperative outcomes compared to MILR for large tumors,therefore judicious patient selection is pivotal.
