Diffusion-weighted magnetic resonance imaging to predict response of hepatocellular carcinoma to chemoembolization

AIM: To investigate whether intra-procedural diffusion- weighted magnetic resonance imaging can predict response of hepatocellular carcinoma (HCC) during trans- catheter arterial chemoembolization (TACE). METHODS: Sixteen patients (15 male), aged 59 ±11 years (range: 42-81 years) underwent a total of 21 separate treatments for unresectable HCC in a hybrid magnetic resonance/interventional radiology suite. Ana- tomical imaging and diffusion-weighted imaging (b = 0, 500 s/mm2) were performed on a 1.5-T unit. Tumor enhancement and apparent diffusion coefficient (ADC, mm2/s) values were assessed immediately before and at 1 and 3 mo after TACE. We calculated the percent change (PC) in ADC values at all time points. We compared follow-up ADC values to baseline values using a paired t test (α = 0.05). RESULTS: The intra-procedural sensitivity, specificity, and positive and negative predictive values (%) for detecting a complete or partial 1-mo tumor response using ADC PC thresholds of ±5%, ±10%, and ±15% were 77, 67, 91, and 40; 54, 67, 88, and 25; and 46, 100, 100, and 30, respectively. There was no clear predictive value for the 3-mo follow-up. Compared to baseline, the immediate post-procedure and 1-mo mean ADC values both increased; the latter obtaining statistical significance (1.48 ± 0.29 mm2/s vs 1.65 ± 0.35 × 10-3 mm2/s, P 15% predicted 1-mo anatomical HCC response with the greatest accuracy, and can provide valuable feedback at the time of TACE.

Hepatocellular carcinoma(HCC)is the sixth most common cancer in the *** majority of patients with HCC present with unresectable *** patients have historically had limited treatment options secondary to HCC demonstrating chemoresistance to the currently available systemic ***, normal liver parenchyma has shown intolerance to tumoricidal radiation doses,limiting the use of external beam *** of these limitations,novel percutaneous liver-directed therapies have emerged. The targeted infusion of radioactive microspheres (radioembolization)represents one such therapy. Radioembolization is a minimally invasive transcatheter therapy through which radioactive microspheres are infused into the hepatic arteries that supply tumor. Once infused,these microspheres traverse the hepatic vascular plexus and selectively implant within the tumor *** within the arterioles, the 90Y impregnated microspheres emit high energy and low penetrating radiation doses selectively to the *** has recently shown promise for the treatment of patients with unresectable HCC. The objective of this review article is to highlight twocurrently available radioembolic devices(90Y,188Rh)and provide the reader with a recent review of the literature.

Angular unconformity in Pennsylvanian strata from 3-D seismic interpretation,Goldsmith Field,West Texas

The Pennsylvanian unconformity,which is a detrital surface,separates the beds of the Permian-aged strata from the Lower Paleozoic in the Central Basin *** data interpretation indicates that the unconformity is an angular unconformity,overlying multiple normal faults,and accompanied with a thrust fault which maximizes the region's structural ***,the Pennsylvanian angular unconformity creates pinch-outs between the beds above and *** computed the spectral decomposition and reflector convergence attributes and analyzed them to characterize the angular unconformity and *** spectral decomposition attribute divides the broadband seismic data into different spectral bands to resolve thin beds and show thickness *** contrast,the reflector convergence attribute highlights the location and direction of the pinch-outs as they dip south at angles between 2° and 6°.After reviewing findings from RGB blending of the spectrally decomposed frequencies along the Pennsylvanian unconformity,we observed channel-like features and multiple linear bands in addition to the faults and *** can be inferred that the identified linear bands could be the result of different lithologies associated with the tilting of the beds,and the faults may possibly influence hydrocarbon migration or act as a flow barrier to entrap hydrocarbon *** identification of this angular unconformity and the associated features in the study area are vital for the following reasons:1)the unconformity surface represents a natural stratigraphic boundary;2)the stratigraphic pinch-outs act as fluid flow connectivity boundaries;3)the areal extent of compartmentalized reservoirs'boundaries created by the angular unconformity are better defined;and 4)fault displacements are better understood when planning well locations as faults can be flow barriers,or permeability conduits,depending on facies heterogeneity and/or seal effectiveness of a fault,which

Two well-preserved petrified palm stems from the latest Maastrichtian(Late Cretaceous) to earliest Danian(Early Paleocene) sediments of the Deccan Intertrappean Beds of Madhya Pradesh, Central India are described. Their significant anatomical characteristics include a Calamus-type general stem pattern,the presence of well-preserved fibrovascular bundles(fvbs) with two wide metaxylem vessel elements(230 μm-250 μm) and one phloem strand, uniform density of fvbs, lack of continuity between protoxylem and metaxylem vessel elements, and an absence of centrifugal differentiation of sclerenchymatous fibrous *** features reveal a close resemblance to those of extant genera of scandent Calamoideae. The permineralized stems are described as a new species namely, Palmoxylon calamoides kumar, Roy et khan sp. *** fossils represent the oldest reliable fossil records of this family, supporting their Gondwanan origin, their importance in tracing their migration pathways from India to Europe and other continents after the docking of the Indian subcontinent with Eurasia during the Paleocene, and an “Out-of-India” dispersal hypothesis. Today the subfamily Calamoideae is disjunctly occurred in Africa, Asia, Australia, Europe, and South America, but the poor deep-time fossil record of this subfamily with a small number of Cenozoic fossils makes hypotheses concerning its origin and dispersal difficult to evaluate. The present study has significant implications for the origin and migration of this subfamily and the paleoclimate.

Effects of soil metal(loid)s pollution on microbial activities and environmental risks in an abandoned chemical smelting site

Abandoned chemical smelting sites containing toxic substances can seriously threaten and pose a risk to the surrounding ecological *** samples were collected from different depths(0 to 13 m)and analyzed for metal(loid)s content and fractionation,as well as microbial *** potential ecological risk indices for the different soil depths(ordered from high to low)were:1 m(D-1)>surface(S-0)>5 m(D-5)>13 m(D-13)>9 m(D-9),ranging between 1840.65-13,089.62,and representing extremely high environmental risks,of which Cd(and probably not arsenic)contributed to the highest environmental risk.A modified combined pollution risk index(MCR)combining total content and mobile proportion of metal(loid)s,and relative toxicities,was used to evaluate the degree of contamination and potential environmental *** the near-surface samples(S-0 and D-1 layers),the MCR considered that As,Cd,Pb,Sb,and Zn achieved high and alarming degrees of contamination,whereas Fe,Mn,and Ti were negligible or low to moderate pollution *** microcalorimetry and enzymatic activity measurements of contaminated soil samples were used to assess the microbial metabolic activity *** analysis elucidated the relationship between metal(loid)s exchangeable fraction or content and microbial activity characteristics(pk)measurements were considered sensitive indicators to reflect the soil microbial activities in abandoned chemical smelting sites.

为准确获知花生贸易中商业批次的黄曲霉毒素水平,本研究通过模拟应用联合国粮食及农业组织(FAO)的农业品类真菌毒素取样模型,以脱壳整粒花生批次为模拟对象,在中国、日本法规限值下,分别设置实验室样品数目、实验室样品量和接受/拒绝限值,模拟每种组合超出法规限值的接收/放行概率。结果表明,在常规黄曲霉毒素风险的产季/产地下,批次能够满足>90%概率符合中国国标和日本法规的取样方案分别为:设置接受/拒绝限值为黄曲霉毒素B15~10 ng·g^(-1),实验室样品数目和样品量分别为2~3个、5~10 kg;设置接受/拒绝限值为黄曲霉毒素总量5 ng·g^(-1),实验室样品数目和样品量分别为3个、10 kg。推荐在以下3种高风险或风险不确定情景下应用此模型制订升级取样方案、调整接收限值、增强采购/销售信心:黄曲霉毒素高风险产季/产地采购;新开发的供应商/采购区域;批次超标预警时对新批次的放行,以免连续超标。本研究结果为中国花生加工/贸易企业提供了科学取样方案,进而为保障食品安全、保护企业经济利益提供了理论参考。
