colorectal cancer (cRc) is a common cancer that affects one in three men and one in four women worldwide. Late-stage detection is associated with significantly lower 5-year survival rates. Although it is well established that cRc mortality rates have decreased in the past several decades, adoption of routine screening continues to lag behind screening for other common cancers such as cervical and breast cancer. The decrease in overall rates has been attributed, in part, to improved primary and secondary prevention efforts, including smoking prevention and cessation programs, nutritional counseling, and the use of evidence-based screening protocols, as well as access to better treatment. Despite the increased screening rates, it is estimated that at least one-third of eligible people do not receive appropriate screening. The objective of this review is to describe the current epidemiology of cRc and to demonstrate effective primary and secondary prevention strategies for the primary care provider.

Minocycline hydrochloride（MH）, a semi-synthetic tetracycline derivative, is a clinically available antibiotic and anti-inflammatory drug that also exhibits potent neuroprotective activities. It has been shown to target multiple secondary injury mechanisms in spinal cord injury, via its anti-inflammatory, anti-oxidant, and anti-apoptotic properties. The secondary injury mechanisms that MH can potentially target include inflammation, free radicals and oxidative stress, glutamate excitotoxicity, calcium influx, mitochondrial dysfunction, ischemia, hemorrhage, and edema. This review discusses the potential mechanisms of the multifaceted actions of MH. Its anti-inflammatory and neuroprotective effects are partially achieved through conserved mechanisms such as modulation of p38 mitogen-activated protein kinase（MAPK） and phosphoinositide 3-kinase（PI3K）/Akt signaling pathways as well as inhibition of matrix metalloproteinases（MMPs）. Additionally, MH can directly inhibit calcium influx through the N-methyl-D-aspartate（NMDA） receptors, mitochondrial calcium uptake, poly（ADP-ribose） polymerase-1（PARP-1） enzymatic activity, and iron toxicity. It can also directly scavenge free radicals. Because it can target many secondary injury mechanisms, MH treatment holds great promise for reducing tissue damage and promoting functional recovery following spinal cord injury.

The throughflow in the canadian Arctic Archipelago(cAA)had a significant impact on the North Atlantic Ocean with the Arctic climate *** findings of physical mechanisms driving the throughflow in the cAA differed and few studies about the impact of sea level pressure(SLP)on the cAA throughflow were made.A high-resolution ice-ocean coupled Arctic Ocean Finite-Volume community Ocean Model(AO-FVcOM)was used over the period 1978-2016 to examine the interannual and seasonal variability of the outflows in the cAA and the mechanism of SLP in driving the variation of the cAA throughflow *** simulated volume transport through Davis Strait,Nares Strait,Lancaster Sound and Jones Sound showed consistent increasing trends over 1978-2016 and the larger flux in winter and spring than in summer and *** variation of volume transport through Nares Strait contributed more than Lancaster and Jones Sound to the variation through Davis *** process-oriented experiments were made to further explore the role of SLP in setting up and controlling the sea surface height(SSH)difference and thus the throughflow transport in the *** SLP was a primary forcing to control the SSH difference and the outflow transport compared with the wind *** memory of the SSH to the SLP was short and an equilibrium state could be reached if the SLP varied with *** upstream and downstream SLP difference,however,made a slight direct contribution to driving the volume transport of the cAA *** addition to the external forcing of SLP and wind,the variability of the cAA outflow was also influenced by the variability of the inflow/outflow and SSH on boundaries connected to the Pacific and Atlantic *** choice of SLP datasets from cORE-v2,EcMWF and NcEP was sensitive to the simulated uncertainty of volume transport.

cognitive Radio （cR） is a multiuser, wireless communication concept that allows for a dynamic and adaptive assignment of spectral resources. The coexistence of multiple users, often transmitting at significantly different power levels, makes cR receivers vulnerable to spectral leakage caused by components＇ nonlinearities and timetruncation of the processed signal records. In this work we propose a method for mitigating the latter with an adaptive choice of the length of the processing block size. With simulations we show that a significant leakage reduction that leads to receiver dynamic range improvement of around 10 dB can be achieved with the proposed method.

AIM： Grapefruit-seed extract （GSE） containing flavonoids, possesses antibacterial and antioxidative properties but whether it influences the gastric defense mechanism and gastroprotection against ethanol- and stress-induced gastric lesions remains unknown. METHODS： We compared the effects of GSE on gastric mucosal lesions induced in rats by topical application of 100% ethanol or 3.5 h of water immersion and restraint stress （WRS） with or without （A） inhibition of cyclooxygenase （cOX）-1 activity by indomethacin and rofecoxib, the selective cOX-2 inhibitor, （B） suppression of NO-synthase with L-NNA （20 mg/kg ip）, and （c） inactivation by capsaicin （125 mg/kg sc） of sensory nerves with or without intragastric （ig） pretreatment with GSE applied 30 min prior to ethanol or WRS. One hour after ethanol and 3.5 h after the end of WRS, the number and area of gastric lesions were measured by planimetry, the gastric blood flow （GBF） was assessed by H2-gas clearance technique and plasma gastrin levels and the gastric mucosal generation of PGE2, superoxide dismutase （SOD） activity and malonyldialdehyde （MDA） concentration, as an index of lipid peroxidation were determined. RESULTS： Ethanol and WRS caused gastric lesions accompanied by the significant fall in the GBF and SOD activity and the rise in the mucosal MDA content. Pretreatment with GSE （8-64 mg/kg i g） dose- dependently attenuated gastric lesions induced by 100% ethanol and WRS; the dose reducing these lesions by 50% （ID50） was 25 and 36 mg/kg, respectively, and this protective effect was similar to that obtained with methyl PGE2 analog （5 μg/kg i g）. GSE significantly raised the GBF, mucosal generation of PGE2, SOD activity and plasma gastrin levels while attenuating IVlDA content. Background:To measure angle-to-angle(ATA)and spur-to-spur(STS)distances along six meridians using highresolution swept-source optical coherence tomography(SS-OcT)and to compare those values with horizontal white-to-white(WTW)***:68 eyes from 68 patients were quantitatively assessed with the Anterion SS-OcT(Heidelberg Engineering,Heidelberg,Germany).ATA and STS distances were measured with the SS-OcT’s B-Scan in six cross-sectional images corresponding to the vertical(6-12 o’clock),1-7 o’clock,2-8 o’clock,horizontal(3-9 o’clock),4-10 o’clock and 5-11 o’clock *** was measured horizontally with the device’s infrared camera.A Pearson correlation analysis was carried out to compare ATA and STS distances with ***:The largest values were found for the vertical meridian and the shortest for the 2-8 o’clock meridian,both for ATA and STS *** statistically significant differences were found between WTW,ATA and STS along the horizontal meridian(p>0.1).However,ATA and STS showed statistically significant differences elsewhere,except for the horizontal and the 2-8 o’clock meridians(p>0.05).Moreover,we found that ATA and STS varied significantly depending on the meridian being assessed,except for ATA at 4-10 versus 3-9 o’clock and for STS at 4-10 versus 3-9 o’clock and at 3-9 versus 2-8 o’clock(p>0.1).R^(2) values ranged from 0.49 to 0.75 for ATA and STS at the different meridians,showing the best correlation at 3-9 o’clock meridian(0.64 and 0.75,respectively)and the worst at 6-12 o’clock meridian(R^(2)=0.49 for both ATA and STS).conclusions:ATA and STS distances vary radially,thus showing that the anterior chamber is vertically ***,it is advisable to measure these two distances along the meridian to be used.

BAcKGROUND QTc interval prolongation with an increased risk of torsade de pointes(Tsd)has been described in coronavirus disease 2019(cOVID-19)patients treated with hydroxychloroquine(HcQ)and azithromycin(AZI)in Western *** the DR congo,few studies have evaluated the safety of this association or proposed new *** To determine the incidence of QTc prolongation and Tsd in cOVID-19 patients treated with HcQ-AZIs vs doubase c(new molecule).METHODS In present randomized clinical trial,we have included patients with mild or moderate cOVID-19 treated with either HcQ-AZI or doubase ***(EcG)changes on day 14 of randomization were determined based on pretreatment *** QTc was defined as≥500 ms on day 14 or an increase of≥80 ms compared to pretreatment *** with cardiac disease,those undergoing other treatments likely to prolong QTc,and those with disturbed EcG tracings were excluded from the *** The study included 258 patients(mean age 41±15 years;52%men;3.4%diabetics,11.1%hypertensive).Mild and moderate cOVID-19 were found in 93.5%and 6.5%of patients,*** baseline,all patients had normal sinus rhythm,a mean heart rate 78±13/min,mean PR space 170±28 ms,mean QRS 76±13 ms,and mean QTc 405±30 *** complaints suggesting cardiac involvement were reported during or after *** four patients(1.5%)experienced QTc interval prolongation beyond 500 ***,only five patients(1.9%)had an increase in the QTc interval of more than 80 *** prolongation was more significant in younger patients,those with high viral load at baseline,and those receiving HcQ-AZI(Pc prolongation without Tsd was observed at a lower frequency in patients treated with HcQ-AZI vs doubase *** absence of comorbidities and concurrent use of other products that are likely to cause arrhythmia may explain our results.
